Purpose: To evaluate efficacy of an anesthetic mucoadhesive film with a polymeric device (PD) in promoting anesthesia compared to conventional local infiltration (LA) in children. Methods: 50 children aged 6-10 years (both genders) needing similar procedures on homologous teeth on the maxilla were included. The parents and children were asked about perception of dental treatment. The child's heart rate per minute (bpm) and blood pressure were evaluated before and after each anesthetic technique (AT) procedure. Anesthesia efficacy was measured by reporting pain using Wong-Baker Faces Scale. Children's behavior and AT preferences were also evaluated. Paired T-test. chi-square and Wilcoxon test were used for statistical comparisons. Results: Fear of anesthesia was reported by 50% of caregivers and by 66% of children. No difference was observed in systolic (P= 0.282) and diastolic (P= 0.251) blood pressure, comparing both AT. Difference was observed regarding the child's behavior when the PD was used (P= 0.0028), Evaluating the face scale, 74% of the children selected the "no pain" (face 01 (P< 0.0001) for PD, and 26% for LA. Objective: The aim of this study was to evaluate the possible association between genetic polymorphisms in DEFB1 (rs1799946 and rs11362) and the development of gingivitis in children from the Amazon region of Brazil. Material and methods: The study included 27 healthy children, 10-12 years old, from public schools in Manaus. For the oral examination, the parameters described in the gingival index of Löe and Silness were used to identify the presence or absence of gingivitis. The dependent variable was categorized as “Yes” for children with gingivitis (experimental group; n=10) and “No” for children without gingivitis (control group; n=17). Genomic DNA was extracted from saliva, the selected polymorphisms in the DEFB1 gene (rs1799946 and rs11362) were genotyped by TaqMan PCR and an endpoint analysis was performed. Genotypic and allelic distribution between groups was performed using the Fisher’s exact test with an established alpha of 5%. Results: It was found that evaluated SNPs in the DEFB1 gene (rs1799946 and rs11362) were not associated with gingivitis (p>0.05). Conclusion: The single nucleotide polymorphisms (SNP) of references rs1799946 and rs11362 in the DEFB1 gene had no function on gingivitis, promoting neither harmful nor beneficial effects, in children. Objective: To evaluate the periapical healing following root canal treatment in teeth with apical periodontitis (in vivo) and the cytotoxic potential of root canal sealers in vitro. Material and Methods: Apical periodontitis was induced in 60 dogs' teeth and root canals were filled with Sealapex (40 roots), EndoREZ (40 roots), intracanal dressing (20 roots), or left untreated (20 roots). After 30 and 90 days, histopathological analyses were made. In vitro, J774.1 macrophages were stimulated with root canal sealers extracts, cytotoxicity was assessed using lactate dehydrogenase assay, and qRT-PCR was used to analyze TNF-α gene expression. Results: In vivo, smaller apical periodontitis and lower inflammatory cell infiltrate were found in teeth treated with Sealapex compared to EndoREZ. In vitro, EndoREZ was cytotoxic and induced TNF-α gene expression by macrophages differently from Sealapex. Conclusion: Sealapex allowed improved tissue repair following root canal treatment in teeth with apical periodontitis compared to EndoREZ. Synthesis of TNF-α induced by LPS was enhanced by EndoREZ, whereas Sealapex prevented pro-inflammatory gene expression. [ABSTRACT FROM AUTHOR]

This study evaluated the effect of antimicrobial photodynamic therapy (aPDT) on the endodontic treatment of apical periodontitis (AP). AP was induced in 48 premolars of 6 dogs. After biomechanical preparation, the teeth were divided into 4 groups: Calcium-Hydroxide (CH)/120d and CH/180d: root canals filled with CH-based dressing for 15 days before obturation; aPDT/120d and aPDT/180d: conditioning with phenothiazine photosensitizer (10 mg/mL) for 1 minute and irradiation with diode laser in the same session as obturation. Root filling was performed with AH Plus sealer. After the experimental periods, animals were euthanized and teeth were submitted for histology. HE staining was performed for descriptive analysis of the periapical region, measurement of apical periodontitis and for inflammatory cells, and blood vessels count. Immunohistochemistry was performed for osteopontin (OPN) and alkaline phosphatase (ALP). Data were analyzed statistically by two-way ANOVA and chi-square test (α = 5%). Teeth in Group CH/120d presented only a slightly enlarged periodontal ligament (PL) with advanced repair. Group aPDT/120d presented the PL moderately enlarged, with moderate inflammatory infiltrate and few collagen fibers. The same pattern was observed at 180 days. AP lesions in CH-treated groups were smaller than those in aPDT-treated groups (p < 0.001) with more blood vessels (p < 0.0001), regardless of the evaluation period, without significant differences in the number of inflammatory cells (p > 0.05). CH-treated groups showed significantly more intense immunostaining for ALP and OPN (p < 0.001) in both periods. Although aPDT stimulated angiogenesis and expression of bone formation markers, the two-session endodontic treatment with CH-based dressing promoted better apical periodontitis repair. Objectives: The aim of this study was to evaluate the association between tooth agenesis and skeletal malocclusions in Brazilian non-syndromic orthodontic patients. Material and Methods: Pretreatment orthodontic records of 348 patients of both genders and with various skeletal malocclusions were examined. Tooth agenesis was evaluated in panoramic radiographs. Angular measurements were taken from lateral cephalometric radiographs to classify the patient's malocclusion as skeletal Class I, Class II and Class III. Subjects were divided into 2 groups, "with tooth agenesis" and "without tooth agenesis". Chi-square or Fisher exact test was used to compare categorical data. ANOVA with Tukey's post-test was used for means comparisons. An alpha of 5% was established. Results: From 348 analysed patients, 28 presented tooth agenesis. There was no difference between genders (P = 0.27) nor mean age (P = 0.16). The most prevalent skeletal malocclusion was Class I (63.11%), followed by Class II (25.94%), and Class III (10.95%). The mean of congenitally missing teeth was 1.3 (SD 0.13). Thirteen subjects had premolar agenesis, 13 upper lateral incisor agenesis, 4 lower incisor agenesis and 2 molars agenesis. The group with tooth agenesis presented A pointnasion- B point (ANB) angle smaller (1.66 [SD 2.52]) than the group without tooth agenesis (2.86 [SD 2.49]) (P = 0.01). ANB angle had a negative correlation with the number of congenitally missing teeth (P = 0.039; r = -0.39). Conclusions: Tooth agenesis is associated with a smaller A point-nasion-B point angle and is negatively correlated with the number of congenitally missing teeth. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study was to evaluate the association between caries and body mass index (BMI) deviations in Brazilian schoolchildren and adolescents. Methods: A total of 237 three- to 15-year-old children, all undergoing treatment in a university pediatric dental clinic, was evaluated. Caretakers answered a questionnaire on sociodemographic, ethnicity, oral hygiene, and dietary information. The children's height was measured in centimeters, the weight in kilograms, and BMIs were calculated. A dental exam was done. All data were analyzed using Epi Info 7.1.5 and Graph Pad Prism 5.0 software with a significance level of five percent. Results: The mean BMI in the studied population was 17.78 (±3.30 SD); all scores fell within the 12.5 to 30.0 BMI range. The distribution of decayed, missing, and filled primary and permanent teeth (dmft/DMFT) was statistically similar among normal weight, underweight, overweight, and obese children (P>0.05). There was no correlation between BMI and caries experience. The univariate and multivariate analysis using modifier factors as covariables did not demonstrate an association between BMI status and caries experience (P>0.05). Purpose To evaluate the effects of radiation therapy on deciduous teeth. Materials and methods The enamel and dentin microhardness (n = 12) was evaluated at 3 depths, both before (control) and after each 10 Gy of irradiation and up to a dose of 60 Gy. The morphology was evaluated via scanning electron microscopy (SEM) (n = 8). The data were analyzed using a two-way analysis of variance (ANOVA) and Tukey's test (α = 5%). Results The enamel microhardness, as a whole, increased (p < 0.05) after a dose of 60 Gy (211.4 KH), mostly in the superficial enamel. There was a significant difference between the values of nonirradiated dentin microhardness (28.9 KH) compared with dentin that was irradiated with doses of 10 Gy (23.8 KH), 20 Gy (25.6 KH), 30 Gy (24.8 KH), and 40 Gy (25.7 KH) (p < 0.05). There was no difference between nonirradiated dentin and dentin irradiated with 60 Gy (p > 0.05). The highest mean value of microhardness (29.9 KH) (p < 0.05) was found in the middle dentin. The groups that were irradiated with doses of 30 and 60 Gy exhibited greater surface changes in their enamel and dentin compared with the nonirradiated groups for all regions, exhibiting an amorphous surface upon increase of the irradiation doses. Conclusions The enamel microhardness increased at a dose of 60 Gy, whereas the value of the dentin microhardness did not change. Lymphangioma is a hamartoma resulting from the proliferation of common lymphatic vessels in the head and neck area, rarely appearing in the lower lips. Its clinical presentation is a nodular mass with a pebbly surface and no defined borders formed by a cluster of slow-growing coalescing vesicles. The purpose of this paper is to present two children with a lesion in the lower lip whose clinical characteristics (single papillary lesion with a pediculated base, same color and consistency of the surrounding mucosa, and negative result for diascopy test) suggested an initial diagnosis of mucocele but were later confirmed as cavernous lymphangiomas. The clinical and microscopic characteristics of the lesions are discussed as well as the differential diagnosis and the treatment approach. These cases reinforce the importance of always performing a confirmatory histopathological analysis, even for lesions with typical clinical features. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this case report is to describe a RUNX2 nonsense mutation associated with cleidocranial dysplasia (CCD) with unusual dental features. The patient was a 12-year-old Brazilian girl who sought dental care due to over-retention of primary teeth and absence of erupted permanent teeth. Clinical and radiographic examinations revealed multiple impacted permanent teeth, a prominent cingulum of the permanent impacted maxillary incisors and enamel defects (hypoplasia and hypomineralization) in addition to skeletal abnormalities. No supernumerary teeth were present. The diagnostic hypothesis of CCD was raised and the patient was referred to the genetic medical service, where the diagnosis was cofirmed. After RUNX2 genetic screening, including polymerase chain reaction and sequencing of both DNA strands, a heterozygous nonsense mutation was identified in exon 2 (c.193 C>T [Q65X]).
