21. Lymphoplasmacytoid lymphoma elaborating lambda chain paraprotein with amyloid deposition in Sjögren's syndrome.

Abstract

A woman presented with painful enlargement of her parotid and submandibular glands. She was shown to have the previously unreported combination of idiopathic thrombocytopenic purpura, Sjögren's syndrome, Hashimoto's disease, and myasthenia gravis. Parotid gland biopsy and bone marrow examination showed the presence of a rare lymphoplasmacytoid lymphoma. There was amyloid deposition in the parotid glands, gums and on muscle biopsy. Immunohistochemical staining of the parotid lymphoma and amyloid was monotypic for lambda light chains, and there was also a lambda chain paraprotein. It is felt that the lymphoplasmacytic lymphoma was responsible for the light chain amyloidosis. [ABSTRACT FROM PUBLISHER]

Abstract

A peripheral blood eosinophilia was found at presentation in 193 of 1260 (15%) patients with Hodgkin's disease who had been entered into clinical studies by the British National Lymphoma Investigation (BNLI). Eosinophilia as a component of a general leucocytosis conferred no survival advantage. Eosinophilia without a general leucocytosis was present in 95 patients, and this selective eosinophilia was associated with a clear survival advantage. The association of selective eosinophilia and improved survival was limited to patients with mixed cellularity and grade I nodular sclerosis histology. Selective eosinophilia was found to be a good prognostic indicator both in local and generalised disease. Abstract

, , 788-793. A man of 37 had an iodized talc pleurodesis carried out for recurrent spontaneous pneumothorax. There was no history of industrial exposure to asbestos. Two years later he presented with pain in the right chest and radiographs at that time showed some localized pleural thickening at the site of the thoracoscopy cannulation for introduction of talc. A provisional diagnosis of talc granuloma, chemical abscess or tumour was made and exploratory thoracotomy revealed a tumour involving the chest wall, lung, and pleura which, on histological examination, showed adenocarcinoma of varying degrees of differentiation and in some parts also presenting a more squamoid appearance. Numerous doubly refractile talc particles were intimately associated with the tumour and fibrous tissue. Shortly after excision the patient developed evidence of systemic dissemination of the disease and died four months later. The possibility of this tumour being induced by the talc is discussed. A brief review is made of the various forms of talc used in surgery over the past 40 years and attention is drawn to the significance of the proportion of asbestos mineral which is present in talc as mined in various parts of the world. 32. Metabolite profiling of sesquiterpene lactones from Lactuca species. Major latex components are novel oxalate and sulfate conjugates of lactucin and its derivatives.

Abstract

Wounding leaves or stems of Lactuca species releases a milky latex onto the plant surface. We have examined the constituents of latex from Lactuca sativa (lettuce) cv. Diana. The major components were shown to be novel 15-oxalyl and 8-sulfate conjugates of the guaianolide sesquiterpene lactones, lactucin, deoxylactucin, and lactucopicrin. The oxalates were unstable, reverting to the parent sesquiterpene lactone on hydrolysis. Oxalyl derivatives have been reported rarely from natural sources. The sulfates were stable and are the first reported sesquiterpene sulfates from plants. Unusual tannins based on 4-hydroxyphenylacetyl conjugates of glucose were also identified. Significant qualitative and quantitative variation was found in sesquiterpene lactone profiles in different lettuce varieties and in other Lactuca spp. The proportions of each conjugate in latex also changed depending on the stage of plant development. A similar profile was found in chicory, in which oxalyl conjugates were identified, but the 8-sulfate conjugates were notably absent. The presence of the constitutive sesquiterpene lactones was not correlated with resistance to pathogens but may have a significant bearing on the molecular basis of the bitter taste of lettuce and related species. The induced sesquiterpene lactone phytoalexin, lettucenin A, was found in the Lactuca spp. but not in chicory.

33. The loss of ferromagnetism in Gd(X)Al2, where X is U, Th or La

Abstract

The results of magnetization, susceptibility and a.c. susceptibility measurements on alloys of GdAl2, in which Gd has been replaced by U, Th or La, are presented. The loss of ferromagnetism in the first two systems is very rapid and is followed by various types of spin-glass behaviour, but they differ in the maintenance of high paramagnetic Curie temperatures in the U system, the sustaining of which is associated with the spin-ftuctuation character of UAl2. The magnetic phase diagram of the LaAl2-GdAl2 system, which is ferromagnetic above about 14% GdAl2, shows a general similarity to that of the f.c.c. Au-Fe system including a re-entrant spin-glass regime.

34. Cell proliferation in human tumours measured by in-vivolabelling with bromodeoxyuridine

Abstract

The causes of failure after radiotherapy or chemotherapy may be related to physical or biological factors. The physical factors will include geographic miss in radiotherapy. The biological reasons may be inadequate drug delivery in chemotherapy or resistance of nutritionally deprived or hypoxic cells, a capacity to repair damage or rapid proliferation of the tumour cells so that repopulation may occur during treatment. There is much current interest in repopulation and it would be of value to identify those tumours which have rapid cell proliferation for, in these cases, a change in drug scheduling or shortening the overall treatment time with radiotherapy may lead to increased success by overcoming the effect of repopulation (Fowler, 1985).We have previously described a method for labelling proliferating cells in human tumours by measuring the uptake of bromodeoxyuridine (BrdUrd) into cells synthesizing DNA, following an intravenous injection, and the potential applications of this procedure to the measurement of proliferation rates in human tumours (Begg et al, 1985; Wilson et al, 1985). In this preliminary communication we report on three human tumours where kinetic studies have been performed and demonstrate the potential advantages of the technique.

35. A comparison of the tumour concentrations obtainable with misonidazole and Ro 03–8799

Abstract

Both Ro 03–8799 and misonidazole were administered to six patients before tumour sampling. Using similar doses, tumour concentrations of Ro 03–8799 were approximately double those of misonidazole. Examination of the data with regard to the relative sensitising efficiencies of these two compounds gives us, as a conservative estimate, a factor of 1.6 in favour of Ro 03–8799. From these observations it is predicted that, when given with radiotherapy in a 25-fraction course Ro 03–8799, is likely to be at least five times more effective as a hypoxic cell sensitiser than misonidazole.

36. Tumour regression as a guide to prognosis: a clinical study

Abstract

The response seen at the end of a course of radiotherapy in 116 patients with advanced carcinoma of the cervix included in hyperbaric oxygen trials was assessed by reduction in bulk and by histological examination. These patients have now been carefully followed for a minimum of three and a maximum of 13 years. The degree of regression, as measured by reduction in bulk at the primary site, was found to predict closely the subsequent local tumour failure, survival and freedom from metastasis. The histological assessment also could be associated with the subsequent course, but the correlation was much weaker. In a trial of hyperbaric oxygen, where radiotherapy was given in 27 fractions to both groups, the immediate response was significantly better in the patients treated in oxygen, so predicting the result of the trial seen in long-term follow up. In the radiotherapy of carcinoma of cervix, reduction in bulk of the primary tumour at the end of treatment is a valuable prognostic index.

37. COMPENSATORY OVULATION IN BLINDED RATS AND/OR THOSE FROM WHICH THE OLFACTORY BULB HAD BEEN REMOVED

Abstract

Thirty-two prepubertal, female rats (Southern Farms) were allocated to intact control, bilateral optic enucleation (blinded), bilateral olfactory bulb removal (anosmic) and blinded—anosmic groups. Olfactory bulbs were surgically removed between 27 and 29 days of age and eyes were removed at 30 days of age. One ovary was removed from each animal between 107 and 112 days of age on Day 2 (metoestrus) of the oestrous cycle. The number of eggs ovulated was determined by flushing the oviducts with normal saline solution. All rats completed one oestrous cycle and were killed at metoestrus of the following cycle. There was no difference in the number of ova shed between the four groups at the time of removal of the first ovary. One cycle later, compensatory ovulation was found at autopsy to have occurred in all animals. Controls ovulated 10·3±0·5 eggs; blinded, 9·6±0·7; anosmic, 10·3±0·3; and blinded—anosmic, 9·7±0·8. Follicular development was quantitatively analysed in both intact and hemispayed blinded and/or anosmic rats. These data suggest that pituitary—ovarian function as evaluated by the number of eggs ovulated is not affected by blinding and/or anosmia.

Abstract

Between 1954 and 1980, 9 patients with non-Hodgkin's lymphoma of the orbit were treated at the Regional Radiotherapy Centre, Mount Vernon Hospital. Following radiotherapy complete local regression of the tumour was seen in every case. Three patients subsequently developed recurrent disease which proved fatal in two cases. Abstract

The article presents a review update of randomized controlled trials focused on the use of hyperbaric oxygen therapy (HBOT) for treating chronic foot ulcers of the lower limb in people with diabetes. The review authors used Conchrane methodology to evaluate the risk of bias of the relevant trials. Results revealed increased in the rate of ulcer healing and confidence interval in a number of patients treated with HBOT. Findings indicated no significant difference in major amputation rate.

50. Cardiovascular and Electroencephalographic Effects of Laudanosine in "Nephrectomized" Cats

Abstract

We studied a cat model simulating laudanosine accumulation in the “anephric” patient. Cardiovascular effects were seen only with the bolus doses of laudanosine 2 mg kg-1, and at plasma laudanosine concentrations unlikely to be achieved clinically. Similarly, EEG and power spectra analysis showed no evidence of epileptiform activity at all plasma laudanosine concentrations achieved. The non-specific EEG/power spectra changes observed may reflect a direct CNS activity with neurodepression, since laudanosine was shown by its presence in CSF to cross the blood-brain barrier. These EEG changes were observed at plasma concentrations eight to 10 times those observed in humans during infusion of atracurium. Thus, laudanosine accumulation and related CNS or cardiovascular toxicity seem unlikely following atracurium administration in the anephric patient.
