L’observation de la dynamique issue de l’appropriation des interventions en planification familiale à Ngaba explique mieux le faible résultat de ces interventions. Chaque couple recourt à une des méthodes de son choix, selon ses raisons propres. Au sein des couples, il est difficile de dégager un consensus ou un positionnement commun en matière de la reproduction. Il existe plusieurs facteurs qui influencent les couples dans la logique locale en contraste avec la logique globale des interventions en planification familiale. Cette réalité sous étude, nous induit à penser à l'anthropologie de la planification familiale pour chaque communauté ou peuple.

A l’heure actuelle dans le contexte de la démocratisation en Afrique, il est aberrant de vivre les crises sociopolitiques qui caractérisent l’espace politique africain. La question centrale dans cette étude est celle de savoir pourquoi l’Afrique demeure encore incapable de capitaliser de dividendes de la démocratisation comme dans les pays occidentaux ? Cette question trouve ses éléments de réponse dans la parenté et l’hybridisme des systèmes politiques en Afrique, au regard de leurs implications fonctionnelles dans la participation politique. En bref, l’africain doit chercher son identité en politique et un modèle adapté pour son épanouissement, sinon la démocratie serait un piège pour l’Afrique comme stipulait Honoré N’gbanda : « Afrique : Démocratie piégée » , alors que l’élection qui en est le mécanisme inconditionnel est « un piège à con » selon Jean Salem . C’est l’expression de l’hybridisme des systèmes politiques en d’autres termes en Afrique.

L’étude relève de l’analyse du discours en contexte médical et s’intéresse aux interactions verbales en milieu hospitalier. Elle explore un ensemble de consultations enregistrées à l’hôpital de La Cavale Blanche (Brest) anticipant une intervention de neurochirurgie éveillée (NCE) dont le but est la résection de tumeurs cérébrales. Les modalités de la NCE – justifiées par la nécessité de s’appuyer sur les rétroactions d’un patient conscient, et même acteur, pour optimiser le geste chirurgical – exigent de l’équipe soignante une attention particulière à l’accompagnement et à la préparation du patient. Dans ce cadre, est décryptée la manière dont « la pathologie est désignée ». Il s’agit de saisir comment les discours sur la pathologie contribuent à construire une relation thérapeutique relevant du partenariat. L’analyse souligne la complémentarité des rôles des différents soignants mise au service de (i) l’acceptation de la maladie et du statut/rôle de patient; (ii) la préparation, sur les plans opérationnel comme psychologique, de l’intervention de NCE; (iii) la verbalisation, voire le désamorçage, des appréhensions relativement à la maladie et au protocole chirurgical.

No consensus exists as to whether cotherapy is effective in the short-term prevention of severe NSAID-related gastroduodenal damage. The aim of this study was to provide a quantitative systematic review of the efficacy of gastroprotective drugs, such as misoprostol, H2-blockers, and proton pump inhibitors (PPI) in preventing the severe acute NSAID-related gastroduodenal damage. Placebo-controlled randomized clinical trials on the use of misoprostol, H2-blockers, and PPIs as preventative agents published between January 1986 and May 1999 were identified through Medline and reference lists from clinical reviews. Studies on patients or healthy subjects were considered to be eligible for data pooling if they were performed in acute NSAID users (not longer than 30 days) and with at least one endoscopic evaluation during therapy that reported results specifically for gastric and duodenal damage. Risk difference (RD), heterogeneity χ2 test, publication bias assessment and number needed to treat (NnT) were calculated for each meta-analysis by a customized program. Twenty-one trials met the inclusion criteria evaluating a total of 636 healthy subjects and 1904 patients with arthritis randomized to active drug or placebo. The baseline risk of NSAID-related gastric (68% vs 16.6%, P < 0.001) and duodenal (22% vs 8.5%, P < 0.001) damage was higher in healthy subjects compared to patients with arthritis. Meta-analysis demonstrated a significant heterogeneity between trials performed in the two populations (P < 0.0001). In healthy subjects the active drug treatment induced a significant prevention of severe gastric (misoprostol RD = 69%, 95% CI = 60.3–77.7, H2-blocker RD = 38.3%, 95% CI = 17.8–58.9 and PPI RD = 43%, 95% CI = 28.2–57.7) and duodenal damage (misoprostol RD = 22.3%, 95% CI = 13.6–31, H2-blocker RD = 13.2%, 95% CI = 5.2–21.3 and PPI RD = 17.7%, 95% CI = 3.5–31.8). NnT values were, respectively, 1, 3, and 2 for gastric and 4, 8, and 6 for duodenal damage. In patients with arthritis lower RD and higher NnT values were found compared to healthy subjects. In conclusions, cotreatment with gastroprotective drugs for short-term prevention of severe gastroduodenal NSAID-related damage was more effective in healthy subjects than in patients with arthritis; misoprostol and PPIs were more effective than H2-blockers in the prevention of both gastric and duodenal severe damage; more studies need to evaluate the role of short-term prevention in patients with arthritis who require acute NSAID treatment. Cette étude prolonge un travail de recherche sur la signification des noms des parties du corps humain (Npc) (Bertin, 2018). On s'intéresse à la construction illustrée par Colin a le front de venir à cette réunion (NPV). Après un inventaire des Npc compatibles avec NPV, on procède à une analyse sémantique : d'une part, on cerne le sens de NPV (en se focalisant sur la contribution du Npc) et, d'autre part, on tire profit de cette première étape pour l'analyse sémantique des Npc eux-mêmes.

In the Democratic Republic of Congo, the incidence of sickle cell anemia (SCA) is estimated to affect 30,000 to 40,000 neonates per year. However, there is paucity of data on acute clinical manifestations in sickle cell children. In these circumstances, it is difficult to develop a health care policy for an adequate management of sickle cell patients. This was a seven years’ retrospective study of children admitted with acute sickle cell crisis in the Department of Pediatrics in University Hospital of Kinshasa, Kinshasa, the Democratic Republic of Congo. A total of 108 patients were identified as having SCA. There were 56 (51%) girls and 52 (49%) boys. Median age was 10.5 years (range 1-24 years). No child was diagnosed by neonatal screening. The median age of diagnosis of sickle cell anemia was 90 months (range: 8-250 months). The median age at the first transfusion was 36 months (range 4-168). In this series, 61 (56.5%) patients were eligible for hydroxyurea. However, this treatment was only performed in 4 (6.6%) of them. Pain episodes, acute anemic crisis and severe infection represent respectively 38.2%, 34.3% and 21.9% of events. Altered sensorium and focal deficit were encountered occasionally and represented 3.4% of acute events. Acute renal manifestations, cholelithiasis and priapism were rarely reported, in this cohort. In Kinshasa, the care of patients suffering from sickle cell anemia is characterized by the delayed diagnosis and low detection of organ complications compared to reports of Western countries. This situation is due to resources deficiencies.
