Abstract

Therapy-related myeloid neoplasms (t-MNs), which develop after cytotoxic, radiation, or immunosuppressive therapy for an unrelated disease, account for 7%-8% of acute myeloid leukemia (AML). Worse outcomes and consequently shortened survival are associated with t-MNs as compared with de novo AML. Therapy-related MNs are being reported with increasing frequency in successfully treated acute promyelocytic leukemia (APL), in particular, before the introduction of all-trans retinoic acid (ATRA) plus arsenic trioxide (ATO). Considering the high curability of APL, t-MNs represent one of the prognosis-limiting factors in this setting of leukemia. We report our experience with a patient who developed t-AML 15 years after treatment for APL. Treatment included three cycles of chemotherapy with CPX-351 (Vyxeos, Jazz Pharmaceuticals) followed, as in remission, by an allogeneic hematopoietic stem cell transplant. 10. The problem of bias in medical research and its relationship with medical education

Biases in medical research are becoming acknowledged as a serious and increasing problem for medicine all over the world. They compromise the evaluation of the real effects of drugs, and jeopardise the possibility of evidence-based decisions and knowledge in medical practice. The current measures adopted to attempt to reduce them, although important and necessary, seem to have had only a limited effect. The fundamental aim of this study is to be a piece of exploratory research on the possible factors involved in medical education that can be related to the existence of bias in medical research. The randomised controlled trial, the main research method in much of medical research, is analysed concerning its strengths and weaknesses as a scientific instrument and the most common biases that may occur in this research method are evaluated. Questionnaires and interviews with students and teachers of five medical schools in Parana State (Brazil) were used to appraise aspects of a potential connection between medical education and the aforementioned problems in medical research. Qualitative and quantitative analyses of the data obtained were performed. The level of awareness about bias in research, in these schools, was evaluated as insufficient and fragmentary. Some critical obstacles, related to the transmission of knowledge about bias to the students, were identified. There is evidence that, at least in the schools involved in this research, the problem of bias is considered as a minor issue, when compared with other structural and educational problems. Possible solutions to the problem of low level of awareness about bias in research were collected by the research instruments employed, and are discussed in regard to their potential efficacy and feasibility.

Abstract

Coagulation screening prior to surgery is performed routinely worldwide to identify patients at risk of bleeding during the procedure. Evidence from medical and surgical literature suggests that the activated partial thromboplastin time (aPTT) alone is suitable for predicting individual bleeding risk during surgery and it is current practice in our hospital to measure this parameter.We retrospectively reviewed aPTT ratio results in 8,069 consecutive adult subjects undergoing elective surgery from January 1 to December 31, 2014 to confirm the validity of this approach.In 7,606 patients (94.2%) the aPTT ratio was within the normal range while it was abnormal in 463 (5.8%). Out of these 463, 223 aPTT ratios were between 1.2 and 1.3 and we considered these results not worthy enough of further investigations. In 240 patients the aPTT ratio was higher than 1.3; in the vast majority of these cases (201/240; 83%) this abnormality was associated with oral anticoagulant treatment. Seventeen of the other 39 cases underwent detailed investigations which revealed lupus anticoagulant (n=7), decompensated chronic liver disease (n=4), factor XII deficiency (n=3), mild combined reduction of FXI and FXII (n=1) and mild haemophilia A (n=2). The other 22 patients underwent successful surgery without further investigation.Our results from a pre-surgical setting seem to confirm the low prevalence of coagulation defects in the general population. Increased aPTT ratios were mainly attributable to oral anticoagulant therapy, with a few cases caused by mild, clinically irrelevant clotting factor deficiencies. A carefully taken personal history, including medications (i.e. oral anticoagulants) and/or previous bleeding symptoms seem more useful than coagulation screening tests to predict the risk of bleeding.
