Abstract

BackgroundDrug-drug interactions (DDIs) involving vitamin K antagonists (VKAs) constitute an important cause of in-hospital morbidity and mortality. However, the list of potential DDIs is long; the implementation of all these interactions in a clinical decision support system (CDSS) results in over-alerting and alert fatigue, limiting the benefits provided by the CDSS. ObjectiveTo estimate the probability of occurrence of international normalized ratio (INR) changes for each DDI rule, via the reuse of electronic health records. MethodsAn 8-year, exhaustive, population-based, historical cohort study including a French community hospital, a group of Danish community hospitals, and a Bulgarian hospital. The study database included 156,893 stays. After filtering against two criteria (at least one VKA administration and at least one INR laboratory result), the final analysis covered 4047 stays. Exposure to any of the 145 drugs known to interact with VKA was tracked and analyzed if at least 3 patients were concerned. The main outcomes are VKA potentiation (defined as an INR≥5) and VKA inhibition (defined as an INR≤1.5). Groups were compared using the Fisher exact test and logistic regression, and the results were expressed as an odds ratio (95% confidence limits). Abstract

Aims: The objective of the study was to describe the impact of a clinical decision support system (CDSS) on antidiabetic drug management by clinical pharmacists for hospitalized patients with T2DM. Methods: We performed a retrospective, single‐centre study in a teaching hospital, where clinical pharmacists analysed prescriptions and issued pharmacist interventions (PIs) through a computerized physician order entry (CPOE) system. A CDSS was integrated into the pharmacists' workflow in July 2019. We analysed PIs during 2 periods of interest: one before the introduction of the CDSS (from November 2018 to April 2019, PIs issued through the CPOE alone) and one afterwards (from November 2020 to April 2021, PIs issued through the CPOE and/or the CDSS). The study covered nondiabetology wards as endocrinology, diabetes and metabolism departments were not computerized at the time of the study. Results: There were 203 PIs related to antidiabetic drugs in period 1 and 319 in period 2 (a 57.5% increase). Sixty‐four of the 319 PIs were generated by the CDSS. Noncompliance/contraindication was the main problem identified by the CDSS (41 PIs, 68.4%), and 57.8% led to discontinuation of the drug. Most of the PIs issued through the CDSS corresponded to orders that had not been flagged up by clinical pharmacists using the CPOE. Conversely, most alerts about indications that were not being treated were detected by the clinical pharmacists using the CPOE and not by the CDSS. Conclusion: Use of CDSS by clinical pharmacists improved antidiabetic drug management for hospitalized patients with T2DM. The CDSS might add value to diabetes care in nondiabetology wards by decreasing the frequency of potentially inappropriate prescriptions and adverse drug reactions. [ABSTRACT FROM AUTHOR]

Abstract

International audience; The DetecIP project aims to implement multifactorial dynamic rules within a computerized decision support system (CDSS) for pharmaceutical analysis of orders to reduce the rate and severity of iatrogenic hyperkalemia and acute kidney injury. However, understanding the impact of this intervention (if any) requires that the way in which it influences the work systems and processes also be studied. This study presents the preliminary results of the analysis of the work contexts in which these rules will be implemented. A series of semi-structured interviews exploring the dimensions of the systems engineering initiative for patient safety (SEIPS) were conducted with healthcare professionals involved in the prevention and management of iatrogenic risks in five hospital units. Data were analyzed to identify current barriers and facilitators to the prevention and management of iatrogenic risks. Preliminary results from a geriatric unit and a cardiology unit reveal that, despite overall similarities in work processes, differences in the availability and location of physicians and clinical pharmacists influence how iatrogenic risks are managed. These contextual differences could influence the impact of the new CDSS rules once implemented.

Abstract

International audience; Clinical decision support systems (CDSS) are tools that have been used for several years by clinical pharmacy teams to support pharmaceutical analysis, with a perspective of contributing to the quality of care in collaboration with the other health care team members. These tools require both technical, logistical and human resources. The growing use of these systems in different establishments in France and in Europe gave birth to the idea of meeting to share our experiences. The days organized in Lille in September 2021 aimed at proposing a time of exchange and reflection on the use of these CDSS in clinical pharmacy. A first session was devoted to feedback from each establishment. These tools are essentially used to optimize pharmaceutical analysis and to secure patient medication management. This session outlined the clear advantages and common limitations of these CDSS. Two research projects were also presented to put the use of these tools into perspective. The second session of these days, in the form of workshops, addressed 4 themes that surround the implementation of CDSS: their usability, the legal aspect, the creation of rules and their possible valorization. Common problems were raised, the resolution of which requires close collaboration. This is a first step proposing a beginning of harmonization and sharing that should be deepened in order not to lose the dynamics created between the different centers. This event ended with the proposal to set up two working groups around these systems: the creation and structuring of rules for the detection of risk situations and the common valorization of the work.; Les systèmes d’aide à la décision médicale (SADM) sont des outils utilisés depuis quelques années par les équipes de pharmacie clinique comme support à l’analyse pharmaceutique, dans l’optique de concourir à la qualité des soins en collaboration avec les autres membres de l’équipe de soins. Ces outils demandent à la fois des ressources techniques, logistiques et humaines. L’utilisation grandissante de ces systèmes dans différents établissements en France et en Europe a fait naitre l’idée de se rencontrer pour partager nos expériences. Les journées organisées à Lille en septembre 2021 ont eu pour objectif de proposer un temps d’échange et de réflexion sur l’utilisation de ces SADM en pharmacie clinique. Une première session a été consacrée aux retours d’expérience de chaque établissement utilisateur. Ces outils sont essentiellement utilisés à des fins d’optimisation de l’analyse pharmaceutique et de sécurisation de la prise en charge médicamenteuse du patient. Cette session a dressé les avantages certains et les limites communes de ces SADM. Deux projets de recherche ont aussi été présentés pour mettre en perspective l’usage de ces outils. La deuxième session de ces journées, sous forme d’ateliers, a abordé 4 thématiques encadrant l’implémentation des SADM : leur utilisabilité, l’aspect juridique, la création des règles et leur valorisation possible. Des problématiques communes ont été soulevées, dont la résolution nécessite de poursuivre une étroite collaboration. Il s’agit d’une première étape proposant un début d’harmonisation et de mutualisation qu’il conviendrait d’approfondir afin de ne pas perdre la dynamique créée entre les différents centres. Cet évènement s’est terminé sur la proposition de constitution de deux groupes de travail autour de ces systèmes : la création et la structuration des règles de détection des situations à risque et la valorisation commune des travaux.

Key summary points: Aim: To update and validate STOPP/START criteria for potentially inappropriate prescribing. Findings: STOPP/START version 3 has been expanded and validated by an international European panel of experts in geriatric pharmacotherapy. Version 3, with 190 criteria, is significantly larger than version 2 (114 criteria), reflecting the expansion of the pharmacopeia and clinical trials evidence base relevant to older people since the publication of version 2. Message: STOPP/START version 3 represents an updated explicit list of potentially inappropriate medications and potential prescribing omissions aimed at optimizing medication and minimizing adverse drug reactions and events during medication review in older people, particularly those with multimorbidity and polypharmacy. Purpose: STOPP/START is a physiological systems-based explicit set of criteria that attempts to define the clinically important prescribing problems relating to potentially inappropriate medications (PIMs–STOPP criteria) and potential prescribing omissions (PPOs–START criteria). The previous two versions of STOPP/START criteria were published in 2008 and 2015. The present study describes the revised and updated third version of the criteria. Methods: A detailed system-by-system review of the published literature from April 2014 to March 2022 was undertaken with the aim of including clinically important new explicit PIM and PPO criteria and removing any criteria considered to be no longer correct or outdated. A panel of 11 academic physicians with recognized expertise in geriatric pharmacotherapy from 8 European countries participated in a Delphi panel with the task of validating the draft criteria. The panel was presented with the draft new criteria using the SurveyMonkey® on-line platform in which panelists were asked to indicate their level of agreement on a five-point Likert scale. Results: Two hundred and four evidence-based draft criteria (one hundred and forty-five STOPP criteria, fifty-nine START criteria) were presented to panelists for assessment using the Delphi validation method. Over the course of four rounds of Delphi validation, the panel achieved consensus on 133 STOPP criteria and 57 START criteria, i.e., 190 STOPP/START criteria in total representing a 66.7% increase in the number of criteria compared to STOPP/START version 2 published in 2015.
