Abstract

Co-designing Infrastructures tells the story of a research programme designed to bring the power of engineering and technology into the hands of grassroots community groups, to create bottom-up solutions to global crises. Four projects in London are described in detail, exemplifying community collaboration with engineers, designers and scientists to enact urban change. The projects co-designed solutions to air pollution, housing, the water-energy-food nexus, and water management. Rich case-study accounts are underpinned by theories of participation, environmental politics and socio-technical systems. The projects at the heart of the book are grounded in specific settings facing challenges familiar to urban communities throughout the world. This place-based approach to infrastructure is of international relevance as a foundation for urban resilience and sustainability. The authors document the tools used to deliver this work, providing guidance for others who are working to deliver local technical solutions to complex social and environmental problems around the world. This is a book for engineers, designers, community organisers and researchers. Co-authored by researchers, it includes voices of community collaborators, their experiences, frustrations and aspirations. It explores useful theories about infrastructure, engineering and resilience from international academic research, and situates them in community-based co-design experience, to explain why bottom-up approaches are needed and how they might succeed.

Abstract

This handbook critically examines spaces of mental health and wellbeing across multiple, often intersecting, domains from green and blue spaces to lived and embodied spaces, creative spaces, work and home spaces, and institutional and post-institutional spaces. The Routledge Handbook on Spaces of Mental Health and Wellbeing features 45 chapters from leading international scholars who collectively interrogate the spatial dimensions of mental health and wellbeing from conceptual and experiential viewpoints. The ways in which these theoretical developments prompt a re-thinking of mental health and wellbeing as concepts is also discussed before presenting some highlights from the handbook’s five main sections – (1) green and blue spaces, (2) lived and embodied spaces, (3) creative spaces, (4) work and home spaces, and (5) institutional and post-institutional spaces. Abstract

The effective management of stress and burnout is a key priority in promoting medical student wellbeing. Psychosocial factors like hope, stress, burnout, locus of control, and intellectual humility as well as lifestyle factors like exercise, sleep, idle time, downtime, and caffeinated beverage intake may be important in protecting against the negative consequences of stress and burnout. The purpose of this cross-sectional study was to explore the relationships between lifestyle factors and five different psychosocial wellbeing indicators. We sent a voluntary, online survey to first- through fourth-year allopathic and osteopathic medical students (n = 205). We used validated psychometric scales and investigator-created lifestyle questions to measure the variables of interest. Hierarchical linear regressions were used to examine the effects of the different variables' associations with hope. We found that both exercise and sleep are potentially buffering the negative impact of high burnout on dispositional hope. We also found that having a high level of independence of intellect and ego may mitigate the negative effects of a more external locus of control on dispositional hope and may be protective of hope in people with higher stress. Future research can increase our understanding in this area by following medical students across time to control for any temporal effects on these variables and to determine if baseline characteristics can accurately predict future outcomes like burnout., Author(s): Ashten R. 28. Wellbeing in Secondary Education (WISE) Study to Improve the Mental Health and Wellbeing of Teachers: A Complex System Approach to Understanding Intervention Acceptability

Abstract

Teaching staff report poorer mental health and wellbeing than the general working population. Intervention to address this issue is imperative, as poor wellbeing is associated with burnout, presenteeism, and adverse student mental health outcomes. The Wellbeing in Secondary Education (WISE) intervention is a secondary school-based programme aimed at improving the mental health and wellbeing of teachers and students. There are three components: awareness-raising for staff; a peer support service delivered by staff trained in Mental Health First Aid (MHFA); and Schools and Colleges Mental Health First Aid (MHFA) training for teachers. A cluster randomised controlled trial with integrated process and economic evaluation was conducted with 25 secondary schools in the UK (2016-2018). The intervention was largely ineffective in improving teacher mental health and wellbeing. This paper reports process evaluation data on acceptability to help understand this outcome. It adopts a complex systems perspective, exploring how acceptability is a dynamic and contextually contingent concept. Data sources were as follows: interviews with funders (n = 3); interviews with MHFA trainers (n = 6); focus groups with peer supporters (n = 8); interviews with headteachers (n = 12); and focus groups with teachers trained in Schools and Colleges MHFA (n = 7). Results indicated that WISE intervention components were largely acceptable. Initially, the school system was responsive, as it had reached a 'tipping point' and was prepared to address teacher mental health. However, as the intervention interacted with the complexities of the school context, acceptability became more ambiguous. The intervention was seen to be largely inadequate in addressing the structural determinants of teacher mental health and wellbeing (e.g. complex student and staff needs, workload, and system culture). Future teacher mental health interventions need to focus on coupling skills training and support with whole school elements that tackle the systemic drivers of the problem.

Abstract

Objective: To evaluate the impact of a virtual registered nurse (ViRN) model on safety and care outcomes. ViRN is a telemedicine intervention that enables an experienced virtual nurse to assist the in-person care team in providing care to patients. Data Sources and Study Setting: Electronic health records data were utilized from the Mayo Clinic during the intervention (December 2020-November 2021) and historical periods (December 2018-November 2019). ViRN was implemented on general medical units at the Mayo Clinic Rochester. We used general medical units at the Mayo Clinic Arizona as the comparison group. Study Design: This study used a difference-in-differences design to evaluate the impact of ViRN compared to usual care on transfer to the intensive care unit (ICU), inpatient mortality, and length of stay (LOS). We used logistic regression for transfer to the ICU and inpatient mortality and negative binomial regression for LOS. We controlled for demographics, patient interaction with the health system, clinical characteristics, and admission characteristics. We clustered standard errors to account for patients who have multiple admissions during the study period. Principal Findings: There were no significant differences for transfer to the ICU (average marginal effect (AME) -0.08 percentage point [95% confidence interval (CI): -1.34, 1.18]), inpatient mortality (AME 0.43 percentage point [95% CI: -0.33, 1.18]), or LOS (AME -0.20 days [95% CI: -0.57, 0.17]). The findings were mostly consistent across the sensitivity analyses. Conclusions: Our results suggest that ViRN led to similar outcomes as usual care in general medical units. These findings support the potential to develop more advanced models of ViRN at the Mayo Clinic and the dissemination of the ViRN model to other systems. In the context of staffing shortages and other disruptions to the delivery of nursing care, it is critical to understand whether new models like ViRN provide nurse staffing alternatives without negatively affecting outcomes. Abstract

To meet the UK's 2050 net zero carbon targets, Whole Life Carbon Assessment (WLCA) is used to calculate the lifetime carbon emissions of building projects. This paper aims to compare different estate regeneration scenarios for meeting the interim 2030 benchmarks for the 2050 target. The research consists of a case study employing co-design and WLCA experiments, and impact evaluation surveys. The results of the WLCA demonstrate the lower operational impacts of fossil-fuel-free scenarios, and lower embodied and overall carbon emissions of retrofitting scenarios. To make an informed decision towards the future of the estates, different regeneration scenarios need to be studied, and the stakeholders should understand the carbon emissions of different materials and systems. The findings of this study can be used to compare regeneration schemes of other building types. Practical application: This study underscores the critical role of early-stage design decisions and engaging with the communities in minimising the carbon footprint of building projects and making informed decisions. The study reinforces the potential superiority of retrofitting over new builds, which serves as a clear call for the inclusion of retrofit options in the analysis of diverse regeneration and redevelopment scenarios. The study's outcomes also highlight the pivotal role of stakeholder engagement in comprehending the Global Warming Potential (GWP) of materials and systems, influencing the participatory development and evaluation of regeneration scenarios.
