Aim: In our study, we aimed to evaluate the frequency of MSI in endometrial cancer and its relationship with prognostic parameters and inflammatory indexes. Material and Methods: MLH1, MSH2, MSH6, and PMS2 mutations were evaluated immunohistochemically in paraffin blocks of 74 patients diagnosed with endometrial cancer. If no staining was detected in any of these proteins, the tumor was considered microsatellite instable (MSI). Demographic and pathological data of the patients were obtained from the file records. Results: MSI was detected in 21 (28.3%) of 74 patients; expression loss of MLH-1 /PMS-2 (18.9%) in 14, PMS-2 (4.03%) in 3, MSH-2 (2.7%) in 2, MSH-6/PMS-2 (1.3%) in 1 and loss of expression of MSH-2/PMS-2 (1.3%) was observed in 1 of them. There was no significant difference between MSS and MSI groups in terms of age, grade, lymph node, stage, LVI, histological subtype. The 5-year survival rates were 73% in the MSS group and 46% in the MSI group, but this difference was not statistically significant (p=0.760). Discussion: In conclusion, in this study, it was determined that MSI status did not affect the prognosis in endometrial cancers. With the widespread use of immunotherapies, the predictive role of knowledge of the MSI status in endometrial cancer comes to the forefront rather than its prognostic value. In addition, knowledge of the MSI status has gained importance in the new molecular classification of endometrial cancer.

PATIENTS AND METHODS: Data of 501 patients diagnosed with stage 2 colon cancer were evaluated retrospectively. It was evaluated whether the patients' age, gender, tumor differentiation, tumor node metastasis (TNM) stage, overall survival rate, and disease-free survival rate had any correlation with horizontal tumor diameter and tumor sidedness. In the ROC analysis performed to determine the cut-off value for the tumor diameter, which we think will predict survival, no significant results were obtained with maximum sensitivity and specificity. Therefore, the median value of the tumor diameter, which is 5 cm, was accepted as the cut-off value. Kaplan-Meier method and Cox regression analysis were used for survival analysis and determination of prognostic factors. RESULTS: When the patients were evaluated in terms of tumor localization, 189 (37.7%) patients had right colon tumors and 312 (62.3%) patients had left colon tumors. OBJECTIVE: In this study, we aimed to evaluate the prognostic value of preoperative serum ferritin level in patients with stage 2 colon cancer who underwent curative surgery. PATIENTS AND METHODS: The data of 120 patients who were stage 2 after curative surgery and whose ferritin levels were measured at the time of diagnosis without starting any treatment were analyzed. Demographic data such as age and gender, histopathological characteristics such as tumor size, lymphovascular invasion (LVI), perineural invasion (PNI), number of removed lymph nodes, tumor grade, and clinical and laboratory data were retrieved from the hospital medical charts or electronic medical records. In the survival analysis, the cut-off level of ferritin was accepted as 150 ng/ml, which is the upper limit determined by the World Health Organization (WHO), as a prognostic factor. RESULTS: Fifty (41.7%) of the patients were female, 70 (58.3%) were male, and the median age was 63.5 (range 24-90) years. There was no significant difference between the low and high ferritin groups regarding age, gender, T stage, tumor localization, histological subtype, PNI, LVI, removal of less than 12 lymph nodes, and tumor size. Disease-free survival and overall survival of patients with high ferritin levels were worse than patients with low ferritin levels, but this difference did not reach statistical significance. CONCLUSIONS: Serum ferritin level is an easily monitored, cost-effective, and reproducible marker. İn this study we found that high ferritin level was associated with poor survival, although it was not statistically significant.

Gebelikte meme kanseri oldukça nadir olmasına rağmen, gebelik sırasında oluşan kanser türleri içinde serviks kanserinden sonra en sık görülen 2. kanserdir. Gebelikte meme kanseri görülme sıklığının artması önemli bir problemdir. Gebelikte ve laktasyon dönemindeki fizyolojik değişiklikler nedeniyle memede meydana gelebilecek patolojilerin tanınması ve tedavisi zorluk göstermektedir. Gebelik ve laktasyon döneminde bir çok benign patoloji oluşabileceği gibi meme kanseri de görülebilir. Bu nedenle gebelikte görülen her kitlenin araştırılması gerekir.Gebelikte saptanan meme kanserinin tedavisi planlanırken hem annenin hem de bebeğin her ikisinin de yaşamını tehtid eden durumlar dikkate alınmalıdır. Tanının gecikmesi nedeniyle prognoz daha kötüdür. Biz bu olgu sunumunda gebelikte meme kanseri metastazına bağlı gelişen nörolojik defisit nedeniyle acil servisimize başvuran hastayı sunmayı ve bu vaka üzerinden literatürü gözden geçirmeyi amaçladık.Anahtar Kelimeler: meme kanseri, gebelik, metastaz, nörolojik defisit

Amaç: Bu araştırmanın amacı Yale Yeme Bağımlılığı Ölçeğinin Türk bariatrik cerrahi hasta grubunda geçerlilik ve güvenilirliğini incelemektir. Yöntem: Bariatrik cerrahi arayışında olan obez hastalara (s=171) Yale Yeme Bağımlılığı Ölçeği uygulandı. Ölçeğin yapı geçerliliği faktör analizi ile, güvenilirliği madde toplam puan korelasyonu ile, tekrarlanabilirliği ise test-tekrar test sonuçları arasında sınıf içi korelasyon katsayısı hesaplanılarak değerlendirildi. Bulgular: Yirmi beş maddeli ölçeğin Kuder-Richardson Formula-20 katsayısı ile hesaplanan iç tutarlılığı (KR-20=0.822) ve Cronbach alfa (0.859) değerleri yeterli bulundu. Bartlett's Sphericity Testine göre geliştirilen faktör yapısının anlamlı olduğu kararına varıldı. Faktör analizine göre altı belirgin faktör olarak kümelenen bu yapı total varyansın %67.51'ini açıklıyordu. Ölçeğin madde toplam korelasyonları 0.214-0.666 arasında idi. Sonuç: Bulgularımız Yale Yeme Bağımlılığı Ölçeği'nin Türkçe formunun bariatrik cerrahi hasta grubunda yeme bağımlılığını ölçmede güvenilir bir araç olduğunu göstermektedir. (Anadolu Psikiyatri Derg 2015; 16(Özel sayı.1):44-53), Objective: The aim of this study was to examine validity and reliability of Turkish version of Yale Food Addiction Scale (YFAS) among Turkish bariatric surgery patients. Methods: The YFAS scale was administered to obese patients (n=171) who were seeking or underwent bariatric surgery. Construct validity of the scale was evaluated with factor analysis and reliability was evaluated with item -total score correlation and repeatability were tested by intraclass correlation (ICC) analysis between test-retest results. Results: Internal concistency was found adequate Kuder-Richardson Formula 20 coefficient (KR-20) 0.822, and Cronbach's alpha 0.859 for the entire 25-item YFAS. As Bartlett's Test of Sphericity was significant, the factor model developed in the present study was decided appro priate. Factor analysis extracted six factor in Turkish YFAS that explained for 67.51% of the total variance. Item total correlation coefficients of scale ranged from 0.214-0.666. Conclusion: Our findings support the use of the Turkish YFAS as a reliable measure of food addiction among bariatric surgery patients. Eventually, all patients with malignant gliomas recur or progress. Unfortunately, the optimal regimen in the salvage setting has not yet been defined. We retrospectively evaluated 52 patients with malignant gliomas who failed temozolomide therapy and were treated with a combination of intravenous carboplatin and oral cyclophosphamide. The median age of all patients, including those with glioblastoma multiforme (GBM) (n= 40) and anaplastic glioma (AG) (n= 12), was 45.5 years (range 23-68). All patients were treated with consolidation temozolomide after chemoradiotherapy. After temozolomide failure, second surgery was performed on 15, reirradiation on four and radiosurgery on three patients. The median number of chemotherapy cycles was 4 (range 1-8), the clinical benefit was 67.3%, a partial response was achieved in 26.9% and stable disease in 40.4%. In the GBM group, median progression-free survival (PFS) and overall survival (OS) were 3 (95% CI, 2.31-3.69) and 8 (95% CI, 4.76-11.24) months, respectively. In the AG group, median PFS and OS were 5 (95% CI, 3.51-6.49) and 11(95% CI, 6.38-15.62) months, respectively. The six-month PFS rate was 25%. Only one patient survived 18 months after treatment. Serious toxicity was mainly hematological.

Gastrik ve Gastro-özefageal Bileşke Kanserlerinde HER2 Overekspresyonunun Klinik ÖnemiAmaç: Bu çalışmada gastrik kanser (GC) ve gastroözefageal bileşke kanserlerinde (GEJC) HER2 overekspresyon oranı, HER2 overekspresyonunun klinik, patolojik parametrelerle ilişkisi ve prognoz üzerine etkisi araştırıldı.Gereç ve Yöntem: Gastrik kanser ve GEJC tanılı 285 (202 erkek, 83 kadın) hastanın cerrahi ya da biyopsi preperatlarında HER2 ekspresyon varlığı immunohistokimyasal (IHK) yöntemle ve silver insitu hibridizasyon (SISH) yöntemiyle değerlendirildi. HER2 (+)'liğinin tümörün çapı, histopatolojisi, grade'i, seroza invazyonu (Sİ), lenfovasküler (LVİ) ve perinöral invazyon (PNİ), Lauren ve Borrmann tipi, tümörün lokalizasyonu, TNM evresi, izlemde lokal nüks (LN) ve metastaz (M) gelişimi ilişkisi, sağkalım (OS) üzerine etkisi araştırıldı.Bulgular: IHK skorları sırasıyla: 194 (%68.1) IHK 0, 34 (%11.9) IHK +1, 30 (%10.5) IHK +2, 27 (%9.5) IHK +3. IHK +2 olan 30 hastanın 12'de SISH (+) (%4.2), 18'inde SISH (-). IHK +3 ve IHK +2 olup SISH (+) hasta sayısı birlikte değerlendirildiğinde HER2 (+)'liği %13.7'di. HER2 (+)'liğiyle yaş, cinsiyet, TNM evresi, tümör çapı, tümörün lokalizasyonu LN ve M gelişimi, LVİ ve PNİ, Borrman tipi arasında ilişki yoktu. İntestinal tip tümörlerde diffüz tipe göre HER2 (+)'liği daha fazlaydı (%16.7 vs %6.9, p=0.075). İyi-orta derecede diferansiye tümörlerde kötü diferansiye tümörlere göre HER2 (+)'liği anlamlı oranda yüksekti (p=0.001). Pür adenokarsinomlarda %19.4, taşlı yüzük hücreli karsinomlarda %4.2, mikst adenokarsinom-taşlıyüzük hücreli karsinomlarda %10 HER2 (+)'di (p=0.013). HER2 (+)'liğinin OS üzerine anlamlı etkisi saptanmadı (p=0.81), ancak erken evre HER2 (+) hastaların median OS'leri daha kısaydı (p=0.022).Sonuç: Gastrik kanserler ve GEJC'de HER2 (+)'liği tümörün histopatolojisi ve diferansiasyon derecesiyle ilişkilidir. Erken evre hastalarda HER2 (+)'liği kötü prognozla ilgilidir. Clinical Significance of HER2 Overexpression in Gastric and Gastroesophageal Junction CancersObjectives: In this study, we investigated the rate of HER2 overexpression in gastric and gastroesophageal junction cancers (GC, GEJC), and the relationship with HER2 expression and clinical, pathological parameters and prognosis.Materials and Methods: Surgery or biopsy specimen of 285 (202 male, 83 female) patients with GC or GEJC, the presence of HER2 overexpression by immunohistochemistry and silver insitu hybridization (SISH) were evaluated. The relationship between HER2 pozitivity and tumor size, histopathology, grade, serosal invasion (SI), lenfovaskular invasion (LVI), perineural invasion (PNI), Lauren and Borrmann type, tumor location, TNM stage, the development of local recurrence and metastatisis during follow up and survival (OS) were investigated.Results: IHC scores were; 194 (68.1%) IHC 0, 34 (11.9%) IHC +1, 30 (10.5%) IHC +2, 27 (9.5%) IHC +3. 12 of 30 (4.2%) patients with IHC +2, SISH positive, and 18 patients SISH negative. The number of patients evaluated with IHC +3 or IHC +2 and SISH (+), HER2 positivity was 13.7%. There was no relationship between HER (+) and the age, gender, TNM stage, tumor size and localization, development of LN and M, LVI, PNI, Borrmann type. HER2 (+) was higher in intestinal type tumors than in diffuz type (16.7% vs 6.9%, p=0.075). HER (+) was significantly higher in well-moderate diferantiated tumors than poorly diferantiated tumors (p=0.001). 19.4% of adenocarcinomas, 4.2% of signet-ring cell carcinomas, 10% of mixed adenocarcinoma-signet ring cell carcinomas were HER (+) (p=0.013). HER2 pozitivity was no significant effect on OS (p=0.81). But the early-stage HER2 (+) patients, median OS was shorter than advanced stage (p=0.022).Conclusion: HER2 positivity is associated with the degree of tumor differantiation and histopathology in GC and GEJC. Patients with early-stage, HER2 positivity is related to poor prognosis. 119
