Abstract

INTRODUCTION/AIMS: Prospective, randomized, controlled trials of intravenous immunoglobulin (IVIG) maintenance therapy in myasthenia gravis (MG) are lacking. In this trial, we evaluated the safety and efficacy of caprylate/chromatography-purified IVIG; (IGIV-C) in patients with generalized MG undergoing standard care. METHODS: Sixty-two patients enrolled in this phase 2, multicenter, international, randomized trial (1:1 IGIV-C [2 g/kg loading dose; 1 g/kg every 3 weeks through week 21] or placebo). Efficacy was assessed by changes in Quantitative MG (QMG) score at week 24 versus baseline (primary endpoint) and percentage of patients with clinical improvement in QMG, MG Composite (MGC), and MG-Activities of Daily Living (MG-ADL) scores (secondary endpoints). Safety assessments reported all adverse events (AEs). RESULTS: The change in QMG at 24 weeks was -5.1 for IGIV-C and -3.1 for placebo (p = .187). Seventy percent of patients in the IGIV-C group had improvement in MG-ADL (≥2-point decrease) versus 40.6% in the placebo group (p = .025). Patients showing clinical improvement in QMG and MGC (≥3-point decrease) were 70.0% for IGIV-C versus 59.4% for placebo (p = .442) and 60.0% for IGIV-C versus 53.1% for placebo (p = .610). IGIV-C was well tolerated; serious AEs were similar between arms. Three of four MG exacerbations requiring hospitalizations occurred in the IGIV-C arm with one death. DISCUSSION: Several efficacy parameters showed numerical results greater than those seen in the placebo group. This was a small study and may have been underpowered to see significant differences. Additional studies may be warranted to fully determine the efficacy of IVIG maintenance therapy in MG.

2. Supercritical density fluctuations and structural heterogeneity in supercooled water-glycerol microdroplets

Abstract

Recent experiments and theoretical studies strongly indicate that water exhibits a liquid-liquid phase transition (LLPT) in the supercooled domain. An open question is how the LLPT of water can affect the properties of aqueous solutions. Here, we study the structural and thermodynamic properties of supercooled glycerol-water microdroplets at dilute conditions ($\chi_g=3.2~\%$ glycerol mole fraction). The combination of rapid evaporative cooling with ultrafast X-ray scattering allows us to outrun crystallization and gain access to the deeply supercooled regime down to $T=229.3$ K. We find that the density fluctuations of the glycerol-water solution or, equivalently, its isothermal compressibility, $\kappa_T$, increases upon cooling. This is confirmed by molecular dynamics simulations, which indicate that the presence of glycerol shifts the temperature of maximum $\kappa_T$ from $T=230$ K in pure water down to $T=223$ K in the solution. Our findings elucidate the interplay between the complex behavior of water, including its LLPT, and the properties of aqueous solutions at low temperatures, which can have practical consequences in cryogenic biological applications and cryopreservation techniques.

Abstract

Understanding protein motion within the cell is crucial for predicting reaction rates and macromolecular transport in the cytoplasm. A key question is how crowded environments affect protein dynamics through hydrodynamic and direct interactions at molecular length scales. Using megahertz X-ray Photon Correlation Spectroscopy (MHz-XPCS) at the European X-ray Free Electron Laser (EuXFEL), we investigate ferritin diffusion at microsecond time scales. Our results reveal anomalous diffusion, indicated by the non-exponential decay of the intensity autocorrelation function $g_2(q,t)$ at high concentrations. This behavior is consistent with the presence of cage-trapping in between the short- and long-time protein diffusion regimes. Modeling with the $\delta\gamma$-theory of hydrodynamically interacting colloidal spheres successfully reproduces the experimental data by including a scaling factor linked to the protein direct interactions. These findings offer new insights into the complex molecular motion in crowded protein solutions, with potential applications for optimizing ferritin-based drug delivery, where protein diffusion is the rate-limiting step.

Abstract

Studying protein interactions at low temperatures has important implications for optimizing cryostorage processes of biological tissue, food, and protein-based drugs. One of the major challenges is related to the formation of ice nanocrystals which can occur even in the presence of cryoprotectants and can lead to protein denaturation. Here, using a combination of small- and wide-angle X-ray scattering (SAXS and WAXS), we investigate the structural evolution of concentrated Lysozyme solutions in a cryoprotected glycerol-water mixture upon cooling from room temperature (T=300 K) down to cryogenic temperatures (T=195 K). Upon cooling, we observe a transition near the melting temperature of the solution (T~245 K), which manifests both in the temperature dependence of the scattering intensity peak position reflecting protein-protein length scales (SAXS) and the interatomic distances within the solvent (WAXS). Upon thermal cycling, a hysteresis is observed in the scattering intensity, which is attributed to the formation of nanocrystallites in the order of 10 nm. The experimental data is well described by the two-Yukawa model, which indicates temperature-dependent changes in the short-range attraction of the protein-protein interaction potential. Our results demonstrate that the nanocrystal growth yields effectively stronger protein-protein attraction and influences the protein pair distribution function beyond the first coordination shell.

Abstract

Hydrated proteins undergo a transition in the deeply supercooled regime, which is attributed to rapid changes in hydration water and protein structural dynamics. Here, we investigate the nanoscale stress relaxation in hydrated lysozyme proteins stimulated and probed by X-ray Photon Correlation Spectroscopy (XPCS). This approach allows us to access the nanoscale dynamic response in the deeply supercooled regime (T = 180 K) which is typically not accessible through equilibrium methods. The relaxation time constants exhibit Arrhenius temperature dependence upon cooling with a minimum in the Kohlrausch-Williams-Watts exponent at T = 227 K. The observed minimum is attributed to an increase in dynamical heterogeneity, which coincides with enhanced fluctuations observed in the two-time correlation functions and a maximum in the dynamic susceptibility quantified by the normalised variance $\chi_T$. Our study provides new insights into X-ray stimulated stress relaxation and the underlying mechanisms behind spatio-temporal fluctuations in biological granular materials.

Abstract

X-ray free-electron lasers (XFELs) with megahertz repetition rate can provide novel insights into structural dynamics of biological macromolecule solutions. However, very high dose rates can lead to beam-induced dynamics and structural changes due to radiation damage. Here, we probe the dynamics of dense antibody protein (Ig-PEG) solutions using megahertz X-ray photon correlation spectroscopy (MHz-XPCS) at the European XFEL. By varying the total dose and dose rate, we identify a regime for measuring the motion of proteins in their first coordination shell, quantify XFEL-induced effects such as driven motion, and map out the extent of agglomeration dynamics. The results indicate that for average dose rates below $1.06\,\mathrm{kGy}\mathrm{\mu s}^{-1}$ in a time window up to $10\,\mathrm{\mu s}$, it is possible to capture the protein dynamics before the onset of beam induced aggregation. We refer to this approach as correlation before aggregation and demonstrate that MHz-XPCS bridges an important spatio-temporal gap in measurement techniques for biological samples., Comment: 22 pages, 6 figures

8. Innovative Method of Biomass Combustion in the Binary Fluidised Bed

Abstract

In this study, a binary fluidised bed made out of quartz sand and cenospheres for the biomass combustion process was created. Materials were fluidised with air to achieve a vertical density profile (from 0.5 g/cm³ to 1.1 g/cm³) resulting from grains segregation. The density profile was selected to ensure optimal control over the location of the combusted fuel particle. This involved positioning the process as close to the bottom sieve as possible. Fluidised bed combustion was carried out at temperatures of 600 °C, 700 °C, 820 °C and 870 °C using straw, willow and sawmill pellets as fuels. Qualitative and quantitative analysis of flue gases was performed using an FTIR spectrometer. Over 90 % carbon conversion from the biomass to carbon dioxide was achieved at 700 °C. At 820 °C and 870 °C, 100 % of biomass carbon left the reactor as CO2. The composition of organic compounds in the process products remained low, reaching a maximum of 3.0 % wt. at 600 °C. To gain further insights into the processes occurring in the immediate vicinity of biomass samples, a complementary TGA/FTIR analysis was conducted. This aimed to clarify the impact of the biomass particle decomposition stage in the fluidised bed combustion process. The proposed mechanism for biomass combustion in the binary fluidised bed contains the particle decomposition stage and the subsequent stage resulting from the coalescence of bubbles containing flammable components and bubbles containing oxidiser.

33. Only minor stem cell mobilization in head and neck irradiated patients treated with hyperbaric oxygen.

Abstract

Introduction: Hyperbaric oxygen, (HBO) is used to treat several conditions including late radiation tissue injury. Previous studies have suggested that HBO mobilizes bone marrow derived stem/progenitor cells (SPC) to the peripheral blood, however possible cumulative effects were highly variable. Methods: We have investigated a possible HBO-induced mobilization of SPCs by determining CD34+CD45dim cell numbers, as well as SPCs in general. The latter were characterized by high aldehyde dehydrogenase (ALDH) activity by use of the Aldefluor® assay. We included ten patients admitted for HBO treatment of radiation tissue injury. Six patients completed the 29-30 HBO treatment exposures. We also investigated possible HBO-induced effects on platelet activation as measured by flow cytometry and functional analyses. Results: We found a weak and insignificant tendency toward mobilization of CD34+CD45dim cells after a single HBO exposure versus before. Additionally, we found an additive effect of 15 HBO exposures on the increase in CD34+CD45dim cells relative to the pre-1st-HBO values. These changes were significantly more than zero but less than a doubling. We could not demonstrate a significant effect of HBO on the content of Aldefluor® positive SPCs in peripheral blood. There was no significant effect on platelet activation overall. However, in patients with increased expression of activation markers at baseline, we found a decrease after one exposure although this was not reflected in functional tests. Conclusion: We found a minor statistically significant mobilizing effect of HBO treatment on the bone marrow derived stem/progenitor cell content in peripheral blood after 15 treatments (n = 10 patients), but no effect after 30 treatments (n = 6 patients). However, because of the low number of patients we cannot confidentially prove or disprove the null hypothesis. The possibility that HBO treatment reduces the number of activated platelets could not be demonstrated nor excluded. [ABSTRACT FROM AUTHOR]

Abstract

A plethora of studies have suggested the involvement of various eicosanoids in heart failure. The aim of this study was to profile eicosanoid release from isolated murine heart at transition and end-stage phases of heart failure (HF) in Tgαq*44 mice as compared with age-matched wild-type FVB mice. Using an UPLC-MS/MS-based method, the concentration of selected eicosanoids was measured in cardiac effluents collected from isolated perfused mice hearts according to the Langendorff technique in Tgαq*44 and FVB mice (8- and 12-month-old) in basal conditions and in response to bolus injection of arachidonic acid (AA), a major substrate for eicosanoids. In basal conditions, only some eicosanoids were detected in the coronary effluents, with 6-keto-PGF1α, PGD2, 12-HETE detected at the highest concentration. In response to AA, a wide range of its metabolites was detected, including not only prostanoids and HETEs, but also EETs and DHETs. Cardiac production of 6-keto-PGF1α, PGD2, PGE2, PGF2α, TXB2 in basal conditions was unchanged at the transition phase of HF, whereas it was increased at the end-stage of disease in Tgαq*44 mice as compared with age-matched FVB mice. In response to AA, the synthesis of PGE2, 12-, 15-HETEs, 8,9-, 11,12-DHETs were also elevated at the end-stage phase of HF in Tgαq*44 mice as compared to healthy animals. AA-induced vasodilation effect was greater at the end-stage phase of HF in Tgαq*44 mice as compared with age-matched FVB mice, but it was not changed at the transition phase of the disease. In conclusion, eicosanoid profiling in isolated perfused heart pointed to PGI2, PGD2 and 12-HETE as the most abundant AA metabolites of the isolated murine heart. In Tgαq*44 mice, the end-stage phase of heart failure was accompanied by major activation of cyclooxygenase pathways (PGI2, TXA2, PGD2, PGE2, PGF2α), 8,9-, 11,12-EET/DHETs pathways and 12-, 15-HETEs pathways in the heart. Abstract

It is well known that exposure to air pollution can be linked to adverse health effects in the population. This has been demonstrated in various epidemiological studies. The associations determined between pollution exposure and health effects rely on the quality of the exposure assessment. Proper assessment of human exposure is therefore crucial for a correct determination of the association between the pollution load of the population and the negative health outcomes. Focus in this chapter is on assessment of the impact of traffic generated air pollution, which is the major source of human exposure in many countries including Denmark. Some of the methodologies for assessing exposure to traffic-induced air pollution are outlined and examples from Danish exposure studies are presented. To model air pollution concentrations in streets within the Danish exposure studies, a number of locally-developed models have been applied, including the regional scale model DEHM, the Urban background model UBM and the street pollution model OSPM. These models are briefly described here. However, one of the major difficulties in exposure modelling is to obtain proper input data for the calculations. In the first Danish exposure studies, these data were obtained manually or through use of questionnaires sent to local authorities. In recent years, a GIS-based tool, AirGIS, has been applied. AirGIS takes advantage of information from the unique Danish register databases that are available together with digital maps for building images and road network. 39. Oxidative decomposition of methanol in a vibroacoustic fluidized bed of Ag-coated cenosphere core-shell catalyst

Abstract

The paper presents results of a study of oxidative decomposition of methanol in a fluidized bed of silver catalyst. The process of methanol oxidation was carried out on Ag-coated cenospheres core-shell catalyst. The catalyst was obtained by precipitation of silver from methanolic solution of silver nitrate on cenospheres. Cenospheres are lightweight, inert, hollow spheres, which can be easily introduced into a fluidized bed. Application of the catalyst in a form of fluidized bed should ensure good temperature equalization which is very important at low temperatures due to the possibility of generation of formaldehyde. It turned out that local hot points occur in that kind of a catalyst (in the form of a fluidized bed but with very low density), thus use of additional acoustic wave is necessary to ensure good control over the temperature of the process. The products of the process of methanol oxidation were monitored on-line by Fourier transform infrared spectroscopy (FTIR). The catalyst has proven to be highly active in the oxidative decomposition of methanol. Full of methanol conversion in reaction of complete oxidation was achieved at 350°C.

Abstract

This study describes in brief the latest extensions of the Danish Geographic Information System (GIS)-based air pollution and human exposure modelling system (AirGIS), which has been developed in Denmark since 2001 and gives results of an evaluation with measured air pollution data. The system shows, in general, a good performance for both long-term averages (annual and monthly averages), short-term averages (hourly and daily) as well as when reproducing spatial variation in air pollution concentrations. Results from an evaluation of three dispersion models are presented: the currently operational OML model, a new, improved 'Research Version' of OML, and the US AERMOD model. The evaluation is based on the Prairie Grass data set. For these data the OML Research Version appears superior to the other two models. Further, the paper discusses problems and pitfalls of the Prairie Grass data set. The criteria for exclusion of data have tremendous impact on evaluation metrics. Abstract

The aim of this paper is to describe the use of a general methodology tailored to the evaluation of micro-scale meteorological models applied to flow and dispersion simulations in urban areas. This methodology, developed within COST 732, has been tested through a large modelling exercise involving many groups across Europe. The major test case used is the Mock Urban Setting Test (MUST) experiment representing an idealised urban area. It is emphasised that a full model evaluation is problem-dependent and requires several activities including a statistical validation that requires a careful choice of the metrics for the comparison with measurements. Abstract

The aim of this study was to compare and evaluate operational open-road Gaussian line-source models currently in use in Norway, Denmark and Finland. Four models, HIWAY2-AQ, OML-Highway, CAR-FMI and WORM, were applied to datasets from three measurement campaigns from each of the mentioned countries. The results were assessed through analysis with regard to normalisation, wind speed, wind direction, horizontal profiles and stability. Generally, the correlation between model estimates and observations decreased when normalising with emissions, due to the significant positive correlation between observed concentrations and emissions. Furthermore, we found a reduction of bias when normalising the Norwegian and Danish data, caused by overestimation of the dispersion at lower emission values. Due to OML-Highway's more advanced parameterisation of traffic-produced turbulence, this model performed best at higher emission values when the influence of traffic density and vehicle speed on traffic produced turbulence was higher. With regard to horizontal profiles, the relative bias for CAR-FMI increased as a function of distance from the road, indicating that the Lagrangian time scales are too short. [ABSTRACT FROM AUTHOR]

Abstract

Summary: This report presents an overview for pathologists of the development and potential applications of a novel Web enabled system allowing indexing and retrieval of pathology specimens across multiple institutions. The system was developed through the National Cancer Institute''s Shared Pathology Informatics Network program with the goal of creating a prototype system to find existing pathology specimens derived from routine surgical and autopsy procedures (“paraffin blocks”) that may be relevant to cancer research. To reach this goal, a number of challenges needed to be met. A central aspect was the development of an informatics system that supported Web-based searching while retaining local control of data. Additional aspects included the development of an eXtensible Markup Language schema, representation of tissue specimen annotation, methods for deidentifying pathology reports, tools for autocoding critical data from these reports using the Unified Medical Language System, and hierarchies of confidentiality and consent that met or exceeded federal requirements. The prototype system supported Web-based querying of millions of pathology reports from 6 participating institutions across the country in a matter of seconds to minutes and the ability of bona fide researchers to identify and potentially to request specific paraffin blocks from the participating institutions. With the addition of associated clinical and outcome information, this system could vastly expand the pool of annotated tissues available for cancer research as well as other diseases. Abstract

Environmental contextTrafficked streets are air pollution hot spots where people experience high exposure to hazardous pollutants. Although monitoring networks provide crucial information about measured pollutant levels, the measurements are resource demanding and thus can be performed at only few selected sites. Fast and easily applied street pollution models are therefore necessary tools to provide information about the loadings in streets without measurement activities. We evaluate the Operational Street Pollution Model, one of the most commonly applied models in air pollution management and research worldwide. AbstractTraffic emissions constitute a major source of health hazardous air pollution in urban areas. Models describing pollutant levels in urban streets are thus important tools in air pollution management as a supplement to measurements in routine monitoring programmes. A widely used model in this context is the fast and easy to apply Operational Street Pollution Model (OSPM). For almost 20 years, OSPM has been routinely used in many countries for studying traffic pollution, performing analyses of field campaign measurements, studying efficiency of pollution abatement strategies, carrying out exposure assessments and as reference in comparisons to other models. OSPM is generally considered as state-of-the-art in applied street pollution modelling. This paper outlines the most important findings in OSPM validation and application studies in literature. At the end of the paper, future research needs are outlined for traffic air pollution modelling in general but with outset in the research performed with OSPM.
