Abstract

Admissions to jails and prisons in the United States number 10 million yearly; persons entering locked correctional facilities have high prevalence of sexually transmitted infections (STIs). These individuals come disproportionately from communities of color, with lower access to care and prevention, compared with the United States as a whole. Following PRISMA guidelines, the authors present results of a systematic review of literature published since 2012 on STIs in US jails, prisons, Immigration and Customs Enforcement detention centers, and juvenile facilities. This updates an earlier review of STIs in short-term facilities. This current review contributed to new recommendations in the Centers for Disease Control and Prevention 2021 treatment guidelines for STIs, advising screening for Trichomonas in women entering correctional facilities. The current review also synthesizes recommendations on screening: in particular, opt-out testing is superior to opt-in protocols. Carceral interventions—managing diagnosed cases and preventing new infections from occurring (eg, by initiating human immunodeficiency virus preexposure prophylaxis before release)—can counteract structural racism in healthcare.

Abstract

Background: Limited data about victims of domestic violence in health care settings hinder development of appropriate services. A comprehensive program was established in a managed care organization to increase identification and referral of domestically abused female patients. Methods: Female victims of domestic abuse were referred to a trained social worker for further assessment. Information about the women was obtained from clinical consultation forms; initial interviews conducted by social workers; a survey administered to a convenience sample of women seen by the program; and medical chart review. Results: Of 265 women who agreed to a domestic violence referral, 177 (67%) were contacted for further evaluation. The study sample was ethnically diverse and included female victims seen for routine care, women who had been assaulted, women who had depression, and women with various somatic symptoms. Responses from 51 of the 177 women showed the most cited reasons for accepting referral were unhappiness with current situation, wanting to leave or change the situation, concern about children who witnessed abuse, and the suggestion by a health care practitioner that the patient's symptoms could be related to the abuse. Most reported having symptoms of depression in the previous year. Conclusions: Comprehensive programs in the health care setting can increase identification of victims of domestic abuse. This descriptive report provides a greater understanding of victims of domestic abuse, their presentation in the medical setting, their motivation for accepting referral, and issues which affect their recovery. Abstract

Genital human papillomavirus (HPV) is a sexually transmitted disease implicated in the development of abnormal growths and cancer of the cervix, vagina, and vulva. Although the epidemiology of HPV is well studied, the incidence of subclinical infection is unknown. This study of 467 college women used both the ViraPap test and polymerase chain reaction (PCR) to determine HPV infection. The women were predominantly white, in their early twenties, single, had never been pregnant, and averaged four sexual partners overall. Most had used oral contraceptives. Ninety-eight percent were at the health clinic for an annual examination, contraception, or menstrual problems. The ViraPap test detected a positive result at the cervix in seven percent. The PCR-based system found a positivity rate of 33 percent at the cervix, although there is some increase in the probably of false-positive results, that is, finding that the infection is present when it is not. The ViraPap test is easier to use, but is less sensitive. Clearly, HPV infection is common among healthy young women, although the prevalence may decrease with age. Genetic and environmental factors may cause progression to cervical cancer, but HPV infection could still be predictive in a particular group of women. How such a group might be identified is not known. HPV infection did not necessarily infect both the cervical and vulvar sites, so it appears that these infections may occur independently. The prevalence of infections with multiple HPV types was underestimated by both testing procedures. A test to detect multiple infections is under development.
