7. Traditional wisdom of conserving biodiversity through sacred groves: Ethnobotanical treasure in Garhwal Himalayas.

Abstract

The present study was conducted to record the ethnomedicinal plants and their uses in sacred groves of Rudraprayag district in Garhwal Himalaya, Uttarakhand. Field surveys were conducted to identify sacred groves and information was collected from local folks using structured questionnaires. Three sacred groves, namely 1) Banjyani, 2) Jakh devta, and 3) Satoli were identified in the region. The survey revealed 31 medicinal plant species representing 26 families and 29 genera. These plant species belong to diverse families and most dominant families in the groves were Lamiaceae, Moraceae, Ranunculaceae and Rosaceae. The medicinal plants used for different ailment in the sacred groves were dominated by herbs, followed by shrubs and trees. To treat different ailments, leaves were used in maximum number of ailments (33.33%), followed by bark (16.67%), root (12.50%), fruit (12.50%), whole plant (10.42%), flowers (6.25%), rhizome (4.17%) and bulb (2.09%). In identified sacred groves plants were used for different medicinal purposes i.e., cough, diarrhea, leprosy, leucorrhoea, jaundice, bronchitis, digestive issues and to improve appetite. The study confirms the dependence of local people on sacred groves for their health care. However, presence of threatened plants highlights the importance of sustainable utilization of medicinal plant resources available in the groves. [ABSTRACT FROM AUTHOR]

Abstract

Introduction. Psoriasis is a chronic inflammatory skin disease with hyperproliferation, abnormal differentiation and inflammatory infiltration in epidermis and dermis. Sometimes it is clinically and histopathologically challenging to distinguish psoriasis from other non-psoriatic psoriasiform dermatoses (NPPD) like eczema, pityriasis rosea, pityriasis rubra pilaris, and lichen simplex chronicus. Ki-67 is a non-histone nuclear protein complex that regulates the cell cycle and is the most widely used proliferation immunohistochemistry (IHC) marker. Its levels have been shown to be raised in psoriasis compared to normal skin. Aim. To elucidate and compare expression of IHC Ki-67 in psoriasis and NPPD, correlate these levels with clinical variants and disease severity in psoriasis and to observe change in levels with demographic and psoriasis-related variables. Material and Methods. Thirty patients, each with clinically diagnosed psoriasis (cases), and NPPD (controls) were enrolled. Biopsy was taken for histopathology and IHC Ki-67 immunohistochemistry. Statistical analysis was performed. Results. We found a significantly higher expression of IHC Ki-67 in psoriasis as compared to all types of NPPD. The higher level of Ki-67 in pustular and erythrodermic psoriasis compared to plaque-type emphasizes the greater severity and activity of these forms. The Ki-67 expression was found to increase with increasing body surface area involvement and disease severity (PASI) in chronic plaque type. Pityriasis rubra pilaris had the highest Ki67 expression among NPPD group. Conclusion. Ki-67 is a promising tool with diagnostic and prognostic utility in psoriasis, particularly when it comes to its differentiation from nonpsoriasis psoriasiform disorders.

22. Teriparatide Therapy in a 4-Month-Old With Severe Hypoparathyroidism.

Abstract

Teriparatide is a human parathyroid hormone analog approved for the treatment of osteoporosis in adult patients. Its use for hypocalcemia and hypoparathyroidism in the pediatric population is described through case reports and small case series; however, larger studies that demonstrate long-term efficacy and safety are limited. At our institution, a 4-month-old premature (gestational age: 32 weeks) infant with multiple congenital anomalies, functional athymia, and severe hypoparathyroidism and receiving calcitriol, vitamin D, and calcium carbonate supplementation was initiated on subcutaneous injection of teriparatide. During the course of treatment, her calcium carbonate, vitamin D, and calcitriol supplementation requirements substantially decreased. Teriparatide effectively increased serum ionized calcium concentrations and decreased serum phosphorus concentrations in the present case-study over a 6-month period. Teriparatide was well tolerated, and no evidence of hypercalcemia was observed throughout treatment. [ABSTRACT FROM AUTHOR]
