8. Prevalence of asymptomatic Leishmania donovani infection and associated factors in Ethiopia: a systematic review and meta-analysis.

Abstract

Background: Visceral leishmaniasis is endemic in Ethiopia and caused by Leishmania donovani. Although the disease manifests with significant clinical variability, a substantial number of individuals are asymptomatic. These individuals can serve as reservoirs, complicating control efforts. However, comprehensive data on asymptomatic L. donovani infections in Ethiopia are lacking, highlighting the need for a systematic review and meta-analysis to consolidate evidence and understand the distribution and determinants of this infection. Methods: PRISMA guidelines followed and registered with PROSPERO (CRD42024531454). Systematically searched electronic databases and grey literature sources up to April 13, 2024. Original research articles in English considered. Statistical analyses performed using STATA version 16. Publication bias evaluated using funnel plots and Egger's regression tests. Pooled estimate of asymptomatic L. donovani infection derived using a random effects model. Study heterogeneity assessed using Chi-square (χ²)-based Q test (p < 0.1) and I² subgroup analysis and meta-regression conducted with significance set at p < 0.05. Result: A total of 1,288 articles were identified, with 11 studies met inclusion criteria. These studies, published between 2012 and 2024, reported data from 17 districts across six regions. Sample sizes ranged from 185 to 1,682, with a total of 7,288. Six types of laboratory testing methods were employed. Prevalence of asymptomatic L. donovani infection per individual study ranged from 0.9 to 15.8%, while district-level prevalence varied from 0 to 31.1%. The overall pooled estimate of asymptomatic L. donovani infection in Ethiopia was 9.0% (95% CI: 6.0-11.0%). The pooled estimate in the Amhara region was 9.0% (95% CI: 8.0-11.0%), compared to 7.0% (95% CI: 3.0-12.0%) in other regions. Living in a household with domestic animals (OR: 2.54; 95% CI: 1.43–3.64) and being male (OR: 2.22; 95% CI: 1.21–3.23) were significantly associated with higher asymptomatic L. donovani infection. Conclusion: A considerable number of asymptomatic L. donovani infections were reported in Ethiopia. Close contact with domestic animals and being male were identified as significant risk factors. Regular screening of people living in close contact with animals. Abstract

Background Ethiopia has a high burden of visceral leishmaniasis. Recently, there was a significant increase in cases in the South Omo Zone. This study aims to assess the prevalence of Leishmania donovani infection and its associated factors. Methods A household-based cross-sectional study was carried out in January 2023 in the South Omo Zone in Ethiopia. Dried blood spot samples were collected from 382 randomly selected study participants. Direct agglutination test (DAT) and kinetoplast DNA real-time PCR tests were performed to detect L. donovani infection. Participants' sociodemographic, clinical and risk factors for L. donovani infection data were collected using questionnaires. Bivariate and multivariate logistic regressions were used to analyze the data. Febrile cases were checked for malaria with a multiplex PCR assay. Results Overall prevalence of L. donovani infection among the sampled population was 32.5% (n=124), of which 41.1% (n=51) was detected by PCR, 33.9% (n=42) by DAT and 25.0% (n=31) by both tests. The majority of the positives were from the Logira (28.2%; n=35) and Dilbayne (29.0%; n=36) villages. Participants residing in Logira (adjusted OR [AOR]: 5.80; 95% CI 1.85 to 18.15) and Dilbayne (AOR: 3.38; 95% CI 1.15 to 9.96) villages and owning cows (AOR: 2.31; 95% CI 1.03 to 5.15) showed an association with Leishmania infection. Plasmodium falciparum was detected in 3.4% (n=2) of 59 febrile participants. Conclusions The prevalence of L. donovani infection in the South Omo Zone is high. Further research on the role of cows in the transmission cycle is needed to design the best strategy to control Leishmania infection in the South Omo Zone. Abstract

Background: Street women are women, who make their living on the streets by begging, sleeping in the streets, or on the sides of roads. They are the most marginalized and neglected segment of society, with little access to health care, including modern contraception, and a lack of knowledge about health services, particularly in Ethiopia. Therefore, this study aimed to examine modern contraceptive utilization and associated factors among street women. Methods: A systematic review and meta-analysis were conducted on observational community-based studies published from January 2010 up to February 1, 2023. PubMed, Google Scholar, HINAR, Scopus, Web of Sciences, and grey literature were used to search primary studies. We used Microsoft Excel for data entry and extracting data. STATA-17 statistical software was used to analyze the data as well and I2 tests evaluated study heterogeneity. The pooled utilization of the modern contraceptive method was predicted using a random-effect model. Results: A total of eight studies were included. The pooled prevalence of modern contraceptive utilization was 51.89% (95% CI: 40.89–62.9). Being married (OR = 4.22, 95%CI, 2.75–6.49); facing a history of sexual assault/rape at a street life (OR = 3.59, 95%CI, 2.46–5.23); age between 25–35 years (OR = 3.27, 95%CI, 1.73–6.18), getting advise from the health professionals (OR = 6.23, 95%CI, 1.44–27.07), having a history of pregnancy in street life (OR = 1.90, 95%CI, 1.28–2.81) and no more children wanted (OR = 2.47, 95%CI, 1.52–5.00) were significantly associated with modern contraceptive utilization. Conclusions: The pooled modern contraceptive utilization was suboptimal. Every concerned body or stakeholder should give more consideration to their lifestyles and living conditions by raising awareness and providing access to contraception. [ABSTRACT FROM AUTHOR]

Abstract

Background: The World Health Organization recommends that all postpartum women be examined for resumed sexual activity. Despite this, postpartum sexual health education and health promotion are not adequately incorporated into current maternal healthcare systems in low- and middle-income nations. There were variations in the prevalence and variables associated with early postpartum sexual intercourse across several studies. Objectives: The purpose of this systematic review and meta-analysis was to evaluate the pooled prevalence and associated factors for early postpartum sexual intercourse in sub-Saharan African countries. Data Sources and Methods: Primary studies were identified using international databases such as Scopus, PubMed, Google Scholar, Embase, and CINAHL. The Newcastle‒Ottawa Scale quality assessment tool was used to evaluate the quality and strength of the included studies. STATA version 17 was used for the meta-analysis. The heterogeneity of the studies and publication bias was examined using I 2 statistics and Egger's regression test. Subgroup analysis decreased the underlying heterogeneity based on the study years and sample sizes. Results: Seventeen primary articles were included in the meta-analysis with 8507 study participants. The pooled prevalence of early postpartum sexual resumption in sub-Saharan Africa was 39.41% (95% CI: 31.55%–47.27%). Primiparous (OR = 3.32; 95% CI: 2.26–5.90), spontaneous vaginal delivery (OR = 5.98; 95% CI: 1.74–20.51), formula feeding (OR = 2.24; 95% CI: 1.46–3.44), family planning (OR = 2.91; 95% CI: 1.89–4.49), husband pressure (OR = 4.99; 95% CI: 1.38–18.05), have no formal education (OR = 2.36; 95% CI: 1.49–3.76), and monogamy (OR = 4.18; 95% CI: 2.27–7.69) were significantly associated with early postpartum sexual resumption. Conclusion: Four out of 10 women had returned to sexual activity within 6 weeks of giving birth. This suggests that a large proportion of women are more vulnerable to unwanted pregnancies and sexual health problems. Sexual health education and counseling should be incorporated into standard postpartum care to increase contraceptive use and delay unplanned pregnancies. Abstract

BACKGROUND: Health-care professions are among the highly exposed group of population affected by COVID-19 pandemic and identified as the target population to get vaccination against the spread of the infection. However, vaccine reluctance is still high, which may negatively affect the efforts to prevent the spread of the infection. Hence, this study was aimed to assess acceptance of COVID-19 vaccine and associated factors among health professionals working in Hospitals of South Gondar Zone, Northwest Ethiopia. METHODS: Institutional-based cross-sectional study was performed among 422 health professionals working in Hospitals of South Gondar Zone, Northwest Ethiopia, from March 1 to 30, 2021. Study participants were selected through simple random sampling techniques. Data were collected through a self-administered questionnaire. Multivariable logistic regression analysis was performed to identified factors associated with acceptance of COVID-19 vaccine with a p-value < 0.05 at 95% CI considered as statistically significant. RESULT: Overall, 45.3% of health professionals accepted COVID-19 vaccine. Age (AOR: 2.55; 95% CI: 1.32–4.92), being male (AOR: 1.729; 95% CI: 1.32–2.34), had higher risk of COVID-19 (AOR: 1.74, 95% CI: 1.00–3.02), and positive attitude (AOR: 3.26, 95% CI: 2.14–4.96) were found to be statistically significant with the acceptance of COVID-19 vaccine. CONCLUSION: The acceptance of COVID-19 vaccine among health professionals was significantly low. Participant’s age, sex, high risk of COVID-19, and attitude toward COVID-19 vaccine were significantly associated with vaccine acceptance. Therefore, the government with respective stakeholders should emphasize addressing the concern of the health professionals and increase attitudes regarding COVID-19 vaccine to scale up vaccine acceptance.

Abstract

Background: Early case detection and prompt treatment are important malaria control and elimination strategies. However, the emergence and rapid spread of drug-resistant strains present a major challenge. This study reports the first therapeutic efficacy profile of pyronaridine-artesunate against uncomplicated Plasmodium falciparum in Northwest Ethiopia. Methods: This single-arm prospective study with 42-day follow-up period was conducted from March to May 2021 at Hamusit Health Centre using the World Health Organization (WHO) therapeutic efficacy study protocol. A total of 90 adults ages 18 and older with uncomplicated falciparum malaria consented and were enrolled in the study. A standard single-dose regimen of pyronaridine-artesunate was administered daily for 3 days, and clinical and parasitological outcomes were assessed over 42 days of follow-up. Thick and thin blood films were prepared from capillary blood and examined using light microscopy. Haemoglobin was measured and dried blood spots were collected on day 0 and on the day of failure. Results: Out of 90 patients, 86/90 (95.6%) completed the 42-day follow-up study period. The overall PCR-corrected cure rate (adequate clinical and parasitological response) was very high at 86/87 (98.9%) (95% CI: 92.2–99.8%) with no serious adverse events. The parasite clearance rate was high with fast resolution of clinical symptoms; 86/90 (95.6%) and 100% of the study participants cleared parasitaemia and fever on day 3, respectively. Abstract

Introduction: Transactional sex is casual sex between two people to receive material incentives in exchange for sexual favors. Transactional sex is associated with negative consequences, which increase the risk of sexually transmitted diseases, including HIV/AIDS, unintended pregnancy, unsafe abortion, and physiological trauma. In Sub-Saharan Africa, several primary studies have been conducted in various countries to examine the prevalence and associated factors of transactional sex among women. These studies had great discrepancies and inconsistent results. Hence, this systematic review and meta-analysis aimed to synthesize the pooled prevalence of the practice of transactional sex among women and its associated factors in Sub-Saharan Africa. Method: Data source: PubMed, Google Scholar, HINARI, the Cochrane Library, and grey literature were searched from March 6 to April 24, 2022, and included studies conducted from 2000 to 2022. The pooled prevalence of transactional sex and associated factors was estimated using Random Effect Model. Stata (version 16.0) was used to analyze the data. The I-squared statistic, a funnel plot, and Egger's test were used to check for heterogeneity and publication bias, respectively. A subgroup analysis was done based on the study years, source of data, sample sizes, and geographical location. Results: The pooled prevalence of transactional sex among women in Sub-Saharan Africa was 12.55% (9.59%–15.52%). Early sexual debut (OR = 2.58, 95% CI: 1.56, 4.27), substance abuse (OR = 4.62, 95% CI: 2.62, 8.08), history of sexual experience (OR = 4.87, 95% CI: 2.37, 10.02), physical violence abuse (OR = 6.70, 95% CI: 3.32, 13.53), orphanhood (OR = 2.10, 95% CI: 1.27, 3.47), and sexual violence abuse (OR = 3.76, 95% CI: 1.08, 13.05) were significantly associated with transactional sex. Conclusion: The prevalence of transactional sex among women in sub-Saharan Africa was high. Alcohol consumption, substance abuse, early sex debuts, having a history of sexual experiences, physical violence, and sexual violence increased the practice of transactional sex.
