Abstract

Most studies on gender and psychosis have focused on gender differences at illness onset or on the long-term outcome, whereas little is known about the impact of gender on the first years after psychosis onset. A total of 185 first episode psychosis (FEP) patients were followed for 5 years after psychosis onset, and gender differences were explored in psychopathology (PANSS), needs for care (CAN), and insight (SAI-E). Male patients showed more negative symptoms than females over time, whereas female patients showed higher levels of depressive symptoms than males throughout the study period. In addition, female patients presented more functioning unmet needs for care, but higher levels of insight into illness than males. Therapy and rehabilitative programs for FEP patients should be gender-targeted, as gender has proved to impact on psychopathology, needs for care, and insight in the very first years following psychosis onset.

3. Cutaneous squamous cell carcinoma in epidermolysis bullosa: A 28 year retrospective study.

Abstract

Introduction &objectives: Some forms of epidermolysis bullosa (EB), notably severe generalized recessive dystrophic EB (RDEB-SG), are associated with an increased risk of developing mucocutaneous squamous cell carcinomas (SCCs) which behave aggressively and are the major cause of mortality in early to midadulthood. We report our centres' experience of EB-associated SCCs over the last 28 years. Materials &methods: An observational, retrospective, double institution case record review of EB patients diagnosed with SCC between July 1991-June 2019. Result(s): Forty-four EB patients with SCC were identified with a total of 221 primary SCCs. They comprised: 31 (70%) with RDEB-SG, 1 (2.3%) with RDEB-pruriginosa, 1 with RDEBinversa (2.3%), 2 (4.5%) with RDEB-generalised intermediate, 3 (6.8%) with junctional EB generalised intermediate (JEB-GI), 5 (11.4%) with dominant dystrophic EB, and 1 (2.3%) with Kindler syndrome. Diagnosis of first SCC was earlier in the RDEB-SG group (median 29.5 years (range 13-52 years)) compared to other groups collectively (median 47.1 years (range 30-89 years)). Most SCCs occurred in the RDEB-SG group, and the majority had multiple tumours (mean 5.8 (range 1-44)). SCC-associated mortality was high in this group (64.5%), with a median survival after diagnosis of first SCC of 2.4 years (range 0.5-12.6 years). Most SCCs were well-differentiated (53.4%) and located on the extremities. Wide local excision was undertaken for the majority of tumours (86.4%). Metastatic disease occurred in 16 of 31 (51.6%) RDEB-SG patients and 1 with JEB-GI. Treatments for metastatic disease included lymph node dissection (n = 6), radiotherapy (n= 5), chemotherapy (n=3), electrochemotherapy (n= 2), and targeted cancer therapies erlotinib (n=1), cetuximab (n =2) and cemiplimab (n= 1). 18. Justified concern or exaggerated fear: the risk of anaphylaxis in percutaneous treatment of cystic echinococcosis-a systematic literature review

Abstract

Percutaneous treatment (PT) emerged in the mid-1980s as an alternative to surgery for selected cases of abdominal cystic echinococcosis (CE). Despite its efficacy and widespread use, the puncture of echinococcal cysts is still far from being universally accepted. One of the main reasons for this reluctance is the perceived risk of anaphylaxis linked to PTs. To quantify the risk of anaphylactic reactions and lethal anaphylaxis with PT, we systematically searched MEDLINE for publications on PT of CE and reviewed the PT-related complications. After including 124 publications published between 1980 and 2010, we collected a total number of 5943 PT procedures on 5517 hepatic and non-hepatic echinococcal cysts. Overall, two cases of lethal anaphylaxis and 99 reversible anaphylactic reactions were reported. Lethal anaphylaxis occurred in 0.03% of PT procedures, corresponding to 0.04% of treated cysts, while reversible allergic reactions complicated 1.7% of PTs, corresponding to 1.8% of treated echinococcal cysts. Analysis of the literature shows that lethal anaphylaxis related to percutaneous treatment of CE is an extremely rare event and is observed no more frequently than drug-related anaphylactic side effects.

Abstract

The present longitudinal study is aimed at analyzing how adolescents change their dysfunctional attitudes towards their body and eating behaviors in relation to personality characteristics across a six-month time span. Via multiple regression analyses we investigated whether MMPI-A Obsessiveness, Low Self-Esteem, Depression, Family Problems and Concern for health are temporal antecedents of EDI-2 eating disorders, and vice versa. Our main findings revealed a bidirectional relationship between eating attitudes and personality. In fact, Obsessiveness, Low self-esteem, Depression, and Family problems significantly predicted changes in different dysfunctional eating attitudes, but also EDI-Interpersonal distrust and Ineffectiveness predicted changes in different MMPI-A scale scores. All together, findings suggest that personality development in adolescence is sensitive to attitudes towards eating and body appearance, and vice versa.

Abstract

During goal oriented web navigation does the competition for web selection depend on all navigation options or only those options which are more likely to be functional for information seeking? Here we provide evidence in favour of the latter alternative. Within a representative set of real web sites of variable breadth, the time required to reach a goal located at the depth of two clicks from the home page is accounted for by C, an objective measure of the complexity of the start page, based on the number of links weighted by the number and type of embedding web elements. Our results demonstrate how focusing on links while ignoring other web elements optimizes the deployment of attentional resources necessary to navigation.

Abstract

Self-discrepancies represent a gap between people’s different selves and evoke negative emotions (Higgins, 1987). In this research project, in agreement with Higgins’ theory, we hypothesized that participants’ actual-self vs. ideal-self discrepancies are primarily related to dejection emotions. We conducted an experimental study where we used explicit measures. After identifying an ideal-self discrepant quality by administering self-report questionnaires in which participants rated themselves on their actual personality attributes and on their ideal qualities, two weeks later we assigned each participant a schematic self-discrepant quality and ask to describe situations in which s/he wished s/he met his/her ideal standard but s/he didn’t (positive discrepancy condition or achieving direction), or to describe a situation in which s/he met right the contrary of his/her ideal standard (negative discrepancy condition or avoidance direction). Via a priming task, we aimed at activating such ideal self-schemata, before asking participants in experimental conditions to rate current emotional states along POMS items (Spielberger, 2012). We expected to observe higher mean scores on POMS Depression scale when participants were exposed to the actual/ideal priming conditions, in comparison to the control-group participants, who only rated their current emotional states on POMS. Preliminary findings cautiously suggest that discrepancies require to be assessed in a more clear and accurate way in order to relate to specific emotions, by taking into account the role of positive discrepancy condition (achieving direction) and negative discrepancy condition (avoidance direction) as well.

Abstract

Metastasis arises from disseminated tumour cells (DTCs) that are characterized by intrinsic phenotypic plasticity and the capability of seeding to secondary organs. DTCs can remain latent for years before giving rise to symptomatic overt metastasis. In this context, DTCs fluctuate between a quiescent and proliferative state in response to systemic and microenvironmental signals including immune-mediated surveillance. Despite its relevance, how intrinsic mechanisms sustain DTCs plasticity has not been addressed. By interrogating the epigenetic state of metastatic cells, we find that tumour progression is coupled with the activation of oncogenic enhancers that are organized in variable interconnected chromatin domains. This spatial chromatin context leads to the activation of a robust transcriptional response upon repeated exposure to retinoic acid (RA). We show that this adaptive mechanism sustains the quiescence of DTCs through the activation of the master regulator SOX9. Finally, we determine that RA-stimulated transcriptional memory increases the fitness of metastatic cells by supporting the escape of quiescent DTCs from NK-mediated immune surveillance. Overall, these findings highlight the contribution of oncogenic enhancers in establishing transcriptional memories as an adaptive mechanism to reinforce cancer dormancy and immune escape, thus amenable for therapeutic intervention., (© 2024. WHAT DOES THIS STUDY ADD?: These guidelines aim to provide information on the surgical and conservative therapeutic hand management of children and adults diagnosed with EB. Abstract

EU and Italian laws dealing for the presence of antibiotics or, more in general, drags in food established limits for different kinds of food. Suitable rules exist about the medical treatment of cattle in relation to the production of milk and meat. The adoption of a procedure to check the respect of the law limits is necessary. In this paper, the presence of different classes of antibiotics in milk and in homogenised meat is investigated. Generally, HPLC methods are applied for this purpose. In this paper, the application of polarographic analysis is studied and the results are compared with the chromatographic ones. The comparison is relative to all the phases of analysis including the sample preparation. The results show the advantage of the proposed procedure.

Abstract

Percutaneous treatment (PT) emerged in the mid-1980s as an alternative to surgery for selected cases of abdominal cystic echinococcosis (CE). Despite its efficacy and widespread use, the puncture of echinococcal cysts is still far from being universally accepted. One of the main reasons for this reluctance is the perceived risk of anaphylaxis linked to PTs. To quantify the risk of anaphylactic reactions and lethal anaphylaxis with PT, we systematically searched MEDLINE for publications on PT of CE and reviewed the PT-related complications. After including 124 publications published between 1980 and 2010, we collected a total number of 5943 PT procedures on 5517 hepatic and non-hepatic echinococcal cysts. Overall, two cases of lethal anaphylaxis and 99 reversible anaphylactic reactions were reported. Lethal anaphylaxis occurred in 0.03% of PT procedures, corresponding to 0.04% of treated cysts, while reversible allergic reactions complicated 1.7% of PTs, corresponding to 1.8% of treated echinococcal cysts. Analysis of the literature shows that lethal anaphylaxis related to percutaneous treatment of CE is an extremely rare event and is observed no more frequently than drug-related anaphylactic side effects.

Abstract

The underlying genetic abnormalities of epidermolysis bullosa (EB) cause destabilization at the dermo-epidermal junction. Patients with EB characteristically are subject to blistering following relatively minor trauma (the Nikolsky sign), and suffer from ulcers and erosions in all areas subject to persistent or repeated friction, such as the hand. Hand deformities occur in most patients with dystrophic EB (DEB), and include adduction contractures of the first web space, pseudosyndactyly, and flexion contractures of the interphalangeal, metacarpophalangeal, and wrist joints. All structures in the hand may be involved. The severity of the deformity worsens with age, and surgical correction becomes increasingly difficult. Recurrent deformity occurs within 2 to 5 years. Meticulous skin care and the use of well-fitted splints supervised within a multidisciplinary team setting are essential. Abstract

The value of ultrasonography as an adjunct for diagnosis and monitoring malaria was investigated. In all, 118 patients (male/female 65/53; age 2-78 years, median 29 years) with malaria underwent a standardised abdominal ultrasound examination at baseline. In 62 out of 118 patients, ultrasonography was repeated 21 days later. In the results at baseline, huge splenomegaly with firm organ consistency, consistent with hyperreactive malarious splenomegaly syndrome, was observed in two Cameroonese children. In the other 116 patients, the most common finding was non-specific splenomegaly (96/116, 82.76%), occurring more frequently in non-immune patients (71/78, 91.03%) than in patients who had grown up in malaria-endemic areas (25/38, 65.79%; P<0.002). No correlation was found between liver or spleen size and any clinical parameter. The results on day 21 show that, although splenomegaly after therapy persisted more frequently in patients with malaria recrudescence or relapse (8/8, 100%) than in patients cured (32/54, 59.26%; P<0.0421), the practical value of this finding is questionable. Ultrasonography cannot be regarded as a first-line diagnostic method in patients with malaria.

Abstract

A new classification of fingertip injuries is presented. The PNB classification separates the injury into its effect on the three components of the fingertip: pulp, nail and bone. This provides a three digit number that accurately describes the injury. This new classification can be used to record and document injuries, without having to resort to lengthy description. It could also be used to provide detailed instructions for treatment and indications for referral. Examples of the application of the classification are given.

Abstract

Absence of the right superior caval vein has been shown by postmortem studies of infants and children to be associated with histological abnormalities of the sinus node. We report the case of a 32-yr-old man who presented with syncope due to disease of the sinus node and was found to have absence of the right superior caval vein. He was successfully managed with implantation of an epicardial atrioventricular sequential pacemaker.
