Objectives Psoriatic arthritis (PsA) has a strong genetic component, and the identification of genetic risk factors could help identify the ~30% of psoriasis patients at high risk of developing PsA. Our objectives were to identify genetic risk factors and pathways that differentiate PsA from cutaneous-only psoriasis (PsC) and to evaluate the performance of PsA risk prediction models. Methods Genome-wide meta-analyses were conducted separately for 5,065 patients with PsA and 21,286 healthy controls and separately for 4,340 patients with PsA and 6,431 patients with PsC. The heritability of PsA was calculated as a single-nucleotide polymorphism (SNP)–based heritability estimate (h2SNP) and biologic pathways that differentiate PsA from PsC were identified using Priority Index software. The generalizability of previously published PsA risk prediction pipelines was explored, and a risk prediction model was developed with external validation. Results We identified a novel genome-wide significant susceptibility locus for the development of PsA on chromosome 22q11 (rs5754467; P = 1.61 × 10−9), and key pathways that differentiate PsA from PsC, including NF-κB signaling (adjusted P = 1.4 × 10−45) and Wnt signaling (adjusted P = 9.5 × 10−58). The heritability of PsA in this cohort was found to be moderate (h2SNP = 0.63), which was similar to the heritability of PsC (h2SNP = 0.61). We observed modest performance of published classification pipelines (maximum area under the curve 0.61), with similar performance of a risk model derived using the current data. Conclusion Key biologic pathways associated with the development of PsA were identified, but the investigation of risk classification revealed modest utility in the available data sets, possibly because many of the PsC patients included in the present study were receiving treatments that are also effective in PsA. Future predictive models of PsA should be tested in PsC patients recruited from primary care.

Clinical, immunological and genetic findings suggest a pathogenic role for interleukin (IL)-1.\ud \ud Objectives:\ud To determine whether anakinra (an IL-1 receptor antagonist) delivers therapeutic benefit in PPP.\ud \ud Methods:\ud This was a randomized (1 : 1), double-blind, two-staged, adaptive, UK multicentre, placebo-controlled trial [ISCRTN13127147 (registered 1 August 2016); EudraCT number: 2015-003600-23 (registered 1 April 2016)]. Participants had a diagnosis of PPP (> 6 months) requiring systemic therapy. Treatment was 8 weeks of anakinra or placebo via daily, self-administered subcutaneous injections. Primary outcome was the Palmoplantar Pustulosis Psoriasis Area and Severity Index (PPPASI) at 8 weeks.\ud \ud Results:\ud A total of 374 patients were screened; 64 were enrolled (31 in the anakinra arm and 33 in the placebo arm) with a mean (SD) baseline PPPASI of 17·8 (10·5) and a PPP investigator’s global assessment of severe (50%) or moderate (50%). The baseline adjusted mean difference in PPPASI favoured anakinra but did not demonstrate superiority in the intention-to-treat analysis [–1·65, 95% confidence interval (CI) –4·77 to 1·47; P = 0·30]. Similarly, secondary objective measures, including fresh pustule count (2·94, 95% CI –26·44 to 32·33; favouring anakinra), total pustule count (–30·08, 95% CI –83·20 to 23·05; favouring placebo) and patient-reported outcomes, did not show superiority of anakinra. When modelling the impact of adherence, the PPPASI complier average causal effect for an individual who received ≥ 90% of the total treatment (48% in the anakinra group) was –3·80 (95% CI –10·76 to 3·16; P = 0·285). No serious adverse events occurred.\ud \ud Conclusions:\ud No evidence for the superiority of anakinra was found. IL-1 blockade is not a useful intervention for the treatment of PPP.

We present a compilation and analysis of 1099 Holocene relative shore-level (RSL) indicators including 867 relative sea-level data points and 232 data points from the Ancylus Lake and the following transitional phase from 10.7 to 8.5 ka BP located around the Baltic Sea. The spatial distribution covers the Baltic Sea and near-coastal areas fairly well, but some gaps remain mainly in Sweden. RSL data follow the standardized HOLSEA format and, thus, are ready for spatially comprehensive applications in, e.g., glacial isostatic adjustment (GIA) modelling. Sampling method The data set is a compilation of rather different samples from geological, geomorphological and archaeological studies. Most of the data was already published in different formats. In this compilation we homogenized the meta information of the available information according to the HOLSEA database format, https://www.holsea.org/archive-your-data, which is a modification of the recommendations given in Hijma et al. (2015). In addition to the reformatting, the majority of samples with radiocarbon dating were recalibrated with oxcal-software using the calib13 and marine13 curves. Furthermore, all sample descriptions were critically checked for consistency in positioning, levelling and indicative meaning by experts of the respective geographic region see Supplement 2. Analytical method In principle, it is a compilation, recalibration and revision of already published data. Data Processing Data of individual compilations were revised and imported into a relational database system. Therein, the data was transferred into the HOLSEA format by specified rules. By this procedure, a homogeneous categorisation was achieved without losing the original data. Also this is stored in the relational database system allowing for later updates of the transfer procedure or a recalibration of the data. Variation in response to biologic therapy for inflammatory diseases, such as psoriasis, is partly driven by variation in drug exposure. Real-world psoriasis data were used to develop a pharmacokinetic/pharmacodynamic (PK/PD) model for the first-line therapeutic antibody ustekinumab. The impact of differing dosing strategies on response was explored. Data were collected from a UK prospective multicenter observational cohort (491 patients on ustekinumab monotherapy, drug levels, and anti-drug antibody measurements on 797 serum samples, 1,590 measurements of Psoriasis Area Severity Index (PASI)). Ustekinumab PKs were described with a linear one-compartment model. A maximum effect (E max) model inhibited progression of psoriatic skin lesions in the turnover PD mechanism describing PASI evolution while on treatment. A mixture model on half-maximal effective concentration identified a potential nonresponder group, with simulations suggesting that, in future, the model could be incorporated into a Bayesian therapeutic drug monitoring “dashboard” to individualize dosing and improve treatment outcomes.

Importance: High-cost biologic therapies have transformed the management of immune-mediated inflammatory diseases. To optimize outcomes and reduce costs, dose adjustment informed by measurement of circulating drug levels has been shown to be effective in various settings. However, limited evidence exists for this approach with the interleukin 12 and interleukin 23 inhibitor ustekinumab. Objective: To evaluate clinical utility of therapeutic drug monitoring for ustekinumab in patients with psoriasis. Design, Setting, and Participants: A prospective observational cohort of 491 adults with psoriasis was recruited to the multicenter Biomarkers of Systemic Treatment Outcomes in Psoriasis study within the British Association of Dermatologists Biologic and Immunomodulators Register from June 2009 to December 2017; samples from some patients were taken between 2009 and 2011 as part of a pilot study with the same inclusion criteria. Exposure: Serum ustekinumab level measured at any point during the dosing cycle using an enzyme-linked immunosorbent assay. Main Outcomes and Measures@ Disease activity measured using the Psoriasis Area and Severity Index (PASI) score. Treatment response outcomes were PASI75 (75% reduction in PASI score from baseline [primary outcome]), PASI90 (90% reduction of PASI score from baseline), and absolute PASI score of 1.5 or less. Results: A total of 491 patients (171 women and 320 men; mean [SD] age, 45.7 [12.8] years) had 1 or more serum samples (total, 853 samples obtained 0-56 weeks from start of treatment) and 1 or more PASI scores within the first year of treatment. Antidrug antibodies were detected in only 17 of 490 patients (3.5%). Early measured drug levels (1-12 weeks after starting treatment) were associated with PASI75 response 6 months after starting treatment (odds ratio, 1.38; 95% CI, 1.11-1.71) when adjusted for baseline PASI score, age, and ustekinumab dose. However, this finding was not consistent across the other PASI outcomes (PASI90 and PASI score of ≤1.5). Conclusions and Relevance: This real-world study provides evidence that measurement of early serum ustekinumab levels could be useful to direct the treatment strategy for psoriasis. Adequate drug exposure early in the treatment cycle may be particularly important in determining clinical outcome.

10.1016/j.envint.2014.11.009 The worldwide ban of several formulations of brominated flame retardants has caused an increase in the production of organophosphorus flame retardants (PFRs) to meet the existing fire regulations for a wide range of household products. This biomonitoring study surveys the occurrence of the metabolites from PFRs and related plasticizers (dialkyl and diaryl phosphates; DAPs) in urine from a Norwegian mother-child cohort (48 mothers and 54 children). Concentrations of DAPs were higher in the children than in their mothers (Wilcoxon signed-rank test p=0.001). Median urinary concentrations of diphenyl phosphate (DPHP) were 1.1 and 0.51ng/mL in children and mothers, respectively, followed by bis(1,3-dichloro-2-propyl) phosphate (BDCIPP) with medians of 0.23 and 0.12ng/mL, respectively. 10.1016/j.aca.2014.06.026 Organophosphate triesters are common flame retardants used in a wide variety of consumer products from which they can migrate and pollute the indoor environment. Humans may thus be continuously exposed to several organophosphate triesters which might be a risk for human health. An analytical method based on direct injection of 5 µL urine into an ultra performance liquid chromatography system coupled to a time-of-flight mass spectrometry has been developed and validated to monitor exposure to organophosphate triesters through their respective dialkyl and diaryl phosphate metabolites (DAPs). The targeted analytes were: di-n-butyl phosphate (DNBP), diphenyl phosphate (DPHP), bis(2-butoxyethyl) phosphate (BBOEP), bis(2-chloroethyl) phosphate (BCEP), bis(1-chloro-2-propyl) phosphate (BCPP) and bis(1,3-dichloro-2-propyl) phosphate (BDCIPP). Separation was achieved in less than 3 min on a short column with narrow diameter and small particle size (50 mm × 2.1 mm × 1.7 µm). Different mobile phases were explored to obtain optimal sensitivity. Acetonitrile/water buffered with 5 mM of ammonium hydroxide/ammonium formate (pH 9.2) was the preferred mobile phase. Quantification of DAPs was performed using deuterated analogues as internal standards in synthetic urine (averaged DAP accuracy was 101%; RSD 3%). Low method limits of quantification (MLQ) were obtained for DNBP (0.40 ng mL-1), DPHP (0.10 ng mL-1), BDCIPP (0.40 ng mL-1) and BBOEP (0.60 ng mL-1), but not for the most polar DAPs, BCEP (~12 ng mL-1) and BCPP (~25 ng mL-1).
