Mulher de 21 anos encaminhada com piora dos sintomas que vinha apresentando há anos: tontura ao levantar-se, sem sintomas vertiginosos, sem queixas de síncope ou alterações neurológicas, melhora em decúbito dorsal e repouso, após poucos minutos. O ecocardiograma transtorácico mostrava presença de massa hiperecogênica na região médio-apical da parede posterior do ventrículo esquerdo e pericárdio normal. A ressonância nuclear magnética cardíaca permitiu o diagnóstico de tumor de ventrículo esquerdo sugestivo de lipoma. Realizado tratamento cirúrgico e ressecado o tumor, a paciente apresentou boa recuperação e mantém-se assintomática.

Purpose: This prospective study aimed to identify risk factors associated with progression to stage 3 chronic kidney disease (CKD) and the occurrence of kidney stone formation or growth following nephrectomy for urolithiasis., Materials and Methods: From January 2006 to May 2013, patients undergoing nephrectomy for urolithiasis were enrolled. Renal function was assessed using estimated glomerular filtration rate (eGFR) via the Chronic Kidney Disease Epidemiology Collaboration equation, while kidney stone events were detected using computed tomography., Results: Among 107 patients followed for an average of 83.5 months, type 2 diabetes mellitus (T2DM) significantly increased the risk of progression to stage 3 CKD by 34.79-fold (p=0.004). Age was associated with a 15% increase in the odds of developing stage 3 CKD per year (p=0.01), while higher preoperative eGFR was protective (OR=0.84, p<0.01). DMSA-99mTc values below 15% were less likely to lead to renal function deterioration. New kidney stone formation occurred in 15.9% of patients and stone growth observed in 12.1%. Contralateral kidney stones (p<0.01) and hypercalciuria (p=0.03) were identified as risk factors for kidney stone events., Conclusions: T2DM and age were predictors of CKD progression, while higher preoperative eGFR was protective. Purpose: To assess the impact of thinness on the outcome of the percutaneous nephrolithotomy (PCNL)., Materials and Methods: A matched case-control study was performed using a prospectively collected database of all patients who underwent PCNL between June 2011 and October 2021. The patients were stratified into two groups according to their phenotypic characteristics, arbitrarily defined according to their body mass index (BMI): <0kg/m2 (Group 1, very thin patients, G<20) and ≥25 kg/m2 (Group 2, non-thin patients, G≥25). Patients were randomly matched based on Guy's Stone Score (GSS) according to case complexity at a ratio of 1:3., Results: A total of 204 patients were enrolled in this study: 51 patients (G<20) and 153 controls (G≥25). Complications occurred in 15.2% of the patients, with 5.4% of these complications classified as major complications (Clavien grade ≥ 3). According to complications there were no significant differences between the groups. The overall complication rates were 17.6% in the G<20 and 14.4% in the G≥25 (p = 0.653). The major complication rates were 3.9% in the G<20 and 5.8% in the G≥25 (p=0.429). No differences in transfusion or urinary fistula rates were found., Conclusions: In this study, very thin patients were not at a higher risk of complications when submitted to PCNL than in those with a BMI of ≥25 kg/m2. We report the case of a 21-year-old female referred to our institution complaining of dizziness when standing up, which improved in the dorsal decubitus position and at rest, after a few minutes. The symptom, which had lasted for years, was not accompanied by vertigo, syncope, or neurological changes, but was gradually getting worse. Transthoracic echocardiogram showed a hyperechoic mass in the middle-apical region of the left ventricular posterior wall and normal pericardium. The cardiac nuclear magnetic resonance allowed the diagnosis of the left ventricular tumor suggestive of lipoma. Surgery was performed and the tumor was resected. The patient recovered well and is currently asymptomatic.
