Additional file 3: Supplemental Figure 2. Random Forest classification (with 10-fold cross-validation) correctly predicts study feeding group 80% (±17.95%) on average. Panel A shows a receiver operating curve, indicating that both groups could be predicted with a high degree of accuracy vs. random chance. Panel B shows the relative importance scores for the top five most predictive sequence variants. The predictive potential was powered by a very small number of sequence variants; the top two sequence variants explain 7.4% and 6.9% of the variation, respectively, and were the same two sequence variants (Lactocaseibacillus spp. and L. rhamnosus) identified by ANCOM as being differentially abundant between the study feeding groups.

Abstract

Objective To evaluate the growth of resident aerobic mesophilic flora and added Enterobacter sakazakii in fresh, unfortified human milk; fresh human milk fortified with two commercial powdered fortifiers differing in iron content; and infant formula prepared from powder. Subjects Eight mothers provided preterm breast milk samples. Methods Breast milk samples were divided into three aliquots: unfortified, fortified with fortifier containing 1.44 mg iron/14 kcal, and fortified with fortifier containing 0.4 mg iron/14 kcal. Aliquots of formula were prepared. Breast milk and formula aliquots were divided into two test samples. Half were inoculated with low amounts of E sakazakii; half were not. All test samples were maintained at room temperature (22[degrees]C), serially diluted, and plated onto agars after 0, 2, 4, and 6 hours. Plates were incubated at 35[degrees]C and enumerated. Statistical analyses Data were analyzed using repeated measures analysis of variance. P<.05 was considered significant. Results There were no differences in colony counts of aerobic bacteria among uninoculated or among inoculated human milk samples at any time; counts did not increase significantly over 6 hours. There were no differences in colony counts of E sakazakii among inoculated human milk samples at any time; counts did not increase significantly over 6 hours. Aerobic bacteria and E sakazakii colony counts from infant formula did not increase significantly over 6 hours. Conclusions During 6 hours at 22[degrees]C, fresh human milk and formula had negligible bacterial growth; fortifying human milk with powdered fortifiers did not affect bacterial growth. 20. Prolonging small feeding volumes early in life decreases the incidence of necrotizing enterocolitis in very low birth weight infants

Abstract

Objective. Approximately 90% of infants who develop necrotizing enterocolitis (NEC) do so after being fed. Previous prospective studies have shown that infants given small enteral feedings for the first 7 to 10 days of feeding do not have an increased risk for NEC compared with those given no feedings. Although neonatologists now commonly increase feeding volumes, no study has compared the risk for NEC between infants fed these small volumes and those fed volumes that are increased slowly. The purpose of this study was to compare the risks and benefits of small and increasing feeding volume. Methods. In a randomized, controlled trial, we randomly assigned 141 preterm infants in the newborn intensive care unit to be fed 10 days using 1 of 2 schedules. One group was fed 20 mL/kg/d for the first 10 study days (minimal). The other group (advancing) was fed 20 mL/ kg/d on study day 1; feeding volume was increased by 20 mL/kg/d up to 140 mL/kg/d, which was maintained until study day 10. The main outcome measure was incidence of NEC; secondary outcomes were maturation of intestinal motor patterns, time to reach full enteral feedings, and incidence of late sepsis. Results. The study was closed early because 7 infants who were assigned to advancing feeding volumes developed NEC, whereas only 1 infant fed minimal feeding volumes did, or 10% versus 1.4%. Although infants who were fed minimal volumes established full enteral feeding volumes later than infants who were fed advancing volumes, maturation of intestinal motor patterns and the incidence of late sepsis and feeding intolerance was similar in the 2 groups. Conclusion. Given that advancing feeding volumes increase the risk of NEC without providing benefits for motor function or feeding tolerance, neonatologists should consider using minimal feeding volumes until future trials assess the safety of advancing feeding volumes. 21. Minimal enteral feeding induces maturation of intestinal motor function but not mucosal growth in neonatal dogs

Abstract

Providing small enteral feedings for parenterally fed preterm infants during the first few weeks of life improves feeding tolerance. However, it is not known whether these feedings achieve this result via stimulation of gut growth and/or maturation of function. The minimal level needed to attain these responses is also critical to identify, because neonatologists often limit feeding volumes to minimize the risk of necrotizing enterocolitis. Thus, we determined the dose-response relationships between enteral feeding volume and gastrointestinal growth and small intestine motor function. Newborn canine pups (n = 51) received 0, 2.5, 5.0, 7.5, 10, 30 or 100% of their daily fluid intake enterally with the remainder given parenterally for 4-5 d. Motor activity was recorded, blood drawn for determination of gastrin and motilin, and intestinal tissue harvested for determination of DNA and protein content and morphology. Feeding volumes that provided 30% or more of daily fluid intake significantly increased small intestinal mucosal growth above that of unfed pups, but feeding volumes that provided as little as 10% of daily fluid intake significantly induced maturation of motor patterns beyond that of unfed pups. Plasma concentrations of gastrin and motilin did not differ among groups. We conclude that small enteral feedings typically used for minimal enteral feeding strategies improve feeding tolerance by triggering maturation of motor function but not gut growth in neonatal dogs. 22. Impact of necrotizing enterocolitis on length of stay and hospital charges in very low birth weight infants

Abstract

Objective. To determine the impact of necrotizing enterocolitis (NEC) on length of stay and hospital charges. Design. Case-control study. Setting. Two neonatal intensive care units in an academic medical center. Patients. Infants born in 1992-1994 with birth weight Measurements and Main Results. We performed odds ratio and t testing. As with previous studies, there was no single factor that increased the risk for developing NEC. However, the diagnosis of NEC increased the risk for death, infection, and the need for central line placement. Infants with surgical NEC had lengths of stay that exceeded those of controls by 60 days, whereas lengths of stay among infants with medical NEC exceeded those of controls by 22 days. Based on length of stay, the estimated total hospital charges for infants with surgical NEC averaged $186 200 in excess of those for controls and $73 700 more for infants with medical NEC. The yearly additional hospital charges for NEC were $6.5 million or $216 666 per survivor. Conclusions. A diagnosis of NEC in the very low birth weight infant imposes a significant additional financial burden to the individual patient as well as the neonatal community as a whole. This expense justifies additional research into preventive measures and potentially costly therapies aimed at reducing the incidence of NEC. These data also provide an estimated cost to compare the cost effectiveness of new preventive measures for NEC. Abstract

Denser infant formulas with more calories may cause reduced intestinal motor activity and explain early feeding intolerance in premature infants. Researchers evaluated the intestinal motor activity in 26 premature infants who received small amounts of slowly infused preterm formulas. The formula with lower caloric content was Similac 20 and the richer formula was Similac Special Care 24 with higher contents of fat, carbohydrates, and protein. Normal increased movement in the intestines occurred in infants who received Similac 20, but the denser formula was associated with lower intestinal motor activity. Seven children on the richer formula and two patients on the lighter formula developed feeding intolerance. After 10 days, this difference disappeared. Infants who fail to tolerate formula initially may respond well after the introduction of heavier milk products somewhat later., Objective. Preterm formulas are nutritionally better for preterm infants; however, it has been observed that these formulas cause more feeding intolerance than do regular formulas. Because intestinal motor activity is responsible for the aboral movement of intraluminal nutrients, the purpose of this study was to evaluate intestinal motor activity responses to two infant formulas: 84 and 100.8 J/oz. Study Design and Results. Intestinal motor activity was recorded in 52 preterm infants who had never been fed and who were randomly assigned to receive small enteral feedings (24 mL/kg per day) for 10 days with one of two commonly used infant formulas. In a subset of 26 of these infants, acute motor responses to both formulas were also evaluated. At the end of the study period, motor activity during fasting did not differ between the two groups of infants. However, motor responses during feeding to the two formulas differed significantly. When infants were fed for the first time, their motor activity increased compared with fasting when they were fed the 84-J/oz formula but decreased when they were fed the 100.8-J/oz formula. These differences in motor responses to the two formulas were not present 10 days later. These differences in motor responses to the 100.8- and 84-J/oz formulas were even more pronounced among the 7 infants who subsequently developed feeding intolerance to the 100.8-J/oz formula during the 10-day study period. Conclusion. Inhibition of motor responses to calorically denser formulas during active feeding in a subset of preterm infants may underlie the feeding intolerance they experience when they are fed these formulas. However, this inhibitory response diminishes with age, suggesting that denser formulas can be reintroduced later in life to these infants. Abstract

Besides calories, vitamins, and minerals, breast milk supplies infants with hormones and small proteins called peptides, which are important for infant health. A number of peptides, including gastric inhibitory peptide (GIP), bombesin, and vasoactive intestinal peptide (VIP), are involved in regulating gastrointestinal (GI) function. The amount of these GI neuropeptides in human breast milk was determined. Levels of GIP in breast milk remained constant throughout the first six postpartum months, while amounts of other GI peptides decreased. Bombesin levels in milk were three times those found in blood, implying that maternal cells actively took up bombesin from the blood or synthesized it. In comparing blood levels of GI peptides in pregnant versus control women, it was found that in pregnant women the levels of GIP were lower and those of VIP were higher. The study shows that breast milk supplies infants with significant amounts of GI peptides, which may improve the intestinal function of newborns in the two to four weeks before they are able to make their own gastrointestinal peptides.
