Abstract

Infection is a common complication of severe trauma and is a major cause of extra hospitalization and death. Patients who have sustained major blunt trauma have been found to have a suppressed immune system, which contributes to the occurrence of secondary infection. Blunt trauma is frequent in children, and while the immune system of children differs from that of adults, changes in its function following severe injury have not been well-studied. Immune system function was studied in 25 children, aged 2 to 14, who were hospitalized in intensive care for major trauma. Infections occurred in 14 children, and three patients died. Skin tests of delayed-type hypersensitivity (DTH) reactions to common bacterial antigens (molecules provoking immune responses) were used as initial indicators of immune function; 14 patients were anergic (absence of immune system reactivity), and these children were significantly more likely to develop infections. Non-anergic trauma patients had significantly fewer positive skin tests than did 10 control children with minor disorders. Specific changes in patient subgroups of lymphocytes, important infection-fighting white blood cells, were also studied. Among another group of 10 children, aged 2 months to 14 years and hospitalized with major head trauma, there was a significant decrease in levels of B lymphocytes, but not T lymphocytes, in the first week following injury. The study indicates that unlike adults with trauma injuries, children similarly injured sustain decreases in B lymphocytes, but not T lymphocytes. Use of the DTH skin test should be helpful in identifying patients at risk for infection and allow treatment focused at preventing infections in these patients. Abstract

Infection is a major complication of severe trauma and may also be a primary cause of prolonged hospitalization and death among trauma patients. The immune system response of 10 infants and children suffering from severe blunt trauma was studied within 36 hours of arrival in an intensive care unit. Possible immune system suppression was present in the form of defective T-helper cells, which could explain the failure of antigen-specific B-cells to proliferate and produce normal amounts of antibody. Other variables may account for the increased rate of infection in these patients. Open wounds and the placement of catheters, monitors, and tubes in the body provide portals for infection. Blood transfusions may also affect immune function, and many of the drugs and anesthetics used in these patients have immunosuppressive effects.

Abstract

Intussusception, the inverse telescoping of the bowel into itself, can cause blockage in infants and children. Enemas given to create pressure in the intestines from the force of the fluid being flushed into the rectum can sometimes relieve ileocolic (between the ileum and the colon) intussusception in the bowel. In cases where initial manipulation of the bowel by enema fails, the use of anesthesia during second enema attempt may prove useful. Over a three-year period, enemas were performed in 62 children. In 31 children of those children a first attempt was unsuccessful. A second try performed in the operating room with the use of anesthesia was successful in 21 children. Manipulation of the bowel during surgery had to be performed in the remaining 10 children. Two children required surgical resection of the bowel. Surgery was avoided in 84 percent of the affected children over the three years. It is suggested that the anesthesia used during the second attempt caused the walls of the intestines to relax during the enemas. The relaxation and the time delay between the two attempts may have contributed to the success of the second attempt. It is concluded that the use of anesthesia during a second attempt for intussusception repair should be included in the standard treatment of ileocolic intussusception in children.

Abstract

Genetic polymorphisms influence the magnitude of the cytokine response after an inflammatory stimulus. To determine whether such polymorphisms might play a role in Kawasaki disease (KD), we analyzed white and Japanese children with KD and control populations for two polymorphic loci in which the A allele is associated with high tumor necrosis factor-a secretion. The lymphotoxin-a+250 A/A genotype was overrepresented among white children with KD compared with controls (0.59 versus 0.36;p = 0.013). The tumor necrosis factor-a-308 A/G genotype was overrepresented among whites with KD who had coronary artery abnormalities compared with those with normal echocardiograms (0.36 versus 0.09;p = 0.044). No significant difference was seen at either locus between Japanese children with KD and Japanese controls. The increased frequency of the high secretor alleles in white children with KD suggests that these loci may be related to susceptibility to KD and to outcome after disease.
