Abstract

Background Handgrip strength, a measure of muscular fitness, is associated with cardiovascular (CV) events and CV mortality but its association with cardiac structure and function is unknown. The goal of this study was to determine if handgrip strength is associated with changes in cardiac structure and function in UK adults. Methods and results Left ventricular (LV) ejection fraction (EF), end-diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV), mass (M), and mass-to-volume ratio (MVR) were measured in a sample of 4,654 participants of the UK Biobank Study 6.3 ± 1 years after baseline using cardiovascular magnetic resonance (CMR). Handgrip strength was measured at baseline and at the imaging follow-up examination. We determined the association between handgrip strength at baseline as well as its change over time and each of the cardiac outcome parameters. After adjustment, higher level of handgrip strength at baseline was associated with higher LVEDV (difference per SD increase in handgrip strength: 1.3ml, 95% CI 0.1±2.4; p = 0.034), higher LVSV (1.0ml, 0.3±1.8; p = 0.006), lower LVM (-1.0g, -1.8 ±-0.3; p = 0.007), and lower LVMVR (-0.013g/ml, -0.018 ±-0.007; p Conclusions Better handgrip strength was associated with cardiac structure and function in a pattern indicative of less cardiac hypertrophy and remodeling. These characteristics are known to be associated with a lower risk of cardiovascular events.

38. Abstract 12340: Utilization and Outcomes of Catheter-directed Thrombolysis for Acute Pulmonary Embolism: A Nationwide Cohort Study

Abstract

Introduction:Catheter-directed thrombolysis (CDT) may improve short-term effectiveness and safety in selected patients with acute pulmonary embolism (PE) who have high-risk features and a treatment plan for thrombolysis. However, there are few data comparing CDT with ultrasound facilitated low-dose thrombolysis (CDT-US) versus standard CDT (S-CDT) with low-dose thrombolysis alone.Hypothesis:S-CDT results in similar clinical outcomes to CDT-US.Methods:We analyzed adults >18 years of age hospitalized with acute PE and treated with thrombolysis in the 2016 Nationwide Readmissions Database. CDT-US and S-CDT were determined using ICD-10-PCS claims codes specific for each procedure. Primary outcomes were in-hospital mortality and 30-day readmission rates. Hierarchical logistic regression models, adjusted for patient demographics, cor pulmonale, and comorbidities, were used to model outcomes according to treatment strategy employed.Results:Among 5,654 patients hospitalized with acute PE, 62.6% (N=3,539) were treated with systemic thrombolysis and 37.4% (N=2,115) with CDT. Among those receiving CDT, 431 (20%) were performed with US. Mean age of those receiving CDT was 60.0 ? 15.2 years and 49.2% were female. Acute cor pulmonale was present in 31.6%, 9.9% had hypotension, and 7.9% were mechanically ventilated. There was no difference in comorbidities, GI bleeding, intracranial hemorrhage, mechanical ventilation and hypotension (p>0.05 for all) between S-CDT and CDT-US. Likewise, in-hospital mortality and 30-day readmission rates were similar for CDT-US and S-CDT (Figure). After adjustment, no treatment differences were identified (p>0.05 for all).Conclusions:Among patients admitted with acute PE in the US, S-CDT had similar outcomes compared with CDT-US. Given the higher cost of CDT-US, a head-to-head randomized trial comparing these therapies seems warranted before CDT-US is applied more broadly. Figure:CDT-US versus S-CDT.
