1. A hybrid echocardiography-computational fluid dynamics framework for ventricular flow simulations

Abstract

Image-based computational fluid dynamics (CFD) has emerged as a powerful tool to study cardiovascular flows while 2D echocardiography (echo) is the most widely used non-invasive imaging modality for diagnosis of heart disease. Here, echo is combined with CFD, i.e., an echo-CFD framework, to study ventricular flows. To achieve this, our previous 3D reconstruction from multiple 2D-echo at standard cross-sections is extended by 1) reconstructing valves (aortic and mitral valves) from 2D-echo images and the superior wall; 2) optimizing the location and orientation of the surfaces to incorporate the physiological assumption of fixed apex as a reference (fixed) point for reconstruction; and 3) incorporating several smoothing algorithms to remove the nonphysical oscillations (ringing) near the basal section observed in our previous study. The main parameters of the reconstructed left ventricle (LV) are all within the physiologic range. Our results show that the mitral valve can significantly change the flow pattern inside the LV during the diastole phase in terms of ring vortex formation/propagation as well as the flow circulation. In addition, the abnormal shape of unhealthy LV can drastically change the flow field during the diastole phase. Furthermore, the hemodynamic energy loss, as an indicator of the LV pumping performance, for different test cases is calculated which shows a larger energy loss for unhealthy LV compared to the healthy one.

27. Right ventricular free wall circumferential strain reflects graded elevation in acute right ventricular afterload

Abstract

This study was designed to evaluate the concurrent changes in the right ventricular free wall (RVFW) movement in experimentally induced, acute mild, moderate, and severe right ventricle (RV) afterload conditions. In 14 open-chest pigs (weight 43 [+ or -] 4 kg) with preserved pericardia, acute mild (>35 and 60 mmHg) increases in RV systolic pressure (RVSP) were induced by constriction of the pulmonary artery. At each step, longitudinal, radial, and circumferential strains and strain rates were measured in both the RVFW and the interventricular septum. The mean RVSPs were 31.0 [+ or -] 4.3 mmHg at baseline and 41.1 [+ or -] 2.7 mmHg during mild, 52.7 [+ or -] 3.4 mmHg during moderate, and 61.7 [+ or -] 1.6 mmHg during severe afterload conditions. The RVFW circumferential strains showed significant differences among baseline, mild, moderate, and severe afterload conditions (-10.5 [+ or -] 3.9, -8.3 [+ or -] 3.3, -5.4 [+ or -] 2.7, and -7.5 [+ or -] 5.3%, respectively, P = 0.008) and had significant linear correlation with RVSP (r = 0.636, P < 0.001) if the severe condition was excluded. Decrease of the RVFW circumferential strain magnitude is the most distinct response in acute mild and moderate RV afterload that could aid in detection of clinical conditions associated with acutely increasing RV afterload. right ventricle; afterload; strain echocardiography

34. Myocardial contractility by strain echocardiography: comparison with physiological measurements in an in vitro model

Abstract

Strain echocardiography (SE) provides the rate and extent of myocardial segment shortening and lengthening. Thus we hypothesized that SE will noninvasively provide estimates of shortening velocity (SV) and length change ([delta]L). We compared SE-derived strain rate (SR) and strain ([epsilon]) to force/length transducer-derived SV and percent [delta]L in isolated muscle strips at multiple load levels and under varying conditions. Electrically stimulated contractions in left ventricular muscle strips (n = 20) were simultaneously recorded with a force/length transducer (to measure SV and percent [delta]L) and an ultrasound transducer (to measure SR and [epsilon]). Measurements were made at baseline, after inotropic stimulation, and during hypoxia at multiple load clamp levels (simulating multiple levels of afterload). Despite a difference in absolute numbers, there was a close correlation between SR and SV at baseline ([R.sup.2] = 0.95), with dobutamine treatment ([R.sup.2] = 0.99), and during hypoxia ([R.sup.2] = 0.99). SR was load dependent at baseline (r = 0.98), with dobutamine treatment (r = 0.99), and during hypoxia (r = 0.92). Similarly, there was a close correlation between [epsilon] and [delta]L at baseline ([R.sup.2] = 0.99), with dobutamine treatment ([R.sup.2] = 0.96), and during hypoxia ([R.sup.2] = 0.87). Percent [epsilon] was load dependent at baseline (r = 0.98), with dobutamine treatment (r = 0.98), and during hypoxia (r = 0.94). Bland-Altman analysis revealed a systematic overestimation of SV by SE-derived SR at baseline and with dobutamine treatment. There was no bias with SR measurements during hypoxia or with [epsilon] measurements. Abstract

Percutaneous transendocardial injections of therapeutic agents into the myocardium may not always be effective. We used an animal model for assessing the efficacy of the injections using linoleic acid as a testing agent. Efficacious delivery into the myocardium of a beating heart was indicated by rapidly developed local myocardial necrosis and wall motion abnormalities using echocardiography. We employed this experimental model to test our innovative technology, an acoustically active injection catheter. The Doppler ultrasound‐guided acoustically active injection catheter effectively delivers the substance to the myocardium but needs further technical improvements to minimize an unwanted systemic distribution of the agent.
