Constipation is a common gastrointestinal complaint among 16% of the world's population. Several conventional treatments are recommended; however, these sometimes do not provide satisfactory results for many patients or cause unpleasant side effects. In this context, Ayurvedic medicine can be an alternative, cost-effective and satisfactory treatment for constipation. Hence, this clinical study aimed to evaluate the efficacy and safety of an Ayurvedic PetSaffa formulation in subjects with functional constipation. Design, Setting, Participants and Intervention: This open-label, non-randomized clinical study was conducted to evaluate the safety and efficacy of an Ayurvedic PetSaffa formulation in healthy volunteers with functional constipation. Subjects were selected based on inclusion and exclusion criteria. Participants received PetSaffa granules for 21 days. Outcome parameters of the study, including frequency of bowel movements, constipation score, constipation symptoms, gas, acidity, and serum level of SGPT, were evaluated at screening and after 21 days of treatment. Endophytes are the microorganisms that are present in living tissue of various plant parts (roots, fruits, stem, seed, leaf etc,). Endophytic microorganisms are good source of antibiotics. Endophytic antagonists were isolated from the roots of healthy acid lime plants collected from major acid lime growing areas of Andhra Pradesh. A total of 8 fungal and 10 bacterial endophytic antagonists were isolated. The antagonists were further subjected to preliminary screening, out of which only 6 endophytic fungal antagonists (EFA 1-6) and 8 endophytic bacterial antagonists (EBA 1-8) isolates showed good inhibitory effect on radial growth of Fusarium solani causing dry root rot in acid lime in vitro. Among them the one of the best fungal and bacterial antagonists that were found to be extremely efficient against Fusarium solani in dual culture assay were selected for further molecular identification. The BLAST results revealed that one of the fungal isolate had shown 100% similarity with Aspergillus fumigatus and one of the bacterial isolate had shown 95.56% similarity with Pseudomonas aeruginosa. 8. The phenomenon of yoga in the imagination of Turkish nursing students: 'The way to place goodness in the heart'

Abstract Background The aim of the research is to shed light on the experiences of a group of nursing students enrolled in a yoga elective course who practiced yoga regularly for 14 weeks, regarding yoga and the phenomenon of doing yoga, with a qualitative approach. Methods This qualitative study was conducted at a public university in Izmir, Turkey. The study sample consisted of 61 students enrolled in the 1st-year yoga course at the Department of Nursing, Faculty of Health Sciences. Except for two students, 59 of them participated in the study. Participants attended a face-to-face yoga course once a week for 1.5 h over a 14-week period. Each session comprised 30 min of theoretical instruction and 60 min of practice. In data collection, an interview form containing five questions was used to understand nursing students' experiences and thoughts about practicing yoga. In addition, A4 sized papers in different colors were presented to the participants. The participants were asked to draw and/or cut a shape by choosing the paper in the color that most evokes yoga. Then they were asked to explain why they chose this color and why they drew this shape. Participants were given one hour. After the data were collected, the pictures drawn by the participants and their descriptions of their drawings were transferred to the computer and included in the analysis. Qualitative data were hand-coded by the researchers. Within in-vivo coding, code names were formed from the participants' expressions. The findings analyzed by content analysis were interpreted with the literature under the themes by presenting quotations. Results It was determined that the participants used nature figures (sun, cloud, tree, sky, flower) (f = 75), people doing yoga (f = 12), and sound, light and other figures (bird sound, wave sound, candle, light bulb, traffic light, heart, eye, left key, peace, swing, India, circle, etc.) (f = 29) in the shapes they drew and cut out on A4 sized papers in different colors to describe their experiences and thoughts about practicing yoga. The participants mostly chose blue-green-yellow colors (n = 41). With qualitative question analysis, a list of codes was created from the answers given by the students to the questions for the phenomenon of “doing yoga” (number of codes = 98). After the analysis of the data, four categories were reached. The category names and frequency numbers explaining the phenomenon of practicing yoga were distributed as “symbol of health and serenity (f = 345)”, “the way to place goodness in your heart (f = 110)”, “the most effective way to meet the self (f = 93)” and “no guarantee of relaxation (f = 71)”, respectively. Conclusions The results indicate that yoga is perceived by nursing students as a multifaceted practice that evokes a range of emotional and physical responses. Most participants associate yoga with symbols of health, serenity, and self-awareness, often using natural elements and blue-green-yellow the colors to represent these feelings. However, there is also recognition that yoga may not guarantee relaxation for everyone, as some students reported difficulties in meditation, physical discomfort, and challenges in focusing. The study highlights the variability in how individuals experience yoga, emphasizing both its positive effects on well-being and the potential challenges in practice. These findings suggest that while yoga is widely valued for its calming and health-promoting benefits, it may not be universally effective in achieving relaxation or mindfulness.

İklim değişikliği, aşırı yağış/sıcaklık, taşkın veya kuraklık gibi ekstrem olaylara sebebiyet verebilmektedir. Bu çalışmada ülkemizin önemli havzalarından biri olan Küçük Menderes Havzasının uzun vadeli bölgesel eğilimlerinin yönünü belirlemek için Yenilikçi Şen (ITA) ve Yenilikçi Poligon Trend (IPTA) Analizi yöntemleri kullanılmıştır. 6 istasyonun (Kuşadası, İzmir Bölge, Çeşme, Ödemiş, Seferihisar ve Selçuk) 1972-2023 periyodu aylık ortalama sıcaklık ve toplam yağış verileri çalışmada analiz edilmiştir. Bu verilerin homojenliği Buishand, Pettitt, Standard Normal Homogeneity Test (SNHT) ve von Neumann Ratio Test yöntemleri ile sınanmıştır. ITA yönteminde veriler düşük, orta ve yüksek olarak sınıflandırılmış ve grafiklere %1,%2 ve %3 eğilim çizgileri çizilmiştir. Toplam yağışlar incelendiğinde orta sınıfta trend gözlenmezken, yüksek sınıfta ortalama %0,33 oranında artan trend gözlenmiştir. Ortalama sıcaklık verileri incelendiğinde ise düşük, orta ve yüksek sınıfta sırasıyla ortalama %0,92, %1,67 ve %2,42 oranında artan trend gözlenmiştir. IPTA yönteminde ise istasyonların ay bazında trend uzunlukları, eğimleri ve yönleri bulunmuştur. Sıcaklık verileri hemen hemen tüm istasyonlarda artan trend bölgesi içinde kalmıştır. Yağış verilerindeyse dinamik bir tabloyla karşılaşılmaktadır. Analizler sonucunda ilerleyen periyotta hem sıcaklık hem de yağışlar için artan trend gözlenmiştir.

10. Retrospective analysis of rapid drug desensitization with biologic agents: A single center experience

Abstract Background Following the increased use of biological agents, a subset of patients experiences hypersensitivity reaction (HSR). We reported our experience with rapid drug desensitization (RDD) to nine biologics (rituximab, infliximab, cetuximab, trastuzumab, pertuzumab, nivolumab, brentuximab, tocilizumab and filgrastim) and identified risk factors for breakthrough reactions (BTRs). Method This was a retrospective review (2013–2022) of patients with immediate HSRs to biological agents. Initial HSRs were classified as grade 1, 2, or 3 in their severity. Skin prick tests (SPT)/intradermal tests (IDT) were performed using implicated agents. The phenotypes of HSRs were defined as Type I, cytokine‐release syndrome (CRS), mixed reactions (cytokine‐release + type I) based on history, clinical presentations and skin tests with implicated biologicals. A 12‐step RDD protocol was used. Results The study comprised 45 patients (F/M: 31/14, median age: 55 (range: 20–69)). Majority of the patients reacted at the first infusion (n: 29/45, 64.4%). The majority of initial HSRs were grade 3 (n: 24/45, 53.3%) and grade 2 (n: 21/45, 46.6%); none were grade 1. Initial reactions were presented as type I (n: 20/45, 44.4%), CRS (n: 12/45, 26.6%) and mixed (n: 13/45, 28.8%). A total of 258 RDDs were performed and 98.4% of them were completed successfully. BTRs occurred in 36/258 (13.9%) infusions of RDDs. There was no significant association between the BTRs and age, drug cycle, SPT and IDT positivity, gender, comorbidities, or atopy. Conclusion In our experience, 98.4% of 258 RDDs to biologics were successfully completed; RDD was safe and effective for our population.

BACKGROUND: In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS: We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS: A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS: This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo. Instrumented battery cells (i.e. those containing sensors) and smart cells (with integrated control and communication circuitry) are essential for the development of the next-generation battery technologies, such as Sodium-ion Batteries (SIBs). The mapping and monitoring of parameters, for example the quantification of temperature gradients, helps improve cell designs and optimise management systems. Integrated sensors must be protected against the harsh cell electrolytic environment. State-of-the-art coatings include the use of Parylene polymer (our reference case). We applied three new types of coatings (acrylic, polyurethane and epoxy based) to thermistor arrays mounted on flexible printed circuit board (PCBs). We systematically analyse the coatings: (i) PCB submersion within electrolyte vials (8 weeks); (ii) analysis of sample inserted into coin cell; (iii) analysis of sensor and cell performance data for 1Ah pouch SIBs. Sodium-based liquid electrolyte was selected, consisting of a 1 M solution of sodium hexafluorophosphate (NaPF _6 ) dissolved in a mixture of ethylene carbonate and diethylene carbonate in a ratio of 3:7 (v/v%). Our novel experiments revealed that the epoxy based coated sensors offered reliable temperature measurements; superior performance observed compared to the Parylene sensors (erroneous results from one sample were reported, under 5 d submersed in electrolyte). Nuclear magnetic resonance (NMR) spectroscopy revealed in the case of most coatings tested, formation of additional species occurred during exposure to the different coatings applied to the PCBs. The epoxy-based coating demonstrated resilience to the electrolytic-environment, as well as minimal effect on cell performance (capacity degradation compared to unmodified-reference, within 2% for the coin cell, and within 3.4% for pouch cell). The unique methodology detailed in this work allows sensor coatings to be trialled in a realistic and repeatable cell environment. This study demonstrated for the first time that this epoxy-based coating enables scalable, affordable, and resilient sensors to be integrated towards next-generation Smart SIBs.

Abstract Background Severe inflammation and one or more extrapulmonary organ dysfunctions have been reported and this clinical picture is defined as "multisystem inflammatory syndrome in adults" (MIS-A) in severe coronavirus disease-2019 (COVID-19). We aimed to determine the effect of LDH/lymphocyte ratio (LLR) on the development of MIS-A. Methods The data of 2333 patients were retrospectively analyzed. Results MIS-A rate was found to be 9.9% and MIS-A related mortality was 35.3%. LRR level above 0.24 was found to predict MIS-A development with 70% sensitivity and 65.2% specificity. The risk of MIS-A development was found to be 3.64 times higher in those with LRR levels above 0.24 compared to those with 0.24 and below. In patients with MIS-A, LRR level above 0.32 predicts mortality with 78% sensitivity and 70% specificity. Conclusions Early detection of MIS-A with high sensitivity and specificity in a practical ratio is very important in terms new studies.

Abstract Background Incontinentia pigmenti (IP) is a rare X-linked dominant disorder affecting the skin and other ectodermal tissues that is caused by a mutation of the IKBKG/NEMO gene. Case presentation We describe a Turkish family with IP. Sanger sequencing was performed in our patient with IP, and we identified DNA variant c.172_173delAA (p.Asn58SerfsTer79) in IKBKG. We found the same mutation in the patient's mother and grandmother. Conclusion Our report expands the mutation spectrum in this disorder and provides valuable information on the importance of the IKBKG. Our study shows that confirmation of the mutation analysis of IP in the suspected cases is necessary for future planning pregnancies.

IntroductionIn the last decades, we have seen a rapid increase in the prevalence of allergic diseases such as asthma, allergic rhinitis, atopic dermatitis, and food allergies. The environmental changes caused by industrialization, urbanization and modernization, including dramatic increases in air pollutants such as particulate matter (PM), diesel exhaust, nitrogen dioxide (NO2), ozone (O3), alarming effects of global warming, change and loss of biodiversity, affect both human health and the entire ecosystem.ObjectiveIn this review, we aimed to discuss the effects of the external exposome on epithelial barriers and its relationship with the development of allergic diseases by considering the changes in all stakeholders of the outer exposome together, in the light of the recently proposed epithelial barrier hypothesis.MethodTo reach current, prominent, and comprehensive studies on the subject, PubMed databases were searched. We included the more resounding articles with reliable and strong results.ResultsExposure to altered environmental factors such as increased pollution, microplastics, nanoparticles, tobacco smoke, food emulsifiers, detergents, and household cleaners, and climate change, loss and change in microbial biodiversity, modifications in the consumption of dietary fatty acids, the use of emulsifiers, preservatives and the decrease in the antioxidant content of the widely consumed western diet may disrupt the epithelial barriers of the skin, respiratory and gastrointestinal tracts, making us more vulnerable to exogeneous allergens and microbes. Epithelial cell activation, microbial dysbiosis and bacterial translocation disrupt the immune balance and a chronic Th2 inflammation ensues.ConclusionDramatic increases in air pollution, worrisome effects of global warming, dysbiosis, changing dietary habits and the complex interactions of all these factors affect the epithelial barriers and local and systemic inflammation. We want to draw attention to the emerging health effects of environmental changes and to motivate the public to influence government policies for the well-being of humans and the nature of the earth and the well-being of future generations.

A study was conducted in Goalpara District of Assam in order to find out the difference in the level of participation of the tribal and non-tribal farm women in animal husbandry activities and also to see if there was any relationship of their socio-economic traits with the level of participation in animal husbandry activities. Out of the 8 development blocks in the district, two viz. Matia and Lakhipur blocks were randomly selected and from each of these two blocks 20 Tribal and 20 non-tribal farm women were randomly selected, thus making the sample size 80.Data were collected from the respondents individually by the researchers by a pre-tested, reliable and valid interview schedule. These data were processed, compiled and subjected to statistical analysis like frequency, mean, standard deviation, t-test, correlation and regression. The findings revealed that the non-tribal farm women had significantly higher level of participation in animal husbandry activities than their tribal counterparts. Out of the six socioeconomic traits only land holding was found to have negative correlation with level of participation in animal husbandry activities. On the other hand, mass media exposure, land holding, livestock units and family size had significant influence on the level of participation in animal husbandry activities. These six variables together could predict about 93.00 per cent of the variation in the level of participation in animal husbandry activities. [ABSTRACT FROM AUTHOR]

Hypoxia ischemia (HI)-related brain injury is the major cause of long-term morbidity in neonates. One characteristic hallmark of neonatal HI is the development of reactive astrogliosis in the hippocampus. However, the impact of reactive astrogliosis in hippocampal damage after neonatal HI is not fully understood. In the current study, we investigated the role of Na +/H+ exchanger isoform 1 (NHE1) protein in mouse reactive hippocampal astrocyte function in an in vitro ischemia model (oxygen/glucose deprivation and reoxygenation, OGD/REOX). 2 h OGD significantly increased NHE1 protein expression and NHE1-mediated H+ efflux in hippocampal astrocytes. NHE1 activity remained stimulated during 1-5 h REOX and returned to the basal level at 24 h REOX. NHE1 activation in hippocampal astrocytes resulted in intracellular Na+ and Ca2+ overload. The latter was mediated by reversal of Na+/Ca2+ exchange. Hippocampal astrocytes also exhibited a robust release of gliotransmitters (glutamate and pro-inflammatory cytokines IL-6 and TNFα) during 1-24 h REOX. Interestingly, inhibition of NHE1 activity with its potent inhibitor HOE 642 not only reduced Na+ overload but also gliotransmitter release from hippocampal astrocytes. The noncompetitive excitatory amino acid transporter inhibitor TBOA showed a similar effect on blocking the glutamate release. Taken together, we concluded that NHE1 plays an essential role in maintaining H + homeostasis in hippocampal astrocytes. Over-stimulation of NHE1 activity following in vitro ischemia disrupts Na+ and Ca2+ homeostasis, which reduces Na+-dependent glutamate uptake and promotes release of glutamate and cytokines from reactive astrocytes. Therefore, blocking sustained NHE1 activation in reactive astrocytes may provide neuroprotection following HI. © 2014 Cengiz et al.

Background: The bumetanide (BMT)-sensitive Na+-K+-2Cl- cotransporter isoform 1 (NKCC1) maintains cell volume homeostasis by increasing intracellular K+ and Cl- content via regulatory volume increase (RVI). Expression levels of NKCC1 positively correlate with the histological grade and severity of gliomas, the most common primary adult brain tumors, and up-regulated NKCC1 activity facilitates glioma cell migration and apoptotic resistance to the chemotherapeutic drug temozolomide (TMZ). However, the cellular mechanisms underlying NKCC1 functional up-regulation in glioma and in response to TMZ administration remain unknown.Methods: Expression of NKCC1 and its upstream kinases With-No-K (Lysine) kinase 1 (WNK1) and oxidative stress-responsive kinase-1 (OSR1) in different human glioma cell lines and glioma specimens were detected by western blotting and immunostaining. Live cell imaging and microchemotaxis assay were applied to record glioma cell movements under different treatment conditions. Fluorescence indicators were utilized to measure cell volume, intracellular K+ and Cl- content to reflect the activity of NKCC1 on ion transportation. Small interfering RNA (siRNA)-mediated knockdown of WNK1 or OSR1 was used to explore their roles in regulation of NKCC1 activity in glioma cells. Results of different treatment groups were compared by one-way ANOVA using the Bonferroni post-hoc test in the case of multiple comparisons.Results: We show that compared to human neural stem cells and astrocytes, human glioma cells exhibit robust increases in the activation and phosphorylation of NKCC1 and its two upstream regulatory kinases, WNK1 and OSR1. siRNA-mediated knockdown of WNK1 or OSR1 reduces intracellular K+ and Cl- content and RVI in glioma cells by abolishing NKCC1 regulatory phospho-activation. Background: COVID-19 (Coronavirus Disease-2019) is a pandemic disease, infecting more than 26.5 million people. Since there is no specific and effective treatment; early diagnosis and optimal isolation of the patient are of vital importance. Real-time polymerase chain reaction-based (RT-PCR) analyses do not achieve sufficient sensitivity in the diagnosis of the disease. Methods: The data from 2217 patients diagnosed as COVID-19 between March 2020 and June 2020 and hospitalized or discharged with home isolation were retrospectively analyzed. Demographic data, comorbidities, PCR results, initial computed tomography (CT), laboratory values, Lactate Dehydrogenase (LDH)/Lymphocyte ratio, initial treatments and last status were recorded. The diagnostic sensitivity of LDH/Lymphocyte ratio, which is the main purpose of the study, was analyzed statistically. Results: In order to test the effectiveness of LDH/Lymphocyte ratio for COVID-19 for diagnostic purposes, CT results were considered as gold standard. The area under the curve (AUC) was found to be 0.706 (p < 0.001; cut-off > 0.06) (Sensitivity: 76.4, specificity: 59.60). For the evaluation of LDH/Lymphocyte ratio in terms of survival, AUC was found to be 0.749 (p < 0.001; cut-off > 0.21) (Sensitivity: 70.59, specificity: 73.88). Conclusion: Studies based on radiological findings have demonstrated that CT involvement has higher sensitivity. LDH/Lymphocyte ratio was analyzed in terms of diagnosis and mortality with using specific CT involvement as gold standard method which was found to be a more sensitive due to PCR false negativity; 0.06 and 0.21 were obtained as cut off values for diagnosis and mortality.

Çalışmanın amacı, farklı mesleklerde başarılı kişilerin düşünme stilleri ile karar verme stilleri arasındaki ilişkiyi ortaya koymak ve düşünme stilleri ile karar verme stillerini cinsiyet ve meslek grubu değişkenleri açısından incelemektir. Araştırma ilişkisel tarama modelinde yürütülmüş olup; çalışma grubu, amaçlı örnekleme yöntemlerinden kartopu örnekleme yöntemi ile seçilen 184 kişiden oluşturmaktadır. Veri toplama aracı olarak Sternberg-Wagner Düşünme Stilleri Ölçeği (SWDSÖ) ve Melbourne Karar Verme Ölçeği I-II (MKVÖ I-II) kullanılmıştır. Veriler, SPSS 22 programı ile analiz edilmiştir. Sonuçlar, farklı mesleklerde başarılı kişilerin düşünme stilleri ve karar verme stilleri arasında anlamlı ilişkiler olduğunu ortaya koymuştur. Mesleğinde başarılı kişilerin düşünme ve karar verme stillerinde, cinsiyet açısından istatistiksel olarak anlamlı sonuçlar ortaya çıkmıştır. Farklı mesleklerde başarılı kişilerin düşünme stillerinde meslek grubuna göre anlamlı farklar olduğu belirlenmiştir.

Sermaye yapısı kararları ve firma karlılığı arasındaki ilişki üzerine yapılan çalışmalarda tartışmalar hala devam etmektedir. Bu çalışmanın da başlangıç noktasını bu tartışmalar oluşturmuştur. Bu çalışmanın amacı sermaye yapısı kararlarının ve firmaların kredi risk düzeylerinin firma karlılığı üzerine etkisi olup olmadığını araştırmaktır. Çalışmada sermaye yapısı, kredi risk düzeyi ve firma karlılığı arasındaki ilişki panel veri analizi yardımı ile değerlendirilmiştir. Firma karlılığını ölçmek için aktif karlılığı oranı seçilmiştir (AK). Sermaye yapısını değerlendirebilmek için kaldıraç oranı ve uzun vadeli borç oranı seçilmiştir. Ayrıca firmaların kredi risklerini değerlendirmek için kukla değişken kullanılmıştır. Firma büyüklüğünün de firma karlılığını etkilediği düşünülerek firma büyüklüğü de kontrol değişkeni olarak seçilmiştir. Çalışma da 2013-2017 yılları arasında Türkiye’de faaliyet gösteren 235 halka açık işletme çalışmanın örneklemini oluşturmuştur. Firma karlılığı ve firma büyüklüğü arasında da istatistiki olarak anlamlı pozitif bir ilişki olduğu tespit edilmiştir. Son olarak firmaların kredi risk düzeylerinin sermaye yapısı ve karlılık arasındaki ilişkide bir farklılık yaratmadığı sonucuna ulaşılmıştır.

- Abstract
Maternal food restriction is associated with the development of obesity in offspring. This study examined how maternal undernutrition in sheep affects the fetal hypothalamic glucocorticoid receptor (GR) and the appetite-regulating neuropeptides, proopiomelanocortin (POMC) and neuropeptide Y, which it regulates. In fetuses from ewes undernourished from -60 to +30 d around conception, there was increased histone H3K9 acetylation (1.63-fold) and marked hypomethylation (62% decrease) of the POMC gene promoter but no change in POMC expression. In the same group, acetylation of histone H3K9 associated with the hypothalamic GR gene was increased 1.60-fold and the GR promoter region was hypomethylated (53% decrease). In addition, there was a 4.7-fold increase in hypothalamic GR expression but no change in methylation of GR gene expression in the anterior pituitary or hippocampus. Interestingly, hypomethylation of both POMC and GR promoter markers in fetal hypothalami was also identified after maternal undernutrition from -60 to 0 d and -2 to +30 d. In comparison, the Oct4 gene, was hypermethylated in both controlandunderfed groups. Periconceptional undernutrition is therefore associated withmarked epigenetic changes in hypothalamic genes. Increase in GR expression in the undernourished group may contribute to fetal programming of a predisposition to obesity, via altered GR regulation of POMC and neuropeptide Y. These epigenetic changes in GR and POMC in the hypothalamus may also predispose the offspring to altered regulation of food intake, energy expenditure, and glucose homeostasis later in life. Background: All platin-based chemotherapeutics can cause hypersensitivity reactions (HSRs). With rapid drug desensitization (RDD), few patients experience breakthrough reactions (BTR) during desensitization. However, data about risk factors for BTRs during RDD in patients with HSRs to platins are limited. We first aimed to describe characteristics of our platin-reactive population and to validate the Brigham and Women's Hospital's (BWH's) RDD protocol in our population along with their outcomes with RDD. Our second aim was to identify the risk factors for BTRs. Method: This was a retrospective chart review (2013–2020) of patients with symptoms of immediate HSRs to platins. Initial HSRs were classified as grade 1, 2, or 3 based on their severity. Skin prick tests (SPT)/intradermal tests (IDT) were performed with implicated platins. A 12-step protocol was used during RDD. Results: The study comprised 65 women and seven men (mean age 57.78 ± 8.73 years). Initial HSRs to carboplatin, cisplatin, and oxaliplatin occurred in 38, 13, and 21 patients, respectively. All patients reacted at the fifth (median) recurrent infusions (min:1, max:20). The median values for carboplatin, cisplatin, and oxaliplatin were 6 (1–20), 3 (1–15), and 3 (1–11), respectively. Most initial HSRs were grade 2 (n = 40, 55.6%) and 3 (n = 27, 37.5%); only 6.9% (n = 5) were grade 1. Patients with grade 1, 2, and 3 initial HSRs had positive platin skin test results at rates of 80%, 74%, and 88%, respectively.A total of 232 RDDs were performed in 72 patients and 98.7% of these desensitizations were completed. BTRs occurred in 56 (24.1%) (grade 1 n = 14, 25%; grade 2 n = 32, 57%; grade 3 n = 10, 18%) of these desensitizations. Breakthrough reactions were more severe in patients with positive SPTs or 1:100 or 1:10 dilutions of IDT (p = 0.014). BTR was not observed during RDD in any of the patients with positive 1:1 dilutions of IDT. Positivity on prick or 1:100 or 1:10 IDT increased the risk of BTR 5.058 times. There was no significant association between the risk of BTRs and age, drug cycle, sex, comorbidities, or atopy. Conclusion: In our experience, 98.7% of 232 RDDs to platins were completed successfully, showing that RDD was safe and effective. Drug skin test positivity is a potential marker for identifying high-risk patients who will have BTRs during RDDs to platins.

We aimed to examine the efficiency for prediction of prognosis and response in non-APL AML cases of the “Samatya-predicting score”. A total of 213 patients diagnosed between January 2010-December 2020 were examined. Of the 158 patients included in the study, the median value of risk score was determined as 2,5. The sensitivity for mortality was 88% and the specificity was 42%. In terms of being non-responder to induction therapy, the sensitivity was 90,1%, the specificity was 25,3%. OS was shorter in those with high risk scores. This study makes an important contribution to the literature in terms of creating a different perspective to predict prognosis in AML.

Instrumented cells, equipped with miniature sensors, are proposed to aid the next stage of electrification in the automotive and aerospace industries. To optimize the energy density available within a lithium ion (li-ion) pack we demonstrate how a power line communication (PLC) network can be formed at an individual cell level. This reduces the need for complex communication cables within a vehicle wiring loom.Here we show a unique prototype smart cell (instrumented cell equipped with interface circuitry and processing capability) can be connected via a PLC network, to enable monitoring of vital parameters (temperature, voltage, current), regardless of cell state of charge (2.5 V to 4.2 V DC operating voltage). In this proof-of-concept study, we show the reliable system (0 errors detected over ∼24 hr experiment, acquired data (logged at 10 Hz) from cells (in a parallel configuration), and comparative data for cell internal and external temperature was recorded. During a prolonged discharge (1C, 5A discharge) a peak core temperature >3 °C hotter than surface temperature was observed, highlighting the need to understand cell operation in cooling system design.
