Abstract

Objective: The aim of the Medical Academy Waldbreitbach is to connect individual and organisational requirements in order to promote an appropriate and multi-locational development of medical competency in the face of the continuously evolving challenges of clinical practice. Integral processes in this are the reduction of organisational learning barriers and the successive integration of competency-oriented learning events in the structures of personnel and organisational development. The modular system for the further development of doctors’ skills serves here as a supplementary and recommendation system for both existing curricula and those defined by regulatory organisations and professional associations.Methods: The Medical Academy’s modular system has a two-dimensional structure. In addition to the axis of biography orientation, the model orients itself around issues relating to the needs of a doctor in any individual professional position, as well as with whom he comes into contact and where his primary challenges lie. In order to achieve better integration in day-to-day routine and a needs-specific orientation of content, the modular system provides a combination of “one, two or three day and two- three- or four-hour training units” depending upon the topic. The transfer of experiential knowledge with the aid of practical exercises is a central element of the didactic model.Results: Through the combined use of summative and formative assessment, the significance of a dialogue-orientated approach in both planning and in the organisational process was highlighted. In feedback discussions and quantitative evaluation sheets, participants identified in particular cross-generational knowledge sharing as a central element for the development of personal values alongside the interdisciplinary transfer of knowledge. The combination of specialist and interdisciplinary topics, for example on team processes or communication, is frequently emphasised, indicating that this had been taught insufficiently and impractically during medical school. Longitudinal evaluations of continuous course units support this, so that the reinforcement of informal learning processes through feedback and exchange of experience is established as an effective and integral learning pattern within the modular system.Conclusion: The of the modular system of the Medical Academy Waldbreitbach – as an institution of the Marienhaus Hospitals Ltd. – is to develop the knowledge, ability and motivation of doctors both individually and professionally. Here, an equally high demand is placed upon the advancement of individual dispositions, attitudes and values, as well as on specialised topics, in order to promote/develop solutions-based and overall medical activity.

Abstract

Shock is a life-threatening condition. This study evaluated if sublingual microcirculatory perfusion on admission is associated with 30-day mortality in older intensive care unit (ICU) shock patients. This trial prospectively recruited ICU patients (≥ 80 years old) with arterial lactate above 2 mmol/L, requiring vasopressors despite adequate fluid resuscitation, regardless of shock cause. All patients received sequential sublingual measurements on ICU admission (± 4 h) and 24 (± 4) hours later. The primary endpoint was 30-day mortality. From September 4th, 2022, to May 30th, 2023, 271 patients were screened, and 44 included. Patients were categorized based on the median percentage of perfused small vessels (sPPV) into those with impaired and sustained microcirculation. 71% of videos were of good or acceptable quality without safety issues. Patients with impaired microcirculation had significantly shorter ICU and hospital stays (p = 0.015 and p = 0.019) and higher 30-day mortality (90.0% vs. 62.5%, p = 0.036). Cox regression confirmed the independent association of impaired microcirculation with 30-day mortality (adjusted hazard ratio 3.245 (95% CI 1.178 to 8.943, p = 0.023). Measuring sublingual microcirculation in critically ill older patients with shock on ICU admission is safe, feasible, and provides independent prognostic information about outcomes. 31. Comparison of nanomechanical properties of in vivo and in vitro keratin 8/18 networks

Abstract

In our work we compare the mechanical properties of the extracted keratin cytoskeleton of pancreatic carcinoma cells with the mechanical properties of in vitro assembled keratin 8/18 networks. For this purpose we use microrheology measurements with embedded tracer beads. This method is a suitable tool, because the size of the beads compared to the meshsize of the network allows us to treat the network as a continuum. Observing the beads motion with a CCD-High-Speed-Camera then leads to the dynamic shear modulus. Our measurements show lower storage moduli with increasing distance between the rim of the nucleus and the bead, but no clear tendency for the loss modulus. The same measurement method applied to in vitro assembled keratin 8/18 networks shows different characteristics of storage and loss moduli. The storage modulus is one order of magnitude lower than that of the extracted cytoskeleton and the loss modulus is higher. Abstract

A detailed understanding of the interface between living cells and substrate materials is of rising importance in many fields of medicine, biology and biotechnology. Cells at interfaces often form epithelia. The physical barrier that they form is one of their main functions. It is governed by the properties of the networks forming the cytoskeleton systems and by cell-to-cell contacts. Different substrates with varying surface properties modify the migration velocity of the cells. On the one hand one can change the materials composition. Organic and inorganic materials induce differing migration velocities in the same cell system. Within the same class of materials, a change of the surface stiffness or of the surface energy modifies the migration velocity, too. For our cell adhesion studies a variety of different, homogeneous substrates were used (polymers, bio-polymers, metals, oxides). In addition, an effective lithographic method, Polymer Blend Lithography (PBL), is reported, to produce patterned Self-Assembled Monolayers (SAM) on solid substrates featuring two or three different chemical functionalities. This we achieve without the use of conventional lithography like e-beam or UV lithography, only by using self-organization. These surfaces are decorated with a Teflon-like and with an amino-functionalized molecular layer. The resulting pattern is a copy of a previously created self-organized polymer pattern, featuring a scalable lateral domain size in the sub-micron range down below 100 nanometers. The resulting monolayer pattern features a high chemical and biofunctional contrast with feature sizes in the range of cell adhesion complexes like e.g. focal adhesion points., Comment: 43 pages, 15 figures
