Zusammenfassung Hintergrund Im Rahmen von Sepsis und septischem Schock kommt es aufgrund der engen Verflechtung von Gerinnung und Entzündung häufig zu einer Koagulopathie. Die sepsisinduzierte Koagulopathie (SIC) stellt hierbei die schwerste, potenziell fatale Form dar. Aufgrund fehlender Evidenz beschränken sich die aktuellen Sepsis-Leitlinien auf Empfehlungen zur medikamentösen Prophylaxe einer venösen Thromboembolie (VTE), während die Behandlung einer SIC nicht thematisiert wird. Methoden Um den Status quo der VTE-Prophylaxe sowie der SIC-Behandlung auf deutschen Intensivstationen zu erheben, wurde von Oktober 2019 bis Mai 2020 eine deutschlandweite Onlineumfrage unter ärztlichen Leitern von Intensivstationen durchgeführt. Diese wurde ab April 2020 durch einen zusätzlichen Fragenblock ergänzt, der sich mit der VTE-Prophylaxe sowie der SIC-Behandlung bei Coronaviruskrankheit(COVID)-19-Patienten befasste. Ergebnisse Die Umfrageergebnisse zeigen eine ausgeprägte Heterogenität in der klinischen Praxis bezüglich Prophylaxe von VTE und Therapie der SIC. Ein systematisches Screening auf SIC findet in den meisten Intensivstationen nicht statt. Bei COVID-19-Patienten fällt v. a. auf, dass bei drei Viertel der teilnehmenden Intensivstationen die gelebte Praxis der medikamentösen VTE-Prophylaxe nicht von Non-COVID-19-Patienten abweicht. Schlussfolgerung Die Heterogenität der in der Umfrage gesammelten Antworten legt nahe, dass es einer systematischen Aufarbeitung dieses Themenfeldes bedarf, um zukünftig über klinische Interventionsstudien die individualisierte Patientenversorgung mit der gebotenen Evidenz zu unterlegen.

ObjectiveTo evaluate the feasibility of continuous paO2 prediction in an intraoperative setting among neurosurgical patients with modern machine learning methods.Materials and MethodsData were extracted from routine clinical care of lung-healthy, neurosurgical patients. We used recursive feature elimination to identify relevant features for the prediction of paO2. Five machine learning algorithms (gradient boosting regressor, k-nearest neighbors regressor, random forest regressor, support vector regression, multi-layer perceptron regressor) and a multivariate linear regression were then tuned and fitted to the selected features. A performance matrix consisting of Spearman’s ρ, mean absolute percentage error (MAPE) and root mean squared error (RMSE) was finally computed based on the test set and used to compare and rank each algorithm.ResultsWe analyzed 4,180 patients with 12,497 observations. A total of 20 features were selected from analysis of the training dataset comprising 836 patients with 9,992 observations. The best algorithm, random forest, was able to predict paO2 values with ρ=0.90, MAPE=10.4%, and RMSE=30.9mmHg, closely followed by gradient boosting and multi-layer perceptron. Support vector regression, k-nearest neighbors regressor and the linear regression did not achieve the performance metrics.DiscussionWe successfully applied and compared several machine learning algorithms to estimate continuous paO2 values in neurosurgical patients. The random forest regressor performed best over all three categories of the performance matrix.ConclusionPaO2 can be predicted by perioperative routine data in neurosurgical patients.

Zusammenfassung Hintergrund Im Januar 2022 wird die 11. Revision der International Classification of Diseases (ICD-11) in Kraft treten. Unter anderem wird darin die SEPSIS-3-Definition implementiert sein, in der Sepsis als „lebensbedrohliche Organdysfunktion, hervorgerufen durch eine fehlregulierte Wirtsantwort auf eine Infektion“ definiert wird. Ziel der vorliegenden Sekundärauswertung einer Umfrage zum Thema „Sepsis-induzierte Koagulopathie“ war es zu evaluieren, ob die SEPSIS-3-Definition (engl. The Third International Consensus Definitions for Sepsis and Septic Shock) 4 Jahre nach ihrer internationalen Einführung im klinischen Alltag anästhesiologisch geführter Intensivstationen in Deutschland angekommen ist und so die Voraussetzungen für die Verwendung des ICD-11 gegeben sind. Methoden Im Rahmen einer deutschlandweiten Umfrage unter ärztlichen Leitern von Intensivstationen, die zwischen Oktober 2019 und Mai 2020 durchgeführt wurde, wurde in einem gesonderten Fragenblock nach der verwendeten Sepsisdefinition gefragt. An 78,9 % der Universitätsklinika und 84,0 % der teilnehmenden Lehrkrankenhäuser ist die SEPSIS-3-Definition im klinischen Alltag integriert. A pre-existing anticoagulation treatment and predisposing diseases for thromboembolic events represent common problems in patients with sepsis or septic shock; however, these conditions are not addressed in current national guidelines for sepsis and septic shock. One of the aims of this nationwide survey in Germany was therefore to determine how intensive care physicians deal with these problems.From October 2019 to May 2020, we conducted a nationwide survey among German medical directors of intensive care units (ICU) addressing anticoagulation and drug-based prophylaxis of venous thromboembolism (VTE) in patients with sepsis and sepsis-induced coagulopathy. One focus was the procedure for patients with a pre-existing anticoagulation treatment or a previously known heparin-induced thrombocytopenia (HIT) type 2 (acute symptomatic vs. dating back years).In most of the participating ICUs pre-existing anticoagulation is largely continued with low molecular weight heparin preparations or unfractionated heparin. In patients with pre-existing HIT type 2 both acute symptomatic and dating back years, argatroban represents the drug of choice. There is a high degree of variability in the definition of the target values, usually being well above the range for pure VTE prophylaxis.Data on the continuation of anticoagulation beyond VTE prophylaxis with a subsequently increased risk of bleeding in patients with sepsis and septic shock is limited and treatment decisions are in many cases subject to individual consideration by the practitioner. The results of our survey imply the need for a systematic work-up of this topic in order to support daily practice in many ICUs with the required evidence.HINTERGRUND: Sowohl eine vorbestehende Antikoagulation als auch prädisponierende Vorerkrankungen für Thromboembolien stellen ein häufiges Problem bei Patienten mit Sepsis dar, wenngleich der Umgang mit diesen Rahmenbedingungen von den aktuellen nationalen und internationalen Leitlinien für Sepsis und septischen Schock nicht adressiert wird. Ein Ziel der hier vorliegenden deutschlandweiten Umfrage war es, den Umgang von Intensivmedizinern mit derartigen Problemen zu eruieren.Von Oktober 2019 bis Mai 2020 führten wir eine deutschlandweite Umfrage unter ärztlichen Leitern von Intensivstationen zum Thema Antikoagulation und medikamentöser Thromboseprophylaxe bei Sepsis und Sepsis-induzierter Koagulopathie durch. Ein Fokus war hierbei, das Vorgehen bei Patienten mit vorbestehender Indikation zur therapeutischen Antikoagulation sowie bei vorbekannter Heparin-induzierter Thrombopenie Typ II (HIT-II) (akut-symptomatisch vs. Jahre zurückliegend) zu eruieren.Die Umfrageergebnisse zeigen, dass auf den meisten der teilnehmenden Intensivstationen eine vorbestehende Antikoagulation größtenteils mit niedermolekularen Heparinpräparaten oder unfraktioniertem Heparin fortgeführt wird. Bei bekannter HIT-II wird Argatroban bevorzugt, unabhängig davon, ob es sich um eine Jahre zurückliegende oder eine akut-symptomatische HIT-II handelt. Eine hohe Variabilität besteht bei der Festlegung der Zielwerte für die Antikoagulation, wobei diese größtenteils deutlich über dem Bereich einer reinen venösen Thromboembolie(VTE)-Prophylaxe zu liegen kommen.Die Datenlage zur Fortführung einer über die reine VTE-Prophylaxe hinausgehenden Antikoagulation mit konsekutiv erhöhtem Blutungsrisiko bei Patienten mit Sepsis und septischem Schock ist eingeschränkt, und Therapieentscheidungen unterliegen in vielen Fällen einer individuellen Abwägung des Behandlungsteams. The 11th revision of the International Classification of Diseases (ICD-11) will come into effect in January 2022. Among other things, The Third International Consensus Definitions for Sepsis and Septic Shock (SEPSIS‑3 definition) will be implemented in it. This defines sepsis as a "life-threatening organ dysfunction caused by a dysregulated host response to infection". The aim of the present secondary analysis of a survey on the topic of "sepsis-induced coagulopathy" was to evaluate whether the SEPSIS‑3 definition, 4 years after its international introduction, has arrived in everyday clinical practice of intensive care units (ICU) run by anesthesiologists in Germany and thus the requirements for its use of the ICD-11 are given.Between October 2019 and May 2020, we carried out a nationwide survey among German medical directors of ICUs. In a separate block of questions we asked about the definition of sepsis used in daily practice. In addition, we asked whether the quick-sequential (sepsis-related) organ failure assessment (qSOFA) score is used in screening for sepsis in the hospital to which to the participating ICU belongs.A total of 50 medical directors from anesthesiological ICUs took part in the survey. In total, the ICUs evaluated stated that they had around 14% of the high-care beds registered in Germany. The SEPSIS‑3 definition is integrated into everyday clinical practice at 78.9% of the university hospitals and 84.0% of the participating teaching hospitals. In contrast, the qSOFA screening test is only used by 26.3% of the participating university hospitals, but at least 52% of the teaching hospitals and 80% of the other hospitals.The data show that both SEPSIS‑3 and qSOFA have become part of everyday clinical practice in German hospitals. Background: In the context of sepsis and septic shock, coagulopathy often occurs due to the close relationship between coagulation and inflammation. Sepsis-induced coagulopathy (SIC) is the most severe and potentially fatal form. Anticoagulants used in prophylactic or therapeutic doses are discussed to potentially exert beneficial effects in patients with sepsis and/or SIC; however, due to the lack of evidence recent guidelines are limited to recommendations for drug prophylaxis of venous thromboembolism (VTE), while treatment of SIC has not been addressed. Methods: In order to determine the status quo of VTE prophylaxis as well as treatment of SIC in German intensive care units (ICU), we conducted a Germany-wide online survey among heads of ICUs from October 2019 to May 2020. In April 2020, the survey was supplemented by an additional block of questions on VTE prophylaxis and SIC treatment in coronavirus disease 2019 (COVID-19) patients. Results: A total of 67 senior doctors took part in the survey. The majority (n = 50; 74.6%) of the responses were from ICU under the direction of an anesthesiologist and/or a department of anesthesiology. Most of the participants worked either at a university hospital (n = 31; 47.8%) or an academic teaching hospital (n = 27; 40.3%). The survey results show a pronounced heterogeneity in clinical practice with respect to the prophylaxis of VTE as well as SIC treatment. In an exemplary case of pneumogenic sepsis, low molecular weight heparins (LMWH) were by far the most frequently mentioned group of medications (n = 51; 76.1% of the responding ITS). In the majority of cases (n = 43; 64.2%), anti-FXa activity is not monitored with the use of LMWH in prophylaxis doses. Unfractionated heparin (UFH) was listed as a strategy for VTE prophylaxis in 37.3% of the responses (n = 25). In an exemplary case of abdominal sepsis 54.5% of the participants (n = 36; multiple answers possible) stated the use of UFH or LMWH and UFH with dosage controlled by PTT is used on two participating ICUs. The anti-FXa activity under prophylactic anticoagulation with LMWH is monitored in 7 participating clinics (10.6%) in abdominal sepsis. Systematic screening for sepsis-associated coagulation disorders does not take place in most hospitals and patterns in the use of anticoagulants show significant variability between ICUs. In the case of COVID-19 patients, it is particularly noticeable that in three quarters of the participating ICUs the practice of drug-based VTE prophylaxis and SIC treatment does not differ from that of non-COVID-19 patients. Background In the SARS-CoV-2 pandemic, viral genomes are available at unprecedented speed, but spatio-temporal bias in genome sequence sampling precludes phylogeographical inference without additional contextual data. Aim We applied genomic epidemiology to trace SARS-CoV-2 spread on an international, national and local level, to illustrate how transmission chains can be resolved to the level of a single event and single person using integrated sequence data and spatio-temporal metadata. Methods We investigated 289 COVID-19 cases at a university hospital in Munich, Germany, between 29 February and 27 May 2020. Using the ARTIC protocol, we obtained near full-length viral genomes from 174 SARS-CoV-2-positive respiratory samples. Phylogenetic analyses using the Auspice software were employed in combination with anamnestic reporting of travel history, interpersonal interactions and perceived high-risk exposures among patients and healthcare workers to characterise cluster outbreaks and establish likely scenarios and timelines of transmission. Results We identified multiple independent introductions in the Munich Metropolitan Region during the first weeks of the first pandemic wave, mainly by travellers returning from popular skiing areas in the Alps. In these early weeks, the rate of presumable hospital-acquired infections among patients and in particular healthcare workers was high (9.6% and 54%, respectively) and we illustrated how transmission chains can be dissected at high resolution combining virus sequences and spatio-temporal networks of human interactions. Conclusions Early spread of SARS-CoV-2 in Europe was catalysed by superspreading events and regional hotspots during the winter holiday season. Genomic epidemiology can be employed to trace viral spread and inform effective containment strategies.

Aims This study aimed to determine whether anthropometric markers of thoracic skeletal muscle and abdominal visceral fat tissue correlate with outcome parameters in critically ill COVID-19 patients. Methods We retrospectively analysed thoracic CT-scans of 67 patients in four ICUs at a university hospital. Thoracic skeletal muscle (total cross-sectional area (CSA); pectoralis muscle area (PMA)) and abdominal visceral fat tissue (VAT) were quantified using a semi-automated method. Point-biserial-correlation-coefficient, Spearman-correlation-coefficient, Wilcoxon rank-sum test and logistic regression were used to assess the correlation and test for differences between anthropometric parameters and death, ventilator- and ICU-free days and initial inflammatory laboratory values.
