It discusses the social, environmental and economic impacts of tobacco considered as commercial determinants of fiscal policy for health. It refers to the tobacco industry's harm reduction narrative, pharmaceuticalisation of business model, and targeted marketing as challenges to sustaining progress on tobacco control and tobacco taxation.

5. A psychoanalytic approach to the study of international relations

This thesis tries to demonstrate that psychoanalytic principles, primarily those developed by Sigmund Freud, can be extrapolated from the individual to the collective level-of-analysis in order to formulate a paradigm of international relations. The first part illustrates that structural concepts of Freudian psychoanalysis provide a model of human psychology by which traditions of international political thinking, both Western and non-Western, can be compared and analysed; it is argued that the 'id-ego-superego' model provides a trans-cultural and trans-historical representation of political philosophy. Similarly, although Freud's writing on political and social themes did not examine the political philosophies of either Western or non-Western traditions in any depth, nor elaborate a theory of international relations, his analyses of political and social affairs, while limited, would seem to have applied his individual models of human psychology to analyse relations between group actors such as states, even though he did not make this explicit. Nonetheless, this thesis extrapolates psychoanalytic principles to the level of state and non-state groupings in order to develop a psychoanalytic theory of international relations based on four main themes: first, that the 'id-ego-superego' model of human socialisation can be used to construct an 'order out of chaos theory' of international society; secondly, that ego psychology can be used to analyse the foreign policy interactions of state apparats; thirdly, that psychoanalytic precepts can be used to construct a 'fourth image' of war, and finally, that the psychoanalytic model developed in this thesis intersects with the three main paradigms of modern international relations theory, namely structuralism, pluralism, and realism/idealism, which suggests that they need not be considered as mutually exclusive bodies of theory. It is concluded that the concepts elaborated in this thesis, which are illustrated by means of historical observations and case studies, constitute a theoretical model that offers a distinct view of world politics.

Abstract

The globalization of public health poses new threats to health but also holds important opportunities in the coming century. This commentary identifies the major threats and opportunities presented by the process of globalization and emphasizes the need for transnational public health approaches to take advantage of the positive aspects of global change and to minimize the negative ones. Transnational public health issues are areas of mutual concern for the foreign policies of all countries. These trends indicate a need for cross-national comparisons (e.g., in the areas of health financing and policy development) and for the development of a transnational research agenda in public health. Abstract

The transnationalization of disease and health risks will require global awareness, analysis, and action and indicates a need for global cooperation. Transnational actions must be built on firm local and national foundations, but they also require new forms of transnational collaboration in order to minimize risks and build on opportunities. In a world characterized by the globalization of public health, countries and communities will need to look beyond their narrow self-interests in defining and confronting the shared problems that are emerging. In fact, a strong case can be made that enlightened self-interest and altruism will converge in the increasingly interdependent world being shaped by the process of globalization. Abstract

This monograph, a joint effort of the U.S. National Cancer Institute and World Health Organization, examines economic issues in tobacco and tobacco control, including the supply and demand of tobacco products. This first chapter frames the issues addressed in the monograph and describes its organization around key topic areas. Each monograph chapter focuses on the global evidence on these issues, particularly the evidence from low- and middle-income countries (LMICs). The closing sections of this chapter present chapter conclusions and major overall conclusions generated by the work presented here. Experts in economics, tobacco control, public policy, public health, and other related fields from every region in the world, including high-income countries and LMICs, were assembled to provide the research and analyses presented within these pages. It is hoped that this monograph will help inform the implementation of global tobacco control efforts in the 21st century.

Introduction The health of populations in all parts of the world is increasingly being [...], Although the application of legal instruments to international health issues -- relative to other areas of international concern -- is still at a rudimentary stage of development, the transnational health impacts of globalization provide a rationale for the codification and implementation of global norms to deal with shared problems. The experience of promulgating international agreements in other areas closely related to international health -- the environment, for example -- demonstrates how evidence-based international agreements can effectively address a range of problems that cross national boundaries. The framework convention-protocol approach is a legally binding, incremental approach to international law-making that has frequently been employed to deal with environmental threats, and is now being adapted to serve purely public health ends. Experience with the recently initiated WHO Framework Convention on Tobacco Control provides a case study of how transnational public health problems can be addressed by an international legal approach. Scientific evidence in public health and economics has provided the foundation for the elaboration of this evidence-based strategy. The present tobacco epidemic poses a range of transnational challenges that are best addressed through coordinated action. In this article, it is argued that the proposed Convention has the potential to be a global 'good' for public health--i.e. it has the potential to yield important global public health benefits--and that it represents a test case for more active involvement of the public health community in international law-making. Introduction Globalization of trade, involving cross-border movement of capital, technology, traded goods and information, [...], Globalization of trade, marketing and investment has important implications for public health, both negative and positive. This article considers the implications of the single package of World Trade Organization (WTO) agreements for public health research and policy, focusing on three themes: commodities, intellectual property rights, and health services. The main aims of the analysis are as follows: to identify how trade issues are associated with the transnationalization of health risks and possible benefits; to identify key areas of research; and to suggest policy-relevant advice and interventions on trade and health issues. The next wave of international trade law will need to take more account of global public health issues. However, to become more engaged in global trade debates, the public health community must gain an understanding of the health effects of global trade agreements. It must also ensure that its own facts are correct, so that public health is not blindly used for political ends, such as justifying unwarranted economic protectionism. 'Healthy trade' policies, based on firm empirical evidence and designed to improve health status, are an important step towards reaching a more sustainable form of trade liberalization.
