The present experiment was conducted to examine the hypothesis that attentional costs of walking vary with lateral balance demands during walking in young and older adults. Twenty young and twenty older adults walked on a treadmill at their preferred walking speed under five conditions: unconstrained normal walking, walking on projected visual lines corresponding to either the participant's preferred step width or 50% thereof (i.e. increased balance demand), and walking within low- and high-stiffness lateral stabilization frames (i.e. lower balance demands). Attentional costs were assessed using a probe reaction-time task during these five walking conditions, normalized to baseline performance as obtained during sitting. Both imposed step-width conditions were more attentionally demanding than the three other conditions, in the absence of any other significant differences between conditions. These effects were similar in the two groups. The results indicate that the attentional costs of walking were, in contrast to what has been postulated previously, not influenced by lateral balance demands. The observed difference in attentional costs between normal walking and both visual lines conditions suggests that visuomotor control processes, rather than balance control, strongly affect the attentional costs of walking. A tentative explanation of these results may be that visuomotor control processes are mainly governed by attention-demanding cortical processes, whereas balance is regulated predominantly subcortically.

Background: Body-worn sensors allow assessment of gait characteristics that are predictive of fall risk, both when measured during treadmill walking and in daily life. The present study aimed to assess differences as well as associations between fall-related gait characteristics measured on a treadmill and in daily life. Methods: In a cross-sectional study, trunk accelerations of 18 older adults (72.3 ± 4.5 years) were recorded during walking on a treadmill (Dynaport Hybrid sensor) and during daily life (Dynaport MoveMonitor). A comprehensive set of 32 fall-risk-related gait characteristics was estimated and compared between both settings. Results: For 25 gait characteristics, a systematic difference between treadmill and daily-life measurements was found. Gait was more variable, less symmetric, and less stable during daily life. Fourteen characteristics showed a significant correlation between treadmill and daily-life measurements, including stride time and regularity (0.48 < r < 0.73; p < 0.022). No correlation between treadmill and daily-life measurements was found for stride-time variability, acceleration range and sample entropy in vertical and mediolateral direction, gait symmetry in vertical direction, and stability estimated as the local divergence exponent by Rosenstein's method in mediolateral direction (r < 0.16; p > 0.25). Conclusions: Gait characteristics revealed less stable, less symmetric, and more variable gait during daily life than on a treadmill, yet about half of the characteristics were significantly correlated between conditions. These results suggest that daily-life gait analysis is sensitive to static personal factors (i.e., physical and cognitive capacity) as well as dynamic situational factors (i.e., behavior and environment), which may both represent determinants of fall risk.

Background: Walking in everyday life requires the ability to adapt walking to the environment. This adaptability is often impaired after stroke, and this might contribute to the increased fall risk after stroke. To improve safe community ambulation, walking adaptability training might be beneficial after stroke. This study is designed to compare the effects of two interventions for improving walking speed and walking adaptability: treadmill-based C-Mill therapy (therapy with augmented reality) and the overground FALLS program (a conventional therapy program). We hypothesize that C-Mill therapy will result in better outcomes than the FALLS program, owing to its expected greater amount of walking practice. Methods: This is a single-center parallel group randomized controlled trial with pre-intervention, post-intervention, retention, and follow-up tests. Forty persons after stroke (≥3 months) with deficits in walking or balance will be included. Participants will be randomly allocated to either C-Mill therapy or the overground FALLS program for 5 weeks. Both interventions will incorporate practice of walking adaptability and will be matched in terms of frequency, duration, and therapist attention. Walking speed, as determined by the 10 Meter Walking Test, will be the primary outcome measure. Secondary outcome measures will pertain to walking adaptability (10 Meter Walking Test with context or cognitive dual-task and Interactive Walkway assessments). Furthermore, commonly used clinical measures to determine walking ability (Timed Up-and-Go test), walking independence (Functional Ambulation Category), balance (Berg Balance Scale), and balance confidence (Activities-specific Balance Confidence scale) will be used, as well as a complementary set of walking-related assessments. The amount of walking practice (the number of steps taken per session) will be registered using the treadmill’s inbuilt step counter (C-Mill therapy) and video recordings (FALLS program).

BACKGROUND: The ability to rise from sitting to standing is critical to an individual's quality of life, as it is a prerequisite for functional independence. The purpose of the current study was to examine the hypothesis that test durations as assessed with the instrumented repeated Sit-To-Stand (STS) show stronger associations with health status, functional status and daily physical activity of older adults than manually recorded test durations.METHODS: In 63 older participants (mean age 83 ±6.9 years, 51 female), health status was assessed using the European Quality of Life questionnaire and functional status was assessed using the physical function index of the of the RAND-36. Physical performance was measured using a wearable sensor-based STS test. From this test, durations, sub-durations and kinematics of the STS movements were estimated and analysed. In addition, physical activity was measured for one week using an activity monitor and episodes of lying, sitting, standing and locomotion were identified. Associations between STS parameters with health status, functional status and daily physical activity were assessed.RESULTS: The manually recorded STS times were not significantly associated with health status (p = 0.457) and functional status (p = 0.055), whereas the instrumented STS times were (both p = 0.009). The manually recorded STS durations showed a significant association to daily physical activity for mean sitting durations (p = 0.042), but not for mean standing durations (p = 0.230) and mean number of locomotion periods (p = 0.218). Furthermore, durations of the dynamic sit-to-stand phase of the instrumented STS showed more significant associations with health status, functional status and daily physical activity (all p = 0.001) than the static phases standing and sitting (p = 0.043-0.422).CONCLUSIONS: As hypothesized, instrumented STS durations were more strongly associated with participant health status, functional status an

Bij roeien en zwemmen treden vermogensverliezen op doordat het water in beweging komt en doordat de voortstuwingssnelheid tijdens de bewegingscycli fluctueert. In dit project wordt met nieuwe meetmethoden onderzocht bij welke bewegingsuitvoering deze verliezen zo klein mogelijk zijn en welke innovatieve feedback kan helpen om deze bewegingsuitvoering te realiseren.

Augmented visual feedback (VF) may offer benefits similar to those of rhythmic external cues in alleviating some mobility-related difficulties in individuals with Parkinson's disease (PD). However, due to an impaired ability to reweigh sensory information under changing circumstances, subjects with PD may be rather vulnerable to incongruity of visual information. In the present study, we investigated whether VF is indeed effective in improving motor functioning in a weight-shifting task during upright stance, and whether subjects with PD are affected more by incongruent VF than healthy controls. Participants performed sideways swaying motions based on tracking of real-time and delayed VF - the first providing congruent, and hence more accurate, visual information than the latter. We analyzed center-of-pressure signals patterns for 28 individuals with PD and 16 healthy, age- and gender-matched controls by estimating task accuracy, movement pattern variability, and normalized movement amplitude. For conditions without feedback and with real-time feedback, subjects with PD performed lateral swaying motions with greater error (F(1, 42) = 12.065, p = .001) and with more variable movement patterns than healthy controls (F(1, 24) = 113.086, p < .001). Error change scores revealed that patients with PD were nevertheless still able to use VF to improve tracking performance (t(24) = -2.366, p = .026). However, whereas controls were able to adapt to a certain amount of visual incongruity, patients with PD were not. Instead, movement amplitude was significantly reduced in this group (F(1.448, 60.820) = 17.639, p < .001). By reducing movement amplitude, subjects with PD appear to resort to a 'conservative' strategy to minimize performance breakdown.

Falls can have devastating consequences for older people. We determined the relationship between the likelihood of fall incidents and daily-life behavior. We used wearable sensors to assess habitual physical activity and daily-life gait quality (in terms of e.g. stability, variability, smoothness and symmetry), and determined their predictive ability for time-to-first-and-second-falls. 319 older people wore a trunk accelerometer (Dynaport MoveMonitor, McRoberts) during one week. Participants further completed questionnaires and performed grip strength and trail making tests to identify risk factors for falls. Their prospective fall incidence was followed up for six to twelve months. We determined interrelations between commonly used gait characteristics to gain insight in their interpretation and determined their association with time-to-falls. For all data -including questionnaires and tests- we determined the corresponding principal components and studied their predictive ability for falls. We showed that gait characteristics of walking speed, stride length, stride frequency, intensity, variability, smoothness, symmetry and complexity were often moderately to highly correlated (r > 0.4). We further showed that these characteristics were predictive of falls. Principal components dominated by history of falls, alcohol consumption, gait quality and muscle strength proved predictive for time-to-fall. The cross-validated prediction models had adequate to high accuracy (time dependent AUC of 0.66-0.72 for time-to-first-fall and 0.69-0.76 for -second-fall). Daily-life gait quality obtained from a single accelerometer on the trunk is predictive for falls. These findings confirm that ambulant measurements of daily behavior contribute substantially to the identification of elderly at (high) risk of falling.

In order to determine whether small-sided game (SSG) locomotor performance can serve as a fitness indicator, we (1) compared 6-a-side (6v6) SSG-intensity of players varying in fitness and skill, (2) examined the relationship of the 6v6-SSG and Yo-Yo IR2 and (3) assessed the reliability of the 6v6-SSG. Thirty-three professional senior, 30 professional youth, 62 amateur and 16 professional woman football players performed 4 × 7 min 6v6-SSGs recorded by a Local Position Measurement system. A substantial subgroup (N = 113) also performed the Yo-Yo IR2. Forty-seven amateur players performed two or three 6v6-SSGs. No differences in 6v6-SSG time-motion variables were found between professional senior and professional youth players. Amateurs showed lower values than professional seniors on almost all time-motion variables (ES = 0.59‒1.19). Women displayed lower high-intensity time-motion variables than all other subgroups. Total distance run during 6v6-SSG was only moderately related to Yo-Yo IR2 distance (r = 0.45), but estimated metabolic power, high speed (>14.4 km · h
