Chronic stress may represent one of the most important factors that negatively affects the health and performance of athletes. Finding a way to introduce psychological strategies to manage stress in everyday training routines is challenging, particularly in junior teams. We also must consider that a stress management intervention should be regarded as “efficacious” only if its application results in improvement of the complex underlying pathogenetic substratum, which considers mechanistically interrelated factors, such as immunological, endocrine and autonomic controls further to psychological functioning and behavior. In this study, we investigated the feasibility of implementing, in a standard training routine of the junior team of the Italian major soccer league, a stress management program based on mental relaxation training (MRT). We evaluated its effects on stress perception and cardiac autonomic regulation as assessed by means of ANSI, a single composite percentile-ranked proxy of autonomic balance, which is free of gender and age bias, economical, and simple to apply in a clinical setting. We observed that the simple employed MRT intervention was feasible in a female junior soccer team and was associated with a reduced perception of stress, an improved perception of overall health, and a betterment of cardiac autonomic control. This data may corroborate the scientific literature that indicates psychological intervention based on MRT as an efficacious strategy to improve performance, managing negative stress effects on cardiac autonomic control.

In questo studio qualitativo descrittivo esploriamo l’esperienza del primo lockdown in alcuni anziani residenti in tre regioni italiane (Lombardia, Piemonte, e Puglia) diversamente colpite dalla pandemia. I dati sono stati raccolti con interviste semi-strutturate, analizzate con la content analysis. Gli intervistati, autosufficienti e con MMSE≥26, hanno mediamente 75,5 anni, sono soprattutto donne (14/18) hanno istruzione media/media secondaria, vivono con il coniuge/partner (9/18) o con altri familiari (5/18). Restituiscono un’esperienza dolorosa della pandemia, in cui si sono sentiti soli, hanno avuto paura e hanno vissuto la mancanza dei loro affetti. Al contempo, gli anziani hanno espresso emozioni positive e dimostrato atteggiamenti resilienti, come coltivare i propri hobby, mantenere le relazioni a distanza, fare movimento a casa, imparare a utilizzare le tecnologie, soprattutto al nord. Gli intervistati sembrano essersi impegnati per mantenere una loro progettualità, nonostante e oltre la pandemia., In this descriptive qualitative study, we explore the experience of the first lockdown in some older adults living in three Italian regions (Lombardy, Piedmont, and Puglia), which were differently affected by the pandemic. Data were collected through semi-structured interviews and analyzed with the content analysis. The interviewees, self-sufficient and with MMSE≥26, are on average 75,5 years old, are mainly women (14/18), have middle/high school education, and live with their spouse/partner (9/18) or with other family members (5/18). They report a painful experience of the pandemic, in which they felt alone, were afraid, and missed their beloved ones. At the same time, older adults expressed positive emotions and demonstrated resilient behaviors, such as cultivating their hobbies, maintaining long-distance relationships, doing movement at home, and learning to use technologies, mainly in the North. The interviewees seem to have committed themselves to preserving their life projects, despite and beyond the pandemic.

Questo studio indaga il tirocinio della Laurea Magistrale (LM) in Scienze Riabilitative delle Professioni Sanitarie (SRPS) nel contesto italiano, attraverso un sondaggio condotto con un questionario somministrato telefonicamente ai Coordinatori. Sono stati coinvolti tutti i 22 corsi attivi nell'AA 2018/19; 15 hanno partecipato (68%). Il tirocinio è un corso annuale obbligatorio, non sempre di 30 CFU, come da normativa italiana, distribuiti in modo diverso tra gli anni di corso e nei piani studi. Nel 73% dei casi, è consentita una formazione autonoma e non strutturata; nel 33%, gli studenti possono andare all'estero, prevalentemente per progetti di ricerca. I tirocini della LM in SRPS hanno differenti modelli organizzativi (esperienza supervisionata/non supervisionata; sede assegnata/scelta autonomamente), spesso combinati uno con l'altro. Sono frequentemente previsti momenti di feedback con il tutor e viene effettuato un esame finale di tirocinio caratterizzato da disomogeneità delle prove e della Commissione. I risultati stimolano la riflessione sulla difformità dei tirocini nelle LM coinvolte, evidenziando la necessità di sviluppare linee guida comuni. Ciò appare particolarmente auspicabile in considerazione dell'imminente revisione dei curricula delle LM delle professioni sanitarie italiane, che non dovrebbe ignorare le innovazioni nella formazione sanitaria guadagnate grazie alla pandemia.

- Abstract
Loneliness is one of the factors that affect quality of life and mental health of older people, especially when they live alone. However, literature seems to focus mainly on “measuring” loneliness in older people, and less on studying their perception of it. We designed a qualitative study on the role of technologies in supporting pre-frail elders, in order to continue living at home, even if alone. Within it, we explored the participants’ experience of loneliness and loss; data were gathered through 25 individual semi-structured interviews and analyzed with the Interpretative Phenomenological Approach. Findings showed that loneliness characterized the interviewees’ life and it was mostly associated with the loss of a beloved one. Although some participants perceived negatively loneliness and its consequences, others emphasized the importance of having space/time for themselves in their lives and loneliness was almost perceived as a conquest. Some interviewees seemed not to want to surrender to solitude, and they showed to have developed some strategies to face it, such as helping others and maintaining hobbies/interests. More details will be reported in our presentation, where we will reflect on the factors that seem to empower pre-frail older people when facing loneliness, with particular attention to digital literacy.

- Abstract
Questa indagine esplora aspettative, percezione dello sviluppo delle conoscenze, situazione lavorativa, sviluppo professionale e upgrading in un gruppo di studenti e di laureati magistrali in Scienze Riabilitative (LMSR) dell’Università di Milano (UNIMI). Sono stati reclutati tutti gli studenti iscritti dall’A.A. 08/09 al 16/17 (n=213) e intervistati con tre questionari creati ad hoc e somministrati all’immatricolazione (T0), alla laurea (T1) e dopo un anno (T2). Gli intervistati sono stati arbitrariamente suddivisi in junior (età < 40) e senior (≥40). A T0-T1 ha risposto il 58.68% della popolazione; a T2, il 68.8%. Abbiamo rilevato: abbassamento dell’età media e maggiore differenziazione dei profili professionali degli iscritti alla LMSR, diminuzione dell’aspettativa dell’upgrading, percezione di incremento delle conoscenze a T1, soprattutto pedagogiche. Nella popolazione generale, l’occupazione passa dal 58 (T0) al 93% (T1) e secondo gli intervistati la LMSR ha avuto un ruolo rilevante. Il 96% è soddisfatto del percorso di studio. A un anno dalla laurea (T2), il 43% migliora/stabilizza la condizione lavorativa. L’autonomia decisionale passa dal 33% (T0) a 55% (T2). Il 19% prosegue gli studi (50% Master; 25% Dottorato). Ulteriore ricerca, anche in altre LM, è necessaria per comprendere come trasformare/migliorare la formazione post-base alla luce di bisogni e carriere degli studenti.

Anche se lo sviluppo di tecnologie dedicate agli anziani è principalmente finalizzato all’assistenza e al monitoraggio della loro salute, le TIC possono anche favorire l’apprendimento continuo e nuove forme di socializzazione. Il presente contributo espone i risultati di un’indagine esplorativa basata su questionari, condotta con un campione persone di anziane, con familiarità con le tecnologie, per raccogliere la percezione di usabilità e il gradimento di alcuni giochi/attività online, analizzando il potenziale percepito di questi dispositivi per la socializzazione., Information and Communication Technologies (ICT), particularly robotics and domotics, are progressively spreading in the contemporary society and their use is increasing in the field of geriatrics too. Even if the implementation of new technologies dedicated to older people is mainly aimed at caring for them and monitor their health, ICT can also sustain continuing learning and develop new practices of socialization. This paper reports the results of an explorative questionnaire survey conducted with a sample of older people who had some familiarity with technologies. Our aim was to investigate their perception about usability and their enjoyment of some online digital games/activities, analyzing the perceived potentialities of those devices for elders’ socialization.

Marfan syndrome (MS) is a dominant autosomal disease caused by mutations in chromosome 15, the locus controlling fibrillin 1 synthesis, and may exhibit skeletal, ocular, cardiovascular, and other manifestations. Pulse wave velocity (PWV) is used to measure arterial elasticity and stiffness and is related to the elastic properties of the vascular wall. Since the practice of exercise is limited in MS patients, it was of interest to analyze the acute effect of submaximal exercise on aortic distensibility using PWV and other hemodynamic variables in patients with MS with either mild or no aortic dilatation. PWV and physiological variables were evaluated before and after submaximal exercise in 33 patients with MS and 18 controls. PWV was 8.51 ± 0.58 at rest and 9.10 ± 0.63 m/s at the end of exercise (P = 0.002) in the group with MS and 8.07 ± 0.35 and 8.98 ± 0.56 m/s in the control group, respectively (P = 0.004). Comparative group analysis regarding PWV at rest and at the end of exercise revealed no statistically significant differences. The same was true for the group that used β-blockers and the one that did not. The final heart rate was 10% higher in the control group than in the MS group (P = 0.01). Final systolic arterial pressure was higher in the control group (P = 0.02). PWV in MS patients with mild or no aortic dilatation did not differ from the control group after submaximal effort.

Chronic stress may represent one of the most important factors that negatively affects the health and performance of athletes. Finding a way to introduce psychological strategies to manage stress in everyday training routines is challenging, particularly in junior teams. We also must consider that a stress management intervention should be regarded as “efficacious” only if its application results in improvement of the complex underlying pathogenetic substratum, which considers mechanistically interrelated factors, such as immunological, endocrine and autonomic controls further to psychological functioning and behavior. In this study, we investigated the feasibility of implementing, in a standard training routine of the junior team of the Italian major soccer league, a stress management program based on mental relaxation training (MRT). We evaluated its effects on stress perception and cardiac autonomic regulation as assessed by means of ANSI, a single composite percentile-ranked proxy of autonomic balance, which is free of gender and age bias, economical, and simple to apply in a clinical setting. We observed that the simple employed MRT intervention was feasible in a female junior soccer team and was associated with a reduced perception of stress, an improved perception of overall health, and a betterment of cardiac autonomic control. This data may corroborate the scientific literature that indicates psychological intervention based on MRT as an efficacious strategy to improve performance, managing negative stress effects on cardiac autonomic control.

The tumor microenvironment (TME) includes immune (T, B, NK, dendritic), stromal, mesenchymal, endothelial, adipocytic cells, extracellular matrix, and cytokines/chemokines/soluble factors regulating various intracellular signaling pathways (ISP) in tumor cells. TME influences the survival/progression of prostate cancer (PC), enabling tumor cell immune-evasion also through the activation of the PD-1/PD-L1 axis. We have performed a systematic literature review according to the PRISMA guidelines, to investigate how the PD-1/PD-L1 pathway is influenced by TME and ISPs. Tumor immune-escape mechanisms include suppression/exhaustion of tumor infiltrating cytotoxic T lymphocytes, inhibition of tumor suppressive NK cells, increase in immune-suppressive immune cells (regulatory T, M2 macrophagic, myeloid-derived suppressor, dendritic, stromal, and adipocytic cells). IFN-γ (the most investigated factor), TGF-β, TNF-α, IL-6, IL-17, IL-15, IL-27, complement factor C5a, and other soluble molecules secreted by TME components (and sometimes increased in patients’ serum), as well as and hypoxia, influenced the regulation of PD-L1. Experimental studies using human and mouse PC cell lines (derived from either androgen-sensitive or androgen-resistant tumors) revealed that the intracellular ERK/MEK, Akt-mTOR, NF-kB, WNT and JAK/STAT pathways were involved in PD-L1 upregulation in PC. Blocking the PD-1/PD-L1 signaling by using immunotherapy drugs can prevent tumor immune-escape, increasing the anti-tumor activity of immune cells.

Simple Summary Vulvar squamous cell carcinoma (VSCC) is the most frequent malignant vulvar tumor, with a peak incidence in the 7–8th decades of life. However, VSCCs can also occur in young women. This unfortunate event is even rarer and more worrisome in pregnant women, being hard to manage for gynecologists, oncologists, and radiotherapists. Very few cases have been reported and we felt the need for an updated review on this topic. Thus, we performed a systematic literature review of VSCCs diagnosed during pregnancy, discussing the clinic-pathologic features, the implications in pregnancy outcomes, and the effects of such a diagnosis in the management of mothers and their babies. Abstract According to our systematic literature review (PRISMA guidelines), only 37 vulvar squamous cell carcinomas (VSCCs) were diagnosed during pregnancy (age range: 17–41 years). The tumor size range was 0.3–15 cm. The treatment was performed after (14/37, 38%), before (10/37, 27%), or before-and-after delivery (11/37, 30%). We found that 21/37 (57%) cases were stage I, 2 II (5%), 11 III (30%), and 3 IVB (8%). HPV-related features (condylomas/warts; HPV infection; high-grade squamous intraepithelial lesion) were reported in 11/37 (30%) cases. We also found that 9/37 (24%) patients had inflammatory conditions (lichen sclerosus/planus, psoriasis, chronic dermatitis). The time-to-recurrence/progression (12/37, 32%) ranged from 0 to 36 (mean 9) months. Eight women died of disease (22%) 2.5–48 months after diagnosis, 2 (5%) were alive with disease, and 23 (62%) were disease-free at the end of follow-up. Pregnant patients must be followed-up. Even if they are small, newly arising vulvar lesions should be biopsied, especially in women with risk factors (HPV, dermatosis, etc.). The treatment of VSCCs diagnosed in late third trimester might be delayed until postpartum. Elective cesarean section may prevent vulvar wound dehiscence. In the few reported cases, pregnancy/fetal outcomes seemed to not be affected by invasive treatments during pregnancy. However, clinicians must be careful; larger cohorts should define the best treatment. Definite guidelines are lacking, so a multidisciplinary approach and discussion with patients are mandatory.

Tracheocele is a herniation of the tracheal mucosa through a defect in the tracheal wall. Familiar and/or genetical factors have never been taken into consideration in the etiopathogenesis of acquired tracheocele. Acquired tracheocele occurred in a brother and a sister from India. CT scan of the neck showed an air cyst next to the right tracheal wall. They underwent surgical excision through a transverse cervical incision. Histopathology of the lesion revealed a fibrous sac lined by a pseudostratified columnar respiratory epithelium in both patients. They had no recurrence of tracheocele at 12months follow up. Genetic counseling did not evidence any significative associated abnormalities. No previous cases of familiar acquired tracheocele have been reported in the literature. Given the rarity of the lesion occurring in more than one member of a family, hereditary factors could be supposed as contributing factors in the etiopathogenesis of acquired tracheocele.

Questa indagine esplora aspettative, percezione dello sviluppo delle conoscenze, situazione lavorativa, sviluppo professionale e upgrading in un gruppo di studenti e di laureati magistrali in Scienze Riabilitative (LMSR) dell’Università di Milano (UNIMI). Sono stati reclutati tutti gli studenti iscritti dall’A.A. 08/09 al 16/17 (n=213) e intervistati con tre questionari creati ad hoc e somministrati all’immatricolazione (T0), alla laurea (T1) e dopo un anno (T2). Gli intervistati sono stati arbitrariamente suddivisi in junior (età < 40) e senior (≥40). A T0-T1 ha risposto il 58.68% della popolazione; a T2, il 68.8%. Abbiamo rilevato: abbassamento dell’età media e maggiore differenziazione dei profili professionali degli iscritti alla LMSR, diminuzione dell’aspettativa dell’upgrading, percezione di incremento delle conoscenze a T1, soprattutto pedagogiche. Nella popolazione generale, l’occupazione passa dal 58 (T0) al 93% (T1) e secondo gli intervistati la LMSR ha avuto un ruolo rilevante. Il 96% è soddisfatto del percorso di studio. A un anno dalla laurea (T2), il 43% migliora/stabilizza la condizione lavorativa. L’autonomia decisionale passa dal 33% (T0) a 55% (T2). Il 19% prosegue gli studi (50% Master; 25% Dottorato). Ulteriore ricerca, anche in altre LM, è necessaria per comprendere come trasformare/migliorare la formazione post-base alla luce di bisogni e carriere degli studenti.

Loneliness is one of the factors that affect quality of life and mental health of older people, especially when they live alone. However, literature seems to focus mainly on “measuring” loneliness in older people, and less on studying their perception of it. We designed a qualitative study on the role of technologies in supporting pre-frail elders, in order to continue living at home, even if alone. Within it, we explored the participants’ experience of loneliness and loss; data were gathered through 25 individual semi-structured interviews and analyzed with the Interpretative Phenomenological Approach. Findings showed that loneliness characterized the interviewees’ life and it was mostly associated with the loss of a beloved one. Although some participants perceived negatively loneliness and its consequences, others emphasized the importance of having space/time for themselves in their lives and loneliness was almost perceived as a conquest. Some interviewees seemed not to want to surrender to solitude, and they showed to have developed some strategies to face it, such as helping others and maintaining hobbies/interests. More details will be reported in our presentation, where we will reflect on the factors that seem to empower pre-frail older people when facing loneliness, with particular attention to digital literacy.

39. Progressive Impairment of Cardiac Autonomic Regulation as the Number of Metabolic Syndrome Components Increases.

- Abstract
Background: Metabolic syndrome (MetS) is associated with an increased risk of cardiovascular diseases. Compelling evidence supports the key role of dysfunction in the autonomic nervous system in that association, as well as mutual correlation among the components of MetS. The autonomic nervous system index (ANSI) is a percentile-ranked unitary proxy of cardiac autonomic regulation (CAR) that is designed to be free of age and sex bias, with higher values indicating better autonomic control. This study investigates CAR using the ANSI in patients with MetS., Methods: A total of 133 patients referred to the Exercise Medicine Clinic of Istituto Auxologico Istituto di Ricovero e Cura a Carattere Scientifico (IRCCS) underwent CAR assessment using the ANSI and answered lifestyle questions in ad hoc questionnaires. The participants were retrospectively subdivided into two groups according to the presence or absence of MetS criteria., Results: Of the subjects, 58 were diagnosed with MetS, and 75 were not (no MetS). The ANSI was significantly impaired (32.9 vs. 44.8, P <0.01) in the MetS group, and ANSI scores showed a decreasing trend ( P =0.004) as the number of MetS components increased. No significant lifestyle differences were found between the groups., Conclusion: The ANSI was significantly reduced in subjects with MetS, and, net of age and sex effects, CAR impairment became progressively more apparent as the number of MetS components increased.

40. Italian Speech-Language Pathologists and Telerehabilitation for Voice Disorders: A Survey on Satisfaction, Effectiveness, Limits, and Future Prospects.

Introduction: The aim of this study was to evaluate the distribution of virtual voice therapy during the coronavirus disease 2019 (COVID-19) lockdown in Italy via the collection of opinions of speech-language pathologists (SLPs). Methods: All SLPs who regularly carried out their professional activity in public hospitals, private hospitals, or private practices in Italy were asked to fill out an online survey consisting of two sections: (1) demographic information (age, gender, work setting, seniority, working time, and regular use of virtual voice therapy) and (2) opinions regarding telerehabilitation (motivation, personal satisfaction, effectiveness, and future needs and uses). Results: A total of 299 SLPs (mean age 39.1 ± 12.4 years) completed the survey. Overall, a regular use of virtual voice therapy was declared by 31.1% (93/299) of SLPs, with the highest prevalence for SLPs working in fully private facilities (46.7%; p < 0.001). Among all respondents, 25.4% had a highly positive opinion on the possible use of virtual voice therapy, even in nonemergency situations, and 55.8% planned to maintain this rehabilitation modality in the future. Discussion: Italian SLPs, regardless of age, had a positive impact with the new telerehabilitation practices. Investments in training and updating SLPs through specific courses would help to break down the strong barriers to telepractice acceptance, such as lack of familiarity with new technologies and lack of adequate preparation. Virtual voice therapy, which had never been experienced in such a way in Italy before the COVID-19 pandemic, promises to be a valuable future addition to the current traditional rehabilitation approaches.

Objectives: To evaluate the role of exercise in the management of fibromyalgia syndrome (FM) by addressing its complex pathogenesis involving central sensitisation, autonomic dysfunction, inflammation, and neurological irregularities, and examining how exercise impacts symptom exacerbation caused by external stressors and comorbid conditions., Methods: This review synthesises evidence from current literature focusing on the benefits of structured and personalised exercise programmes in FM management. It discusses the importance of specifying exercise type, intensity, frequency, duration, and progression tailored to individual patient needs and clinical objectives., Results: Regular physical activity effectively mitigates core aetiopathogenetic mechanisms of FM and improves associated conditions such as stress and obesity. It also provides benefits for preventing other chronic diseases, enhancing well-being, and promoting healthy ageing. Structured and personalised exercise programmes that start with a low-demand protocol and gradually increase exercise volume are most beneficial, by improving patient compliance and reducing the risk of adverse effects., Conclusions: Effective management of FM requires a patient-centred approach integrating both pharmacological and non-pharmacological treatments, with exercise playing a pivotal role. Personalised exercise prescriptions that consider FM patients' specific needs and limitations are crucial for optimising treatment outcomes and enhancing quality of life.

Aims: To assess body composition by means of BOD POD in the large cohort of Italian Olympic athletes of many sport disciplines (studied at the same time), and to provide possible reference values for body composition in elite athletes., Methods: 1556 elite athletes, who took part in the selection procedure for the 2016 Rio Olympic Games for the National Italian Olympic Committee (CONI), were retrospectively studied. Body composition was determined using air plethysmography-based BOD POD., Results: We observed that Fat Mass (FM) and Fat-free Mass (FFM) should be considered as two mutually independent domains in elite athletes. By performing Principal Component Analysis, we defined two independent main domains (respectively, representing FM and FFM), which presented different trends according to gender and static or dynamic exercise load. Background: Ovarian cancer is metastatic at presentation in about 62% of cases, but brain metastases are rare, reported in 3.3-4% of patients. Brain metastasis seems to be more frequent in advanced stages at diagnosis and in patients with BRCA1/2 mutation., Case Presentation: We present a case of a 47-year-old Caucasian woman, BRCA wild type, with an ovarian cancer that started with single cerebellar metastasis., Conclusion: Brain metastases in ovarian cancer are rare and complex for diagnosis and management. Objective: Teachers have an increased prevalence of voice disorders. The present study aimed to investigate the impact of remote teaching on perceived voice fatigue among Italian teachers of all grades during the lockdown due to the COVID-19 pandemic., Method: s The participants were 195 female teachers (M
age =48.77; SDage =9.61) in primary, secondary, and high schools. They completed the Vocal Fatigue Index (VFI) through an online survey. The tool includes 19 items grouped into three dimensions: tiredness of voice and avoidance of voice use (TA); physical discomfort (PD); and improvement of symptoms with rest (IS). The participants reported their perceived voice fatigue during remote teaching. They were also asked to provide data about voice fatigue as perceived in previous classroom teaching. The data were analyzed through two sets of independent one-way ANOVAs, with voice fatigue subscales as criterion variables and school grade as a between-subjects factor., Results: The teachers involved in the study reported higher voice fatigue scores than vocally healthy adults from the general population. Primary school teachers showed higher voice fatigue during remote teaching than both secondary and high school teachers, specifically for the TA and PD dimensions, whereas no difference emerged for IS. - Abstract
Introduction: Fusobacterium nucleatum is a gram-negative anaerobe, a constituent of the oral microflora, responsible for chronic periodontal diseases. Case Report: We describe a preterm infant with premature rupture of membranes at 23 weeks of gestational age due to F. nucleatum . The newborn died soon after birth. Placental histopathology showed severe necrotizing chorioamnionitis and funisitis with gram-negative bacilli. After autopsy, F. nucleatum was microbiologically isolated from the lung. The mother had dental hygiene 1 day before delivery, presenting mild and diffuse gingivitis. At admission, she had leukocytosis, foul-smelling vaginal discharge, but no fever. Conclusion: This case highlights the possibility of F. nucleatum spreading from oral cavity after a dental procedure to the placenta with chorioamnionitis and fetal infection. This raises the question of whether dental procedures during pregnancy should be accompanied by prophylactic antibiotics.

46. Moderate Aerobic Exercise Reduces the Detrimental Effects of Hypoxia on Cardiac Autonomic Control in Healthy Volunteers.

- Abstract
Physical inactivity increases cardiometabolic risk through a variety of mechanisms, among which alterations of immunological, metabolic, and autonomic control systems may play a pivotal role. Physical inactivity is frequently associated with other factors that may further worsen prognosis. The association between physical inactivity and hypoxia is particularly interesting and characterizes several conditions-whether physiological (e.g., residing or trekking at high altitude and space flights) or pathological (e.g., chronic cardiopulmonary diseases and COVID-19). In this randomized intervention study, we investigated the combined effects of physical inactivity and hypoxia on autonomic control in eleven healthy and physically active male volunteers, both at baseline (ambulatory) conditions and, in a randomized order, hypoxic ambulatory, hypoxic bedrest, and normoxic bedrest (i.e., a simple experimental model of physical inactivity). Autoregressive spectral analysis of cardiovascular variabilities was employed to assess cardiac autonomic control. Notably, we found hypoxia to be associated with an impairment of cardiac autonomic control, especially when combined with bedrest. In particular, we observed an impairment of indices of baroreflex control, a reduction in the marker of prevalent vagal control to the SA node, and an increase in the marker of sympathetic control to vasculature.

47. Metabolic Profile of Whole Unstimulated Saliva in Patients with Sjögren's Syndrome.

- Abstract
Primary Sjögren's Syndrome (pSS) is a multi-system autoimmune disease that involves the exocrine glands. Lymphocytes infiltrate the gland tissue, leading to anatomical modification and hypofunction. Even if the prognosis of pSS is favorable, quality of life is typically reduced due to the diverse manifestations of the disease. The aim of this study is to compare the salivary metabolomes of pSS with healthy controls (HCs). Seven cases were selected from a cohort of pSS patients, and six age- and sex-matched HCs were recruited from a cohort of volunteers. Whole unstimulated saliva was collected for NMR analysis. Our metabolomic analysis focused on 360 ms total echo 1D
1 H NMR CPMG spectra. Metabolites detected with CPMG NMR spectra were assigned through 2D NMR spectra (COSY, TOCSY, and HSQC). About 50 metabolites were detected and assigned. Unsupervised exploratory PCA returned partial clustering, and PLS-DA improved the separation between pSS and HCs, highlighting a pool of metabolites distinctly describing each group. Despite the limited number of samples, the presented preliminary data are promising. PLS-DA indicated well-defined group separation, suggesting that the application of1 H-NMR metabolomics is suitable for the study of pSS.- Published

Exercise is one of the major determinants of a healthy lifestyle, which is particularly important in childhood and serves as a powerful preventive tool. On the other hand, obesity and arterial hypertension rates are increasing in children, representing a huge risk for developing major cardiovascular and metabolic diseases in adult life. Of fundamental importance is the modality and volume of exercise required to obtain benefits. In this feasibility study, we considered a group of obese children, studied before and after a 12-week online exercise training program, and subdivided the participants into two groups considering the volume of exercise performed (above or below 1200 MET·min/week). This threshold level was applied in two different ways: subdivision A considered the total weekly physical activity volume (considering both time spent walking for at least 10 min consecutively and time spent performing structured exercise) and subdivision B considered only the weekly volume of structured exercise. We assessed autonomic and metabolic control and auxological and lifestyle parameters. We observed that the improved volume of structured exercise was associated with reduced arterial pressure percentile only in subdivision B and an improvement in markers of vagal and metabolic control was evident. Moreover, the 12-week online exercise training program, defined considering individual fitness level and progressively adapted as the goal was reached, proved to be sustainable from an economical and organizational point of view.

49. Benefits in Cardiac Function from a Remote Exercise Program in Children with Obesity.

- Abstract
Physical activity (PA) is a crucial factor in preventing and treating obesity and related complications. In this one-arm pre−post longitudinal prospective study, we evaluated the effects of a 12-week online supervised training program on cardiac morphology, function and blood pressure (BP) in children with obesity. The training program consisted of three sessions per week, each lasting 60 min. Advanced echocardiographic imaging (tissue Doppler and longitudinal strain analysis) was used to detect subclinical changes in heart function. Categorical variables were described as counts and percentages; quantitative variables as the mean and standard deviation (SD) as they were normally distributed (Shapiro−Wilks test). Pre−post comparisons were made with a paired t-test. A total of 27/38 (71%) enrolled patients (18M/9F, 11 ± 2 years) completed the training protocol and were considered in the analysis. At baseline, no hypertensive patient was noted; all echocardiographic variables were within the normal range. After training, we observed a significant reduction in BP parameters, including systolic BP values and Z-score, diastolic BP values, centiles and Z-score, and mean arterial pressure (all p < 0.05). Significant variations in echocardiographic interventricular septum (IVSd) thickness (p = 0.011), IVSd Z-score (p = 0.001), left ventricular (LV) end-diastolic diameter (p = 0.045), LV posterior wall thickness Z-score (p = 0.017), and LV global longitudinal strain (p = 0.016) were detected. No differences in LV diastolic function and right ventricular strain were noted. PA plays a decisive role in improving BP control and has benefits on left ventricle systolic function, representing a strategic approach to limit CV risk. Online exercise could be an excellent method of training in children with obesity.

50. Psychological Intervention Based on Mental Relaxation to Manage Stress in Female Junior Elite Soccer Team: Improvement in Cardiac Autonomic Control, Perception of Stress and Overall Health.

- Abstract
Chronic stress may represent one of the most important factors that negatively affects the health and performance of athletes. Finding a way to introduce psychological strategies to manage stress in everyday training routines is challenging, particularly in junior teams. We also must consider that a stress management intervention should be regarded as "efficacious" only if its application results in improvement of the complex underlying pathogenetic substratum, which considers mechanistically interrelated factors, such as immunological, endocrine and autonomic controls further to psychological functioning and behavior. In this study, we investigated the feasibility of implementing, in a standard training routine of the junior team of the Italian major soccer league, a stress management program based on mental relaxation training (MRT). We evaluated its effects on stress perception and cardiac autonomic regulation as assessed by means of ANSI, a single composite percentile-ranked proxy of autonomic balance, which is free of gender and age bias, economical, and simple to apply in a clinical setting. We observed that the simple employed MRT intervention was feasible in a female junior soccer team and was associated with a reduced perception of stress, an improved perception of overall health, and a betterment of cardiac autonomic control. This data may corroborate the scientific literature that indicates psychological intervention based on MRT as an efficacious strategy to improve performance, managing negative stress effects on cardiac autonomic control.
