[Aims] Patients with adult congenital heart disease (ACHD) are a potentially vulnerable patient cohort in case of COVID-19. Some cardiac defects may be associated with a poor COVID-19 outcome. Risk estimation in ACHD is currently based on expert opinion. The aim of this study was to collect clinical outcome data and to identify risk factors for a complicated course of COVID-19 in patients with ACHD., [Methods] Twenty-five ACHD centres in nine European countries participated in the study. Consecutive patients with ACHD diagnosed with COVID-19 presenting to one of the participating centres between 27 March and 6 June 2020 were included. A complicated disease course was defined as hospitalisation for COVID-19 requiring non-invasive or invasive ventilation and/or inotropic support, or a fatal outcome., [Results] Of 105 patients with a mean age of 38±13 years (58% women), 13 had a complicated disease course, of whom 5 died. In univariable analysis, age (OR 1.3, 95% CI 1.1 to 1.7, per 5 years), ≥2 comorbidities (OR 7.1, 95% CI 2.1 to 24.5), body mass index of >25 kg/m2 (OR 7.2, 95% CI 1.9 to 28.3) and cyanotic heart disease (OR 13.2, 95% CI 2.5 to 68.4) were associated with a complicated disease course. In a multivariable logistic regression model, cyanotic heart disease was the most important predictor (OR 60.0, 95% CI 7.6 to 474.0)., [Conclusions] Among patients with ACHD, general risk factors (age, obesity and multiple comorbidities) are associated with an increased risk of complicated COVID-19 course. Congenital cardiac defects at particularly high risk were cyanotic lesions, including unrepaired cyanotic defects or Eisenmenger syndrome., The European Collaboration for Prospective Outcome Research in Congenital Heart Disease is funded by internal grants without support from the pharmaceutical industry.

Die aktualisierte zweite Auflage des „Curriculum Kardiologie“ – Erstauflage 2013 – möchte aus Sicht der Fachgesellschaft aufzeigen, welche Kompetenzen ein Kardiologe heute beherrschen sollte. Sehr zu begrüßen ist, dass in dieser zweiten Auflage nun auch Vertreter der Young DGK als Autoren mitgearbeitet haben. Die zunehmende Spezialisierung innerhalb der Kardiologie darf dabei jedoch nur die eine Seite der Medaille darstellen: Auch weiterhin muss es ein gemeinsames Fundament der Kardiologie geben, eingebettet in das Fach „Innere Medizin“. Dieses Fundament beinhaltet den Grundstock an theoretischen Kenntnissen, an praktischen Fähigkeiten (Kompetenzlevel I–III) und an beruflich-professioneller Einstellung des (angehenden) Kardiologen. Neues für die Weiterbildung ist seit der Erstauflage des Curriculums 2013 hinzugekommen, so z. B. ein Kapitel „Digitale Kardiologie“, die neu in die Muster-Weiterbildungsordnung für den Kardiologen aufgenommene Weiterbildung in „Psychokardiologie“ und schließlich auch die explizite Formulierung der Partizipativen Entscheidungsfindung („Shared Decision Making“) im Interesse des Herzpatienten. Dem Weiterzubildenden soll das Curriculum die Möglichkeit geben, seine Weiterbildungszeit so effizient wie möglich zu strukturieren und das Erlernte anschließend im Sinne eines „berufslebenslangen“ Qualifizierens zu bewahren und auszubauen. Das Curriculum möchte aber auch die Weiterbilder und die Ärztekammern erreichen und aufzeigen, welche Inhalte und Fertigkeiten nach Ansicht der Deutschen Gesellschaft für Kardiologie – Herz- und Kreislaufforschung (DGK) in der Weiterbildung zum Kardiologen vermittelt werden sollen. The American Heart Association and American College of Cardiology published practice guidelines for the management of adult congenital heart disease in 2018 and the European Society of Cardiology published analogous guidelines in 2020. Although there are broad areas of consensus between the 2 documents, there are important differences that impact patient management. This review discusses key areas of agreement and disagreement between the 2 guidelines, with discussion of possible reasons for disagreement and potential implications. Aims The post-approval MELODY Registry aimed to obtain multicentre registry data after transcatheter pulmonary valve implantation (TPVI) with the Melody™ valve (Medtronic plc.) in a large-scale cohort of patients with congenital heart disease (CHD). Methods and results Retrospective analysis of multicentre registry data after TPVI with the Melody™ valve. Eight hundred and forty-five patients (mean age: 21.0 ± 11.1 years) underwent TPVI in 42 centres between December 2006 and September 2013 and were followed-up for a median of 5.9 years (range: 0–11.0 years). The composite endpoint of TPVI-related events during follow-up (i.e. death, reoperation, or reintervention >48 h after TPVI) showed an incidence rate of 4.2% per person per year [95% confidence interval (CI) 3.7–4.9]. Transcatheter pulmonary valve implantation infective endocarditis (I.E.) showed an incidence rate of 2.3% per person per year (95% CI 1.9–2.8) and resulted in significant morbidity and in nine deaths. In multivariable Cox proportional hazard models, the invasively measured residual right ventricle (RV)-to-pulmonary artery (PA) pressure gradient (per 5 mmHg) was associated with the risk of the composite endpoint (adjusted hazard ratio: 1.21, 95% CI 1.12–1.30; P 2 improved significantly from 36 [interquartile range (IQR) 24–47] to 12 (IQR 7–17) mmHg and 47 to 1%, respectively (P Conclusion The post-approval MELODY Registry confirms the efficacy of TPVI with the Melody™ valve in a large-scale cohort of CHD patients.
