Aortic arch anomalies represent a range of congenital vascular malformations resulting from disruptions in the typical embryological development of the aortic arch and its branches. These anomalies, which vary widely in their presentation, can lead to significant clinical symptoms depending on their structure and position. We report the case of a 75-year-old male with intermittent hypertension, palpitations, and episodic warmth in the upper body. Computed tomography (CT) angiography revealed an atypical aortic arch anatomy with a unique right subclavian artery anomaly. The aortic arch displayed a typical orientation but included an additional arterial branch arising from the medial wall of the descending aorta. This aberrant branch with a tortuous aspect coursed posteriorly around the esophagus and merged with the subclavian branch of the brachiocephalic trunk, forming a vascular ring. A possible embryological hypothesis requires the persistence of both the distal segment of the right dorsal aorta and the right seventh intersegmental artery, as well as the right fourth aortic arch; however, the imaging aspect of our patient is not that of a classic double aortic arch. This case emphasizes the importance of advanced imaging techniques, such as CT angiography, in identifying and managing rare vascular anomalies that may influence patient care and clinical outcomes. Aortic arch anomalies encompass a diverse spectrum of conditions. Elucidating the prevalence of these anomalies, their impact on patient wellbeing, and the most effective diagnostic tools are crucial steps in ensuring optimal patient care. This paper aims to explore the various presentations of aortic arch anomalies, emphasizing the remarkable utility of computed tomography (CT) angiography in their definitive diagnosis and characterization. We conducted a retrospective study on patients who were submitted to the CT angiography of the thoracic aorta or supra-aortic trunks, or the contrast-enhanced CT scans of the thorax and/or cervical region between January 2021 and February 2024 in our Hospital. Out of the total of 2350 patients, 18 were diagnosed with aortic arch anomalies, with an average age of approximately 55 years. The aortic arch anomalies identified in the study were as follows: left aortic arch with the aberrant origin of the right subclavian artery, right aortic arch (types I and II), double aortic arch, aortic coarctation, aortic pseudocoarctation, and ductus diverticulum. Although often asymptomatic, aortic arch anomalies require recognition and CT using advanced post-processing techniques is the optimal diagnostic method with the ability to also identify other associated cardiac or vascular malformations. Paraneoplastic limbic encephalitis is a rare condition reported in practice. It is most commonly associated with small cell lung cancer (SCLC). This case report is offered to aid physicians in making informed decisions about timing and type of treatment and is evident, that quick diagnosis is critical for both, neurologists and oncologists. The presentation reviews the case of a female patient diagnosed with limbic encephalitis. Further research is needed to establish clinical, laboratory and instrumental criteria that may be related to outcomes. The purpose of this paper is to present the potential repercussions of a delayed diagnosis and highlight the beneficial results of specific investigations and symptomatic therapy. Comprehensive familiarity with clinical presentations and the limitations of current diagnostic procedures is imperative for neurologists. Equally essential is this understanding for radiologists, serving as the basis for accurate diagnostic analyses derived from imaging findings. The intricate nature of neurological disorders sometimes necessitates the cooperation of neurologists, radiologists, and, in this particular instance, oncologists, in order to achieve precise diagnosis and develop successful treatment approaches. We report a case of a 63-year-old male patient with multiple cardiovascular risk factors and previous myocardial infarction who was referred to the emergency department on September 2023 with symptoms and clinical and biological data consistent with an acute coronary event. A coronary angiography revealed severe ostial stenosis of the left anterior descending artery (LAD) and intrastent thrombotic occlusion in the first two segments of the LAD. Two drug-eluting stents were implanted and the patient was discharged when hemodynamically stable; however, three weeks later, he returned to the emergency department complaining of fever, anterior chest pain, dyspnea at rest, and high blood pressure values at home. High levels of troponin T, C-reactive protein, and NT-proBNP were detected and blood cultures showed methicillin-resistant Staphylococcus aureus. The computed tomography (CT) examination showed a saccular dilatation had developed between two fragments of a stent mounted at the level of the LAD, surrounded by a hematic pericardial accumulation. LAD pseudoaneurysm ablation and a double aortocoronary bypass with inverted saphenous vein autograft were performed and the patient showed a favorable postoperative evolution. In this case, surgical revascularization was proven to be the appropriate treatment strategy, demonstrating the need to choose an individualized therapeutic option depending on case-specific factors. Background: The aim of the study was to determine the contribution of 3-dimensional computer tomography (3D-CT) in the diagnosis of the articular fractures compared with X-rays and computer tomography (CT), and to establish if 3D-CT can confirm or modify the preoperative plans. Methods: We have conducted a retrospective study between 2020 - 2022, 46 patients with articular fractures. We have asked 5 independent surgeons to make comparative analyses of the information required for a correct diagnosis and preoperative planning, obtained from X-ray, CT and 3D-CT. Each time, the surgeon was requested to categorize the fracture using specific classification systems and articulate their recommended treatment approach. Results: The study showed us the importance of 3D-CT to achieve a correct diagnosis for the articular fractures by ana-lyzing the discrepancies between X-ray, CT and 3D-CT, with direct influence upon the presurgical plan. In 37% of the cases, fracture lines were not apparent on X-rays but were visible on CT scans. However, in more than 27% of these instances, specific bone injuries, like central articular depression or unique split-comminution patterns, were not evident on standard CT scans but could be identified using 3D-CT scans. Conclusions: To establish the correct diagnosis and the proper therapeutical plan for the articular fractures, it is necessary to make a complete ra-diological evaluation, which consists of X-ray, CT and 3D-CT. The purpose of the present study was to emphasize significant concerns, specifically for neurologists and radiologists, due to the difficulties involved in identifying this disorder. Neurologists must have an extensive understanding of the clinical manifestations and constraints of current diagnostic tests. Furthermore, this understanding is equally essential for radiologists, as it serves as the foundation for precise diagnostic interpretations derived from imaging findings. The present study focused on examining the association between the SARS-CoV-2 virus, responsible for the COVID-19 pandemic, and cerebral venous thrombosis (CVT), a specific form of stroke that affects the brain's vessels and sinuses. While COVID-19 is primarily recognized for its respiratory impact, it may also affect other organs, including the brain. One notable aspect of COVID-19 is its association with coagulopathy, an abnormal condition of blood clotting. Coagulopathy may result in various complications, including neurological ones such as stroke. The study analyzed data obtained from patients admitted to a neurology department who had confirmed neurological pathologies along with COVID-19. It specifically examined the cases of three patients with neurological conditions and COVID-19, discussing their risk factors and how their conditions progressed clinically. The study concluded that COVID-19 infection increases the likelihood of stroke, particularly within the initial 10 days after infection. CVT in particular is strongly linked to COVID-19 and its underlying mechanisms involve immune systemic processes, cytokine storms, increased blood thickness, thrombogenesis, hypercoagulability and inflammation. The presence of SARS-CoV-2 infection may worsen the procoagulant cascade, thereby affecting the clinical condition of patients with CVT. The study underscores the importance of recognizing this uncommon but treatable consequence of COVID-19 infection. Furthermore, it highlights the uniqueness of the study in evaluating COVID-19 infection in patients with CVT from Romania and South-East Europe. The findings support the existence of neurological disorders, including clotting complications in the brain's sinuses and vessels, in individuals infected with SARS-CoV-2. Several risk factors contribute to the development of CVT, such as infections, oral contraceptives, pregnancy, hematological disorders, trauma, autoimmune disorders and malignancies. The aim of the present study was to accurately measure the diameter of the ophthalmic artery (OA) and investigate whether bilateral variations in diameter can be recorded in relation to patient age and sex. A retrospective study including 80 computed tomography angiographic (CTA) examinations and a total of 160 arteries was conducted to demonstrate the morphometric aspects of the OAs analyzed bilaterally by CTA examinations, while considering the references of the internal carotid artery (ICA) caliber. Precise measurements performed on the OA and the ICA below and above the ophthalmic emergence revealed an OA diameter of 1.38±0.24 mm and a narrowing of the ICA between the origin of the OA of 1.5±0.25 mm. Variations in the OA and the ICA calibers were studied in subjects with normal cervical vasculature on CTA. After a thorough statistical study, variations in OA and ICA caliber on each side were identified, between both sex and age-related groups, revealing morphometric parameters of the OA in relation to the ICA. Coronary arteries have a wide range of anatomical variability, and their spectrum ranges from asymptomatic cases to those predisposed to hemodynamic compromise or even sudden cardiac death. This paper aims to review the classification of coronary artery anomalies (CAAs) and illustrate their imaging characteristics by highlighting the important role of CT coronary angiography. Some of the coronary anomalies usually met in current practice are the high origin coronary artery, multiple ostia, aberrant origin from the opposite/non-coronary Valsalva sinus, single coronary artery, ALCAPA syndrome, duplications of the left anterior descending artery, coronary fistulas, and extracardiac terminations. CT coronary angiography is a non-invasive diagnostic modality for CAAs. The complex anatomy of these anomalies can be accurately described by employing 3D reconstructions and post-processing techniques. Knowledge of the imaging characteristics and potential functional impact of these anomalies is essential for accurate diagnosis and therapeutic planning of patients.
