Abstract

Oxygenation monitoring of deep biological tissues with a bedside non-invasive method is still a crucial issue that is not resolved in routine clinics. It is particularly important in several major clinical applications with significant societal and financial implications such as the management of severe traumatic brain injury or stroke, the monitoring of hemodynamic parameters during or after birth or the post-operative monitoring after breast reconstruction surgery. However, the chances of recovery are directly proportional to the rapidity of diagnosis and subsequent remedial treatment. We propose an optical Time-Resolved probe, which monitor noninvasively the level of oxygenation in deep tissues. Our solution combines a measurement device, a stethoscope-type probe and an analysis software that provides biomarkers of tissue oxygenation. It is validated on a cohort of 15 pigs with the detection and identification of all arterial and venous events induced by the experiments’ protocol. We implement two analysis methods– one is real-time and aim at detecting and identifying the occlusion; it is based on a Time-Resolved windowing approach. The other post-processes the data and focuses on the quantification purpose; it is based on a tomographic approach.

Abstract

We compare two methods for the optical characterization of turbid media. The estimates of the absorption and reduced scattering coefficients ([[mu].sub.a] and [[mu]'.sub.s]) by a spatially resolved method and a time-resolved method are performed on tissue-like phantoms. Aqueous suspension of microspheres and Intralipid are used as scattering media with the addition of ink as an absorber. [[mu]'.sub.s] is first measured on weakly absorbing media. The robustness of these measurements is then tested with respect to a variation of [[mu].sub.a]. The spatially resolved method gave more accurate estimates for [[mu]'.sub.s] whereas the time-resolved method gave better results for [[mu].sub.a] estimates. Abstract

We report a method for deriving the absolute value of absorption coefficients at depth in bilayered media. The method was simplified from that of time-resolved diffuse optical tomography (TR-DOT) into one dimension to validate and set up the main parameters with the help of simulations, and to test it in an easy preclinical model. The method was applied to buried flaps as used in reconstructive surgery, and absolute chromophore concentrations in the flap and in the upper (skin and fat) layer were derived. The encouraging results obtained lay a foundation for developing more complex multidimensional models.

Abstract

Solutions of the time-dependent diffusion equation were developed to take into account the depth of the source and the detector inside a semi-infinite medium. These solutions permitted an evaluation of optical properties at different depths below the surface by fitting time-resolved data. Measurements were performed on liquid optical phantoms with optical fibers for delivering and collecting light. A time-correlated single-photon-counting chain was used for electronic detection. The determination of optical properties underlines the continuity between the surface model and the infinite model and shows the depth at which the derived solutions can be applied. 12. Photochemical patterning of biological molecules inside a glass capillary

Abstract

A simple way for photochemical patterning of biological molecules onto the inner wall of fused-silica capillary is described. The method is based on a modification of the inner capillary surface with photoactive benzophenone (BP) derivative. The IN irradiation at 365 nm of the capillary filled with a sample solution results in cross-linking of the solutes to the BP moiety via a stable covalent bond. As a proof of concept, oligonucleotides and proteins were arrayed inside the capillary using an inverted microscope as an irradiation device. We demonstrated that the capillary arrays produced in this way are functional and could be used in different bioassays including DNA hybridization, protein interaction studies, and immunoassays. Having a sensitivity comparable to the fluorophore-based assays in a planar format, the capillary array possesses several advantages including submicroliter sample volume and a short assay time. The capillary format should therefore be considered as a possible alternative to a planar format in a number of low-density array applications such as mutation detection and diagnostic immunoassays.

Abstract

International audience; Surgery is the only therapy that offers the possibility of cure for patients with hepatic metastatic diseases from colorectal or neuroendocrine carcinoma. Five-year survival rates after resection of all detectable liver metastases can reach 50%. In contrast, the role of hepatectomy in patients with liver metastases from noncolorectal and nonneuroendocrine carcinoma is not well defined. Scientific literature showed that in highly selected patients, resection of noncolorectal and nonneuroendocrine liver metastases can be done safely with survival similar to colorectal metastases. Therefore, preoperative and intraoperative staging are critical parameters in order to anticipate an optimized resectability and a favorable outcome.Indocyanine green (ICG) is a fluorescent agent approved in the clinic for liver function testing, as well as measurement of cardiac output and retinal angiography. It is a water-soluble, anionic, amphilic tricarbocyanine probe with a hydrodynamic diameter of 1.2 nm; the excitation and emission wavelengths in the presence of serum are 778 and 830 nm, respectively. Fluorescent navigation systems with ICG injection are used to detect sentinel lymph nodes. It has only recently shown real interest for optical-guided surgical oncology. Several studies report now that this technique can be useful for assisted intraoperative resection of otherwise undetectable liver cancers (such as hepatocellular and metastatic carcinomas) in patients who received and intravenous injection of ICG 4 to 7 days before surgery for a routine preoperative liver function test.We developed a new, miniaturized version of the Fluobeam (see www.fluoptics.com), which can be used for intra-operative surgery of cancer. We used this system and ICG in a clinical trial on 2 patients with liver metastasis, in order to demonstrate that ICG can efficiently label the rim of these tumors, when it is injected intravenously or infused ex-vivo on the surgical liver specimen. Furthermore, we discuss the quality of the ICG labeling, and the possibility to anticipate it by performing conventional diffusion-weighted magnetic resonance imaging, X-ray imaging and 18-FDG metabolic imaging of these patients before optical guided surgery.
