9. Ischaemic preconditioning improves upper-body endurance performance without altering V̇O2 kinetics.

Abstract

Purpose: Whilst pre-exercise ischaemic preconditioning (IPC) can improve lower-body exercise performance, its impact on upper-limb performance has received little attention. This study examines the influence of IPC on upper-body exercise performance and oxygen uptake (V̇O2) kinetics. Methods: Eleven recreationally-active males (24 ± 2 years) completed an arm-crank graded exercise test to exhaustion to determine the power outputs at the ventilatory thresholds (VT1 and VT2) and V̇O2peak (40.0 ± 7.4 ml·kg−1·min−1). Four main trials were conducted, two following IPC (4 × 5-min, 220 mmHg contralateral upper-limb occlusion), the other two following SHAM (4 × 5-min, 20 mmHg). The first two trials consisted of a 15-minute constant work rate and the last two time-to-exhaustion (TTE) arm-crank tests at the power equivalents of 95% VT1 (LOW) and VT2 (HIGH), respectively. Pulmonary V̇O2 kinetics, heart rate, blood-lactate concentration, and rating of perceived exertion were recorded throughout exercise. Results: TTE during HIGH was longer following IPC than SHAM (459 ± 115 vs 395 ± 102 s, p =.004). Mean response time and change in V̇O2 between 2-min and end exercise (ΔV̇O2) were not different between IPC and SHAM for arm-cranking at both LOW (80.3 ± 19.0 vs 90.3 ± 23.5 s [p =.06], 457 ± 184 vs 443 ± 245 ml [p =.83]) and HIGH (96.6 ± 31.2 vs 92.1 ± 24.4 s [p =.65], 617 ± 321 vs 649 ± 230 ml [p =.74]). Heart rate, blood-lactate concentration, and rating of perceived exertion did not differ between conditions (all p ≥.05). Conclusion: TTE was longer following IPC during upper-body exercise despite unchanged V̇O2 kinetics. Highlights Whilst pre-exercise ischaemic preconditioning can improve lower-body exercise performance and alter V̇O2 kinetics, its impact on upper-limb performance has received little attention. An acute bout of ischaemic preconditioning prior to arm-crank ergometry exercise significantly improved time to exhaustion compared to a sham control condition. Abstract

Objective: The aim of this article is to summarize the main radiological evidences induced by multiple myeloma at the jaws level. Materials and Methods: The analysis examines the main case reports and reviews on the matter. All relevant publications were searched through the PubMed search engine, using the following keywords: "Multiple Myeloma" and "Radiological evidences", "Maxilla", "Jaw", "TMJ". The purpose of our review is to highlight how oral radiological investigation is extremely useful in detecting any bone alterations associated with cases of multiple myeloma. In addition, we investigate the sites of the jaws where direct bone alteration is more frequently, and we perform a differential diagnosis with other focal alterations of the jaws or those that are the result of certain drug therapies (bisphosphonates). Our research stresses the concept that early diagnosis of multiple myeloma at the jaws level may prevent unnecessary and inadequate treatments. Results: From the evaluation of our research, it is evident that osteolytic lesions are one of the fundamental signs of multiple myeloma, especially in areas undergoing intense hematopoietic activity. Among the radiological techniques used for diagnosis, the most sensitive one appears to be the three-dimensional technique, such as CT or Cone Beam, whereas two-dimensional radiographs (orthopantomography) appear to be exhaustive only when the lesion is very extensive. Conclusions: It is evident that the dentist figure plays not only a supportive role, but also a fundamental part in the diagnostic process, as lesions that may anticipate a systemic manifestation are often encountered at the oral level. Abstract

With the aim of investigating the possible moderating effect of job control and dispositional mindfulness between different sources of organizational stress and job satisfaction, a correlational study was designed involving health care workers (HCWs). The following questionnaires were administered and completed by 237 HCWs: (1) Occupational Stress Indicator (OSI), to measure the sources of stress at work (managerial role, climate power, climate structure, internal relationships), and job satisfaction, (2) Mindfulness Attention Awareness Scale (MAAS) to assess the individual&rsquo, s level of attention to what is taking place in the present, (3) Job Control Scale (JCS) to assess the perceived control at work. Hierarchical regression analyses were used to test the hypothesized relationships between variables, the results showed that, between the different sources of stress, the organizational climate dimension was negatively associated with job satisfaction, moreover, mindfulness attention moderated the relationship between climate stress and job satisfaction, unexpectedly, the interaction between job control and the organizational climate dimension was not significant in affecting job satisfaction. This study can provide useful information for Human Resources Management (HRM) practices regarding job and mental control interventions and empowerment, and possibly offer a new interpretation of the role of attention to what is happening in the present moment and autonomy between climate stressors and occupational satisfaction.

Abstract

In wild-type Rhizobium leguminosarum, the sitABCD operon specifies a [Mn.sup.2+] transporter whose expression is severely reduced in cells grown in the presence of this metal. Mutations in the R. leguminosarum gene, mur (manganese uptake regulator), whose product resembles the Fur transcriptional regulator, cause high-level expression of sitABCD in the presence of [Mn.sup.2+]. In gel-shift mobility assays, purified R. leguminosarum Mur protein bound to at Least two regions near the sitABOD promoter region, although this DNA has no conventional consensus Fur-binding sequences (fur boxes). Thus, in contrast to [gamma]-proteobacteria, where Fur binds [Fe.sup.2+], the R. leguminosarum Fur homologue, Mur, act as a [Mn.sup.2]-responsive transcriptional regulator.
