Background The implementation of quality of life (QoL) concepts in routine care, is still an open matter. We followed the Medical Research Council framework for complex interventions to implement a model of QoL diagnosis and therapeutic options, and investigated its effectiveness in patients with colorectal cancer. Methods This randomised, single-blind, multicentre, clinical trial enrolled patients diagnosed with primary colorectal cancer aged 18 years or older who were surgically treated in one of four recruiting hospitals in Germany. All patients received aftercare from one of 178 coordinating practitioners (CPs) who had access to 75 healthcare professionals providing tailored therapies. QoL was measured (EORTC QLQ-C30, QLQ-CR29) in all patients after surgery (baseline) and during aftercare (3, 6, 12, 18 months). Patients were randomised (1:1) into two groups: a care pathway, including QoL-profiles consisting of 13 QoL scales plus specific therapeutic recommendations forwarded to the patient’s CP or standard postoperative care adhering to the German national guideline for colorectal cancer (control). The primary endpoint was the proportion of patients in each group with a need for QoL therapy 12 months after surgery. Analyses were done in the intention-to-treat population. This trial is registered with ClinicalTrials.gov , number NCT02321813 and closed to accrual. Findings Between Jan 13, 2014, and Oct 28, 2015, 220 patients were enrolled and randomly assigned (n = 110 per group). At baseline (in hospital after surgery), a need for QoL therapy was diagnosed in 92/103 (89%) of intervention and 86/104 (83%) of control group patients. At 12 months (primary endpoint) the proportion of patients with a need for QoL therapy was 35/83 (42%; 95% CI 31–54%) in the intervention group versus 50/87 (57%; 95% CI: 46–68%) in the control group (p = 0·046, number needed to treat = 7; 95% CI 3–225). Interpretation Patients profited from the diagnosis of QoL deficits and tailored therapeutic options in their treatment of colorectal cancer. This trial confirmed the results of a previous RCT in breast cancer patients. The implementation of QoL concepts should become standard in treatment guidelines on cancer care. Intraoperative verification of parathyroid glands relies on visual identification by the surgeon and, with some time delay, on serum parathormon measurements and frozen section. Fluorescence imaging, however, is an instant on-table method for direct visualization of parathyroid tissue which is known to exhibit increased autofluorescence intensity when exposed to near-infrared light. In this retrospective observational study, we evaluate the clinical use of this method in a series of patients with primary and secondary hyperparathyroidism. A total of 66 adenomatous and hyperplastic parathyroid glands were examined with intraoperative autofluorescence in 39 patients with primary and secondary hyperparathyroidism using a near-infrared system (KARL STORZ GmbH & Co. KG). The specimens were verified by conventional histology. Fifty-seven of 66 histologically proven adenomatous/hyperplastic glands exhibited autofluorescence. The sensitivity of near-infrared autofluorescence was 0.9 in pHPT and 0.83 in sHPT, respectively. The positive predictive value was 0.93 in pHPT and 1.0 in sHPT, respectively. Near-infrared autofluorescence guidance presents an innovative instant surgical imaging tool with sensitivity in detecting adenomatous and hyperplastic parathyroid glands comparable to current intraoperative methods. Due to its elegant and tracer-free design combined with low follow-up costs, this method can be useful for routine use.

Objective: Recent studies have documented the presence of bone marrow-derived endothelial progenitor cells (EPC) in the circulation of several species. This study was designed to evaluate the use of engineered EPC for vascular gene delivery into angioplasty-induced arterial lesions. Methods and Results: EPC could easily be isolated from whole bone marrow and peripheral blood of adult rats. Differentiation was induced by culture on fibronectin in the presence of endothelial specific growth factors. Rat EPC shared several phenotypic and functional properties with mature endothelial cells. Recombinant retroviruses were generated encoding for the anticoagulants tissue-type plasminogen activator (tPA) and hirudin. Efficient (>90%) ex vivo gene transfer could be achieved resulting in high levels of transgene production. Engineered EPC were locally infused into freshly balloon-injured carotid arteries. Analysis of day 7 vessels showed 73±10% luminal coverage of the lesioned arterial bed with transduced EPC. Sustained secretion of both anticoagulants could be detected in organ cultures of explanted arteries. EPC seeding inhibited dilation of the injured arterial segment and prevented reduction of media thickness. However, rapid repopulation with EPC failed to attenuate neointima formation in this model. Conclusions: Peripheral blood and bone marrow can be used as source for endothelial lineage cells. Cultured EPC can be genetically engineered by retroviral gene transfer and serve as cellular vehicles for vascular gene and drug delivery of anticoagulants. Local transplantation of EPC attenuates reendothelialization of angioplasty-injured arteries but fails to inhibit neointima proliferation.
