3. Are the Clinical Course and Drug Toxicity Different in Late-Onset Inflammatory Bowel Disease Patients? From Tertiary Reference Center Real-Life Data.

Abstract

Objective: The rate of onset at a late age in Inflammatory Bowel Diseases (IBD) has increased over the years. The disease course and treatment outcomes remain uncertain in this group. We aimed to evaluate the disease course, drug toxicity, and prognosis of early and late-onset IBD patients. Methods: The methodology involves a comprehensive retrospective review of medical charts of 1,060 patients diagnosed with IBD. Thirty-nine patients with late-onset IBD (=60 years) were included in the study. As the control group, 50 early-onset patients with similar demographic characteristics to the late-onset patient group were included. Disease type, demographic data, treatments, treatment-related adverse effects and toxicities, mortality rates, extraintestinal and hepatobiliary involvement, and complications were compared between the early and late-onset groups. Results: The mean age of the patients at the time of diagnosis was 49.2±17.5 years, and 53 (59.6%) were male. Among the 39 late-onset patients, 25 (64.1%) had UC; among the 50 early-onset patients, 28 (56.0%) had UC. AZT (azathioprine) toxicity (28% vs. 20.5%) and biological therapy toxicity (16% vs. 5.1%) were similar in both groups (P>0.05). Mortality was higher in late-onset patients (33.3% vs. 2%), as were complication rates in late-onset UC (32% vs. 7.1%) compared to early-onset patients (P<0.05). The malignancy rate was higher in late-onset patients, but this difference was not statistically significant (15.4% vs. 6%, P>0.05). Conclusion: Late-onset IBD patients are similar to early-onset patients in terms of drug-related adverse effects. Abstract

Summary Background Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In an earlier phase 2 induction study, etrolizumab significantly improved clinical remission relative to placebo in patients with moderately to severely active ulcerative colitis. The HIBISCUS studies aimed to compare the efficacy and safety of etrolizumab to adalimumab and placebo for induction of remission in patients with moderately to severely active ulcerative colitis. Methods HIBISCUS I and HIBISCUS II were identically designed, multicentre, phase 3, randomised, double-blind, placebo-controlled and active-controlled studies of etrolizumab, adalimumab, and placebo in adult (18–80 years) patients with moderately to severely active ulcerative colitis (Mayo Clinic total score [MCS] of 6–12 with an endoscopic subscore of ≥2, a rectal bleeding subscore of ≥1, and a stool frequency subscore of ≥1) who were naive to tumour necrosis factor inhibitors. All patients had an established diagnosis of ulcerative colitis for at least 3 months, corroborated by both clinical and endoscopic evidence, and evidence of disease extending at least 20 cm from the anal verge. In both studies, patients were randomly assigned (2:2:1) to receive subcutaneous etrolizumab 105 mg once every 4 weeks; subcutaneous adalimumab 160 mg on day 1, 80 mg at week 2, and 40 mg at weeks 4, 6, and 8; or placebo. Randomisation was stratified by baseline concomitant treatment with corticosteroids, concomitant treatment with immunosuppressants, and baseline disease activity. All patients and study site personnel were masked to treatment assignment. The primary endpoint was induction of remission at week 10 (defined as MCS of 2 or lower, with individual subscores of 1 or lower, and rectal bleeding subscore of 0) with etrolizumab compared with placebo. Pooled analyses of both studies comparing etrolizumab and adalimumab were examined for several clinical and endoscopic endpoints. Efficacy was analysed using a modified intent-to-treat population, defined as all randomly assigned patients who received at least one dose of study drug. These trials are registered with ClinicalTrials.gov , NCT02163759 (HIBISCUS I), NCT02171429 (HIBISCUS II). Findings Between Nov 4, 2014, and May 25, 2020, each study screened 652 patients (HIBISCUS I) and 613 patients (HIBISCUS II). Each study enrolled and randomly assigned 358 patients (HIBISCUS I etrolizumab n=144, adalimumab n=142, placebo n=72; HIBISCUS II etrolizumab n=143; adalimumab n=143; placebo n=72). In HIBISCUS I, 28 (19·4%) of 144 patients in the etrolizumab group and five (6·9%) of 72 patients in the placebo group were in remission at week 10, with an adjusted treatment difference of 12·3% (95% CI 1·6 to 20·6; p=0·017) in favour of etrolizumab. In HIBISCUS II, 26 (18·2%) of 143 patients in the etrolizumab group and eight (11·1%) of 72 patients in the placebo group were in remission at week 10, with an adjusted treatment difference of 7·2% (95% CI –3·8 to 16·1; p=0·17). In the pooled analysis, etrolizumab was not superior to adalimumab for induction of remission, endoscopic improvement, clinical response, histological remission, or endoscopic remission; however, similar numerical results were observed in both groups. In HIBISCUS I, 50 (35%) of 144 patients in the etrolizumab group reported any adverse event, compared with 61 (43%) of 142 in the adalimumab group and 26 (36%) of 72 in the placebo group. In HIBISCUS II, 63 (44%) of 143 patients in the etrolizumab group reported any adverse event, as did 62 (43%) of 143 in the adalimumab group and 33 (46%) in the placebo group. The most common adverse event in all groups was ulcerative colitis flare. The incidence of serious adverse events in the pooled patient population was similar for etrolizumab (15 [5%] of 287) and placebo (seven [5%] of 144) and lower for adalimumab (six [2%] of 285). Two patients in the etrolizumab group died; neither death was deemed to be treatment related. Interpretation Etrolizumab was superior to placebo for induction of remission in HIBISCUS I, but not in HIBISCUS II. Etrolizumab was well tolerated in both studies. Funding F Hoffmann-La Roche.

Abstract

Background and Aim: Chronic hepatitis B virus (HBV) infection is a major cause of hepatocellular carcinoma (HCC). Circulating cell-free DNA (cfDNA) methylation of tumor suppressor genes are emerging potential biomarkers in HCC. We aimed to evaluate the cfDNA methylation status of RASSF1 and CDKN2AIP genes in patients with liver cirrhosis (LC) with or without HCC caused by HBV. Materials and Methods: A total of 47 patients with HBV cirrhosis were included in the study. Patients were divided into two groups: HCC and LC (HCC+LC, n=22) and HBV cirrhosis only (LC, n=25). cfDNA was isolated from the plasma samples of the patients. Methylation analysis was performed for RASSF1 and CDKN2AIP genes. Results: Mean methylation percentage of CDKN2AIP gene was 0.001±0.004% in the HCC+LC group and 0.008±0.004 % in the LC only group. The mean methylation percentage of RASSF1 gene was 5.1±16.1% in the HCC+LC group and 9.7±25.9% in the LC only group. The methylation rate of CDKN2AIP was significantly lower in the HCC+LC group (p=0.027). A positive correlation was found with the absence of cfDNA methylation of CDKN2AIP gene in the presence of HCC (R=0.667, p=0.018). Conclusion: cfDNA methylation of CDKN2AIP and RASSF1 genes may provide important diagnostic information regarding the development of HCC in the setting of HBV cirrhosis. [ABSTRACT FROM AUTHOR]

Abstract

INTRODUCTION: Osteoporosis still remains as an important complication after liver transplantation (OLT), and the risk is deemed to be high particularly in the first 6–12 months following the procedure. Therefore, we aimed to assess bone health in liver transplant recipients at a tertiary care center. METHODS: In this cross-sectional study, which was conducted between November 2016 and October 2017, 72 liver transplant recipients were enrolled. All relevant information, including history of fractures, demographics, clinical features and laboratory characteristics were collected. Bone mineral density (BMD) in the lumbar spine (L1–L4) and hip was obtained with dual energy X-ray absorptiometry (DXA) using a Hologic QDR 4500C bone densitometer (Hologic Inc., Bedford, MA, USA). Osteoporosis was defined as a BMD T score ≤ −2.5 in the lumbar spine, total hip or femoral neck. RESULTS: Demographic, clinical and laboratory features of patients are summarized in the table. Of these 72 patients, 40 (55.6%) were male, and median age was 55 (IQR: 44–62) years. Osteoporosis was found in 15 patients (20.8%), nine of which were female. Three patients (4.2%) experienced vertebral (n=1) and femoral (n=2) fractures. Patients with osteoporosis had lower BMI values (p=0.036) and tendency to have more calcium and vitamin D supplements (p=0.016) when compared with patients without osteoporosis. Post-transplant corticosteroid use was 90% in osteoporotic and 71% in non-osteoporotic OLT recipients (p=0.177); however patients with osteoporosis had a longer exposure to corticosteroids with a median of 6 (5.25–22) months as compared to patients without osteoporosis [3.5 (2–6) months, p=0.050]. There were no statistically significant differences between osteoporotic and non-osteoporotic patients in terms of tacrolimus (p=0.892), cyclosporine (p=0.728), mycophenolate mofetil (p=0.181), sirolimus (p=0.490), and everolimus (p=0.887) uses. All patients were stratified into 5 groups according to their durations of post-transplant follow-up at the time of study visit: 0–1 year (3 patients), 1–5 years (21 patients), 5–10 years (20 patients), 10–15 years (20 patients), and 15–20 years (8 patients). Osteoporosis was found in 2 (66.7%), 5 (23.8%), 1 (5%), 3 (15%), and 4 (50%) patients in these groups, respectively (p=0.021). CONCLUSIONS: Duration of post-transplant corticosteroid use remains as an important risk factor for osteoporosis. Bone health of OLT recipients should be carefully monitored throughout the years, since osteoporosis can be encountered at the late follow-up.

Abstract

Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In an earlier phase 2 induction study, etrolizumab significantly improved clinical remission relative to placebo in patients with moderately to severely active ulcerative colitis. The HIBISCUS studies aimed to compare the efficacy and safety of etrolizumab to adalimumab and placebo for induction of remission in patients with moderately to severely active ulcerative colitis.

Abstract

Aim: To investigate clinical, etiological, and prognostic features in patients with hepatocellular carcinoma. Methods: Patients with hepatocellular carcinoma who were followed-up from 2001 to 2011 were included in the study. The diagnosis was established by histopathological and/or radiological criteria. We retrospectively reviewed clinical and laboratory data, etiology of primary liver disease, imaging characteristics and treatments. Child-Pugh and Barcelona Clinic Liver Cancer stage was determined at initial diagnosis. Kaplan-Meier survival analysis was done to find out treatment effect on survival. Risk factors for vascular invasion and overall survival were investigated by multivariate Cox regression analyses. Results: Five hundred and forty-five patients with hepatocellular carcinoma were included in the study. Viral hepatitis was prevalent and 68 patients either had normal liver or were non-cirrhotic. Overall median survival was 16 (13-19) mo. Presence of extrahepatic metastasis was associated with larger tumor size (OR = 3.19, 95% CI: 1.14-10.6). Independent predictor variables of vascular invasion were AFP (OR = 2.95, 95% CI: 1.38-6.31), total tumor diameter (OR = 3.14, 95% CI: 1.01-9.77), and hepatitis B infection (OR = 5.37, 95% CI: 1.23-23.39). Liver functional reserve, tumor size/extension, AFP level and primary treatment modality were independent predictors of overall survival. Transarterial chemoembolization (HR = 0.38, 95% CI: 0.28-0.51) and radioembolization (HR = 0.36, 95% CI: 0.18-0.74) provided a comparable survival benefit in the real life setting. Surgical treatments as resection and transplantation were found to be associated with the best survival compared with loco-regional treatments (log-rank, P < 0.001). Abstract

A 70-year-old man was admitted to our clinic with a history of rectal bleeding and constipation, his colonoscopy revealed varicosities and bluish nodular lesions of the rectum (Figure 1). Abdominal CT showed multiple nodular lesions beginning from the distal descending colon and extending to the rectum, calcifications suggesting phleboliths were also seen in these lesions. A contrast enhanced pelvic MRI demonstrated multiple tubular lesions showing hyperintensity on T2-weighted images and hypointensity on T1-weighted images, consistent with the affected areas on the CT scan (Figure 2). It was a diffuse cavernous hemangioma, which mostly affects the rectosigmoid colon in the gastrointestinal tract, and can clinically mimic internal hemorrhoids, ulcerative colitis or cancer (1). Gastrointestinal hemangioma is a rare benign vascular neoplasm, and might be associated with a congenital disorder like Osler-Weber-Rendu disease, Maffucci-s syndrome, Klippel-Trenaunay syndrome, or the congenital blue rubber bleb nevus syndrome (2). Even though there are different medical treatment options targeting VEGF and FGF-mediated pathways such as bevacizumab and thalidomide, and endoscopic approaches like sclerotherapy and electrocautery; complete resection of the hemangioma is the only curative treatment method (1, 3). Therefore, the patient was referred to department of surgery for a definitive treatment, and lost to follow-up.

Abstract

Introduction Gastroesophageal reflux disease (GERD) is more frequent in patients with diabetes mellitus (DM).The aim of the present study was to evaluate gastroesophageal reflux (GER) in asymptomatic patients with DM using 24-h pH impedance. Materials and Methods 19 healthy controls and 35 patients with DM without typical GERD symptoms were enrolled in the study. A 24-h pH-impedance study, esophageal manometry and gastroscopy were performed on all patients with DM. In the control group, an impedance study was performed on all subjects, and gastroscopy and esophageal manometry were performed on those who consented to the procedures. Patients with diabetes were categorized as obese [body mass index (BMI)>30 kg/m2 ] or non-obese (BMI<30 kg/m2) and both groups were compared with healthy controls. Results The mean BMI was similar in the control group (27.3±2.6 kg/m2) and the diabetic group (28.7±5 kg/m2) (p>0.05).Erosive esophagitis was found in 7.5% of the DM group. Esophageal dysmotility was higher in diabetics compared to the control group (45.5 vs. 11%, p=0.04). Neuropathy was found to be an independent risk factor for dysmotility. The mean DeMeester score (DMS) (25.6±32.5 vs. 11.2±17, p=0.01) and bolus exposure time (2.1±1.3 vs.1.3±1.3 min, p=0.009) were higher in the DM group compared with the control group.The difference was mainly observed between obese diabetics and the control group (p<0.05). The mean DMS, pathologic acid reflux, and esophageal dysmotility rate were higher in patients without complications of DM (p<0.05). BMI was higher in these patients than in patients with complications.
