1. CLADE: Cycle Loss Augmented Degradation Enhancement for Unpaired Super-Resolution of Anisotropic Medical Images

Abstract

Three-dimensional (3D) imaging is popular in medical applications, however, anisotropic 3D volumes with thick, low-spatial-resolution slices are often acquired to reduce scan times. Deep learning (DL) offers a solution to recover high-resolution features through super-resolution reconstruction (SRR). Unfortunately, paired training data is unavailable in many 3D medical applications and therefore we propose a novel unpaired approach; CLADE (Cycle Loss Augmented Degradation Enhancement). CLADE uses a modified CycleGAN architecture with a cycle-consistent gradient mapping loss, to learn SRR of the low-resolution dimension, from disjoint patches of the high-resolution plane within the anisotropic 3D volume data itself. We show the feasibility of CLADE in abdominal MRI and abdominal CT and demonstrate significant improvements in CLADE image quality over low-resolution volumes and state-of-the-art self-supervised SRR; SMORE (Synthetic Multi-Orientation Resolution Enhancement). Quantitative PIQUE (qualitative perception-based image quality evaluator) scores and quantitative edge sharpness (ES - calculated as the maximum gradient of pixel intensities over a border of interest), showed superior performance for CLADE in both MRI and CT. Qualitatively CLADE had the best overall image quality and highest perceptual ES over the low-resolution volumes and SMORE. This paper demonstrates the potential of using CLADE for super-resolution reconstruction of anisotropic 3D medical imaging data without the need for paired 3D training data.

6. The role of magnetic resonance enterography and small bowel ultrasound in Crohn's disease

Abstract

Crohn's disease (CD), a life-long condition resulting in recurrent gastrointestinal inflammation, is estimated to affect one in every 650 people in the UK. Accurate assessment of the disease is crucial to optimise medical and surgical management. Small bowel assessment is heavily reliant on radiological imaging. As the condition predominantly affects young patients, who are more sensitive to the harmful effects of ionizing radiation, ultrasound, and Magnetic Resonance Imaging (MRI) assessment are the preferred radiological investigations. The thesis explores the utility of MR enterography (MRE) and small bowel ultrasound (SBUS) in Crohn's disease. The thesis is structured as follows: Section A provides an overview on the utility of MRE and SBUS in Crohn's disease including review and evaluation of the pre-existing literature. It outlines methodological details of the METRIC (MREnterography or ulTRasound in Crohn's disease) trial: a multicentre, non-randomised, single-arm, prospective comparison study of magnetic resonance enterography and small bowel ultrasound compared to a reference standard in patients aged over 16 years and briefly outlines its main results. Section B details specific substudies of the METRIC trial, investigating the inter-observer variability of the two techniques, influence of oral contrast type and volume on patient experience and quality of luminal distension at MRE and the influence of Diffusion Weighted Imaging and Contrast Enhanced T1 sequences on the diagnostic accuracy of MRE. Section C presents further complementary studies which undertake more in depth analysis of the two imaging techniques: (1) evaluating the relationship between contrast enhanced MRE texture analysis parameters and the presence or absence of histological markers of hypoxia and angiogenesis and (2) evaluating changes in the Dynamic Contrast Enhanced pharmacokinetic and Diffusion Weighted Imaging parameters at MRE relative to clinical response to anti-TNF medication and (3) providing histological validation of SBUS features of inflammation and fibrosis. Section D concludes and summarises the thesis and makes recommendations for future research.

Abstract

Summary: Background: Stricturing Crohn's disease (CD) occurs most commonly in the terminal ileum and poses a clinical problem. Cross‐sectional imaging modalities such as intestinal ultrasound (IUS), computed tomography enterography (CTE), and magnetic resonance enterography (MRE) allow for assessment of the entire bowel wall and associated peri‐enteric findings. Radiologic definitions of strictures have been developed for CTE and MRE; their reliability and responsiveness are being evaluated in index development programs. A comprehensive assessment strategy for strictures using IUS is needed. Aims: To provide a detailed summary of definitions, diagnosis and monitoring of strictures on IUS as well as technical aspects of image acquisition. Methods: We searched four databases up to 6 January 2024. Two‐stage screening was done in duplicate. We assessed risk of bias using QUADAS‐2. Results: There were 56 studies eligible for inclusion. Definitions for strictures on IUS are heterogeneous, but the overall accuracy for diagnosis of strictures is high. The capability of IUS for characterising inflammation versus fibrosis in strictures is not accurate enough to be used in clinical practice or trials. We summarise definitions for improvement of strictures on IUS, and discuss parameters for image acquisition and standardisation. Abstract

Background and Aims The simplified magnetic resonance enterography [MRE] index of activity [sMARIA], London, and 'extended' London, scoring systems are widely used in Crohn's disease [CD] to assess disease activity, although validation studies have usually been single-centre, retrospective, and/or used few readers. Here, we evaluated these MRE indices within a prospective, multicentre, multireader, diagnostic accuracy trial. Methods A subset of participants [newly diagnosed or suspected of relapse] recruited to the METRIC trial with available terminal ileal [TI] biopsies was included. Using pre-specified thresholds, the sensitivity and specificity of sMARIA, London, and 'extended' London scores for active and severe [sMARIA] TI CD were calculated using different thresholds for the histological activity index [HAI]. Results We studied 111 patients [median age 29 years, interquartile range 21-41, 75 newly diagnosed, 36 suspected relapse] from seven centres, of whom 22 had no active TI CD [HAI = 0], 39 mild [HAI = 1], 13 moderate [HAI = 2], and 37 severe CD activity [HAI = 3]. In total, 26 radiologists prospectively scored MRE datasets as per their usual clinical practice. Sensitivity and specificity for active disease [HAI >0] were 83% [95% confidence interval 74% to 90%] and 41% [23% to 61%] for sMARIA, 76% [67% to 84%] and 64% [43% to 80%] for the London score, and 81% [72% to 88%] and 41% [23% to 61%] for the 'extended' London score, respectively. The sMARIA had 84% [69-92%] sensitivity and 53% [41-64%] specificity for severe CD. Conclusions When tested at their proposed cut-offs in a real-world setting, sMARIA, London, and 'extended' London indices achieve high sensitivity for active TI disease against a histological reference standard, but specificity is low. Abstract

Summary: Background: Healthcare service provision in inflammatory bowel disease (IBD) is often designed to meet targets set by healthcare providers rather than those of patients. It is unclear whether this meets the needs of patients, as assessed by patients themselves. Aims: To assess patients' experience of IBD and the healthcare they received, aiming to identify factors in IBD healthcare provision associated with perceived high‐quality care. Methods: Using the 2019 IBD standards as a framework, a national benchmarking tool for quality assessment in IBD was developed by IBD UK, comprising a patient survey and service self‐assessment. Results: 134 IBD services and 9757 patients responded. Perceived quality of care was lowest in young adults and increased with age, was higher in males and those >2 years since diagnosis. No hospital services met all the national IBD standards for recommended workforce numbers. Key metrics associated with patient‐reported high‐ quality care were: identification as a tertiary centre, patient information availability, shared decision‐ making, rapid response to contact for advice, access to urgent review, joint medical/surgical clinics, and access to research (all p < 0.001). Higher numbers of IBD nurse specialists in a service was strongly associated with patients receiving regular reviews and having confidence in self‐management and reporting high‐ quality care. Conclusions: This extensive patient and healthcare provider survey emphasises the importance of aspects of care less often measured by clinicians, such as communication, shared decision‐ making and provision of information. Abstract

Purpose: To prospectively evaluate interobserver agreement for small bowel ultrasound (SBUS) in newly diagnosed and relapsing Crohn's disease. Methods: A subset of patients recruited to a prospective trial comparing the diagnostic accuracy of MR enterography and SBUS underwent a second SBUS performed by one of a pool of six practitioners, who recorded the presence, activity and location of small bowel and colonic disease. Detailed segmental mural and extra-mural observations were also scored. Interobserver variability was expressed as percentage agreement with a construct reference standard, split by patient cohort, grouping disease as present or absent. Prevalence adjusted bias adjusted kappa (PABAK), and simple percentage agreement between practitioners, irrespective of the reference standard, were calculated. Results: Thirty-eight patients (11 new diagnosis, 27 relapse) were recruited from two sites. Overall percentage agreement for small bowel disease presence against the consensus reference was 82% (52–95% (95%CI)), kappa coefficient (κ) 0.64, (substantial agreement) for new diagnosis and 81%, κ 0.63 (substantial agreement) for the relapsing cohort. Agreement for colonic disease presence was 64%, κ 0.27 (fair agreement) in new diagnosis and 78%,κ 0.56 (moderate agreement) in the relapsing cohort. Simple agreement between practitioners was 84% and 87% for small bowel and colonic disease presence respectively. Practitioners agreed on small bowel disease activity in 24/27 (89%) where both identified disease. Kappa agreement for detailed mural observations ranged from κ 0.00 to 1.00. Conclusion: There is substantial practitioner agreement for small bowel disease presence in newly diagnosed and relapsing CD patients, supporting wider dissemination of enteric US. Abstract

Cross sectional Imaging plays an increasingly important role the diagnosis and management of Crohn's disease. Particular emphasis is placed on MRI and Ultrasound as they do not impart ionising radiation. Both modalities have reported high sensitivity for disease detection, activity assessment and evaluation of extra-luminal complications, and have positive effects on clinical decision making. International Guidelines now recommend MRI and Ultrasound in the routine management of Crohn's disease patients. This article reviews the current evidence base supporting both modalities with an emphasis on the key clinical questions.
