Abstract: The current standard model of cosmology successfully describes a variety of measurements, but the nature of its main ingredients, dark matter and dark energy, remains unknown. Euclid is a medium-class mission in the Cosmic Vision 2015-2025 programme of the European Space Agency (ESA) that will provide high-resolution optical imaging, as well as near-infrared imaging and spectroscopy, over about 14,000 deg^2 of extragalactic sky. In addition to accurate weak lensing and clustering measurements that probe structure formation over half of the age of the Universe, its primary probes for cosmology, these exquisite data will enable a wide range of science. This paper provides a high-level overview of the mission, summarising the survey characteristics, the various data-processing steps, and data products. Abstract: In this followup analysis, we update previous constraints on the Transitional Planck Mass (TPM) modified gravity model using the latest version of EFTCAMB and provide new constraints using SPT and Planck anisotropy data along with Planck CMB lensing, BAO, SNe Ia, and an $H_0$ prior from local measurements. We find that large shifts in the Planck mass lead to large suppression of power on small scales that is disfavored by both SPT and Planck. Using only SPT TE-EE data, this suppression of power can be compensated for by an upward shift of the scalar index to $n_s = 1.003 \pm 0.016$ resulting in $H_0 = 71.94^{+0.86}_{-0.85}$ kms$^{-1}$Mpc$^{-1}$ and a $\sim7\%$ shift in the Planck mass. Including Planck TT $\ell \leq 650$ and Planck TE-EE data restricts the shift to be $<5\%$ at $2\sigma$ with $H_0 = 70.65 \pm 0.66$ kms$^{-1}$Mpc$^{-1}$. Excluding the $H_0$ prior, SPT and Planck data constrain the shift in the Planck mass to be $<3\%$ at $2\sigma$ with a best-fit value of $0.04\%$, consistent with the $\Lambda$CDM limit. In this case $H_0 = 69.09^{+0.69}_{-0.68}$ kms$^{-1}$Mpc$^{-1}$, which is partially elevated by the dynamics of the scalar-field in the late universe. This differs from EDE models that prefer higher values of $H_0$ when high $\ell$ Planck TT data are excluded. Abstract: The future Euclid space satellite mission will offer an invaluable opportunity to constrain modifications to Einstein's general relativity at cosmic scales. We focus on modified gravity models characterised, at linear scales, by a scale-independent growth of perturbations while featuring different testable types of derivative screening mechanisms at smaller non-linear scales. We considered three specific models, namely JBD, a scalar-tensor theory with a flat potential, the nDGP gravity, a braneworld model in which our Universe is a four-dimensional brane embedded in a five-dimensional Minkowski space-time, and $k$-mouflage (KM) gravity, an extension of $k$-essence scenarios with a universal coupling of the scalar field to matter. In preparation for real data, we provide forecasts from spectroscopic and photometric primary probes by Euclid on the cosmological parameters and the additional parameters of the models, respectively, $\omega_{\rm BD}$, $\Omega_{\rm rc}$ and $\epsilon_{2,0}$. The forecast analysis employs the Fisher matrix method applied to weak lensing (WL); photometric galaxy clustering (GCph), spectroscopic galaxy clustering (GCsp) and the cross-correlation (XC) between GCph and WL. In an optimistic setting at 68.3\% confidence interval, we find the following percentage relative errors with Euclid alone: for $\log_{10}{\omega_{\rm BD}}$, with a fiducial value of $\omega_{\rm BD}=800$, 27.1\% using GCsp alone, 3.6\% using GCph+WL+XC and 3.2\% using GCph+WL+XC+GCsp; for $\log_{10}{\Omega_{\rm rc}}$, with a fiducial value of $\Omega_{\rm rc}=0.25$, we find 93.4\%, 20\% and 15\% respectively; and finally, for $\epsilon_{2,0}=-0.04$, we find 3.4\%, 0.15\%, and 0.14\%. Data on 45 pts treated between 2022– 2023 were collected. All pts underwent the third-generation A-CT after AH-RT 15– 5 fractions with or without a boost. Acute toxicity was scored according to CTCAE v5.0 for skin, pulmonary, and cardiac adverse events. Univariate and multivariate analyses were conducted to assess significant prognosticators for skin/lung/heart acute toxicities in the AH-RT 5– 15 fractions arms and CT (p < 0.005).Results: A reduction in the time to PORT initiation and overall adjuvant treatment time was recorded. RT was initiated 5 median weeks after surgery, and A-CT was performed 9 median weeks after surgery. The median duration of the entire adjuvant treatment was 35 weeks after surgery. At 6 months mean follow-up, no significant differences in G2–G3 toxicity were noted between the different hypofractionated RT arms, irrespective of the CT schedules, irradiated volumes, or boost (SIB or sequential) in univariate and multivariate analyses. In the multivariate analysis, no significant effects in CT schedules and AH-RT 5– 15 arms for skin/lung acute toxicities (p = 0.077 and p = 0.68; 0.67 and 0.87, respectively) were recorded.Conclusion: As a new PORT approach in BC, AH-RT up-front to the third-generation A-CT appeared safe with a low acute toxicity profile, providing an advantage in shortening the time from surgery to PORT initiation and the overall adjuvant treatment time.Keywords: PORT, third-generation chemotherapy, hypofractionated radiotherapy

Abstract: IntroductionObesity aligned with quadriceps muscle weakness contributes to the high incidence of knee osteoarthritis (KOA), which is prevalent in women. Although molecular signatures of KOA have been suggested, the association between biopsychosocial responses and the plasma metabolomic profile in overweight/ obese women with KOA remains in its early stages of investigation. This study aims to associate the plasma metabolome with biopsychosocial parameters of overweight/obese women diagnosed with KOA.MethodsTwenty-eight overweight/obese women (Control-n = 14; KOA-n = 14) underwent two visits to the laboratory. Functional tests and questionnaires assessing biopsychosocial parameters were administered during the first visit. After 48 h, the participants returned to the laboratory for blood collection. Specific to the KOA condition, the Numerical Pain Rating Scale (NPRS), Tampa Scale for Kinesiophobia (TSK), and Knee injury and Osteoarthritis Outcome Score (KOOS) were appliedResultsThirteen molecules were different between groups, and four correlated with KOA’s biopsychosocial parameters. DG 22:4-2OH and gamma-Glutamylvaline were inversely associated with KOSS leisure and TSK score, respectively. LysoPE 18:0 and LysoPE 20:5 were positively associated with KOSS symptoms and TSK score, respectively.DiscussionWhile the correlations of LysoPE 18:0 and gamma-Glutamylvaline are supported by existing literature, this is not the case for DG 22:4-2OH and LysoPE 20:5. Abstract: The discipline of process mining has a solid track record of successful applications to the healthcare domain. Within such research space, we conducted a case study related to the Intensive Care Unit (ICU) ward of the Uniklinik Aachen hospital in Germany. The aim of this work is twofold: developing a normative model representing the clinical guidelines for the treatment of COVID-19 patients, and analyzing the adherence of the observed behavior (recorded in the information system of the hospital) to such guidelines. We show that, through conformance checking techniques, it is possible to analyze the care process for COVID-19 patients, highlighting the main deviations from the clinical guidelines. The results provide physicians with useful indications for improving the process and ensuring service quality and patient satisfaction. Abstract: Fat embolism syndrome (FES) can be challenging to diagnose by forensic pathologists. For the diagnosis of FES, there is no benchmark test. Postmortem diagnosis requires a full autopsy and specific ancillary examination. However, the high variability in the clinical presentation of FES represents a relevant issue, and there is no consensus on the postmortem assessment. This is the case of a 33-year-old man who died of FES one week after a car accident. He suffered multiple fractures, but was hemodynamically stable and showed no neurological changes. The patient died a few days after hospital discharge. Additionally, he had osteogenesis imperfecta type III, a genetic disorder associated with bone fragility. To the best of our knowledge, no study has assessed whether and how osteogenesis imperfecta contributes to the onset of FES. Despite the heterogeneous manifestations of FES, the present case met many of the proposed clinical and histological diagnostic criteria. Therefore, we briefly review FES diagnostic criteria, show the postmortem diagnostic workup, and discuss the hypothetical link between osteogenesis imperfecta and FES.

Abstract: IntroductionNeonatal sepsis, classified into early-onset and late-onset based on symptom timing, poses significant risks of morbidity and mortality, especially in low birth weight infants. Effective clinical risk management protocols are crucial in reducing these risks.MethodsThis before-and-after study evaluated the impact of a newly implemented clinical risk management protocol in the Neonatology and Neonatal Intensive Care Unit (NICU) at Policlinico Hospital-University of Bari. The study included 399 neonates over three years, comparing pre- and post-protocol outcomes. Data collection focused on maternal and neonatal demographics, infection rates, and hospital stay lengths. Statistical analysis included t-tests, Wilcoxon-Mann–Whitney tests, and logistic regression models.ResultsThe study found no significant differences in neonatal pathologies or demographics between pre- and post-protocol groups. However, post-protocol implementation showed a notable reduction in umbilical venous catheter (UVC) infections (p = 0.018) and improved hospital stay lengths. Blood and urine cultures did not show significant changes in microbial patterns post-protocol.DiscussionThe findings underscore the effectiveness of structured clinical risk management protocols in enhancing neonatal outcomes, particularly in reducing specific infection risks. Despite the study’s limitations, including its observational nature and sample size, the results advocate for broader adoption and further research on these protocols in diverse healthcare settings. 16. An exploration of an early gravity transition in light of cosmological tensions

Abstract: We study a step-like transition in the value of the effective Planck mass (or effective gravitational constant) on cosmological scales prior to recombination. We employ CMB, BAO, and SNIa data and find they are sufficient to strongly constrain our implementation of the Effective Field Theory of Dark Energy and Modified Gravity, used to model the transition, to a limited parameter space. The data prefer a $\sim 5\%$ shift in the value of the effective Planck mass ($<10 \%$ at $2 \sigma$) prior to recombination. This Transitional Planck Mass (TPM) model is free to undergo its transition at any point over multiple decades of scale factor prior to recombination, $\log_{10}(a) = -5.32^{+0.96}_{-0.72}$ (68\% CL). This lowers the sound horizon at last scattering, which increases the Hubble constant to $71.09 \pm 0.75$ km $\textrm{s}^{-1}\textrm{Mpc}^{-1}$ with a combination of local measurements as prior and to $69.22^{+0.67}_{-0.86}$ km $\textrm{s}^{-1}\textrm{Mpc}^{-1}$ when the prior is excluded. The TPM model improves $\chi^2$ with respect to $\Lambda$CDM by $\Delta \chi^2 = -23.72$ with the $H_0$ prior and $\Delta \chi^2 = -4.8$ without the prior. The model allows for both $H_0 > 70$ km$\textrm{s}^{-1}\textrm{Mpc}^{-1}$ and $S_8 < 0.80$ simultaneously with lower values of $S_8$ due to a reduction in the matter density $\Omega_m$ to offset the increase in $H_0$ relative to $\Lambda$CDM. 18. Communicating Climate Change Risk to Children: A Thematic Analysis of Children's Literature

Explorations of children's climate change literature is limited, despite the push from experts to involve children in climate change education and action. A thematic analysis of picturebooks books on the topic was conducted. Data were identified from online websites and accessibility was confirmed via academic and internet search engines. Findings suggest that children's books about climate change lack informational material and overlook the human consequences of the climate crisis. 19. Probing Modified Gravity with Integrated Sachs-Wolfe CMB and Galaxy Cross-correlations

Abstract: We use the cross-correlation power spectrum of the integrated Sachs-Wolfe (ISW) effect in the cosmic microwave background (CMB) temperature anisotropy and galaxy fluctuations to probe the physics of late-time cosmic acceleration. For this purpose, we focus on three models of dark energy that belong to a sub-class of Horndeski theories with the speed of gravity equivalent to that of light: Galileon Ghost Condensate (GGC), Generalized Cubic Covariant Galileon (GCCG), and K-mouflage. In the GGC and GCCG models, the existence of cubic-order scalar self-interactions allows a possibility for realizing negative ISW-galaxy cross-correlations, while the K-mouflage model predicts a positive correlation similar to the $\Lambda$-cold-dark-matter (LCDM) model. In our analysis, we fix the parameters of each model to their best-fit values derived from a baseline likelihood analysis with observational data from CMB, baryon acoustic oscillations, and supernovae type Ia. Then we fit those best-fit models to the ISW-galaxy cross-correlation power spectrum extracted from a collection of photometric redshift surveys. We find that both GGC and GCCG best-fit models degrade the fit to the ISW-galaxy cross-correlation data compared to LCDM best-fit model. This is attributed to the fact that, for their best-fit values constrained from the baseline likelihood, the cubic-order scalar self-interaction gives rise to suppressed ISW tails relative to $\Lambda$CDM. Abstract: Establishing the manner of death is one of the most challenging tasks for forensic pathologists. We present the case of a 24-year-old woman found dead in the early morning on a flyover. The body was sitting on the ground with the back leaning against a wall. The neck was encircled by a white phone charger cable knotted to the staircase’s handrail. The victim had argued with her boyfriend and tried to jump out of his car while coming home from a wedding party the night before. After that, she left home alone with her phone charger in her hand. Due to self-harm behaviors, the first hypothesis was suicide by hanging. However, the ligature crossed immediately beneath the thyroid cartilage and encircled the neck twice horizontally; the two ends of the cable overlapped, forming a cross-over point in the front-right of the neck. Then, the ligature passed obliquely through the nape, gradually disappearing, forming a gap in the mark. The mark was sharply defined, stiff, yellow, and parchment-like. The investigators performed a three-dimensional scene reconstruction using the Trimble X7 Laser Scanner and the PC-Crash Multibody System. Even though the geometry of the ligature mark in the present case raised doubts about the manner of death, the three-dimensional reconstruction confirmed that the hanging was feasible without any external intervention.

23. Healthcare professional and manager perceptions on drivers, benefits, and challenges of telemedicine: results from a cross-sectional survey in the Italian NHS

Abstract: Abstract Background The Covid-19 pandemic provided new challenges and opportunities for patients and healthcare providers while accelerating the trend of digital healthcare transformation. This study explores the perspectives of healthcare professionals and managers on (i) drivers to the implementation of telemedicine services and (ii) perceived benefits and challenges related to the use of telemedicine across the Italian National Health Service. Methods An online cross-sectional survey was distributed to professionals working within 308 healthcare organisations in different Italian regions. Quantitative and qualitative data were collected through a self-administered questionnaire (June-September 2021). Responses were analysed using summary statistics and thematic analysis. Results Key factors driving the adoption of telemedicine have been grouped into (i) organisational drivers (reduce the virus spread-80%; enhance care quality and efficiency-61%), (ii) technological drivers (ease of use-82%; efficacy and reliability-64%; compliance with data governance regulations-64%) and (iii) regulatory drivers (regulations’ semplification-84%). Nearly all respondents perceive telemedicine as useful in improving patient care (96%). The main benefits reported by respondents are shorter waiting lists, reduced Emergency Department attendance, decreased patient and clinician travel, and more frequent patient-doctor interactions. However, only 7% of respondents believe that telemedicine services are more effective than traditional care and 66% of the healthcare professionals believe that telemedicine can’t completely substitute in-person visits due to challenges with physical examination and patient-doctor relationships. Other reported challenges include poor quality and interoperability of telemedicine platforms and scarce integration of telemedicine with traditional care services. Moreover, healthcare professionals believe that some groups of patients experience difficulties in accessing and using the technologies due to socio-cultural factors, technological and linguistic challenges and the absence of caregivers. Conclusions Respondents believe that telemedicine can be useful to complement and augment traditional care. However, many challenges still need to be overcome to fully consider telemedicine a standard of care. Strategies that could help address these challenges include additional regulations on data governance and reimbursements, evidence-based guidelines for the use of telemedicine, greater integration of tools and processes, patient-centred training for clinicians, patient-facing material to assist patients in navigating virtual sessions, different language options, and greater involvement of caregivers in the care process.

26. Constraint-coupled Optimization with Unknown Costs: A Distributed Primal Decomposition Approach

Abstract: In this paper, we present a distributed algorithm for solving convex, constraint-coupled, optimization problems over peer-to-peer networks. We consider a network of processors that aim to cooperatively minimize the sum of local cost functions, subject to individual constraints and to global coupling constraints. The major assumption of this work is that the cost functions are unknown and must be learned online. We propose a fully distributed algorithm, based on a primal decomposition approach, that uses iteratively refined data-driven estimations of the cost functions over the iterations. The algorithm is scalable and maintains private information of agents. We prove that, asymptotically, the distributed algorithm provides the optimal solution of the problem even though the true cost functions are never used within the algorithm. The analysis requires an in-depth exploration of the primal decomposition approach and shows that the distributed algorithm can be thought of as an epsilon-subgradient method applied to a suitable reformulation of the original problem. Finally, numerical computations cor- roborate the theoretical findings and show the efficacy of the proposed approach.

Abstract: Lung neuroendocrine neoplasms (L-NEN) are heterogeneous tumors. While bone metastases (BM) have been associated with worse prognosis in other NEN, their role in L-NEN deserves in-depth analysis. This study analyzes the clinical presentation, treatment and survival outcomes of L-NEN, focusing on patients with BM compared with patients without metastases or with metastases in other sites (OtherMtx). The clinicopathological and survival data of L-NEN admitted to the Federico II University were retrospectively evaluated. Fifty L-NEN were included. Among 27 metastatic patients (54%), 13 (26%) had BM, more commonly occurring in males than females and in primary bilateral L-NEN or L-NEN > 26 mm, with higher Ki67. Atypical carcinoid and hypovitaminosis D were associated with BM. The number of metastatic sites was higher in patients with BM than OtherMtx. Synchronous metastases were associated with shorter overall survival (OS). The median progression-free survival (PFS) and OS in patients with BM were similar to OtherMtx, but a two-times increased risk of shorter OS was detected. BM do not impact PFS or OS more than OtherMtx, but the increased risk of shorter OS in patients with BM should be considered. Periodic bone evaluation in L-NEN should be recommended.

Abstract: Abstract Background Sudden cardiac death is a major public health concern. The incidence of sudden cardiac death in young adults remains unclear and is generally underestimated. Case presentation This is a case report of a 24-year-old man with a silent pathological history, who suddenly collapsed in a restaurant during dinner with his workmates. Autopsy and ancillary examination revealed focal atrioventricular node fibrosis and thymic hyperplasia. Conclusions Thymic hyperplasia has been considered a cause of sudden death in the past century. The mode of death was explained through nosographic entities such as status lymphaticus or thymic asthma, which are currently consigned to history books. Nevertheless, recent studies have sought to determine the relationship between thymic hyperplasia and sudden unexpected deaths in adults. Moreover, isolated fibrosis of the atrioventricular node is a rare condition that can remain undiagnosed even after a full autopsy. This report aims to provide a concise review of the existing literature concerning sudden cardiac death and discuss the so-called “thymic death” theory, which is now considered a myth. Was the finding of thymic hyperplasia and atrioventricular node fibrosis a coincidence? Should the myth surrounding “thymic death” be re-examined?

30. Can Late Dark Energy Transitions Raise the Hubble constant?

Abstract: Late times dark energy transitions at redshifts $z \ll 0.1$ can raise the predicted value of the Hubble constant to the SH0ES value, $74.03\pm 1.42$ (km s$^{-1}$ Mpc$^{-1})$ or more, while providing an equally good fit as $\Lambda$CDM at $67.73 \pm 0.41$ to higher redshift data, in particular from the cosmic microwave background and baryon acoustic oscillations. These models however do not fully resolve the true source of tension between the distance ladder and high redshift observations: the local calibration of supernovae luminosities well out into the Hubble flow. When tested in this manner by transferring the SH0ES calibration to the Pantheon supernovae dataset, the ability of such transitions to raise the Hubble constant is reduced to $69.17 \pm 1.09$. Abstract: Restrictions imposed during the COVID-19 pandemic might have changed recreational habits. In this study, the results of toxicological tests for alcohol and drugs in blood were compared among drivers stopped at roadside checks in the periods before (1 January 2018 to 8 March 2020) and after the lockdown measures (9 March 2020 to 31 December 2021). A total of 123 (20.7%) subjects had a blood alcohol level above the legal limit for driving of 0.5 g/l, 21 (3.9%) subjects tested positive for cocaine, and 29 (5.4%) subjects positive for cannabis. In the COVID-19 period, the mean blood alcohol level was significantly higher than in the previous period. Cannabis use, which was more frequent among younger subjects, was statistically associated with cocaine use. There has also been a quantitative increase in alcohol levels in the population with values above the legal limits, indicative of greater use of alcohol in the population predisposed to its intake.

Etiologies, Child-Pugh Score (7 vs 7) and MELD scores (11 vs 12, p=0.3679) did not differ. Anticoagulation was administered to 43% HCC vs 42% nonHCC. The extension of PVT in the main portal trunk was similar: partial/total involvement was 73.3/6.7% in HCC vs 67.4/6.1% in nonHCC, p=0.760. The remainder had intrahepatic PVT. The recanalization rate was 61.5% and 60.7% in HCC/nonHCC in anticoagulated patients (p=1). Overall PVT recanalisation, including treated and untreated patients, was observed in 30% of HCC vs 37.9% of nonHCC, p=0.530. Major bleeding incidence was almost identical (3.3% vs 3.8%, p=1). Progression of PVT after stopping anticoagulation did not differ (10% vs 15.9%, respectively, HCC/nHCC, p=0.109).Conclusion: The course of bland non-malignant PVT in cirrhosis is not affected by the presence of active HCC. Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

36. Acoustic Dark Energy: Potential Conversion of the Hubble Tension

Abstract: We discuss the ability of a dark fluid becoming relevant around the time of matter radiation equality to significantly relieve the tension between local measurements of the Hubble constant and CMB inference, within the $\Lambda$CDM model. We show that the gravitational impact of acoustic oscillations in the dark fluid balance the effects on the CMB and result in an improved fit to CMB measurements themselves while simultaneously raising the Hubble constant. The required balance favors a model where the fluid is a scalar field that converts its potential to kinetic energy around matter radiation equality which then quickly redshifts away. We derive the requirements on the potential for this conversion mechanism and find that a simple canonical scalar with two free parameters for its local slope and amplitude robustly improves the fit to the combined data by $\Delta\chi^2 \approx 12.7$ over $\Lambda$CDM. 37. Cosmological constraints and phenomenology of a beyond-Horndeski model

Abstract: We study observational constraints on a specific dark energy model in the framework of Gleyzes-Langlois-Piazza-Vernizzi theories, which extends the Galileon ghost condensate (GGC) to the domain of beyond Horndeski theories. In this model, we show that the Planck cosmic microwave background (CMB) data, combined with datasets of baryon acoustic oscillations, supernovae type Ia, and redshift-space distortions, give the tight upper bound $|\alpha_{\rm H}^{(0)}| \le {\cal O}(10^{-6})$ on today's beyond-Horndeski (BH) parameter $\alpha_{\rm H}$. This is mostly attributed to the shift of CMB acoustic peaks induced by the early-time changes of cosmological background and perturbations arising from the dominance of $\alpha_{\rm H}$ in the dark energy density. In comparison to the $\Lambda$-cold-dark-matter ($\Lambda$CDM) model, our BH model suppresses the large-scale integrated-Sachs-Wolfe (ISW) tail of CMB temperature anisotropies due to the existence of cubic Galileons, and it modifies the small-scale CMB power spectrum because of the different background evolution. We find that the BH model considered fits the data better than $\Lambda$CDM according to the $\chi^2$ statistics, yet the deviance information criterion (DIC) slightly favors the latter. Abstract: Background & Aims: Alcohol abuse and metabolic disorders are leading causes of hepatocellular carcinoma (HCC) worldwide. Alcohol-related aetiology is associated with a worse prognosis compared with viral agents, because of the lower percentage of patients diagnosed with HCC under routine surveillance and a higher burden of comorbidity in alcohol abusers. This study aimed to describe the evolving clinical scenario of alcohol-related HCC over 15 years (2006–2020) in Italy. Abstract: BackgroundIn the age of digitalization and big data, personal health information is a key resource for health care and clinical research. This study aimed to analyze the determinants and describe the measurement of the willingness to disclose personal health information.MethodsThe study conducted a systematic review of articles assessing willingness to share personal health information as a primary or secondary outcome. The review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analysis protocol. English and Italian peer-reviewed research articles were included with no restrictions for publication years. Findings were narratively synthesized.ResultsThe search strategy found 1,087 papers, 89 of which passed the screening for title and abstract and the full-text assessment.ConclusionNo validated measurement tool has been developed for willingness to share personal health information. The reviewed papers measured it through surveys, interviews, and questionnaires, which were mutually incomparable. The secondary use of data was the most important determinant of willingness to share, whereas clinical and socioeconomic variables had a slight effect. The main concern discouraging data sharing was privacy, although good data anonymization and the high perceived benefits of sharing may overcome this issue.

Abstract: The clustering of time series with geo-referenced data requires a suitable dissimilarity matrix interpreting the comovements of the time series and taking into account the spatial constraints. In this paper, we propose a new way to compute the dissimilarity matrix, merging both types of information, which leverages on the Wasserstein distance. We then make a quasi-Gaussian assumption that yields more convenient formulas in terms of the joint correlation matrix. The method is illustrated in a case study involving climatological data.

Abstract: We investigate cosmological constraints on K-mouflage models of modified gravity. We consider two scenarios: one where the background evolution is free to deviate from $\Lambda$CDM (K-mouflage) and another one which reproduces a $\Lambda$CDM expansion (K-mimic), implementing both of them into the EFTCAMB code. We discuss the main observational signatures of these models and we compare their cosmological predictions to different datasets, including CMB, CMB lensing, SNIa and different galaxy catalogues. We argue about the possibility of relieving the $H_0$ and weak lensing tensions within these models, finding that K-mouflage scenarios effectively ease the tension on the Hubble Constant. Our final 95\% C.L. bounds on the $\epsilon_{2,0}$ parameter that measures the overall departure from $\Lambda$CDM (corresponding to $\epsilon_{2,0}=0$) are $-0.04\leq \epsilon_{2,0} <0$ for K-mouflage and $0< \epsilon_{2,0} <0.002$ for K-mimic. In the former case the main constraining power comes from changes in the background expansion history, while in the latter case the model is strongly constrained by measurements of the amplitude of matter perturbations. The sensitivity of these cosmological constraints closely matches that of solar system probes. Radiation recall syndrome is a limited field inflammatory reaction that occurs in a volume that was irradiated several months or years previously before being induced by a triggering factor. The most frequently reported phenomena are skin reactions; however, other organs could be involved, such as the lungs in radiation recall pneumonitis (RRP). It is a well-described inflammatory reaction that occurs within a pulmonary volume that was irradiated several months or years previously via radiotherapy (RT), triggered by factors such as drugs, including chemotherapy agents, immunotherapy, or vaccination. Indeed, during the COVID-19 pandemic, RRP following anti-COVID-19 vaccination or SARS-CoV2 infection was recently reported. ACE receptor-rich tissues such as lung or skin tissues were mainly involved. Herein, we present a case of RRP triggered by COVID-19 pulmonary infection in a woman who previously underwent adjuvant breast cancer radiotherapy. Although symptoms were typical, pulmonary CT findings depicted a unique distribution of ground-glass opacities (GGOs) throughout the previous radiation portals and mirror-like the radiation fields. Abstract: Abstract Background Whole-genome sequencing has shown that the Mycobacterium tuberculosis infection process can be more heterogeneous than previously thought. Compartmentalized infections, exogenous reinfections, and microevolution are manifestations of this clonal complexity. The analysis of the mechanisms causing the microevolution —the genetic variability of M. tuberculosis at short time scales— of a parental strain into clonal variants with a patient is a relevant issue that has not been yet completely addressed. To our knowledge, a whole genome sequence microevolution analysis in a single patient with inadequate adherence to treatment has not been previously reported. Case presentation In this work, we applied whole genome sequencing analysis for a more in-depth analysis of the microevolution of a parental Mycobacterium tuberculosis strain into clonal variants within a patient with poor treatment compliance in Argentina. We analyzed the whole-genome sequence of 8 consecutive Mycobacterium tuberculosis isolates obtained from a patient within 57-months of intermittent therapy. Nineteen mutations (9 short-term, 10 fixed variants) emerged, most of them associated with drug resistance. The first isolate was already resistant to isoniazid, rifampicin, and streptomycin, thereafter the strain developed resistance to fluoroquinolones and pyrazinamide. Surprisingly, isolates remained susceptible to the pro-drug ethionamide after acquiring a frameshift mutation in ethA, a gene required for its activation. We also found a novel variant, (T-54G), in the 5′ untranslated region of whiB7 (T-54G), a region allegedly related to kanamycin resistance. Notably, discrepancies between canonical and phage-based susceptibility testing to kanamycin were previously found for the isolate harboring this mutation. In our patient, microevolution was mainly driven by drug selective pressure. Rare short-term mutations fixed together with resistance-conferring mutations during therapy. Conclusions This report highlights the relevance of whole-genome sequencing analysis in the clinic for characterization of pre-XDR and MDR resistance profile, particularly in patients with incomplete and/or intermittent treatment.
