Background: Diabetes Mellitus is a common comorbidity in older persons population and is responsible for major macrovascular as well as microvascular complications. Diabetic Neuropathy occurs in 50% of patients with long standing type 2 diabetes mellitus which causes a significant disability in older persons. Diagnosis of neuropathy is often delayed till it becomes advanced and puts the foot at risk for diabetic ulcer and diabetic foot. In addition, altered proprioception causes imbalance and increased risk of falls. There are no specific guidelines for screening diabetic neuropathy. The aim of this study is to screen diabetics for signs of neuropathy and study the prevalence of diabetic neuropathy in geriatric population. Neurotouch is a portable and handy point of care device which combines four parameters of quantitative sensory testing in one device. It gives a real time display of results along with data transfer facility. It can perform tactile sensation threshold, vibration, thermal testing threshold and skin temperature measurement. Type-Cross-sectional observational study Inclusion criteria: Older adults more than 60 years with type 2 diabetes mellitus of duration =5 years Exclusion criteria: • Patient with CKD, EGFR <30 mL/min/1.73m² • Patient with any other proven cause of neuropathy • On drugs known to cause neuropathy Sample Size: 50 Aim: To screen older adults with diabetes mellitus for diabetic neuropathy To educate individuals at risk or with neuropathy regarding foot care and glycemic control to prevent further progression. Methods: Detailed history and clinical profile of individuals along with clinical examination with specific emphasis on foot examination was done. The device uses 6 points (1st toe hallux, 1st/3rd/5th metatarsal head, medial aspect of foot arch, posterior aspect of heel) in sole of each foot and tested for tactile sensation using monofilament, vibration perception, hot and cold perception, skin temperature. Foot examination is done for corn, callus, ulcer, infection, cracks/fissures, toe deformities, ankle reflex and peripheral pulse. Data from the device is transferred to the phone and report is generated. Results: Percentage of patients with low, moderate and high risk for peripheral neuropathy. Conclusion: The most common neuropathy encountered with diabetes is diabetic symmetrical peripheral neuropathy. It is associated with factors like poor glycemic control, obesity, dyslipidemia, alcohol and smoking and increasing age. There is no FDA approved management or treatment of diabetic neuropathy. Prevention is of paramount importance in such cases. About 50% of diabetics with peripheral neuropathy are asymptomatic and hence emphasis on detailed clinical history and examination is necessary for early detection. [ABSTRACT FROM AUTHOR]
