Abstract

International audience; The typing of proteinuria is one of the complementary examinations carried out during the exploration of proteinuria. It aims to separate and identify the different proteins, or fractions of proteins, that make up proteinuria. The nature and relative importance of the proteins present reflect the location of the renal involvement and help to determine the etiology. The typing of a proteinuria also allows the detection of a monoclonal component in urine and its quantification. Finally, it allows highlighting the existence of a proteinuria of overload that can occur in the absence of kidney damage. Many methods allow the typing of proteinuria, and these have benefited in recent years from technological advances. The purpose of this review is to summarize typing methods currently used, their benefits and limitations, and the help that these diagnostic tools can provide to the management of patients.; Le typage des protéinuries constitue un des examens complémentaires mis en œuvre au cours de l’exploration d’une protéinurie. Il vise à séparer et à identifier les différentes protéines, ou fractions de protéines, qui composent la protéinurie. La nature et l’importance relative des protéines présentes reflètent la localisation de l’atteinte rénale et contribuent à en déterminer l’étiologie. Le typage d’une protéinurie permet également la détection d’un composant monoclonal urinaire ainsi que sa quantification. Enfin, elle permet de mettre en évidence l’existence d’une protéinurie de surcharge autre que les chaînes légères libres d’immunoglobulines pouvant survenir en l’absence de lésion rénale. De nombreuses méthodes permettent le typage des protéinuries, et celles-ci ont bénéficié ces dernières années d’avancées technologiques. L’objet de cette revue est de faire la synthèse des méthodes de typage actuellement utilisées, de leurs avantages et de leurs limites, et de l’aide que ces analyses peuvent apporter à la prise en charge des malades.

Abstract

OBJECTIVE: We assessed the performance of the ratio of peripheral arterial oxygen saturation to the inspired fraction of oxygen (SpO(2)/FiO(2)) to predict the ratio of partial pressure arterial oxygen to the fraction of inspired oxygen (PaO(2)/FiO(2)) among patients admitted to our emergency department (ED) during the SARS-CoV-2 outbreak. METHODS: We retrospectively studied patients admitted to an academic-level ED in France who were undergoing a joint measurement of SpO(2) and arterial blood gas. We compared SpO(2) with SaO(2) and evaluated performance of the SpO(2)/FiO(2) ratio for the prediction of 300 and 400 mmHg PaO(2)/FiO(2) cut-off values in COVID-19 positive and negative subgroups using receiver-operating characteristic (ROC) curves. RESULTS: During the study period from February to April 2020, a total of 430 arterial samples were analyzed and collected from 395 patients. The area under the ROC curves of the SpO(2)/FiO(2) ratio was 0.918 (CI 95% 0.885-0.950) and 0.901 (CI 95% 0.872-0.930) for PaO(2)/FiO(2) thresholds of 300 and 400 mmHg, respectively. The positive predictive value (PPV) of an SpO(2)/FiO(2) threshold of 350 for PaO(2)/FiO(2) inferior to 300 mmHg was 0.88 (CI95% 0.84-0.91), whereas the negative predictive value (NPV) of the SpO(2)/FiO(2) threshold of 470 for PaO(2)/FiO(2) inferior to 400 mmHg was 0.89 (CI95% 0.75-0.96). No significant differences were found between the subgroups. CONCLUSIONS: The SpO(2)/FiO(2) ratio may be a reliable tool for hypoxemia screening among patients admitted to the ED, particularly during the SARS-CoV-2 outbreak.

Abstract

Anatomic and metabolic changes in central nervous system induced by 14 wk of vitamin A deprivation (VAD) were monitored and quantified in rats. In vivo brain magnetic resonance imaging (4.7T) was performed at 5, 7, 9, 11, and 14 wk of each diet after weaning in the following: 1) VAD group; 2) control pair-fed group; and 3) control group that consumed the diet ad libitum (1.15 [micro]g retinol/g diet). After 14 wk, high-resolution magic angle spinning proton NMR spectroscopy (11.7T) was performed on small samples of cortex, hippocampus, and striatum. Serum retinol concentrations remained stable and cerebral volume (CV) increased as a linear function of body weight in the ad libitum group ([R.sup.2] = 0.78; P= 0.047) and pair-fed controls ([R.sup.2] = 0.78; P = 0.046). In VAD rats, retinol decreased from the onset of deprivation (2.2 [+ or -] 0.14 [micro]mol/L) to reach 0.3 [+ or -] 0.13 [micro]mol/L at wk 5, followed by a stopping of body weight gain from wk 7. In VAD rats, the CV decreased from wk 5 and reached a value 11% lower than that of the control group (P < 0.001) at wk 14 and was correlated with retinol status ([R.sup.2] = 0.99; P = 0.002). The VAD hippocampal volume decreased beginning at wk 9 and was 22% lower than that of the control group at wk 14 (P < 0.001). Compared with the control, VAD led to lower N acetyl aspartate: creatine+phosphocreatine (Cr) in cortex (-36%), striatum (-22%), and hippocampus (-19%) and higher myoinositol:Cr in cortex (+127%) and striatum (+150%). VAD induced anatomic and metabolic changes comparable to those associated with neurodegenerative disorders. By wk 7 of deprivation, the slowing in cerebral growth that correlated with the retinol level could be considered as a predictive marker of brain disorders, confirmed by metabolic data from VAD rats after 14wk.

Abstract

International audience; Confronted with the COVID-19 crisis, healthcare professionals have had to tackle an epidemic crisis of a huge magnitude for which they were not prepared. Medical laboratories have been on the front line, from collecting samples to performing the analysis required to diagnose this new pathology. Responding to the needs and to the urgency of the situation, the authorities relied on the network of private laboratories. In France, private laboratory medicine represents 70% of overall activity, and with a network of more than 4,000 local laboratories, private laboratory medicine has been the cornerstone of the « screen-trace-isolate » strategy. This article gives feedback from private laboratory medicine professionals, directly involved in the reorganization carried out at the pre-analytical, analytical and post-analytical stages, during the crisis from March to October 2020.

Abstract

La Société française de biologie clinique « Marqueurs biochimiques de COVID-19 » a constitué un groupe de travail ayant pour but premier de faire le point, d’analyser, de suivre l’évolution des prescriptions biologiques en fonction du parcours de soins du patient et de rechercher des marqueurs d’évolutivité et de gravité de la maladie. Cette étude recouvre tous les secteurs publics et privés de la biologie médicale situés en France métropolitaine et ultra-marine et s’étend également à la francophonie. Dans cet article, sont présentés les témoignages et données obtenus pour le sous-groupe de travail « Outre-mer et francophonie » composé de 45 correspondants volontaires, répartis dans 20 régions du monde. Au vu d’une propagation décalée du virus SARS-CoV-2, les régions d’Outre-mer et les régions francophones ont bénéficié des retours d’expériences des premiers territoires confrontés au COVID-19. Ainsi, l’entrée du virus ou sa propagation sous forme épidémique ont pu être évitées grâce à la fermeture rapide des frontières. Les territoires ultramarins dépendent très fortement des liaisons aériennes et/ou maritimes avec la métropole ou avec le continent voisin. L’isolement de ces pays est responsable de difficultés d’approvisionnement en réactifs et a nécessité des commandes en urgence et la mise en place de stocks de plusieurs mois, afin d’éviter les pénuries et de maintenir une prise en charge adéquate des patients. De plus, dans les pays situés en zones tropicales ou intertropicales, le diagnostic de COVID-19 est compliqué par la présence de diverses zoonoses (dengue, Zika, paludisme, leptospirose, etc.)., [Management of a global health crisis: first COVID-19 disease feedback from Overseas and French-speaking countries medical biologists]. The French society of clinical biology "Biochemical markers of COVID-19" has set up a working group with the primary aim of reviewing, analyzing and monitoring the evolution of biological prescriptions according to the patient's care path and to look for markers of progression and severity of the disease. This study covers all public and private sectors of medical biology located in metropolitan and overseas France and also extends to the French-speaking world. This article presents the testimonies and data obtained for the "Overseas and French-speaking countries" sub-working group made up of 45 volunteer correspondents, located in 20 regions of the world. In view of the delayed spread of the SARS-CoV-2 virus, the overseas regions and the French-speaking regions have benefited from feedback from the first territories confronted with COVID-19. Thus, the entry of the virus or its spread in epidemic form could be avoided, thanks to the rapid closure of borders. The overseas territories depend very strongly on air and/or sea links with the metropolis or with the neighboring continent. The isolation of these countries is responsible for reagent supply difficulties and has necessitated emergency orders and the establishment of stocks lasting several months, in order to avoid shortages and maintain adequate patient care. In addition, in countries located in tropical or intertropical zones, the diagnosis of COVID-19 is complicated by the presence of various zoonoses (dengue, Zika, malaria, leptospirosis, etc.).

36. Effects of hyperglycemia on glucose metabolism before and after oral glucose ingestion in normal men

Abstract

The plasma glucose excursion may influence the metabolic responses after oral glucose ingestion. Although previous studies adressed the effects of hyperglycemia in conditions of hyperinsulinemia, it has not been evaluated whether the route of glucose administration (oral vs. intravenous) plays a role. Our aim was to determine the effects of moderately controlled hyperglycemia on glucose metabolism before and after oral glucose ingestion. Eight normal men underwent two oral glucose clamps at 6 and 10 mmol/l plasma glucose. Glucose turnover and cycling rates were measured by infusion of [[sup.2][H.sub.7]]glucose. The oral glucose load was labeled by D-[6,[6-.sub.2][H.sub.2]]glucose to monitor exogenous glucose appearance, and respiratory exchanges were measured by indirect calorimetry. Sixty percent of the oral glucose load appeared in the systemic circulation during both the 6 and 10 mmol/l plasma glucose tests, although less endogenous glucose appeared during the 10 mmol/l tests before glucose ingestion (P < 0.05). This inhibitory effect of hyperglycemia was not detectable after oral glucose ingestion, although glucose utilization was increased (+28%, P < 0.05) due to increased nonoxidative glucose disposal [10 vs. 6 mmol/l: +20%, not significant (NS) before oral glucose ingestion; +40%, P < 0.05 after oral glucose ingestion]. Glucose cycling rates were increased by hyperglycemia (+ 13% before oral glucose ingestion, P < 0.001; + 31% after oral glucose ingestion, P < 0.05) and oral glucose ingestion during both the 6 (+ 10%, P < 0.05) and 10 mmol/1 (+26%, P < 0.005) tests. A moderate hyperglycemia inhibits endogenous glucose production and contributes to glucose tolerance by enhancing nonoxidative glucose disposal. Abstract

International audience; Today, medical prescriptions of proteinuria or albuminuria, which are depending on medical status of patients, still depend on doctors. Indeed, according to their medical specialty or to the recommendations they are following, they could prescribe either proteinuria or albuminuria. The consequences are practical for laboratories and economical for patients and doctors. This article resumes different situations, which result in an erroneous interpretation of medical prescriptions.; Aujourd’hui, les prescriptions médicales de protéinuries ou d’albuminuries, qui ne devraient dépendre que des pathologies des patients, dépendent encore trop du prescripteur. Selon sa spécialité ou les recommandations qu’il suit, l’examen sera différent. Les conséquences sont à la fois pratiques au niveau du laboratoire exécutant et économiques pour les patients et les prescripteurs. Cet article fait le point sur les différentes prescriptions encore pratiquées de nos jours qui posent problème pour leur interprétation.

Abstract

OBJECTIVE Acute glucose fluctuations are associated with hypoglycemia and are emerging risk factors for cardiovascular outcomes. However, the relationship between glycemic variability (GV) and the occurrence of midterm major cardiovascular events (MACE) in patients with diabetes remains unclear. This study investigated the prognostic value of GV in patients with diabetes and acute coronary syndrome (ACS). RESEARCH DESIGN AND METHODS This study included consecutive patients with diabetes and ACS between January 2015 and November 2016. GV was assessed using SD during initial hospitalization. MACE, including new-onset myocardial infarction, acute heart failure, and cardiac death, were recorded. The predictive effects of GV on patient outcomes were analyzed with respect to baseline characteristics and cardiac status. RESULTS A total of 327 patients with diabetes and ACS were enrolled. MACE occurred in 89 patients (27.2%) during a mean follow-up of 16.9 months. During follow-up, 24 patients (7.3%) died of cardiac causes, 35 (10.7%) had new-onset myocardial infarction, and 30 (9.2%) were hospitalized for acute heart failure. Multivariable logistic regression analysis showed that GV >2.70 mmol/L, a Synergy between PCI with Taxus and Cardiac Surgery (SYNTAX) score >34, and reduced left ventricular ejection fraction of 140 was not (OR 1.07 [0.77–1.49]; P = 0.69). CONCLUSIONS A GV cutoff value of >2.70 mmol/L was the strongest independent predictive factor for midterm MACE in patients with diabetes and ACS.
