2. Predictive models for starting antiseizure medication withdrawal following epilepsy surgery in adults

Abstract

More than half of adults with epilepsy undergoing resective epilepsy surgery achieve long-term seizure freedom and might consider withdrawing antiseizure medications. We aimed to identify predictors of seizure recurrence after starting postoperative antiseizure medication withdrawal and develop and validate predictive models. We performed an international multicentre observational cohort study in nine tertiary epilepsy referral centres. We included 850 adults who started antiseizure medication withdrawal following resective epilepsy surgery and were free of seizures other than focal non-motor aware seizures before starting antiseizure medication withdrawal. We developed a model predicting recurrent seizures, other than focal non-motor aware seizures, using Cox proportional hazards regression in a derivation cohort (n = 231). Independent predictors of seizure recurrence, other than focal non-motor aware seizures, following the start of antiseizure medication withdrawal were focal non-motor aware seizures after surgery and before withdrawal [adjusted hazard ratio (aHR) 5.5, 95% confidence interval (CI) 2.7-11.1], history of focal to bilateral tonic-clonic seizures before surgery (aHR 1.6, 95% CI 0.9-2.8), time from surgery to the start of antiseizure medication withdrawal (aHR 0.9, 95% CI 0.8-0.9) and number of antiseizure medications at time of surgery (aHR 1.2, 95% CI 0.9-1.6). Model discrimination showed a concordance statistic of 0.67 (95% CI 0.63-0.71) in the external validation cohorts (n = 500). A secondary model predicting recurrence of any seizures (including focal non-motor aware seizures) was developed and validated in a subgroup that did not have focal non-motor aware seizures before withdrawal (n = 639), showing a concordance statistic of 0.68 (95% CI 0.64-0.72). Abstract

Environmental issues have become increasingly important to industry and business in recent days. This trend forces the companies to take responsibility for product recovery, and proper recycling and disposal, moving towards the design of sustainable green supply chains. This paper addresses the backward stream in transportation of products, by means of reverse logistics applied to vehicle routing. This problem, called single vehicle routing problem with deliveries and selective pickups, consists in finding a route that starts from the depot and visits all delivery customers. Some pickup customers may also be visited, since the capacity of the truck is not exceeded, and there is also a revenue associated with each pickup. We develop an algorithm inspired on the variable neighbourhood search metaheuristic that explores the power of modern graphics processing unit (GPU) to provide routes in reasonable computational time. The proposed algorithm called four-neighbourhood variable neighbourhood search (FN-VNS) includes a novel high-quality initial solution generator, a CPU–GPU integrated perturbation strategy and four different neighbourhood searches implemented purely in GPU for the local search phase. Our experimental results show that FN-VNS is able to improve the quality of the solution for 51 instances out of 68 instances taken from the literature. Finally, we obtained speedups up to 14.49 times, varying from 17.42 up to 76.84 for each local search, measured over a set of new large-size instances. [ABSTRACT FROM PUBLISHER]

Abstract

Rapid eye movement (REM) sleep usually occurs after 90 minutes of consolidated sleep. Sleep onset REM Period (SOREMP) occurs when REM sleep latency is less than 15 minutes. It is a rare phenomenon, mostly associated with sleep deprivation and narcolepsy. Patients undergoing routine Electroencephalogram (EEG) studies for other reasons than sleep disorders can occasionally have SOREMPs. The objective of this study was to determine the frequency of patients with SOREMPS in routine EEG and to evaluate its possible etiologies. This is a retrospective study of all EEGs performed in the EEG/sleep laboratory from Hospital de Santa Maria-Lisbon, during last 6 years. EEGs with REM sleep were selected. Both ambulatory and EEGs performed in the acute setting were included along with Data for cranial CT-scan or MRI. In 6 years, only 8 patients had SOREMPs, among them, it was possible to obtain cranial MRI or CT-scan in 7 patients (one died). One patient with a psychiatric disorder, showed no lesions on MRI. The other 6 had lesions involving brainstem (n=5) or diencephalon (n=1). The etiologies were inflammatory (2), vascular (3) and infectious (1). All lesions except one were acute. These results suggest that when SOREMPs are detected, lesions involving REM sleep structures should be searched for.

Abstract

Copyright © 2020 International League Against Epilepsy, Our goal was to assess the interrater agreement (IRA) of photoparoxysmal response (PPR) using the classification proposed by a task force of the International League Against Epilepsy (ILAE), and a simplified classification system proposed by our group. In addition, we evaluated IRA of epileptiform discharges (EDs) and the diagnostic significance of the electroencephalographic (EEG) abnormalities. We used EEG recordings from the European Reference Network (EpiCARE) and Standardized Computer-based Organized Reporting of EEG (SCORE). Six raters independently scored EEG recordings from 30 patients. We calculated the agreement coefficient (AC) for each feature. IRA of PPR using the classification proposed by the ILAE task force was only fair (AC = 0.38). This improved to a moderate agreement by using the simplified classification (AC = 0.56; P = .004). IRA of EDs was almost perfect (AC = 0.98), and IRA of scoring the diagnostic significance was moderate (AC = 0.51). Our results suggest that the simplified classification of the PPR is suitable for implementation in clinical practice.
