Abstract

OBJECTIVE: This study aimed to improve the post-marketing surveillance on mRNA anti-SARS-CoV-2 vaccines, characterizing the adverse events (AEs) after the first dose of mRNA BNT162b vaccine. The associations between the AEs and individuals' characteristics were explored. PATIENTS AND METHODS: All adult healthcare workers at Niguarda Hospital (Milan, Italy) who were referred for the first dose of vaccine were offered to participate in a cross-sectional survey during the second-dose administration, between 18 January and 7 February 2021. All participants completed a questionnaire about age, gender, weight, height, medical history, concurrent therapies, employment status, previous diagnosis/testing for SARS-CoV-2 infection, and a list of 24 AEs (solicited AEs). The development of at least one solicited AEs was the main outcome. AEs were stratified by the presence of injection-site symptoms, systemic symptoms or both, and the differences between strata were assessed as a secondary outcome. Biometric data and reports of a previous diagnosis of SARS-CoV-2 infection were also explored, as predictors of the main outcome. RESULTS: 7,014 healthcare workers were included. An incidence of 3 per 10.000 persons for serious AEs following the first administration of the mRNA BNT162b vaccine was found. An association between the development of non-serious AEs with young age, female gender, low body mass index, and previous history of SARSCoV- 2 was described. CONCLUSIONS: This real-life study supported data on the safety profile of the BNT162b2 mRNA vaccine. Our findings on the associations between the development of non-serious AEs with some individual characteristics may help physicians and patients make educated and informed medical decisions towards anti-COVID-19 vaccination.

Abstract

Introduction: Breast carcinoma is the most common malignancy in women and the main cause of tumor deaths. Despite early recognition by screening and improvement in treatment regimens, many patients will eventually develop metastases, either locoregional recurrences or distant metastases, most frequently in the skeleton, liver and lung. Metastases to the gastrointestinal tract are infrequent and not even mentioned in the major oncologic textbooks. Case presentation: We report a case of metastasis to the stomach from occult breast cancer. The diagnosis was established by computed tomography (CT), histologic and immunohistochemical analyses of biopsies of the stomach lesion. The patient was treated with pre-operative hormonal therapy, undergoes breast surgery and then treated with systemic chemotherapy. She is still alive 2 years after initial symptoms of dyspepsia. Conclusion: The differentiation between primary gastric cancer and a metastatic breast tumor to the stomach is important for planning of treatment and to spare the patient unnecessary gastric surgery.

Abstract

A previous Delphi survey from the Rational Use of Analgesics (RUA) project involving Italian palliative care specialists revealed some discrepancies between current guidelines and clinical practice with a lack of consensus on items regarding the use of strong opioids in treating cancer pain. Those results represented the basis for a new Delphi study addressing a better approach to pain treatment in patients with cancer. The study consisted of a two-round multidisciplinary Delphi study. Specialists rated their agreement with a set of 17 statements using a 5-point Likert scale (0 = totally disagree and 4 = totally agree). Consensus on a statement was achieved if the median consensus score (MCS) (expressed as value at which at least 50% of participants agreed) was at least 4 and the interquartile range (IQR) was 3–4. This survey included input from 186 palliative care specialists representing all Italian territory. Consensus was reached on seven statements. More than 70% of participants agreed with the use of low dose of strong opioids in moderate pain treatment and valued transdermal route as an effective option when the oral route is not available. There was strong consensus on the importance of knowing opioid pharmacokinetics for therapy personalization and on identifying immediate-release opioids as key for tailoring therapy to patients’ needs. Limited agreement was reached on items regarding breakthrough pain and the management of opioid-induced bowel dysfunction. These findings may assist clinicians in applying clinical evidence to routine care settings and call for a reappraisal of current pain treatment recommendations with the final aim of optimizing the clinical use of strong opioids in patients with cancer.

10. Sick leave request following anti-COVID-19 vaccine administration is low among healthcare workers: results from a retrospective cross-sectional monocentric study.

Abstract

OBJECTIVE: Anti-COVID-19 vaccines were mainly associated with non-serious adverse events (AEs), whose prevalence was reported to be up to 70% in healthcare workers (HCWs). This may lead to sick leave requests, but this impact has never been quantified. This study aimed to investigate the absence from work among HCWs following anti-COVID-19 vaccination. Its association with age and previous COVID-19 infection was also assessed. PATIENTS AND METHODS: This is a retrospective observational cross-sectional study on administrative data about sick leave requests after anti-COVID-19 vaccination. All the HCWs employed at the Niguarda Hospital (Milan, Italy) who received the vaccine from December 27, 2020 to February 28, 2021 were included. RESULTS: In total, 4,088 HCWs received the first dose of the vaccine and 4,043 completed the vaccination cycle. After the first injection, 1.6% of HCWs requested sick leave, while after the second injection, the number of requests significantly increased (+6.1%, p<0.001). A significant increase in sick leave was detected for those who have had SARS-CoV-2 infection after the first injection (+2.3%, p<0.001). After the second dose, a significant increase in sick leave was observed in the 20-30-year-old group compared to >30 years (+3.6%, p=0.017), if HCWs without a history of SARS-CoV-2 infection were considered. CONCLUSIONS: The requests for sick leave among HCWs following the anti-COVID-19 vaccine were limited and higher after the second injection. This may help the management of the human resources when the large-scale administration of the anti-COVID-19 vaccines will involve other categories of workers. [ABSTRACT FROM AUTHOR]

Abstract

Background: Autonomic nervous system dysfunction that affects 70–80% of Parkinson’s disease (PD) patients, causes significant morbidity and it is correlated with poor quality of life. Objective: We assessed in a consecutive series of PD patients frequency of autonomic symptoms by means of the Scale for Outcomes for Parkinson’s disease AUTonomic (SCOPA-AUT) and we correlated it with the results of noninvasive urological studies (nUS). Methods: PD patients with known conditions that might have influenced urinary function were excluded. Clinical assessment of PD patients included the H&Y staging, UPDRS, BDI, NPI, PDQ- 39, PDSS, ESS, and the SCOPA-AUT scale. nUS consisted of uroflowmetry and ultrasound of the urinary tract with measurement of postvoid residual (PVR) urine volume. The quantitative parameters measured during the voiding phase included maximum flow rate (Qmax) and postvoid residual (PVR). Results: 45 PD patients (26 males and 19 females) were included (mean age at interview 62.1 ± 10.6; mean disease duration: 6.5 ± 4.3 years). PD patients had a mean SCOPA-AUT score of 14.1 ± 7.1. PD patients with urinary dysfunction, had higher SCOPA-AUT scale score, longer disease duration and an higher dosage of levodopa calculated as levodopa equivalent dose (LED). At least one clinical symptom of dysautonomia was found in all of our PD patients and urinary symptoms were the most common complain (92.8%). The mean Qmax was 17.9 ml/s (SD 9.1 ml/s), and the mean PVR was 33.7 ml (SD 68.7 ml), with no patient having a PVR>200 ml. Ultrasound documented possible causes of urinary disorders in 44% of patients (prostate hypertrophy was observed in 11 of 26 males). Conclusion: Our findings suggest that urinary symptoms and abnormal findings in nUS are common in PD patients. The autonomic dysfunction might be related to nigrostriatal degeneration, though urinary dysfunction in patients PD could be, especially in males,attributed to other conditions

Abstract

Introduction: To date, orally administered chemotherapy and biologic agents represent a significant percentage of all antineoplastic treatments in several types of cancer, which are most likely to increase in the near future. In this scenario, the issue of adherence and persistence to oral therapy is a key issue since poor compliance to oral antineoplastic treatments may negatively influence patients' clinical outcomes and, in turn, cause an increase in costs, number of hospitalizations and time spent in the hospital. Areas covered: The issue of adherence to new oral chemotherapeutic and/or biologic agents has not been deeply evaluated and data published in medical literature are quite scarce. Adherence is a multidimensional phenomenon, which may be influenced by patient-and health-care provider-related factors, anticancer therapy itself, education and socioeconomic aspects. Patients' selection plays, therefore, a key role in maximizing adherence and persistence to oral therapies. Treating health-care practitioners should first evaluate patient reliability to avoid prescribing oral treatments to patients with socioeconomic and medical conditions, which may predict poor adherence. Expert opinion: Adherence and persistence to new oral biologic agents, which are linked to several side effects and whose use is constantly widening, should represent a main endpoint of clinical research in the nearest future. © 2012 Informa UK, Ltd.

Abstract

Biopreservation by saccharides is a widely studied issue due to its scientific and technological importance; in particular, ternary amorphous protein-saccharide-water systems are extensively exploited to model the characteristics of the in vivo biopreservation process. We present here a differential scanning calorimetry (DSC) study on amorphous trehalose-water systems with embedded different proteins (myoglobin, lysozyme, BSA, hemoglobin), which differ for charge, surface, and volume properties. In our study, the protein/trehalose molar ratio is kept constant at 1/40, while the water/sugar molar ratio is varied between 2 and 300; results are compared with those obtained for binary trehalose-water systems. DSC upscans offer the possibility of investigating, in the same measurement, the thermodynamic properties of the matrix (glass transition, T(g)) and the functional properties of the encapsulated protein (thermal denaturation, T(den)). At high-to-intermediate hydration, the presence of the proteins increases the glass transition temperature of the encapsulating matrix. The effect mainly depends on size properties, and it can be ascribed to confinement exerted by the protein on the trehalose-water solvent. Conversely, at low hydration, lower T(g) values are measured in the presence of proteins: the lack of water promotes sugar-protein interactions, thus weakening the confinement effect and softening the matrix with respect to the binary system. A parallel T(den) increase is also observed; remarkably, this stabilization can reach ∼70 K at low hydration, a finding potentially of high biotechnological relevance. A linear relationship between T(g) and T(den) is also observed, in line with previous results; this finding suggests that collective water-trehalose interactions, responsible for the glass transition, also influence the protein denaturation.

Abstract

The purpose of this paper is the simulation of vehicle – pedestrian crash test, executed by Visual Nastran. The evaluation of the damage caused to pedestrian is obtained following the EURONCAP directive. The vehicle virtual model, DIMA, follows closely the characteristics of a saloon car belonging to the small cars category. A total of 16 simulation tests are conduced; each characterized by different initial conditions. The vehicle advances with several speeds, at constant speed and in braking. The anthropomorphic model Dummy impacts with the vehicle in lateral or perfectly frontal position with respect to the front of the vehicle.

Abstract

The purpose of this work is the achievement of an anthropomorphic model for the virtual simulation of pedestrian – vehicle crash test. “Dummy” construction in Visual Nastran room does possible the preparation of crash test with several vehicle models. Its three-dimensional geometry is implemented by video animation software, extracting the geometry element for element, and its characteristics are in keeping with EURONCAP protocol. Every bodily segment properties are determined using the reference material on the movement biomechanics. At last fall tests from three different heights are simulated for the “Dummy” validation, obtaining a reliable result.
