4. Community-supported teaching on the topic of transgender identity in undergraduate medical education - a pilot project

Abstract

Introduction and objectives: Future physicians are insufficiently prepared for the topic of transgender identity during their studies. Relevant courses during undergraduate medical education are heterogeneous and not widely established within the curricula. At our university, we investigated if students' knowledge of transgender identity could be increased through medical specialist teaching and teaching delivered by representatives of the trans* community (community-supported teaching).Methods: During summer semester 2021 (SS21), the knowledge level on transgender identity of 134 medical students in their fifth clinical semester was evaluated (phase 1). In addition, knowledge gain on gender incongruence through the module "psychiatry" was retrospectively surveyed across two dimensions: 1. diagnostic criteria, 2. treatment/care. During winter semester 2021/22 (WS 21/22), a 90-minute seminar on transgender identity was held either community-supported or by medical specialists (phase 2). Following the psychiatry exam, a re-evaluation was carried out by 115 students (phase 3).Results: The students in SS21 did not feel sufficiently educated in the topic of transgender identity through their studies, but rated the relevance of the topic for their later profession as high. Learning gain improved after the introduction of the seminar in WS21/22 compared to the previous semester (both dimensions p <.001). Community-supported and specialist teaching achieved equivalent results.Conclusion: One 90-minutes seminar led to a significant learning gain regarding the topic of transgender identity. Community-supported teaching is a promising way to impart knowledge in a qualified manner: Medical faculties should use this form of teaching to convey established knowledge to students in future curricula., Zielsetzungen: Zukünftige Ärzt*innen werden im Studium unzureichend auf das Thema Transidentität vorbereitet. Lehrveranstaltungen sind im internationalen Vergleich heterogen und nicht an allen Universitäten etabliert. An unserer Universität wurde untersucht, ob sich das Wissen der Studierenden zu diesem Thema durch fachärztliche Lehre sowie durch Lehre, die von Vertreter*innen der Trans*Community gehalten wurde (communitygestützte Lehre) in ähnlicher Form steigern lässt.Methoden: Im Sommersemester 2021 (SS21) wurde der Wissensstand zu Transidentität von 134 Medizinstudierenden im fünften klinischen Semester evaluiert (Phase 1). Zudem wurde der Lernzuwachs zu Genderinkongruenz durch das Modul Psychiatrie retrospektiv in zwei Dimensionen erfragt: 1. Diagnosekriterien, 2. Behandlung/Begleitung. Im Wintersemester 2021/22 (WS 21/22) wurde ein 90-minütiges Seminar zum Thema Transidentität fachärztlich oder communitygestützt durchgeführt (Phase 2). Im Anschluss an die Klausur Psychiatrie erfolgte eine erneute Evaluation durch 115 Studierende (Phase 3).Ergebnisse: Die Studierenden im SS21 fühlten sich durch ihr Studium nicht ausreichend im Thema Transidentität ausgebildet, schätzten die Relevanz des Themas für ihren späteren Beruf aber hoch ein. Der Lernzuwachs verbesserte sich nach Einführung des Seminars im WS 21/22 im Vergleich zum vorherigen Semester (beide Dimensionen p <,001). Die communitygestützte und die fachärztliche Lehre erzielte gleichwertige Lernzuwächse.Schlussfolgerung: Ein 90-minütiges Seminar führte bei Studierenden zu einem signifikanten Lernzuwachs bezüglich Transidentität. Communitygestützte Lehre ist hierbei eine gute Möglichkeit, Wissen zu vermitteln. Medizinische Fakultäten sollten diese Form der Lehre nutzen, um in zukünftigen Curricula etabliertes Wissen gezielt an Studierenden zu vermitteln.

12. Design and comprehensive characterization of novel fiber-optic sensor systems using fast-response luminescence-based O2 probes

Abstract

The detection of O2 using optical techniques has now become very important and significant progress has been made in this area over the past few years. The measurement of O2 concentration is a key parameter for diverse applications such as in life sciences, environmental monitoring, industry and biological research, and luminescence-based sensors are a valuable tool for measurements of this type. Often commercial sensors are unable to respond within a short time frame and drift over time - this demonstrates the importance of developing and validating a new technological approach. Given that need, a new sensor has been developed, with a specially-formed tip design coated with a very thin Platinum(II) 5,10,15,20-Tetrakis(2,3,4,5,6-pentafluorophenyl)-porphyrin (PtTFPP)-containing a polystyrene layer. The performance and reproducibility of the sensor design has been studied by comparing three identically fabricated sensor probes, showing that the new probe design has extreme fast response times – of a maximum value, Δt90max ≈ 390 ms for increasing and Δt90max ≈ 250 ms for decreasing O2 concentrations. The sensors created had an accuracy of better than ± 0.03 % O2 over the detection range investigated (between 0 % and 20 % O2) and showed a very stable performance over many hours of use. Examining this in detail through extended continuous use, over 12 h in a 0 % O2 solution, the probe stability was confirmed, showing a very low drift of luminescence decay time of ≈ 0.025 %/h (0.01 µs/h). Furthermore, the temperature ‘cross-talk’ has been studied in the important region between 25 °C and 40 °C (so that temperature corrections can be applied) and its mechanical stability, as well as allowing for compensation for any effects seen, has been confirmed. Given the success of the design developed, this work offers both an effective probe design and illustrates many important criteria for the selection of highly effective O2-sensitive coating materials and thus their use as a measurement tool for different quality control (QC) applications in industry.

Abstract

This study compares the Lucerne German allophones of Middle High German using data from the crowdsourcing smartphone app Dialäkt Äpp, to those of the Linguistic Atlas of German-Speaking Switzerland. In doing so we examine language change in real-time across c. 70 years. In the north of the Canton of Lucerne, use of /l/-vocalisation has receded, whereas in the west, and in and around the city of Lucerne, /l/-vocalisation occurs more frequently than at the time of the historic atlas. Both age and sex differences were found to be statistically non-significant, though older speakers tended to vocalise more. Generally, however, we report substantial areal heterogeneity which we suspect is due to the Canton’s geographical location in a dialectal transition zone, increased personal mobility, and the growing presence of broadcast media in dialect.

49. Ca2+-dependent desensitization of insulin secretion by strong potassium depolarization.

Abstract

Depolarization by a high K+ concentration is a widely used experimental tool to stimulate insulin secretion. The effects occurring after the initial rise in secretion were investigated here. After the initial peak a fast decline occurred, which was followed by a slowly progressive decrease in secretion when a strong K+ depolarization was used. At 40 mM KCl, but not at lower concentrations, the decrease continued when the glucose concentration was raised from 5 to 10 mM, suggesting an inhibitory effect of the K+ depolarization. When tolbutamide was added instead of the glucose concentration being raised, a complete inhibition down to prestimulatory values was observed. Equimolar reduction of the NaCl concentration to preserve isoosmolarity enabled an increase in secretion in response to glucose. Unexpectedly, the same was true when the Na+-reduced media were made hyperosmolar by choline chloride or mannitol. The insulinotropic effect of tolbutamide was not rescued by the compensatory reduction of NaCl, suggesting a requirement for activated energy metabolism. These inhibitory effects could not be explained by a lack of depolarizing strength or by a diminished free cytosolic Ca2+ concentration ([Ca2+]i). Rather, the complexation of extracellular Ca2+ concomitant with the K+ depolarization markedly diminished [Ca2+]i and attenuated the inhibitory action of 40 mM KCl. This suggests that a strong but not a moderate depolarization by K+ induces a [Ca2+]idependent, slowly progressive desensitization of the secretory machinery. In contrast, the decline immediately following the initial peak of secretion may result from the inactivation of voltage-dependent Ca2+ channels. [ABSTRACT FROM AUTHOR]
