Background/Aims: Although gastric neuroendocrine tumors (NETs) are uncommon neoplasms, their prevalence is increasing. The clinical importance of the World Health Organization (WHO) classification of gastric NETs, compared with NETs in other organs, has been underestimated. This study aimed to systematically evaluate the clinical and pathologic characteristics of gastric NETs based on the 2019 WHO classification and to assess the survival outcomes of patients from a single-center with a long-term follow-up. Methods: The medical records of 427 patients with gastric NETs who underwent endoscopic or surgical resection between January 2000 and March 2020 were retrospectively reviewed. All specimens were reclassified according to the 2019 WHO classification. The clinicopathologic characteristics, treatment, and oncologic outcomes of 139 gastric NETs were analyzed. Results: The patients’ median age was 53.0 years (interquartile range [IQR], 46.0 to 63.0 years). The median follow-up period was 36.0 months (IQR, 15.0 to 63.0 months). Of the patients, 92, 44, and 3 had grades 1, 2, and 3 NETs, respectively. The mean tumor size significantly increased as the tumor grade increased (p=0.025). Patients with grades 2 and 3 gastric NETs more frequently had lymphovascular invasion (29.8% vs 10.9%, p=0.005) and deeper tissue invasion (8.5% vs 0%, p=0.012) than those with grade 1 tumors. The overall disease-specific survival rate was 100%. Two patients with grades 2-3 gastric NETs experienced extragastric recurrence. Conclusions: Although gastric NETs have an excellent prognosis, grade 2 or grade 3 gastric NETs are associated with a larger size, deeper invasion, and extragastric recurrence, which require active treatment.

Exoskeleton robots that provide physical assistance to workers at industrial sites have recently been advanced by utilizing various bio-signal measurement sensors and artificial intelligence. However, their commercialization speed is slow compared to the pace of technological development. One of the reasons for this is the motion mismatch that occurs between the control of the exoskeleton and the worker’s response. To solve this issue, the worker’s intended motion could be predicted, and proactive control of the exoskeleton robot could be implemented. An experiment was conducted with 35 subjects, with the data of 30 subjects used for unsupervised learning and the data of the remaining 5 subjects used for supervised learning. To predict the intended motion of the subjects, the data from IMU sensors were used to segment the motion elements through a k-means clustering algorithm, employing a motion segmentation technique. The lower extremity motion was understood as a sequence composed of motion elements. The potential for predicting motion intention and sequence was demonstrated by comparing the results from unsupervised learning with those of an MLP model that predicted motion sequences from new, unused data. Additionally, it was confirmed that proactive control of the exoskeleton robot using the motion segmentation technique was possible when the duration of the element motion constituting the lower extremity motion sequence exceeded 200ms. Therefore, this study conducted preliminary research to develop a system for predicting a worker’s intended motion and motion sequence for proactive control of an exoskeleton robot.

The recovery of biocompatible implantable biomaterials from biowaste is a significant method for protecting both human health and the environment concerns. This study aimed to evaluate the possibility of using hydroxyapatite (HAp) granules derived from planktonic exoskeleton bone in the ocean (OceanBone-HAp) as a bone graft substitute. The morphology and physiochemical properties of the OceanBone-HAp were evaluated using SEM, XRD and FT-IR. In addition, the absence of heavy metals and abundance of trace elements in OceanBone-HAp was confirmed using ICP-OES analysis. Moreover, in vitro results showed that OceanBone-HAp improved cell proliferation, adhesion, differentiation, and infiltration, compared to stoichiometric synthetic HAp granules (pure HAp). Additionally, OceanBone-HAp improved the expression of osteogenic related markers, by upregulation of ALP, collagen type-1αI(Col1αI), osteocalcin (OCN), and bone sialoprotein (BSP), which was confirmed using quantitative real-time polymerase chain reaction. Furthermore, in vivo bone regeneration was also evaluated using a rabbit calvarial defect model, and the results showed that the area of mature bone formation was significantly higher for OceanBone-HAp than pure HAp. These results demonstrated that OceanBone-HAp granules have good potential for use in bone healing as a bone graft material.

Abstract Dual-functional quantum-dots light emitting diodes (QLEDs) have been fabricated using solution processable vanadium oxide (V2O5) hole injection layer to control the carrier transport behavior. The device shows selectable functionalities of photo-detecting and light-emitting behaviors according to the different operating voltage conditions. The device emitted a bright green light at the wavelength of 536 nm, and with the maximum luminance of 31,668 cd/m2 in a forward bias of 8.6 V. Meanwhile, the device could operate as a photodetector in a reverse bias condition. The device was perfectly turned off in a reverse bias, while an increase of photocurrent was observed during the illumination of 520 nm wavelength light on the device. The interfacial electronic structure of the device prepared with different concentration V2O5 solution was measured in detail using x-ray and ultraviolet photoelectron spectroscopy. Both the highest occupied molecular orbital and the gap state levels were moved closer to the Fermi level, according to increase the concentration of V2O5 solution. The change of gap state position enables to fabricate a dual-functional QLEDs. Therefore, the device could operate both as a photodetector and as a light-emitting diode with different applied bias. The result suggests that QLEDs can be used as a photosensor and as a light-emitting diode for the future display industry.

In wireless sensor networks, tree-based routing can achieve a low control overhead and high responsiveness by eliminating the path search and avoiding the use of extensive broadcast messages. However, existing approaches face difficulty in finding an optimal parent node, owing to conflicting performance metrics such as reliability, latency, and energy efficiency. To strike a balance between these multiple objectives, in this paper, we revisit a classic problem of finding an optimal parent node in a tree topology. Our key idea is to find the best parent node by utilizing empirical data about the network obtained through Q-learning. Specifically, we define a state space, action set, and reward function using multiple cognitive metrics, and then find the best parent node through trial and error. Simulation results demonstrate that the proposed solution can achieve better performance regarding end-to-end delay, packet delivery ratio, and energy consumption compared with existing approaches.

Whitlockite (WH) is a calcium-phosphate-based Mg-containing ceramic with good mechanical properties, rapid resorption, and good osteogenicity. Recently, we successfully synthesized highly porous WH granules using a marine plankton exoskeleton (MP-WH). In the present study, we improved the osteoinductive activity of MP-WH granules by bone morphogenetic protein2 (BMP2) (MP-WH/BMP2). The surface morphology and composition of the fabricated MP-WH/BMP2 granules were characterized using scanning electron microscopy (SEM), X-ray diffraction, and Fourier transform infrared (FT-IR) spectroscopy. The biocompatibility and osteogenic effects were evaluated using human mesenchymal stem cells (hMSCs). BMP2 was absorbed on the surfaces of the MP-WH/BMP2 granules. Immobilized BMP2 was released at a moderate rate over 30 days. hMSCs seeded on MP-WH/BMP2 granules became biocompatible, with a better proliferation and adhesion for MP-WH/BMP2, compared with MP-WH. Bone-specific markers Runx2, type I collagen, osteocalcin, and osteopontin were significantly upregulated following BMP2 incorporation. Similar observations were made regarding the alkaline phosphatase activity. This study suggests that BMP2 incorporation improves the osteoinductive activity of marine-plankton-derived WH granules for bone tissue repair.

Summary: Background: Laparoscopic antireflux surgery is a standard surgical treatment method for gastroesophageal reflux disease (GERD) in Western countries. However, the procedure has not been actively carried out in Korea because of concerns regarding surgical complications. Here, we conducted this study to evaluate short-term surgical outcomes of laparoscopic Nissen fundoplication (LNF) performed at a single institution. Methods: From January 2010 to August 2016, totally 29 patients underwent LNF for GERD performed by three surgeons at Asan Medical Center, Korea. All patients participated in a telephonic survey including the presence of symptoms and the status of quality of life (QOL). Results: The median age at operation and BMI were 54 years and 23.8 kg/m2, respectively. Preoperatively, 27 patients (93.1%) underwent PPI therapy. All patients had typical or atypical GERD symptoms. One patient (3.4%) required reoperation because of total gastroesophageal junction obstruction. 24 patients (82.8%) reported complete or partial resolution of symptoms, whereas 21 (72.4%) reported partial or complete improvement in quality of life. In patients with reflux esophagitis, GERD symptoms were significantly relieved after LNF (P = 0.005). LNF responders tended to perceive an improvement in quality of life post operation (P

Powder-based 3D printing is an excellent technique for the fabrication of complex structural shapes. The outstanding bone remodeling capacity of calcium phosphate bioceramics is a desirable characteristic for such fabrication. Whitlockite (WH) is a calcium phosphate-based ceramic that contains Mg ions and possesses good mechanical properties, rapid resorbability, and promotes osteogenesis. The aim of this study was to fabricate 3D-printed scaffolds using marine plankton-derived WH (MP-WH) powder. The surface morphology and composition of the fabricated scaffolds were characterized by scanning electron microscopy and X-ray diffraction. The biocompatibility and osteogenic effects were evaluated using human mesenchymal stem cells. We successfully obtained a 3D porous scaffold using MP-WH. The MP-WH 3D scaffold showed improved compressive strength compared to the tricalcium phosphate (TCP) 3D scaffold. The in vitro results showed that compared with TCP 3D scaffolds, MP-WH 3D scaffolds were biocompatible and enhanced cell proliferation and adhesion. In addition, alkaline phosphatase activity and real-time polymerase chain reaction assays demonstrated that osteoblast differentiation was improved on the MP-WH scaffold. These results suggest that marine plankton-derived WH is useful for fabricating 3D-printed scaffolds for bone tissue engineering applications.

Background: Streptococcus mutans is one of the most important cariogenic bacteria associated with dental caries. Sabinene is a major component of several herbal essential oils. However, the anti-cariogenic effects of sabinene and the underlying mechanism remain to be elucidated. Objectives: We investigated the inhibitory effects of sabinene on the cariogenic activity and studied the underlying mechanism. Design: S. mutans were treated with various concentrations of sabinene and the inhibitory effects were evaluated based on the bacterial growth, acid production and biofilm formation. Real-time polymerase chain reaction (PCR) was performed for several virulence factors. Results: The growth and adherence of S. mutans were inhibited by sabinene. Consistent with the inhibitory effects on bacterial adhesion, gbpB level significantly decreased. Acid production and biofilm formation was also inhibited. In line with the inhibitory effects of sabinene on biofilm formation and pH tolerance, real-time PCR results showed the down regulation in the expression levels of gtfB, gtfC, gtfD, vicR, brpA, and relA. Moreover, high concentrations of sabinene exhibited bactericidal activity. Conclusion: Together our results suggest that sabinene serves as a useful component in the inhibition of the cariogenic activity of S. mutans, indicative of its possible applications in the development of oral healthcare products.

Many applications are able to obtain enriched information by employing a wireless multimedia sensor network (WMSN) in industrial environments, which consists of nodes that are capable of processing multimedia data. However, as many aspects of WMSNs still need to be refined, this remains a potential research area. An efficient application needs the ability to capture and store the latest information about an object or event, which requires real-time multimedia data to be delivered to the sink timely. Motivated to achieve this goal, we developed a new adaptive QoS routing protocol based on the (m,k)-firm model. The proposed model processes captured information by employing a multimedia stream in the (m,k)-firm format. In addition, the model includes a new adaptive real-time protocol and traffic handling scheme to transmit event information by selecting the next hop according to the flow status as well as the requirement of the (m,k)-firm model. Different from the previous approach, two level adjustment in routing protocol and traffic management are able to increase the number of successful packets within the deadline as well as path setup schemes along the previous route is able to reduce the packet loss until a new path is established. Our simulation results demonstrate that the proposed schemes are able to improve the stream dynamic success ratio and network lifetime compared to previous work by meeting the requirement of the (m,k)-firm model regardless of the amount of traffic.

To improve the packet delivery ratio in wireless sensor networks, many approaches such as multipath, opportunistic, and learning-based routing protocols have been proposed. However, the performance of the existing protocols are degraded under long-hop wireless sensor networks because the additional overhead is proportional to the number of hops. To deal with the overhead, we propose an opportunistic multipath routing that forecasts the required number of paths, as well as bifurcation based on opportunistic routing according to the reliability requirement. In the proposed scheme, an intermediate node is able to select a different node for each transmission and to handle path failure adaptively. Through a performance evaluation, we demonstrate that the proposed scheme achieves a higher packet delivery ratio and reduces the energy consumption by at least approximately 33% and up to approximately 65% compared with existing routing protocols, under the condition of an 80% link success ratio in the long-hop sensor network.

The present study investigated the effect of retinal eccentricity on visually induced motion sickness (VIMS) and postural control. Participants wore a head-mounted display masked for the central 10° (peripheral vision), the peripheral except for the central 10° (central vision), or unmasked (control) to watch a highly immersive 3D virtual reality (VR) ride along China’s Great Wall. The Simulator Sickness Questionnaire was administered to assess VIMS symptoms before and after the VR exposure. In addition, postural sway data were collected via sensors attached to each participant’s head, torso, and hip. Results demonstrated that peripheral vision triggered the most severe symptoms of motion sickness, whereas full vision most perturbed posture. The latter finding contradicts previous research findings demonstrating the peripheral advantage of postural control. Although the source of compromised postural control under peripheral stimulation is not clear, the provocative nature of visual stimulation depicting a roller-coaster ride along a rugged path likely contributed to the contradictory findings. In contrast, motion sickness symptoms were least severe, and posture was most stable, under central vision. These findings provide empirical support for the tactic assumed by VR engineers who reduce the size of the field of view to ameliorate the symptoms of motion sickness.

Before discovering meaningful knowledge from big data systems, it is first necessary to build a data-gathering infrastructure. Among many feasible data sources, wireless sensor networks (WSNs) are rich big data sources: a large amount of data is generated by various sensor nodes in large-scale networks. However, unlike typical wireless networks, WSNs have serious deficiencies in terms of data reliability and communication owing to the limited capabilities of the nodes. Moreover, a considerable amount of sensed data are of no interest, meaningless, and redundant when a large number of sensor nodes is densely deployed. Many studies address the existing problems and propose methods to overcome the limitations when constructing big data systems with WSN. However, a published paper that provides deep insight into this research area remains lacking. To address this gap in the literature, we present a comprehensive survey that investigates state-of-the-art research work on introducing WSN in big data systems. Potential applications and technical challenges of networks and infrastructure are presented and explained in accordance with the research areas and objectives. Finally, open issues are presented to discuss promising directions for further research.

In wireless body area networks, temperature-aware routing plays an important role in preventing damage of surrounding body tissues caused by the temperature rise of the nodes. However, existing temperature-aware routing protocols tend to choose the next hop according to the temperature metric without considering transmission delay and data loss caused by human posture. To address this problem, multiple research efforts exploit different metrics such as temperature, hop count and link quality. Because their approaches are fundamentally based on simple computation through weighted factor for each metric, it is rarely feasible to obtain reasonable weight value through experiments. To solve this problem, we propose an enhanced mobility and temperature-aware routing protocol based on the multi-criteria decision making method. The proposed protocol adopts the analytical hierarchy process and simple additive weighting method to assign suitable weight factors and choose the next hop while considering multiple routing criteria. Simulation results are presented to demonstrate that the proposed protocol can efficiently improve transmission delay and data loss better than existing protocols by preventing the temperature rise on the node.

The molecular mechanisms controlling the differentiation of bone marrow stromal stem cells into osteoblasts remain largely unknown. In this study, we investigated whether bone marrow stromal antigen 2 (BST2) influences differentiation toward the osteoblasts lineage. BST2 mRNA expression in human alveolar-derived bone marrow stromal cells (hAD-BMSCs) increased during differentiation into osteoblasts. hAD-BMSCs differentiation into osteoblasts and the mRNA expression of the bone-specific markers alkaline phosphatase, collagen type α 1, bone sialoprotein, osteocalcin, and osterix were reduced by BST2 knockdown using siRNA. Furthermore, BST2 knockdown in hAD-BMSCs resulted in decreased RUNX2 mRNA and protein expression. We hypothesized that BST2 is involved in differentiation of into osteoblasts via the BMP2 signaling pathway. Accordingly, we evaluated the mRNA expression levels of BMP2, BMP receptors (BMPR1 and 2), and the downstream signaling molecules SMAD1, SMAD4, and p-SMAD1/5/8 in BST2 knockdown cells. BMP2 expression following the induction of differentiation was significantly lower in BST2 knockdown cells than in cells treated with a non-targeting control siRNA. Similar results were found for the knockdown of the BMP2 receptor- BMPR1A. We also identified significantly lower expression of SMAD1, SMAD4, and p-SMAD1/5/8 in the BST2 knockdown cells than control cells. Our data provide the first evidence that BST2 is involved in the osteogenic differentiation of bone marrow stromal cells via the regulation of the BMP2 signaling pathway.

Recently, various unicast routing protocols have been proposed to deliver measured data from the sensor node to the sink node within the predetermined deadline in wireless sensor networks. In parallel with their approaches, some applications demand the specific service, which is based on broadcast to all nodes within the deadline, the feasible real-time traffic model and improvements in energy efficiency. However, current protocols based on either flooding or one-to-one unicast cannot meet the above requirements entirely. Moreover, as far as the authors know, there is no study for the real-time broadcast protocol to support the application-specific traffic model in WSN yet. Based on the above analysis, in this paper, we propose a new (m, k)-firm-based Real-time Broadcast Protocol (FRBP) by constructing a broadcast tree to satisfy the (m, k)-firm, which is applicable to the real-time model in resource-constrained WSNs. The broadcast tree in FRBP is constructed by the distance-based priority scheme, whereas energy efficiency is improved by selecting as few as nodes on a tree possible. To overcome the unstable network environment, the recovery scheme invokes rapid partial tree reconstruction in order to designate another node as the parent on a tree according to the measured (m, k)-firm real-time condition and local states monitoring. Finally, simulation results are given to demonstrate the superiority of FRBP compared to the existing schemes in terms of average deadline missing ratio, average throughput and energy consumption.

Wireless Body Area Networks (WBANs) have attracted research interests from the community, as more promising healthcare applications have a tendency to employ them as underlying network technology. While taking design issues, such as small size hardware as well as low power computing, into account, a lot of research has been proposed to accomplish the given tasks in WBAN. However, since most of the existing works are basically developed by assuming all nodes in the static state, these schemes therefore cannot be applied in real scenarios where network topology between sensor nodes changes frequently and unexpectedly according to human moving behavior. However, as far as the authors know, there is no survey paper to focus on research challenges for mobility support in WBAN yet. To address this deficiency, in this paper, we present the state-of-the-art approaches and discuss the important features of related to mobility in WBAN. We give an overview of mobility model and categorize the models as individual and group. Furthermore, an overview of networking techniques in the recent literature and summary are compiled for comparison in several aspects. The article also suggests potential directions for future research in the field.

Excessive nitric oxide (NO) production is toxic to the cochlea and induces hearing loss. However, the mechanism through which NO induces ototoxicity has not been completely understood. The aim of this study was to gain further insight into the mechanism mediating NO-induced toxicity in auditory HEI-OC1 cells and in ex vivo analysis. We also elucidated whether and how epigallocatechin-3-gallate (EGCG), the main component of green tea polyphenols, regulates NO-induced auditory cell damage. To investigate NO-mediated ototoxicity, S-nitroso-N-acetylpenicillamine (SNAP) was used as an NO donor. SNAP was cytotoxic, generating reactive oxygen species, releasing cytochrome c, and activating caspase-3 in auditory cells. NO-induced ototoxicity also mediated the nuclear factor (NF)-κB/caspase-1 pathway. Furthermore, SNAP destroyed the orderly arrangement of the 3 outer rows of hair cells in the basal, middle, and apical turns of the organ of Corti from the cochlea of Sprague-Dawley rats at postnatal day 2. However, EGCG counteracted this ototoxicity by suppressing the activation of caspase-3/NF-κB and preventing the destruction of hair cell arrays in the organ of Corti. These findings may lead to the development of a model for pharmacological mechanism of EGCG and potential therapies against ototoxicity.

Purpose: Textbook outcome is a comprehensive measure used to assess surgical quality and is increasingly being recognized as a valuable evaluation tool. Delta-shaped anastomosis (DA), an intracorporeal gastroduodenostomy, is a viable option for minimally invasive distal gastrectomy in patients with gastric cancer. This study aims to evaluate the surgical outcomes and calculate the textbook outcome of DA. Materials and Methods: In this retrospective study, the records of 4,902 patients who underwent minimally invasive distal gastrectomy for DA between 2009 and 2020 were reviewed. The data were categorized into three phases to analyze the trends over time. Surgical outcomes, including the operation time, length of post-operative hospital stay, and complication rates, were assessed, and the textbook outcome was calculated. Results: Among 4,505 patients, the textbook outcome is achieved in 3,736 (82.9%). Post-operative complications affect the textbook outcome the most significantly (91.9%). The highest textbook outcome is achieved in phase 2 (85.0%), which surpasses the rates of in phase 1 (81.7%) and phase 3 (82.3%). The post-operative complication rate within 30 d after surgery is 8.7%, and the rate of major complications exceeding the Clavien–Dindo classification grade 3 is 2.4%. Conclusions: Based on the outcomes of a large dataset, DA can be considered safe and feasible for gastric cancer. [ABSTRACT FROM AUTHOR]

Periodontitis, which results from infection and inflammation, is one of the most common inflammatory diseases. Some studies have shown that plant extracts can inhibit inflammation in periodontitis. However, the anti-inflammatory effects of black rice extract (BRE) have not yet been investigated. Here, we investigated the anti-inflammatory activity of BRE in vitro on Aggregatibacter actinomycetemcomitans (A. a) stimulated RAW 264.7 macrophages. The results showed that BRE did not affect cell viability at concentrations ranging from 0.015 to 0.5 mg/mL, but toxicity was observed >1 mg/mL. A. a-induced nitric oxide (NO) production and iNOS protein expression were inhibited by treatment with BRE. In addition, BRE inhibited the gene expression of pro-inflammatory cytokines, such as IL-6, IL-1β and TNF-α, and blocked NF-κB p65 translocation as well as IκBα and IKK α/β phosphorylation. Moreover, the DPPH and ABTS radical scavenging activity results showed that BRE exhibits strong antioxidant activity in a dose-dependent manner. This study found that BRE exerts anti-inflammatory activity via inhibition of NF-κB signaling and antioxidant activity. Background: To determine how patients who underwent bariatric surgery at a tertiary hospital in Korea first considered and then decided to get the surgery and identify information gaps among patients and healthcare professionals. Methods: This study included 21 patients who underwent bariatric surgery to treat morbid obesity (body mass index [BMI] ≥35 or ≥30 kg/m2 together with obesity-related comorbidities) between August 2020 and February 2022. A telephone interview was conducted with the patients after at least 6 months had elapsed since the surgery. We asked how the patients decided to undergo bariatric surgery. We also inquired about their satisfaction with and concerns about the surgery. Results: Seventy-one percent of the patients were introduced to bariatric surgery following a recommendation from healthcare professionals, acquaintances, or social media. Most of the patients (52%) decided to undergo bariatric surgery based on recommendations from healthcare professionals in non-surgical departments. Satisfaction with the information provided differed among the patients. Post-surgical concerns were related to postoperative symptoms, weight regain, and psychological illness. Conclusion: Efforts are needed to raise awareness about bariatric surgery among healthcare professionals and the public. Tailored pre- and postoperative consultation may improve quality of life after bariatric surgery. [ABSTRACT FROM AUTHOR]

Purpose: Despite scientific evidence regarding laparoscopic gastrectomy (LG) for advanced gastric cancer treatment, its application in patients receiving neoadjuvant chemotherapy remains uncertain. Materials and Methods: We used the 2019 Korean Gastric Cancer Association nationwide survey database to extract data from 489 patients with primary gastric cancer who received neoadjuvant chemotherapy. After propensity score matching analysis, we compared the surgical outcomes of 97 patients who underwent LG and 97 patients who underwent open gastrectomy (OG). We investigated the risk factors for postoperative complications using multivariate analysis. Results: The operative time was significantly shorter in the OG group. Patients in the LG group had significantly less blood loss than those in the OG group. Hospital stay and overall postoperative complications were similar between the two groups. The incidence of Clavien– Dindo grade ≥3 complications in the LG group was comparable with that in the OG group (1.03% vs. 4.12%, P=0.215). No statistically significant difference was observed in the number of harvested lymph nodes between the two groups (38.60 vs. 35.79, P=0.182). Multivariate analysis identified body mass index (odds ratio [OR], 1.824; 95% confidence interval [CI], 1.029–3.234; P=0.040) and extent of resection (OR, 3.154; 95% CI, 1.084–9.174; P=0.035) as independent risk factors for overall postoperative complications.
