BackgroundBedside ultrasonography, also known as point-of-care ultrasound (PoCUS), is a promising technological tool that enhances clinical assessment, enriching diagnostic capabilities and clinical reasoning. Its use in nursing spans various patient populations and health care settings, providing nurses with a valuable health assessment tool to improve care quality and patient safety. Despite its growing integration into clinical practice, PoCUS training has mainly focused on physicians, leaving a gap for trained nurses who demonstrate similar proficiency in conducting scans and interpreting images. Previous research highlights the value of digital tools in PoCUS training, showing their role in improving professionals’ and students’ knowledge, image interpretation skills, and clinical acumen. ObjectiveThis study aimed to (1) establish an assessment instrument gauging nurses’ competency milestones in PoCUS and evaluate its content and appearance validity, (2) develop a series of 5 educational videos focused on PoCUS and assess their content and appearance validity, and (3) construct an online learning environment tailored to nurses’ PoCUS training needs and evaluate its content and appearance validity. MethodsWe will conduct a methodological study of technological production guided by Rogers’ diffusion of innovations theory. Subproject 1 will design and validate a comprehensive assessment tool for evaluating nurses’ competency milestones in PoCUS use. For this purpose, a scoping review will be conducted. The review will be based on JBI Collaboration guidelines and reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extended for Scoping Reviews (PRISMA-ScR) checklist. Subproject 2 involves an evaluation of content and appearance validity for a series of 5 educational videos on PoCUS, designed specifically for nurses about applying peripherally inserted central catheter lines, inserting nasogastric feeding tubes, assessing gastric residual volume, assessing pressure injuries and soft tissue conditions, and assessing muscle mass to monitor patient nutritional status. In subproject 3, a comprehensive online learning environment dedicated to PoCUS training for nurses will be developed and validated. The launch of an online learning environment represents a cornerstone of our dissemination strategy, scheduled to coincide with the inaugural Brazilian PoCUS symposium for nurses, an event organized by the project members. This platform will serve as a pivotal resource for continuous learning and professional development. ResultsSubproject 1 will start in the second half of 2024 and is expected to be completed by mid-2025. Subproject 2 is currently ongoing and is expected to be completed in early 2026. Subproject 3 is set to begin in early 2025 and is planned to be completed by 2026. ConclusionsThrough these concerted efforts, the project aims to bridge the existing gap in PoCUS training for nurses, thereby fostering their proficiency and enhancing patient care outcomes. ABSTRACT: Leptospirosis is a zoonotic disease that must be studied on the One Health point of view. It is possible that there are particularities in the epidemiology of leptospirosis in Caatinga biome, where the environment is often unfavorable and challenges the adaptability of Leptospira spp. Overall, 100 dogs (47 shelter and 53 stray dogs) selected based on sampling calculation were used to evaluate the Leptospira spp. seroprevalence and associated factors. The microscopic agglutination test (MAT) was applied to detect anti-Leptospira spp. antibodies (cut-off titer 50). Risk factors were identified using the robust Poisson regression analysis. Twenty-four animals (24%; 95% CI = 15.6% - 32.4%) were seroreactive, antibody titers ranged from 50 to 200, and the reacting serogroups were Ballum (17%), Autumnalis (6%) and Djasiman (1%). The factors/categories associated with seropositivity were the environment where the animal stay/soil (prevalence ratio [PR] = 6.03; 95% CI for PR: 1.86 - 7.69; P < 0.001) and access to polluted water/yes (PR = 3.79; 95% CI for PR: 1.85 - 24.22; P = 0.011). The results suggested leptospirosis as a concern in the One Health context in stray and shelter dogs from the Caatinga biome despite the adverse conditions of this biome for the survive of Leptospira spp. on the environment. Moreover, despite being social and government issues, factors such as environment where the animal stay (soil) and access to polluted water must be carefully deemed and corrected to avoid the transmission of leptospires to animals and humans.

BACKGROUND AND OBJECTIVES: This research stems from the scarcity of official data regarding the homeless population and the challenging realities faced by this group, such as social invisibility and precarious access to healthcare services. The main objective is to conduct a narrative review to understand the interconnection between chronic pain (CP), depression, and sleep disorders in this population, identifying key factors that play a role in these conditions. CONTENTS: The study employs a qualitative narrative review, conducting a comprehensive search in the Web of Science and Scopus databases without temporal restrictions. It utilizes English descriptors related to homelessness, depression, and sleep disorders. Globally, over 100 million people are homeless, with Brazil particularly affected by this phenomenon due to the economic crisis. The prevalence of CP in this group is linked to precarious living conditions, discrimination, and exposure to violence. The impairment of sleep quality, depression, and the bidirectional relationship between CP and depressive disorders are highlighted. Social vulnerability, lack of access to health resources, and the adversities of living conditions contribute to the complexity of challenges faced by the homeless population, negatively impacting overall health. CONCLUSION: Individuals experiencing homelessness face increased vulnerability to injuries, CP, depression, and sleep disorders. The bidirectional relationship between pain and depression is exacerbated by the adversities they endure. It is crucial to implement specific and effective health policies to improve living conditions, promoting equity and social justice for this vulnerable and marginalized population.

Cannabinoids ,Pain ,Treatment outcome ,Review ,Anesthesiology ,RD78.3-87.3 - Abstract
There is growing interest in using cannabinoids across various clinical scenarios, including pain medicine, leading to the disregard of regulatory protocols in some countries. Legislation has been implemented in Brazil, specifically in the state of São Paulo, permitting the distribution of cannabinoid products by health authorities for clinical purposes, free of charge for patients, upon professional prescription. Thus, it is imperative to assess the existing evidence regarding the efficacy and safety of these products in pain management. In light of this, the São Paulo State Society of Anesthesiology (SAESP) established a task force to conduct a narrative review on the topic using the Delphi method, requiring a minimum agreement of 60% among panelists. The study concluded that cannabinoid products could potentially serve as adjuncts in pain management but stressed the importance of judicious prescription. Nevertheless, this review advises against their use for acute pain and cancer-related pain. In other clinical scenarios, established treatments should take precedence, particularly when clinical protocols are available, such as in neuropathic pain. Only patients exhibiting poor therapeutic responses to established protocols or demonstrating intolerance to recommended management may be considered as potential candidates for cannabinoids, which should be prescribed by physicians experienced in handling these substances. Special attention should be given to individual patient characteristics and the likelihood of drug interactions.

The aim of this paper is to analyse the concept of testimony and to promote reflections that support the understanding of this linguistic act through the lens of the concept of Zeitzeuge/Zeitzeugenschaft. Our analyses aim to observe how the value of the witness is constructed in the paratext of Ruth Klüger’s work weiter leben, from the German original and its paperback version (1992; 1997) to the later versions in French (1997), English (2001) and Portuguese (2005). In addition to drawing on the work of various scholars on the notion of testimony, this paper is based on the principles of dialogic discourse theory in conjunction with the category of paratext proposed by Genette (2009). Among other things, this study shows how discourse in the testimony is directly influenced by the valorisation of the role of the witness, which in the case analysed leads to different ways of reading the “same work”.

Background: This study aimed to investigate the analgesic impact of S(+)-ketamine on pain behavior and synovial inflammation in an osteoarthritis (OA) model. Methods: Animals were grouped as follows: OA-Saline (n = 24) and OA-Ketamine (n = 24), OA induced via intra-articular sodium monoiodoacetate (MIA); a Non-OA group (n = 24) served as the control. On the 7th day post OA induction, animals received either saline or S(+)-ketamine (0.5 mg.kg−1). Behavioral and histopathological assessments were conducted up to day 28. Results: S(+)-ketamine reduced allodynia from day 7 to 28 and hyperalgesia from day 10 to 28. It notably alleviated weight distribution deficits from day 10 until the end of the study. Significant walking improvement was observed on day 14 in S(+)-ketamine-treated rats. Starting on day 14, OA groups showed grip force decline, which was countered by S(+)-ketamine on day 21. However, S(+)-ketamine did not diminish synovial inflammation. Conclusion: Low Intra-articular (IA) doses of S(+)-ketamine reduced MIA-induced OA pain but did not reverse synovial histopathological changes. IRB approval number: 23115 012030/2009-05.

Summary: Background: Patients admitted to the Intensive Care Unit (ICU) are at greater risk of developing nosocomial infections due to their investigations, treatment and changes in the immune system. One of the most prevalent nosocomial infections is respiratory tract infection, such as hospital acquired pneumonia and ventilator-associated pneumonia (VAP). The bacteria commonly found in the oral cavity in the hospital environment are Streptococcus viridians, Staphylococcus aureus, Pseudomonas aeruginosa, Enterococcus spp., and Klebsiella pneumoniae. There is a need to test and define appropriate standard protocols for oral hygiene in patients undergoing mechanical ventilation in ICUs through the intervention of a dental specialist, preventing the proliferation of microorganisms into the respiratory tract, thus reducing hospitalization time, the use of antibiotics, and increased morbidity/mortality. Objective: This study aimed to evaluate the effectiveness of dental brushing in the reduction of the pathogenic buccal microbiota associated with mechanical ventilation in patients admitted to the Evangelical Hospital from Londrina, Paraná, Brazil. Methodology: The sample consisted of 90 patients (of both sexes), mean age of 65 years, under mechanical ventilation by orotracheal tube and tracheostomized patients, without suspected or confirmed diagnosis of pneumonia. Patients were randomized ∗∗∗ Results: Results showed that oral hygiene using a toothbrush by suction, with chlorhexidine gel 0.12% (Group B), was more effective than conventional hygiene using gauze soaked with chlorhexidine 0.12% (Group A) in reducing pathogenic buccal microbiota. Conclusions: There was a reduction of the pathogenic buccal microbiota in mechanically ventilated patients receiving oral hygiene using a toothbrush by suction with chlorhexidine gel 0.12% (Group B)

Docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA) supplementation has been used to improve cognitive outcomes in diseases associated with cognition. However, the influence of APOEε4 genotype on these relationships remains unclear. Therefore, this systematic review aims to discuss the evidence regarding the effects of DHA and EPA supplementation on cognitive decline variables and relate them to APOEε4 genotype in middle-aged and older adults. The Medline (via PubMed), Scopus, Web of Science, and Bireme/Lilacs databases were searched for studies, retrieving 1863 articles. After the step selection, two randomized clinical trials (RCTs) that evaluated the effects of EPA and DHA supplementation on cognitive variables were included. All the methodological steps were performed by two raters. The results of the RCTs were not congruent, as the non-carriers in the APOEε4 supplemented group performed worse in the quick-choice reaction time cognitive domain, whereas the carriers improved in the reasoning domain. The increase in DHA concentration in the supplemented group was lower in APOEε4 carriers than in individuals with other genotypes. Additionally, a smaller decline in right hippocampal brain volume correlated with higher EPA concentrations after supplementation in APOEε4 non-carriers. The dosage and supplementation time (18 months) were similar between studies. In conclusion, DHA and EPA supplementation showed no pragmatic effects on cognitive variables when considering the presence of the APOEε4 allele.

Background and Aim: Treatment of endometriosis involves pain relief which is achieved through the administration of analgesics and non-steroidal anti-inflammatory drugs, with or without the addition of hormone therapy. At present, studies investigating endometriosis pain management using experimental rat models and the use of medications are scarce. Therefore, this study aimed to systematically evaluate research trends and critical points in the field of endometriosis pain management using experimental models. Materials and Methods: A total of 30 publications related to this topic that were published from 2012 to 2022 were retrieved from various databases, including Web of Science, Scopus, PubMed, Embase, and CINAHL, using appropriate English keywords. The quality of the publications was evaluated using impact metrics, productivity, term density mapping, and author network. Results: The average publication rate was three articles per year, reaching its peak in 2021 at five articles per year. The United States and China were found to be the most productive countries, with 12 and 10 publications per year, respectively. The field of medicine (37.0%) was the most abundant, although the H-index was relatively low (13:13). Term density mapping involved the search of 542 keywords, of which 35 were selected, with only 8 exhibiting significant density. Conclusion: In the past decade, there has been a shortage of publications that have addressed pain in endometriosis in experimental models and, within this context the majority of the production and publication in this field has been performed by the United States and China. After performing this bibliometric review, it can be inferred that more research is required in this field, to develop new approaches and treatments for endometriotic pain.

The pequi (Caryocar brasiliense) fruit peel, despite being frequently discarded, has a high content of bioactive compounds, and therefore has a high nutritional value. The present study aimed to explore the bioactivities in the pequi peel, particularly their potential health benefits at the level of antioxidant activity. The exploitation of this fruit could also present significant economic benefits and applications of pequi by-products would represent a reduction in waste, having a positive impact on the environment. Phenolic compounds present in the pequi exocarp and external mesocarp were identified by paper spray mass spectrometry (PS-MS) and quantified by HPLC. The total phenolic content (TPC) along with the amount of 2,2-diphenyl-1-picrylhydrazyl (DPPH), Ferric Reducing Antioxidant Power (FRAP), and the amount of 2,2′-azino-bis(3-ethylbenzothiazoline-6-sulphonic acid (ABTS) were also determined in peel extracts. Epicatechin was the most abundant phenolic compound found, followed by the caffeic, salicylic, and gallic acids. In addition, fingerprinting revealed compounds related to several beneficial health effects. In short, the results obtained were encouraging for potential applications of pequi peel in the field of functional foods.

ABSTRACT: Leptospirosis, a disease of significant economic impact on livestock and public health concern, may exhibit unique epidemiological characteristics in indigenous villages, particularly those located within semiarid climates such as the Caatinga biome in Brazil. This environment often presents conditions unfavorable for leptospire survival. This study conducted serological diagnoses of livestock in indigenous villages within the Caatinga biome, Northeastern Brazil. The survey was carried out in the Mãe Maria and Santa Helena indigenous villages, home to the Xukuru de Cimbres tribe, in the county of Pesqueira, Pernambuco State, Brazil, within the Caatinga biome. Blood samples were collected from cattle (n = 42), goats (n = 41), and sheep (n = 19). Out of the 102 samples tested, 73 (71.6%; 95% CI = 62.8% - 80.3%) were seroreactive (cut-off ≥ 100). The seropositivity rates for cattle, goats, and sheep were 54.8%, 80.5%, and 89.5%, respectively. A statistically significant difference was observed in the proportion of seropositive animals between cattle and goats and cattle and sheep (P < 0.05). Icterohaemorrhagiae was identified as the most probable serogroup across all animal species. The study revealed that livestock in indigenous villages within the Caatinga biome are frequently exposed to leptospires. This finding underscores the importance of a One Health approach, emphasizing the need for comprehensive studies in areas where large human and animal populations coexist. Such studies are crucial for investigating zoonotic infections and for planning and implementing control measures against livestock-associated leptospirosis.

Leptospirosis is an infectious disease that affects domestic animals, wild animals, and humans. It represents a public health problem and has an important economic impact on livestock. This study aims to investigate the importance of genital and transplacental infection in the epidemiology of leptospirosis in cows maintained in Caatinga biome conditions, Northeastern Brazil, as well as reporting organs colonized by Leptospira spp. in embryos and fetuses. Blood, urinary tract (urine, bladder, and kidney), and reproductive tract (vaginal fluid, uterus, uterine tube, ovary, and placenta) samples were collected from 15 slaughtered pregnant cows. Two embryos and 13 fetuses were sampled. Central nervous system and choroid ovoid samples were collected from embryos. Blood, central nervous system, lung, peritoneal liquid, abomasal content, liver, spleen, urine, bladder, kidney, and reproductive system samples were collected from fetuses. Diagnostic methods included the microscopic agglutination test (MAT) using a collection of 24 serovars belonging to 17 different pathogenic serogroups of five species as antigens, as well as polymerase chain reaction (PCR). Anti-Leptospira spp. antibodies were found in 9 cows (60%), while 13 cows (86.67%) had at least one organ or urine with leptospiral DNA. No fetus was seroreactive. Among the embryos and fetuses, 13 (86.67%) presented leptospiral DNA, proving a high frequency of transplacental infection (100%). For cows, the most frequent biological materials regarding Leptospira spp. DNA detection were placenta (13 out of 15 samples; 86.7%), uterus (10 out of 15 samples; 66.7%), and vaginal fluid (5 out of 15 samples; 33.3%), while, for fetuses/embryos, the most frequent PCR-positive samples were choroid ovoid (1/2; 50%), spleen (6/13; 46.2%), kidney (5/13; 38.5%), and central nervous system (5/15; 33.3%). Sequenced samples based on the LipL32 gene presented 99% similarity with L. borgpetersenii. The results indicate that transplacental infection is an efficient way of spreading Leptospira spp. in cows maintained in Caatinga biome conditions. Therefore, prevention and control strategies must include actions that interrupt transmission through this alternative route.

Osteoarthritis (OA) remains a chronic incurable condition, presenting substantial challenges in treatment. This study explores a novel strategy by investigating the concurrent use of cuminaldehyde, a natural compound, with indomethacin in animal models of MIA-induced OA. Our results demonstrate that the co-administration of cuminaldehyde and indomethacin does indeed produce a superior effect when compared to these compounds individually, significantly enhancing therapeutic outcomes. This effect is evidenced by a marked reduction in pro-inflammatory cytokines IL-6 and IFN-γ, alongside a significant increase in the anti-inflammatory cytokine IL-10, compared to treatments with each compound alone. Radiographic analyses further confirm the preservation of joint integrity and a reduction in osteoarthritic damage, highlighting the association’s efficacy in cartilage-reducing damage. These findings suggests that the association of cuminaldehyde and indomethacin not only slows OA progression but also offers enhanced cartilage-reducing damage and fosters the production of protective cytokines. This study underscores the potential benefits of integrating natural products with pharmaceuticals in OA management and stresses the importance of further research to fully understand the mechanisms underlying the observed potentiated effects.

The jump is one of the main body elements in Rhythmic Gymnastics (RG). To perform it correctly, gymnasts must develop appropriate force and velocity levels to reach enough jump height to show the body shape during flight, as defined by the RG Code of Points. Jumping performance is influenced by the mechanical force-velocity (F-V) profile and the maximum power generated by the lower limbs. The F-V profile identification can provide a more accurate and complete mechanical representation of its capabilities and needs. This study aimed to analyse the F-V profile during the counter movement jump of RG athletes, identify the magnitude and direction of the imbalance between the two variables (force and velocity), and compare the jumping ability and mechanical variables of different age groups. Eighteen Portuguese gymnasts (average age 12.2±1.8 years) were evaluated, according to age group: beginner and youth gymnasts (G1- ages 10 to 12) and junior and senior gymnasts (G2 - ages 13 to 16), and according to the F-V profile imbalance detected. The data collection was performed after a covid-19 lockdown period. Results showed anthropometric differences between age groups but no differences in the F-V profile related variables. When gymnasts were compared according to the deficit, differences were found in variables force and velocity. Furthermore, 72.3% of the gymnasts presented force deficit, 11% presented velocity deficit and 16.6% were balanced. Considering the deficits found, the demands of the sport and of each athlete, it is essential to include strength training in the regular training routines of rhythmic gymnasts.

In this scenario, this study aimed to describe those aspects in patients with post-surgery PSPS and non-surgical PSPS.Methods: This study included patients with PSPS, and non-surgical PSPS, over 18 years, with quarterly evaluations at the Chronic Pain Clinic. After evaluation, demographic and clinical characteristics were obtained. The Beck Depression Inventory-II, Beck Anxiety Inventory, Douleur neuropathique 4 questions, Visual Analog Pain Scale, and Fear-Avoidance Beliefs Questionnaire–Brazilian Version (FABQ-Brazil) were used to evaluate psychological aspects. Results: Forty-six patients were included, 23 patients with post-surgery PSPS and 23 with non-surgical PSPS. Both groups had high scores in the physical and work domains of the FABQ, high rates of absenteeism and most patients in these groups had moderate-to-severe neuropathic pain and some degree of anxiety and/or depression. The groups showed no statistically significant difference (p > 0.05) when comparing all questionnaires.Discussion: This is one of the first studies to evaluate FAB and other associated psychological factors, such as anxiety and depression, in patients with post-surgery PSPS in a follow-up several years after surgery and compare with patients diagnosed with non-surgical PSPS. In this study, most patients in both groups had high scores in the FABQ domains, not having statistically relevant difference between groups. Conclusion: Even though there was no statistically relevant difference between the PSPS patient with or without surgical history in terms of the assessed outcome measures, the described scores for fear-avoidance beliefs, pain, anxiety and depression were high, showing an interference in the daily life activities of those patients. Keywords: fear, depression, anxiety, spine surgery, back pain

Abstract We aimed to determine the herd and animal levels seroprevalence and associated factors for Toxoplasma gondii infections in cattle from the state of Paraíba, Northeastern Brazil. Herds (n = 434) and cows aged ≥ 24 months (n = 1,895) were randomly selected, and serum samples were tested with the immunofluorescence antibody test (IFAT) using as cutoff of 64. Of the 434 farms investigated, 197 had at least one seropositive cow (prevalence of 49.0%; 95% CI = 44.3%-53.8%), and the prevalence at animal level was 18.0% (95% CI = 5.3%-21.1%). The antibody titers ranged from 64 to 1024, with the most frequent titers being 64 (10.8%) and 128 (3.7%). The risk factors identified were property located in Sertão region (odds ratio [OR] = 3.07), property located in Agreste/Zona da Mata regions (OR = 2.00), animal purchasing (OR = 2.68), herd size of 34-111 animals (OR = 2.91) and herd size > 111 animals (OR = 6.97). The results suggest the wide distribution of T. gondii infections in cattle throughout the state of Paraíba, and the risk factors identified are not possible to correct.

Leukemia is among the most common types of hematological cancers and the use of herbal medicines to prevent and treat leukemia are under quick development. Among several molecular pathways involved in leukemia pathogenesis and exacerbations, purinergic signaling is revealed as a key component. In the present study, the effects of two doses (5 ug/mL and 10 ug/mL) of Immunity-6™, a phytocomplex composed by beta-glucan, green tea (Camelia sinensis), chamomile (Matricaria chamomilla), and ascorbic acid (vitamin C) was tested in vitro, using chronic myelogenous leukemia cell line (K-562; 5 ×104/mL/well), which were challenged with lipopolysaccharide (LPS; 1 ug/mL) for 24 h. The results demonstrated that both doses of Immunity-6™ inhibited ATP release (p

Background: Heart failure demands self-care skills and behaviors that can be negatively impacted by a low level of perceived control (PC), a belief about having the necessary resources to deal with negative events. Having valid and reliable instruments to measure PC is important to support interventions that improve self-care and related outcomes. The Control Attitudes Scale-Revised (CAS-R)was developed in the United States to measure PC in cardiac conditions. In Brazil, there are no instruments available to measure this construct. Objective: The aims of this study were to translate and adapt the CAS-R to the Brazilian population and to assess the content validity of the adapted version. Methods: The CAS-R was translated, back-translated, and assessed by an expert committee for linguistic equivalences. An agreement > 80%was considered adequate. Content validity (clarity, theoretical relevance, and practical pertinence) wasassessedbyboth an expertprofessionalpanel (n=6-8) and a panel of patientswith heart failure (n = 40). A content validity coefficient > 0.70was considered acceptable. Results: The translations to Brazilian Portuguesewere considered consistentwith the original CAS-R. In the third round of linguistic equivalence assessment, all items achieved acceptable agreement, except for 2 items. After modifications to the instrument to achieve adequate equivalences, the adapted version had a final content validity coefficient of 0.93. Most patients were able to understand the instrument. Conclusions: The CAS-R-Brazilian version is equivalent to the original CAS-R and has satisfactory evidence of content validity. Additional psychometric testing will be performed to allow for the assessment of PC in individuals with heart failure in Brazil. [ABSTRACT FROM AUTHOR]

ABSTRACT: Animal hoarding disorder, especially dogs and cats, is a complex issue that occurs in almost all communities, and it is necessary to collect data that contribute to the understanding of leptospirosis within the One Health initiative. In order to determine the seroprevalence of Leptospirasp., 71 dogs and 39 cats from 33 people with animal hoarding profile in the municipality of Patos, state of Paraíba, semi-arid region of Northeastern Brazil were evaluated by using the microscopic serum agglutination test (MAT) as diagnostic test. Thirty-eight animals (34.5%; 95% CI = 25.7 - 43.4%) were seroreactive, with 46.5% in dogs and 12.8% in cats (P < 0.01), with the highest frequency of the Icterohaemorrhagiae serogroup in both species. The high seroprevalence reported suggests the need for public policies for this vulnerable population in order to prevent the transmission of zoonoses.
