Abstract

This study examined the congruence between the preoperative teaching received and valued by patients undergoing same day surgery. A convenience sample of 116 patients was interviewed using the 26-item preoperative teaching interview guide, which explores five dimensions of preoperative information, including situational/procedural information, sensation/discomfort information, patient role information, psychosocial support, and skills training. The correlation between preoperative teaching received and valued was r = 0.34, P < .01. Significant relationships were observed between income level and preference for situational/procedural information and between gender and preference for psychosocial support information. The findings are being used to frame a program of research in perioperative nursing and to restructure preoperative teaching in one facility. 10. Health-related quality of life in chronic disorders: a comparison across studies using the MOS SF-36.

Abstract

The purpose of this report is to examine health-related quality of life (HRQoL) as measured by the Medical Outcomes Study Short Form-36, across patient populations with chronic disorders and to compare quality of life (QoL) in these subjects with normative data on healthy persons. Six studies, within the Center for Research in Chronic Disorders at the University of Pittsburgh School of Nursing, in patients with urinary incontinence, prostate cancer, chronic obstructive pulmonary disease (COPD), acquired immune deficiency syndrome (AIDS), fibromyalgia and hyperlipidaemia provided the data for analysis. The results demonstrated that not only did the prostate cancer and hyperlipidaemia patients have the highest QoL across the chronic disorders, but their QoL was comparable to normative data on healthy persons. Homebound, elderly, incontinent patients had the lowest QoL for physical functioning, whereas patients hospitalized with AIDS had the lowest QoL in general health and social functioning. Patients with COPD had the lowest QoL in role-physical, role-emotional and mental health. Patients with fibromyalgia had the lowest QoL in bodily pain and vitality. Compared to normative data, patients with urinary incontinence, COPD, AIDS and fibromyalgia generally had lower QoL. Prostate cancer and hyperlipidaemia patients had QoL comparable to normative data. Compared to normative data, patients with urinary incontinence, COPD, AIDS and fibromyalgia had more variability for role-emotional. AIDS patients had more variability on physical functioning, bodily pain and social functioning compared to the normative data. These data suggest that patients with various chronic disorders may have QoL that is lower in most domains compared to a healthy population. 11. Relationship of Cardiac Inpatients' Outcomes to Mood State

Abstract

This descriptive study used a computerized charge capture system (CCCS) to explore the differences of cost and length of stay (LOS) between cardiac inpatients with a diagnosis of depression (n144) and cardiac inpatients without depression (n9,099). Level of severity, gender, and mood state (depression vs. nondepression) were also compared. A matched sample of 352 nondepressed patients was compared with a sample of 94 depressed patients. There were no significant differences between the depressed and nondepressed groups. However, the study did indicate interesting findings regarding mood state, gender, and cardiac outcomes. Depression was significantly overrepresented among females (x224.0, df= I, P< 0.05). When gender and mood state were considered together, women with cardiac disease who were depressed had significantly longer lengths of stay (LOSs) and increased costs than men with depression (F6.6, dfI, P0.01). A major unanticipated finding was the extremely low incidence of depression detected in these patients (1.6) when compared with patients in other studies. One possible reason for the low incidence of depression was related to the use of a financial, rather than a clinical, data set.
