Abstract: We estimate a matching model of teachers and elementary schools with rich data on teachers' applications and principals' ratings from a large, urban district in North Carolina. Both teachers’ and principals’ preferences deviate from those that would maximize the achievement of economically disadvantaged students: teachers prefer schools with fewer disadvantaged students, and principals' ratings are weakly related to teacher effectiveness. In equilibrium, these two deviations combine to produce a surprisingly equitable current allocation, where teacher quality is balanced across advantaged and disadvantaged students. To close achievement gaps, policies that address deviations on one side alone are ineffective or harmful, while policies that address both could substantially increase the achievement of disadvantaged students.

BateThe “Thrive Inland SoCal: Labor Market Analysis” offers a comprehensive exploration of the labor market in Southern California’s Inland Southern California Region, made up of Riverside and San Bernardino Counties. Using a mixed methods approach, this report focuses on understanding the various challenges the region’s labor market is faced with, in addition to identifying current efforts to support workers. Using publicly available data, the team examined several variables to understand the context in which the region’s residents currently operate. The team additionally conducted a series of interviews with a variety of stakeholders, in order to better understand the unique challenges and needs of the region. In particular, this report focuses on understanding the high-road approach, which in a workforce context refers to efforts that aim to improve job quality and increase access, especially for workers who experience barriers to employment. Complemented by a comprehensive literature review, an analysis of the data gathered identified the following four areas as major barriers: • Financial: childcare and eldercare, transportation, housing cost, and technology • Social: discrimination, immigration status and language, cultural capital, social stigma, awareness of opportunities, social networking, and youth job seekers • Educational: access to higher education, general attainment, and financial barriers • Environmental: climate-related changes, heat, unpredictable weather

This report provides a snapshot of the labor market and working conditions in the Inland Empire (IE). We document how changes over the last five years, including those related to investment, inflation, and the COVID-19 pandemic, have impacted workers’ employment outcomes, working conditions, and well-being. This analysis marries diverse perspectives from personal narratives from workers collected through in-depth interviews (collected in English and Spanish) with a vast array of quantitative data on labor market conditions.

“The State of Work: Transportation, Distribution, andLogistics in the Inland Empire” offers a comprehensiveexploration of the Transportation, Distribution, and Logistics (TDL) sector in Southern California’s Inland Empire. This pivotal sector, integral to both national and globaleconomies, includes transportation services, warehousing, and logistics services. Despite recent size decline, the TDL sector has shown rapid expansion over the last five years, particularly in warehousing and transportation, significantly impacting the region’s employment landscape.This report delves into the various challenges and opportunities presented by the TDL sector. Notably, the concentration of warehouses and distribution centers has sparked concerns about environmental impacts, traffic congestion, and job quality. The authors also examine the geographic spread of warehouse developments and their community and environmental ramifications, highlighting the complex balance between economic growth and maintaining quality of life.

Summary: We propose a per‐cluster instrumental variable (PCIV) approach for estimating linear correlated random coefficient models in the presence of contemporaneous endogeneity and two‐way fixed effects. This approach estimates heterogeneous effects and aggregates them to population averages. We demonstrate consistency, showing robustness over standard estimators, and provide analytic standard errors for robust inference. In Monte Carlo simulation, PCIV performs relatively well in finite samples in either dimension. We apply PCIV in estimating the price elasticity of gasoline demand using state fuel taxes as instrumental variables. We find significant elasticity heterogeneity and more elastic gasoline demand on average than with standard estimators.

Objectives: Patients with Type A aortic dissections are often overwhelmed by the influx of new information and changes necessary to adjust to life with an aortic disease. This article uses psychoeducation about life after dissection to dispel common myths about aortic disease and empower patients to live full lives. It is written in clear and easy-to-understand language to maximise patient comprehension and knowledge of their condition to help them feel more confident after Type A aortic dissection. Design: An interdisciplinary group of professionals (including cardiothoracic surgeons, a clinical cardiac psychologist, clinical cardiac psychology trainees, and genetic counsellors) were asked about their interest in creating an article written for patients who have undergone Type A aortic dissection repair. Those who agreed were then asked to report some of the myths commonly heard in practice about the procedure and its ramifications. Setting: The myths confronted in this article come from professionals working for a comprehensive heart care institute, the East Carolina Heart Institute in Greenville, NC, USA and Johns Hopkins Hospital in Baltimore, MD, USA. Methods: A multi-disciplinary group of professionals focused on aortic disease contributed myths commonly reported by patients seen in practice. Results: Common myths are presented and include safety fears about physical activity, diet, and recurrence of aortic dissection. By educating yourself about the facts of your diagnosis, you can feel as though you have more control over your health and your life. Conclusion: The takedown of common myths about life post-dissection can be helpful for patients with aortic disease and their families. Understanding the facts about a loved one's new diagnosis can help partners and caregivers to be more fully involved in the patient's care and feel confident. An important limitation of the present work is the lack of prior research on patient quality of life and mental health following Type A aortic dissection. Future work should explore the unique mental health needs of patients with aortic disease to create interventions that target mental health symptomatology to help patients live longer, healthier and richer lives.

Heavy load carrying of water, firewood, and sand/stones is a ubiquitous activity for women living in developing countries. Although the intra-abdominal pressure associated with heavy load carrying is hypothesized to increase the risk of pelvic organ prolapse (POP) among women, relevant epidemiologic data are lacking. We conducted a comparative study involving two exploratory cross-sectional studies among convenience samples of women carrying heavy loads, with different characteristics: (1) as part of their activities for daily living, in Shinyanga region, Tanzania; and (2) working as sand miners in Pokhara, Nepal. Women were categorized has having low or high load-carrying exposures based on the measured weights of the loads being carried at the time of the survey, as well as on self-reported duration and frequency of load carrying. A summary score for lower abdominal discomfort suggestive of POP was generated using questions from the Pelvic Organ Prolapse Distress Inventory (POPDI-6). Women with higher load carrying exposures had on average higher discomfort scores in both Tanzania (adjusted prevalence difference (PDa) = 3.7; 95% CI: -3.8-11.3; p = 0.33) and Nepal (PDa = 9.3; 95% CI: -4.9-23.6; p = 0.18). We identified trends suggestive of an association between increasing heavy load carrying exposures and symptoms of lower abdominal discomfort. Our findings underscore the need for larger epidemiologic studies of the potential adverse reproductive health effects of heavy load carrying activities on women in developing countries.

BackgroundHeavy load carrying has been associated with musculoskeletal discomfort (MSD) and disability. However, there is a lack of research investigating this association in resource-constrained settings where heavy load carrying by women is common.ObjectivesWe assessed the impact of heavy load carrying on musculoskeletal pain and disability among women in Shinyanga Region, Tanzania, in an exploratory cross-sectional study.MethodsEligible participants were a convenience sample of women, at least 18 years of age, who passed a study recruitment site carrying a load. We collected information on load-carrying practices, including frequency and time spent carrying water, wood, agricultural products, coal, sand, or rocks, and measured the weight of the load carried at the time. Outcomes included self-reported MSDs, defined as experiencing pain lasting >3 days in the neck, head, back, knees, feet and/or ankles within the last 1 year, and related disability. Using multivariable logistic regression we assessed for associations between load carrying exposures and MSDs and disability.FindingsResults showed a high prevalence of MSDs across the body regions assessed and evidence to suggest a relationship of back pain and related disability with several measures of load-carrying, including duration, frequency, and weight. Multivariable analyses revealed associations of increased load carrying exposures with low back pain (LBP) and related disability, including statistically significant increases in odds of LBP with increasing weight, total duration of load carrying/week and cumulative loads/week.ConclusionsFindings indicate a substantial burden of MSDs and disability in this population of women who carry heavy loads daily. The extent of discomfort and disability increased with increasing exposure to various load-carrying measures, especially for LBP. Larger epidemiologic studies that definitively assess relationships of load carrying with MSDs and disability are warranted.

BackgroundExposure to household air pollution (HAP) from cooking with biomass fuel affects billions of people. We hypothesized that HAP from woodsmoke, compared to other household fuels, was associated with adverse cardiovascular outcomes, of which there have been few studies.MethodsA cross-sectional study was completed in 299 females aged 40-70 years in Kaski District, Nepal, during 2017-18. All participants underwent a standard 12-lead ECG, ankle and brachial systolic blood pressure measurement, and 2D color and Doppler echocardiography. Current stove type was confirmed by inspection. Blood pressure, height, and weight were measured using a standardized protocol. Hypertension was defined as ≥140/90 mmHg or prior diagnosis. Hemoglobin A1c (HbA1c) was obtained, with diabetes mellitus defined as a prior diagnosis or HbA1C ≥ 6.5%. We used adjusted linear and logistic multivariable regressions to examine the relationship of stove type with cardiac structure and function.ResultsThe majority of women primarily used liquified petroleum gas (LPG) stoves (65%), while 12% used biogas, and 23% used wood-burning cook-stoves. Prevalence of major cardiovascular risk factors was 35% with hypertension, 19% with diabetes mellitus, and 15% current smokers. After adjustment, compared to LPG, wood stove use was associated with increased indexed left atrial volume (β = 3.15, 95% CI 1.22 to 5.09) and increased indexed left ventricular end diastolic volume (β = 7.97, 95% CI 3.11 to 12.83). There was no association between stove type and systemic hypertension, left ventricular mass, systolic dysfunction, diastolic dysfunction, pulmonary hypertension, abnormal ankle-brachial index, or clinically significant ECG abnormalities.ConclusionBiomass fuel use was associated with increased indexed left atrial volume and increased indexed left ventricular diastolic volume in Nepali women, suggesting subclinical adverse cardiac remodeling from HAP in this cross-sectional study. We did not find evidence of an association with hypertension or typical cardiac sequelae of hypertension. Future studies to confirm these results are needed.

METHODS: Community-based case-control study of 448 women (170 cases of uterine prolapse; 278 controls) aged 18-60 years in Kaski district, Nepal was conducted. Women diagnosed with uterine prolapse were cases. Two controls were recruited for each case, frequency-matched by residential area and age. Multivariate logistic regression was used to investigate associations between outcome and exposures. RESULTS: No association of heavy load carrying with uterine prolapse was observed; women who never used a patuka had lower odds of uterine prolapse (odds ratio = 0.18, 95% confidence interval = 0.05-0.71). Women working in a sitting position had higher odds than those working in a standing position (odds ratio = 2.94, 95% confidence interval = 1.74-4.96), as did women who mainly worked in a bending position (odds ratio = 2.45, 95% confidence interval = 1.12-5.34). Housewives were more prone to uterine prolapse than women engaged in farming (odds ratio = 2.13, 95% confidence interval = 1.31-3.47). CONCLUSION: Using a patuka, occupation, and body position during work were all associated with uterine prolapse. No association was found with heavy load carrying, although that might be attributable to the cross-sectional nature of study recruitment.

Automotive technicians are commonly exposed to organic and chlorinated solvents, particularly through use of cleaning products. Mainly during the period 1989-2002, n-hexane was a component of some of these products. In other occupational contexts, n-hexane has been shown to be a cause of peripheral neuropathy. The purpose of the present study was to investigate whether previous exposures to low concentrations of n-hexane were a cause of persistent peripheral neuropathy in automotive technicians. Enrolled in the study were 830 San Francisco Bay Area automotive technicians. Each participant underwent a battery of tests to investigate peripheral nervous system impairment. Test results regressed against estimated hexane and total solvent exposures showed only limited evidence of association with solvent exposures. Exposures to both hexane and general solvents were well below their occupational exposure limits. Generally, our results provide reassurance about persistent peripheral neuropathic effects in automotive technicians who previously used hexane-containing automotive cleaning products. This may reflect repair processes, since the exposures occurred some years previous to the study. However, we cannot exclude the possibility that the absence of observed effect in this study may be attributable to low exposures, exposure misclassification and/or the healthy worker survivor effect.

BackgroundThe risk of developing latent tuberculosis infection (LTBI) associated with cooking with solid fuels is unknown. This study examined the relationship between household fuel uses and LTBI in adults living in Nepal, a country with a high incidence of tuberculosis.MethodsParticipants were 1088 adults aged 18-70 years, members of the control group of a population-based case-control study of pulmonary TB (PTB) in people without previous TB, living in Kaski and neighboring districts of Nepal. Participants were interviewed in their homes with a standardized questionnaire. Blood samples were tested for LTBI using an interferon-gamma release assay. Multivariate unconditional logistic regression was used to examine associations between household fuel sources and LTBI.ResultsThe overall prevalence of LTBI in the study population was 36%. Using liquefied petroleum gas (LPG) as the baseline cooking fuel type, the adjusted odds ratio (aOR) for using a primary wood cookstove was 1.13 (95%CI: 0.73,1.77) for all participants and, in women only, 1.14 (0.62, 2.09). Corresponding figures for biogas stoves were 0.64 (0.34,1.20) and 0.59 (0.24,1.45), respectively. Household sources of air pollution positively associated with LTBI included traditional oil lamps (diyos) used during power outages, for which the aOR in all participants was 2.53 (1.20, 5.31), although the number of users was small. Use of candles for lighting was also associated with increased risk of LTBI among men (aOR = 1.61, 95% CI:1.01, 2.56).ConclusionsNo association was found between use of wood for cooking and LTBI. However, there was some evidence that biogas cookstoves were associated with reduced odds of LTBI. Some exposures at the time of actual infection will have been different than the current exposures used in the analysis, biasing results towards the null. Results are sufficient for the use of diyos to be discouraged for lighting purposes. Overall, results suggest that household cooking fuel use is likely to have more effect on moving from the infected state to PTB than on becoming infected with the M. tuberculosis complex. Further research, including longitudinal studies with serial LTBI testing would be useful to more accurately assess the relationships between exposures and infection.

BackgroundBovine leukemia virus (BLV) infection is widespread in cattle globally and is present in marketed beef and dairy products. Human infection with BLV has been reported in breast and lung cancer tissues and was significantly associated with breast cancer in 3 case-control studies. The purpose of this current research was to determine if BLV is present in human blood cells and if antibodies to BLV are related to blood cell infection.MethodsStandard liquid PCR and Sanger DNA sequencing were used to test for BLV in buffy coat cells (leukocytes and platelets) of blood specimens from 95 self-selected female subjects.Enzyme-linked immunosorbent assay (ELISA) for IgG, IgM, and IgA was used to detect antibodies to BLV in the plasma of the corresponding blood samples.ResultsBLV DNA was detected in the buffy coat cells of blood in 33/95 (38%) of the subjects by PCR and DNA sequencing. IgG antibodies were detected in 30/95(32%), IgM in 55/95(58%), and IgA in 30/95(32%) of the subjects. There was no significant correlation between presence of the antibodies and presence of BLV DNA.ConclusionsThis first report of BLV in human blood raises the question of whether infection of leukocytes could conceivably lead to leukemia as it does in infected cattle. Also, system wide circulation of infected blood cells could facilitate BLV transit to various internal tissues/organs with potential for their infection and subsequent development of cancer. The most likely route of BLV transmission to humans would be zoonotic, as a foodborne infection. Although eradicated from cattle in some countries, BLV still has a high rate of infection in the Americas, the Middle East, and parts of Europe and Asia. This report of BLV in the blood layer containing human leukocytes/platelets adds important information which could be useful to elucidate possible routes of transmission of BLV to humans and to prevent further human infection.

Diet diversity and overlap was investigated in the sympatric skinks Trachylepis punctatissima and Trachylepis varia from central South Africa using stomach and intestinal contents of museum specimens. Simpson's Index and the Shannon–Wiener Index were used to determine their food niche breadths and Pianka's Index to determine the extent to which they shared the food resource. Food items were identified from head capsules, mouthparts and appendages to order level. The two species contained similar prey types with T. varia having the wider niche breadth. Females also had wider niche breadths than males and isopterans were present in significantly higher quantities than the rest of the food items in both species. Larvae were found in both, but arachnids only in T. varia. Trophic niche overlap between the two was fairly high (Pianka's Index, 0.6915) and therefore did not support the hypothesis that similar species sharing a food resource should have well differentiated diets to reduce competition. This finding however requires further verification as the two species may consume different species of the corresponding orders identified. Intraspecifically trophic niche overlap between the sexes was low (T. punctatissima, 0.3889; T. varia, 0.4633) suggesting strong competition for the food resource. Notably lower niche breadth in males compared to females may be due to morphological and behavioural differences and should be further investigated. [ABSTRACT FROM AUTHOR]

Intraspecific variation in colour patterns may reflect adaptive responses to local environmental regimes that favour selection of different trade-offs between visual communication, thermoregulation, and anti-predatory functions. Understanding the drivers of colour pattern variation within species can therefore provide valuable insights about adaptation. However, the ecogeographic correlates, and thus the ecological drivers, associated with the segregation of different colour morphs for most reptile species, including snakes, are not well known. Here, we examined differences in environmental niches associated with occurrence records of preserved museum specimens and georeferenced photographic images of common egg-eaters (Dasypeltis scabra) to characterise ecogeographic divergence between typically patterned, patternless, and faintly marked individuals. We gathered 1707 records that we assigned as either patterned, plain, or intermediate morphs. Most records were of patterned individuals, which were widely distributed in southern Africa. Climate niches differed between morphs as plain and intermediate snakes were restricted to high-elevation areas in the Highveld grasslands of South Africa, and woodland-dominated areas in the central and eastern parts of both Zambia and Zimbabwe, but there were no areas where only plain or intermediate snakes occurred in isolation from patterned snakes. Environmental niche modelling predicted minimal areas suitable for co-habitation of colour morphs. We speculate that plain or weakly-marked colour patterns likely developed as an adaptation to increase camouflage ability in open areas in grassland and certain woodland habitats. BackgroundWhether cooking with solid fuels, as occurs widely in developing countries, including Nepal, is a risk factor for pulmonary tuberculosis (PTB) is uncertain. Epidemiologic studies have produced variable results. This case-control study sought to resolve this issue with a large sample size and a population-based control group.MethodsPTB cases (N = 581), aged 18-70 were recruited from diagnostic centers in Kaski and neighboring districts of Nepal. Population-based controls (N = 1226) were recruited. Persons who had previously been diagnosed with TB were excluded. Questionnaires were administered at participants' homes.ResultsUsing liquefied petroleum gas (LPG) as the cookstove reference fuel, for women the odds ratio (OR) for having a primary cookstove that used wood was 0.21 (95% CI: 0.08,0.52); for men the corresponding OR was 0.80 (0.37, 1.74). For biogas, the OR for women was 0.24 (0.06,0.87) and for men, 1.41 (0.61, 3.23).ConclusionsThe unexpected finding of a higher risk for women using LPG cookstoves, relative to wood or biogas-burning cookstoves, may be attributable to excluding persons with prior TB. A possible explanation is that emissions, such as ultrafine particles, formed during LPG combustion promote PTB manifestation in infected people who have not previously had PTB. The damage from the initial PTB leaves them susceptible to the PTB-promoting effects of smoke from wood fires. Further studies, excluding participants who have previously had TB are needed to confirm these findings. Use of exhaust hoods to the outdoors for all stoves, well-ventilated kitchens, and gas stoves raised above ground would reduce exposures.

Evidence suggests that early-life exposure to pesticides inside the home may be associated with childhood leukemia, however data from Latin American countries are limited. We examined whether self-reported maternal residential pesticide use and nearby pesticide applications-before and after childs birth-were associated with acute lymphoblastic leukemia (ALL) in the Costa Rican Childhood Leukemia Study (CRCLS), a population-based case-control study (2001-2003). The purpose of this study was to assess occupational injury characteristics and predictors among career firefighters. A total of 249 firefighters from central Texas and northern California participated in this Internet-based survey. Approximately 27% of firefighters had reported an occupational injury within the previous 12 months. The majority of injuries occurred on the scene of a non-fire call while performing an activity that required lifting, pushing, or pulling. Firefighters' backs were most frequently injured. Of the reported injuries, approximately 18% returned to work on modified duty, but 46% were not allowed to work due to their occupational injuries. Firefighters who reported occupational injuries were more likely to be older and experiencing occupational stress compared with their coworkers who did not report occupational injuries. Injured firefighters were also more likely to report fewer job rewards (money/salary), overcommitment, less esteem (respect and support), and fewer promotional prospects. These injury factors should be incorporated into interventions to reduce or prevent workplace injuries.

The mechanism of toxicity of hydrogen sulfide (H2S) gas is thought mainly to operate through effects on the nervous system. The gas has high acute toxicity, but whether chronic exposure causes effects, including peripheral neuropathy, is yet unclear. The city of Rotorua, New Zealand, sits on an active geothermal field and the population has some of the highest measured ambient H2S exposures. A previous study in Rotorua provided evidence that H2S is associated with peripheral neuropathy. Using clinical methods, the present study sought to investigate and possibly confirm this association in the Rotorua population. The study population comprised 1635 adult residents of Rotorua, aged 18-65. Collected data relevant to the peripheral neuropathy investigation included symptoms, ankle stretch reflex, vibration sensitivity, as measured by the timed-tuning fork test and a Bio-Thesiometer (Bio-Medical Instrument Co., Ohio), and light touch sensitivity measured by monofilaments. An exposure metric, estimating time-weighted H2S exposure across the last 30 years was used. Principal components analysis was used to combine data across the various indicators of possible peripheral neuropathy. The main data analysis used linear regression to examine associations between the peripheral nerve function indicators and H2S exposure. None of the peripheral nerve function indicators were associated with H2S exposure, providing no evidence that H2S exposure at levels found in Rotorua is a cause of peripheral neuropathy. The earlier association between H2S exposure and peripheral neuropathy diagnoses may be attributable to the ecological study design used. The possibility that H2S exposure misclassification could account for the lack of association found cannot be entirely excluded.

PurposeHydrogen sulfide (H2S) is a highly toxic gas with well-established, acute irritation effects on the eye. The population of Rotorua, New Zealand, sited on an active geothermal field, has some of the highest ambient H2S exposures in the world. Evidence from ecological studies in Rotorua has suggested that H2S is associated with cataract. The purpose of the present study was, using more detailed exposure characterization, clinical examinations, and anterior eye photography, to more directly investigate this previously reported association.MethodsEnrolled were 1637 adults, ages 18 to 65, from a comprehensive Rotorua primary care medical register. Patients underwent a comprehensive ophthalmic examination, including pupillary dilation and lens photography to capture evidence of any nuclear opacity, nuclear color, and cortical and posterior subcapsular opacity. Photographs were scored for all four outcomes on the LOCS III scale with decimalized interpolation between the exemplars. H2S exposure for up to the last 30 years was estimated based on networks of passive samplers set out across Rotorua and knowledge of residential, workplace, and school locations over the 30 years. Data analysis using linear and logistic regression examined associations between the degree of opacification and nuclear color or cataract (defined as a LOCS III score ≥2.0) in relation to H2S exposure.ResultsNo associations were found between estimated H2S exposures and any of the four ophthalmic outcome measures.ConclusionsOverall, results were generally reassuring. They provided no evidence that H2S exposure at the levels found in Rotorua is associated with cataract. The previously found association between cataract and H2S exposure in the Rotorua population seems likely to be attributable to the limitations of the ecological study design. These results cannot rule out the possibility of an association with cataract at higher levels of H2S exposure.
