- Abstract
Background: Ventricular fibrillation (VF) waveform analysis has been proposed as a potential non-invasive guide to optimize timing of defibrillation. Methods: The AMplitude Spectrum Area (AMSA) trial is an open-label, multicenter randomized controlled study reporting the first in-human use of AMSA analysis in out-of-hospital cardiac arrest (OHCA). The primary efficacy endpoint was the termination of VF for an AMSA ≥ 15.5 mV-Hz. Adult shockable OHCAs randomly received either an AMSA-guided cardiopulmonary resuscitation (CPR) or a standard-CPR. Randomization and allocation to trial group were carried out centrally. In the AMSA-guided CPR, an initial AMSA ≥ 15.5 mV-Hz prompted for immediate defibrillation, while lower values favored chest compression (CC). After completion of the first 2-min CPR cycle, an AMSA < 6.5 mV-Hz deferred defibrillation in favor of an additional 2-min CPR cycle. AMSA was measured and displayed in real-time during CC pauses for ventilation with a modified defibrillator. Findings: The trial was early discontinued for low recruitment due to the COVID-19 pandemics. A total of 31 patients were recruited in 3 Italian cities, 19 in AMSA-CPR and 12 in standard-CPR, and included in the data analysis. No difference in primary outcome was observed between the two groups. Termination of VF occurred in 74% of patients in the AMSA-CPR compared to 75% in the standard CPR (OR 0.93 [95% CI 0.18–4.90]). No adverse events were reported. Interpretation: AMSA was used prospectively in human patients during ongoing CPR. In this small trial, an AMSA-guided defibrillation provided no evidence of an improvement in termination of VF. Trial registration: NCT03237910. Visuo-motor integration shapes our daily experience and underpins the sense of feeling in control over our actions. The last decade has seen a surge in robotically and virtually mediated interactions, whereby bodily actions ultimately result in an artificial movement. But despite the growing number of applications, the neurophysiological correlates of visuo-motor processing during human-machine interactions under dynamic conditions remain scarce. Here we address this issue by employing a bimanual robotic interface able to track voluntary hands movement, rendered in real-time into the motion of two virtual hands. We experimentally manipulated the visual feedback in the virtual reality with spatial and temporal conflicts and investigated their impact on (1) visuo-motor integration and (2) the subjective experience of being the author of one's action (i.e., sense of agency). Using somatosensory evoked responses measured with electroencephalography, we investigated neural differences occurring when the integration between motor commands and visual feedback is disrupted. Our results show that the right posterior parietal cortex encodes for differences between congruent and spatially-incongruent interactions. The experimental manipulations also induced a decrease in the sense of agency over the robotically-mediated actions. These findings offer solid neurophysiological grounds that can be used in the future to monitor integration mechanisms during movements and ultimately enhance subjective experience during human-machine interactions.

- Abstract
Objective: To quantify how the first public announcement of confirmed coronavirus disease 2019 (COVID-19) in Italy affected a metropolitan region's emergency medical services (EMS) call volume and how rapid introduction of alternative procedures at the public safety answering point (PSAP) managed system resources. Methods: PSAP processes were modified over several days including (1) referral of non-ill callers to public health information call centers; (2) algorithms for detection, isolation, or hospitalization of suspected COVID-19 patients; and (3) specialized medical teams sent to the PSAP for triage and case management, including ambulance dispatches or alternative dispositions. Call volumes, ambulance dispatches, and response intervals for the 2 weeks after announcement were compared to 2017–2019 data and the week before. Results: For 2 weeks following outbreak announcement, the primary-level PSAP (police/fire/EMS) averaged 56% more daily calls compared to prior years and recorded 9281 (106% increase) on Day 4, averaging ∼400/hour. The secondary-level (EMS) PSAP recorded an analogous 63% increase with 3863 calls (∼161/hour; 264% increase) on Day 3. The COVID-19 response team processed the more complex cases (n = 5361), averaging 432 ± 110 daily (∼one-fifth of EMS calls). Although community COVID-19 cases increased exponentially, ambulance response intervals and dispatches (averaging 1120 ± 46 daily) were successfully contained, particularly compared with the week before (1174 ± 40; P = 0.02). Conclusion: With sudden escalating EMS call volumes, rapid reorganization of dispatch operations using tailored algorithms and specially assigned personnel can protect EMS system resources by optimizing patient dispositions, controlling ambulance allocations and mitigating hospital impact. Prudent population-based disaster planning should strongly consider pre-establishing similar highly coordinated medical taskforce contingencies.

16. Emerging Information Societies in an Interdependent World.

- Abstract
Commenting on the trends toward the interlocking of emerging information societies and the growing interdependence of countries, this paper suggests the role that "informatics" (the rational and systematic use of information for planning and decision making) may play in the transition of societies into the information age. Two paradoxes challenging the builders of information societies are identified: countries must mold their own particular social situations in a context of wide international commitments, and countries involved in international cooperation agreements must also maintain their diversity. Suggestions for meeting the challenge include changing the basis for measuring the development of emerging countries from gross national product, literacy, or personal income criteria to how well the countries manage and apply their information sources and developing systems of national information resource management. The role of information in emerging information societies is outlined, including the results of the Intergovernmental Conference on Strategies and Policies for Informatics (SPIN) and an Intergovernmental Bureau for Informatics (IBI) survey on international informatics. The role of IBI is defined, and the paper concludes with a call for collaboration between IBI and the International Institute of Communications. A table of characteristics of ten newly industrialized countries is appended. (AEA)

- Abstract
Non-disjoined chromosomes 15 from 115 cases of uniparental disomy (ascertained through Prader-Willi syndrome) and 13 cases of trisomy of maternal origin were densely typed for microsatellite loci spanning chromosome 15q. Of these 128 cases a total of 97 meiosis I (MI) errors, 19 meiosis II (MII) errors and 12 mitotic errors were identified. The genetic length of a map created from the MI errors was 101 cM, as compared with a maternal length of 137 cM based on CEPH controls. No significant differences were detected in the distribution of recombination events along the chromosome arm and a reduction was seen for most of the chromosome 15 intervals examined. It was estimated that 21% of tetrads leading to MI non-disjunction were achiasmate, which may account for most or all of the reduction in recombination noted. The mean age of mothers of cases involving MI errors which showed no transitions from heterodisomy to isodisomy was significantly lower (32.7) than cases showing one or more observable transitions (36.3) (P < 0.003, t-test). However, even among chiasmate pairs the highest mean maternal age was seen for multiple exchange tetrads. Chromosome-specific differences in maternal age effects may be related to the normal distribution of exchanges (and their individual susceptibilities) for each chromosome. However, they may also reflect the presence of multiple factors which act to ensure normal segregation, each affected by maternal age in a different way and varying in importance for each chromosome

BACKGROUND: Few data are available regarding the use of antithrombotic strategies in coronary artery disease patients with atrial fibrillation (AF) in everyday practice. We sought to describe the prevalence of AF and its antithrombotic management in a contemporary population of patients with stable coronary artery disease.METHODS AND FINDINGS: CLARIFY is an international, prospective, longitudinal registry of outpatients with stable coronary artery disease, defined as prior (≥12 months) myocardial infarction, revascularization procedure, coronary stenosis >50%, or chest pain associated with evidence of myocardial ischemia. Overall, 33,428 patients were screened, of whom 32,954 had data available for analysis at baseline; of these 2,229 (6.7%) had a history of AF. Median (interquartile range) CHA2DS2-VASc score was 4 (3, 5). Oral anticoagulation alone was used in 25.7%, antiplatelet therapy alone in 52.8% (single 41.8%, dual 11.0%), and both in 21.5%. OAC use was independently associated with permanent AF (pCONCLUSIONS: In this contemporary cohort of patients with stable coronary artery disease and AF, most of whom are theoretical candidates for anticoagulation, oral anticoagulants were used in only 47.2%. Half of the patients received antiplatelet therapy alone and one-fifth received both antiplatelets and oral anticoagulants. - Abstract
Objective: We have investigated the role that improvement in arterial oxygenation has, consequent on positive end-expiratory pressure (PEEP), in the reduction of cardiac index (CI) determined by applying PEEP. Design: 2 × 2 factorial trial. Setting: Department of intensive care medicine at a university hospital. Patients: 13 patients on mechanical ventilation for acute lung injury. Interventions: Four experimental conditions, each one characterized by one level of PEEP and one level of PaO2: LOLP = Low PaO2 (∼ 50 mmHg) Low PEEP (∼ 1 cmH2O) LOHP = Low PaO2 (∼ 50 mmHg) High PEEP (∼ 10 cmH2O) HOLP = High PaO2 (∼ 80 mmHg) Low PEEP (∼ 1 cmH2O) HOHP = High PaO2 (∼ 80 mmHg) High PEEP (∼ 10 cmH2O) Measurements and results: Hemodynamic and gas exchange data were collected for each experimental condition. CI showed a 13% decline from LOLP (7.0 ± 1.71/min per m2) to HOHP (6.1 ± 1.31/min per m2). Both the direct effect of PEEP on the CI (LOLP + HOLP vs LOHP + HOHP, p < 0.01) and the indirect effect related to the improvement in oxygenation (LOLP + LOHP vs HOLP + HOHP. p < 0.01) contributed to the reduction in CI. Conclusions: In evaluating CI changes induced by PEEP we should take into account the indirect effect of arterial oxygenation upon CI. This should be considered, at least in part, as a physiological adjustment rather than as impaired cardiovascular performance

There is growing evidence for a link between body weight and cancer risk, but there is not a clear consensus yet.We studied the association between body mass index (BMI) and overall, lung, prostate and colon cancer mortality. In 1971/73, weight and height were measured in 2974 men working in Basel, Switzerland. In 1990, the vital status of all participants was assessed.290 men had died from cancer, 87 from lung, 30 from prostate, and 22 from colon cancer. In the predefined Cox Proportional Hazards Regression Models for survival analysis, a baseline hazard was modified multiplicatively by covariates, i.e. the untransformed continuous variable "BMI" was chosen as covariate. In addition it was assumed that the baseline hazard may be different for smokers, non-smokers and different age groups (age at entry into study). Thus, multiple strata, i.e. combinations of smoking status and age groups were allowed. With increasing BMI overall cancer mortality did not change. Accordingly, the relative risk (RR) per 1-unit increase of BMI (unit = 1 kg/m2) was 1.03 (95% CI: 0.99-1.07). In relation to lung cancer, mortality did neither increase nor decrease with increasing BMI (RR = 1.0; 95% CI 0.93- 1.07). The results for prostate cancer mortality were similar, i.e. no correlation with BMI was observed (RR = 0.95; 95% CI: 0.93-1.18). The same was true for colon cancer mortality (RR = 1.09; 95% CI: 0.92-1.24).This investigation provides little evidence of an association between BMI and mortality of all cancers combined, cancer of the lung, the prostate and the colon.

32. Meiotic origin of trisomy in confined placental mosaicism is correlated with presence of fetal uniparental disomy, high levels of trisomy in trophoblast, and increased risk of fetal intrauterine growth restriction

Molecular studies were performed on 101 cases of confined placental mosaicism (CPM) involving autosomal trisomy. The origin of the trisomic cell line was determined in 54 cases (from 51 pregnancies), 47 of which were also analyzed for the presence of uniparental disomy (UPD) in the disomic cell line. An additional 47 cases were analyzed for parental origin in the disomic cell line only. A somatic (postmeiotic) origin of the trisomy was observed in 22 cases and included the majority of cases with CPM for trisomy 2, 7, 8, 10, and 12. Most cases of CPM involving trisomy 9, 16, and 22 were determined to be meiotic. Fetal maternal UPD was found in 17 of 94 informative CPM cases, involving trisomy 2 (1 case), 7 (1 case), 16 (13 cases), and 22 (2 cases). The placental trisomy was of meiotic origin in all 17 cases associated with fetal UPD (P = .00005). A meiotic origin also correlated with the levels of trisomy in cultured chorionic villi samples (CVS) (P = .0002) and trophoblast (P = .00005). Abnormal pregnancy outcome (usually IUGR) correlated with meiotic origin (P = .0003), the presence of fetal UPD (P = 4 x 10(-7)), and the level of trisomy in trophoblast (P = 3 x 10(-7)) but not with the level of trisomy in CVS or term chorion. The good fit of somatic errors with the expected results could have been observed only if few true meiotic errors were misclassified by these methods as a somatic error. These data indicate that molecular determination of origin is a useful predictor of pregnancy outcome, whereas the level of trisomy observed in cultured CVS is not. In addition, UPD for some chromosomes may affect prenatal, but not postnatal, development, possibly indicating that imprinting effects for these chromosomes are confined to placental tissues.

This paper presents the results of a broad, transversal, observational survey on the social cost of female urinary incontinence in the general female population, resident in the ASL3-Monza area. In this population, in an 18-65 year old age group, the prevalence of U.I. observed is 28.5% (LIFE cases) and 18.6% (YEAR cases). The total number of incontinent women in the ASL3-Monza area and in the Lombardy Region can be estimated as follows: - ASL3-Monza 125,288 "LIFE" cases, 81,767 (18.6% of 439,609) "YEAR" cases. - Lombard Region 1,119,480 "LIFE" cases, 730,608 (18.6% of 3,928, 709) "YEAR" cases. In the light of the data collected, we can estimate that in the Lombardy Region and in the ASL3-Monza area, at least 379, 600 (52% of 730,608) and 42,518 (52% of 81, 767) incontinent women use protection and 175,200 (24% of 730,608) and 19,624 (24% of 81,767) consider using a pad to be the only useful solution to the problem. The "estimated" mean/annual cost per woman in an incontinent population with characteristics similar to those of the general female population of the ASL3-Monza is € 358,03, for a mean/life cost per woman of € 11.099,20. When there is no effective treatment of the symptom, we can estimate the following total cost/life in the ASL3-Monza area to be: 65,41 × 31 × € 358,03 = € 726.025.407. If the symptom disappears and the treatment is effective, this is also an advantage, apart from the obvious requirement for public health related to it, from a strictly pharmacological-economic point of view. The estimated total/life cost for the treatment (with the most effective medical, rehabilitative and/or surgical therapy available today) of a sample of 1,000 incontinent women of the ASL3-Monza area is significatively lower than the total cost/life expected in the case of persistence and "assistance" of the symptom: € 4.502.812, 10 vs. € 23.451.270,02. A 25-year-old normal healthy male was karyotyped because five of his wife's pregnancies terminated in spontaneous abortions at 6-14 wk of gestation. Cytogenetic investigation disclosed a de novo balanced Robertsonian t(22q;22q) translocation. Molecular studies revealed maternal only inheritance for chromosome 22 markers. Reduction to homozygosity for all informative markers indicates that the rearranged chromosome is an isochromosome derived from one of the maternal chromosomes 22. Except for the possibility of homozygosity for recessive mutations, maternal uniparental disomy 22 does not seem to have an adverse impact on the phenotype, apart from causing reproductive failure. It can be concluded that no maternally imprinted genes with major effect map to chromosome 22.

- Abstract
One hundred and ten consecutive patients who had come to the Urogynaecological outpatients surgery of Desio Hospital with stress urinary incontinence were studied and treated with uro-rehabilitation at the Neuro-rehabilitation Division of Seregno Hospital between 1998 and 2000. The study only included the patients studied and treated in that three year period of research who suffered from stress urinary incontinence (genuine or mixed) with a 0/1 degree cystocele according to the Baden-Walker HWS classification with type I/II incontinence according to Blassas, with no suspect intrinsic clinical instrumental urethral defect and of whom all the subjective and objective parameters of evaluation of the gravity of the IUS adopted were available (micturition diary, VAS, Pad test according to the ICS, stress test according to Ferrari, I-S score, PC test) before and after treatment. All the patients voluntarily accepted to follow the combined therapeutic rehabilitation protocol at the Outpatients Surgery: One cycle of physiochinesitherapy associated to a three weekly cycle as outpatients of electro myographic biofeedback and FES with vaginal probe. The comparison between the initial and final numerical values studied after treatment show how the degree of the disorder, subjective and/or objective of the patients had significantly improved: the mean value of VAS passed from 6.8+/-2.1 to 3.5+/-2.6; the values of the PAD and stress tests went from 9.8±16.8 2.8±1.3 to 5.9±13.7 and 1.8±1.6 respectively an inverse trend being shown by the PC-test which rise from 1.1±1 to 2.2±0.9. In particular, 55% of the patients during their first visit mentioned a VAS equal to or above 7; while after the rehabilitation treatment in 51% of the population a VAS equal to or less than 3 was registered.
