Abstract

Sudden cardiac death is a leading cause of death worldwide, whereby myocardial infarction is considered the most frequent underlying condition. Percutaneous coronary intervention (PCI) is an important component of post-resuscitation care, while uninterrupted high-quality chest compressions are key determinants in cardiopulmonary resuscitation (CPR). In our paper, we evaluate a case of a female patient who suffered aborted cardiac arrest due to myocardial infarction. The ambulance crew providing prehospital care for sudden cardiac arrest used a mechanical chest compression device during advanced CPR, which enabled them to deliver ongoing resuscitation during transfer to the PCI laboratory located 20 km away from the scene. Mechanical chest compressions were continued during the primary coronary intervention. The resuscitation, carried out for 2 h and 35 min, and the coronary intervention were successful, as evidenced by the return of spontaneous circulation and by the fact that, after a short rehabilitation, the patient was discharged home with a favorable neurological outcome. Our case can serve as an example for the effective and safe use of a mechanical compression device during primary coronary intervention.

Abstract

The inhibition of cancer-related carbonic anhydrase (CA) activity is a promising way to intensify anti-tumor responses. In vitro data suggest improved efficacy of cytotoxic drugs in combination with CA-inhibitors in several cancer types. Despite accumulating data on CA-expression, experimental or clinical studies towards B-cell lymphoma therapy are missing. We therefore decided to test the effect of the CA-inhibitor acetazolamide (AA) on the conventional CHOP treatment regimen using the A20/BalbC in vivo syngeneic mouse lymphoma model. Tumor growth characteristics, 18F-MISO-PET activity, histomorphology, cell proliferation, and T-cell immune infiltrate were determined following single or multiple dose combinations. All results point to a significant increase in the anti-tumor effect of CHOP+AA combinations compared with the untreated controls or with the single CHOP or AA treatments. CD3+ and CD8+ T-cell immune infiltrate increased 3&ndash, 4 times following CHOP+AA combination compared with the classical CHOP protocol. In conclusion, CA-inhibitor AA seems to act synergistically with the anti-tumor treatment CHOP in aggressive lymphoma. Further to a cytotoxic effect, AA and other more selective blockers potentially support tumor-associated immune responses through the modification of the microenvironment. Therefore, CA-inhibitors are promising candidates as adjuvants in support of specific immunotherapies in lymphoma and other malignancies.

31. Mapping changes of in vivo connectivity patterns in the human mediodorsal thalamus: correlations with higher cognitive and executive functions

Abstract

The mediodorsal thalamic nucleus is recognized as an association hub mediating interconnections with mainly the prefrontal cortex. Tracer studies in primates and in vivo diffusion tensor tractography findings in both humans and monkeys confirm its role in relaying networks that connect to the dorsolateral prefrontal, orbitofrontal, frontal medial and cingulate cortex. Our study was designed to use in vivo probabilistic tractography to describe the pathways emerging from or projecting to the mediodorsal nucleus; moreover, to use such information to automatically define subdivisions based on the divergence of remote structural connections. Diffusion tensor MR imaging data of 156 subjects were utilized to perform connectivity-based segmentation of the mediodorsal nucleus by employing a k-means clustering algorithm. Two domains were revealed (medial and lateral) that are separated from each other by a sagittally oriented plane. For each subject, general assessment of cognitive performance by means of the Wechsler Abbreviated Scale of Intelligence and measures of Delis-Kaplan Executive Function System (D-KEFS) test was utilized. Inter-subject variability in terms of connectivity-based cluster sizes was discovered and the relative sizes of the lateral mediodorsal domain correlated with the individuals' performance in the D-KEFS Sorting test (r = 0.232, p = 0.004). Our results show that the connectivity-based parcellation technique applied to the mediodorsal thalamic nucleus delivers a single subject level descriptor of connectional topography; furthermore, we revealed a possible weak interaction between executive performance and the size of the thalamic area from which pathways converge to the lateral prefrontal cortex

Abstract

After months of restrictive containment efforts to fight the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) epidemic, European countries are planning to reopen. To support the process, we conducted a cross-sectional survey among the Hungarian population to estimate the prevalence of infectious cases and prior SARS-CoV-2 exposure. A representative sample (n = 17,787) for the Hungarian population of 14 years or older living in private households (n = 8,283,810) was selected. The study was performed within 16 days after 50 days of restrictions, when the number of confirmed cases was stable low. Naso- and oropharyngeal smears and blood samples were collected for PCR and antibody testing. The testing was accompanied by a questionnaire about symptoms, comorbidities, and contacts. Design-based prevalence estimates were calculated. In total, 10,474 individuals (67.7% taken into account a sample frame error of 2315) of the selected sample participated in the survey. Of the tested individuals, 3 had positive PCR and 69 had positive serological test. Population estimate of the number of SARS-CoV-2 infection and seropositivity were 2421 and 56,439, respectively, thus active infection rate (2.9/10,000) and the prevalence of prior SARS-CoV-2 exposure (68/10,000) was low. Self-reported loss of smell or taste and body aches were significantly more frequent among those with SARS-CoV-2. In this representative, cross-sectional survey of the Hungarian population with a high participation rate, the overall active infection rate was low in sync with the prevalence of prior SARS-CoV-2 exposure. Abstract

Background: The aim of this study was to reveal potential sources of systematic motion artifacts in stroke functional magnetic resonance imaging (fMRI) focusing on those causing stimulus-correlated motion on the individual-level and separate the motion effect on the fMRI signal changing from the activation-induced alteration at population level.Methods: Eleven ischemic stroke patients were examined by fMRI. The fMRI paradigm was based on passive ankle movement on both the healthy and the paretic leg's side. Three individual-level motion correction strategies were compared and we introduced five measures to characterize each subjects' in-scanner relative head movement. After analyzing the correlation of motion parameters and the subjects' physiological scale scores, we selected a parameter to model the motion-related artifacts in the second-level analysis.Results: At first (individual) level analysis, the noise-component correction-based CompCor method provided the highest -log10(p) value of cluster-level occurrence probability at 12.4/13.6 for healthy and paretic side stimulus, respectively, with a maximal z-value of 15/16.3. Including the motion parameter at second (group) level resulted in lower cluster occurrence values at 10.9/5.55 while retaining the maximal z-value.Conclusions: We proposed a postprocessing pipeline for ischemic stroke fMRI data that combine the CompCor correction at first level with the modeling of motion effect at second-level analysis by a parameter obtained from fMRI data. Our solution is applicable for any fMRI-based stroke rehabilitation study since it does not require any MRI-compatible motion capture system and is based on commonly used methods.
