Abstract

Abstract: Purpose: Multifractal spectrum analysis is a discriminating tool that may distinguish volunteers and epileptic patients. In this study, our aim was to detect epileptogenic sources by computing the singularity spectrum. Materials and methods: Multifractal analysis based on generalized concepts of fractals has been applied to evaluate biological tissues composed of complex structures. This analysis can provide a precise quantitative description for a broad range of heterogeneous phenomena. This study investigates the possibility of using a mathematical model (multifractal analysis) for image texture to detect the underlying source changes on SPECT images. Previously, reported on application of 3D multifractal analysis to describe the multifractal character of brain SPECT (Single-Photon Emission Computed Tomography) images and we showed that the multifractal spectrum is a discriminating tool to distinguish volunteers and epileptic patients. The experiment is divided into two phases. First, we developed a 3D method for the singularity spectrum compute. In the test phase, we applied this multifractal spectrum to the detection of epileptogenic sources in SPECT images. Results: The results obtained on a group of seven patients show that the proposed method is feasible and the results are encouraging. The epileptic fit sources obtained using the system were in agreement with the opinion of expert diagnosticians. Conclusion: Multifractal spectrum analysis provides a means of detecting and localizing epileptogenic foci in brain SPECT scans.

34. Anxiety in Parkinson's disease is associated with changes in the brain fear circuit

Abstract

Background: Anxiety is frequent in Parkinson's disease (PD) and has a negative impact on disease symptoms and quality of life. The underlying mechanisms remain largely unknown. The aim of this study was to identify anatomical and functional changes associated to PD-related anxiety by comparing the volume, shape and texture of the amygdala, the cortical thickness as well as the functional connectivity (FC) of the fear circuit in patients with and without clinically relevant anxiety.Methods: Non-demented PD patients were recruited, and anxiety was quantified using the Parkinson Anxiety Scale. Structural MRI was used to compare cortical thickness and amygdala structure and resting-state functional MRI to compare FC patterns of the amygdala and resting-state functional networks in both groups.Results: We included 118 patients: 34 with (A+) and 84 without (A-) clinically relevant anxiety. Clusters of cortical thinning were identified in the bilateral fronto-cingulate and left parietal cortices of the A+ group. The texture and the shape of the left amygdala was different in the A+ group but the overall volume did not differ between groups. FC between the amygdala and the whole brain regions did not differ between groups. The internetwork resting-state FC was higher between the "fear circuit" and salience network in the A+ group.Conclusion: Anxiety in PD induces structural modifications of the left amygdala, atrophy of the bilateral fronto-cingulate and the left parietal cortices, and a higher internetwork resting-state FC between the fear circuit and the salience network.

Abstract

International audience; Glioblastoma Multiform (GBM) is a high-grade brain tumour with local recurrence and poor outcome. Photodynamic therapy to treat glioblastoma has been recently reported to improve overall survival. However, controlled study to evidence treatment efficacy is still expected and treatment delivery has to be optimized. Several parameters might be optimized such as fluence rate, dose escalation or fractionation of light delivery. Specially, fractionation of light delivery is expected to enhance treatment efficiency by restoring tissue oxygenation. Thus, we have evaluated light fractionation on a preclinical rodent model harbouring grafted GBM. Thirty-nine " Nude " rats were grafted with human U87 cells. Cells were stereotactically grafted in rat brain. After the presence and the size of the tumour were assessed by microMRI, PDT was delivered interstitially. Light was delivered through an optical fibre introduced into the tumour after PS precursor intake (5-ALA). The rats were randomized in three groups: (1) without illumination, (2) with 2-steps light fractionation and (3) with 5-steps light fraction-ation. Brain immunohistology and microMRI images were achieved to evaluate the treatment effects. On diffusion MRI, elevated diffusion values in tumour centre among treated animals were observed, especially for the 5-steps fractionation group. Perfusion decreased around the treatment site, especially in the 5-steps fractionation group. Histology confirmed our MRI findings, with a more extensive necrosis and apopto-sis associated with a rarefiedangiogenic network in the treatment area, after 5-steps fractionation PDT. However, more surrounding oedema and neovascularization in the peripheral ring after 5-steps fractionation PDT were also noticed. Finally we observed that interstitial PDT induced more tumour necrosis and apoptosis when applying 5-steps fractionation. However , this scheme also led to significant peripheral oedema and neovascularization. Diffusion and perfusion were able to predict the histological lesions. Nevertheless, apoptosis cells death remains the preferred effect in order to decrease potential side effects. We hypothesize a lower fluence rate with the same light fractionation scheme might increase apoptosis while decreasing necrosis and oedema.
