8. How the position of a cycle lane installed on an urban residential street impacts on the behaviour of young drivers.

Abstract

In this driving simulation study, we evaluated the change in driver behaviour of forty young drivers (20 females) following the conversion of a 2-way local street into a 1-way one with a cycle lane in a typical residential neighbourhood. Twenty drivers who were also used to commuting/travelling by bicycle were selected for the study. We compared three road layouts: (i) a standard 2-way urban street with no cycle lane (baseline), (ii) a 1-way street with a cycle lane on the right side, and (iii) a 1-way street with a left-side cycle lane. Driver behavioural metrics when interacting with cyclists including overtaking speed, lateral position in the lane, and vehicle-cyclist clearance were analysed using linear mixed-effect models. Speed and lateral position in the lane were also analysed in the non-overtaking phase. Significantly better driver behaviour was observed in layouts with a dedicated cycle lane, manifesting itself in greater lateral distance values for safer overtaking and better trajectory control. Drivers were more likely to stay within their lanes, eliminating the need to alter trajectory when passing cyclists, thus promoting better longitudinal and lateral control. The presence of a cycle lane significantly improves the interaction between drivers and cyclists, regardless of the position of the cycle lane, i.e., Ieftvs. right side Withrespectto the vehicle lane. Whiletheresults do not favour a particular side, designers should also consider cyclist preferences and driver expectations at intersections. Further research is crucial to validate findings and explore lane positioning effects near intersections, and greater emphasis should be placed on determining cyclist preferences in terms of infrastructure design. 29. The ATP-bound conformation of the Mre11-Rad50 complex is essential for Tel1/ATM activation

Abstract

Activation of the checkpoint protein Tel1 requires the Mre11-Rad50-Xrs2 (MRX) complex, which recruits Tel1 at DNA double-strand breaks (DSBs) through direct interaction between Tel1 and Xrs2. However, in vitro Tel1 activation by MRX requires ATP binding to Rad50, suggesting a role also for the MR subcomplex in Tel1 activation. Here we describe two separation-of-functions alleles, mre11-S499P and rad50-A78T, which we show to specifically affect Tel1 activation without impairing MRX functions in DSB repair. Both Mre11-S499P and Rad50-A78T reduce Tel1-MRX interaction leading to poor Tel1 association at DSBs and consequent loss of Tel1 activation. The Mre11-S499P variant reduces Mre11-Rad50 interaction, suggesting an important role for MR complex formation in Tel1 activation. Molecular dynamics simulations show that the wild type MR subcomplex bound to ATP lingers in a tightly 'closed' conformation, while ADP presence leads to the destabilization of Rad50 dimer and of Mre11-Rad50 association, both events being required for MR conformational transition to an open state. By contrast, MRA78T undertakes complex opening even if Rad50 is bound to ATP, indicating that defective Tel1 activation caused by MRA78T results from destabilization of the ATP-bound conformational state.

Abstract

Background:There is consensus that dementia is the most burdensome disease for modern societies. Few cost-of-illness studies examined the complexity of Alzheimer's disease (AD) burden, considering at the same time health and social care, cash allowances, informal care, and out-of-pocket expenditure by families.Methods:This is a comprehensive cost-of-illness study based on the baseline data from a randomized controlled trial (UP-TECH) enrolling 438 patients with moderate AD and their primary caregiver living in the community.Results:The societal burden of AD, composed of public, patient, and informal care costs, was about €20,000/yr. Out of this, the cost borne by the public sector was €4,534/yr. The main driver of public cost was the national cash-for-care allowance (€2,324/yr), followed by drug prescriptions (€1,402/yr). Out-of-pocket expenditure predominantly concerned the cost of private care workers. The value of informal care peaked at €13,590/yr. Socioeconomic factors do not influence AD public cost, but do affect the level of out-of-pocket expenditure.Conclusion:The burden of AD reflects the structure of Italian welfare. The families predominantly manage AD patients. The public expenditure is mostly for drugs and cash-for-care benefits. From a State perspective in the short term, the advantage of these care arrangements is clear, compared to the cost of residential care. However, if caregivers are not adequately supported, savings may be soon offset by higher risk of caregiver morbidity and mortality produced by high burden and stress. Abstract

The availability of family caregivers of older people is decreasing in Italy as the number of migrant care workers (MCWs) hired by families increases. There is little evidence on the influence of socioeconomic factors in the employment of MCWs.We analyzed baseline data from 438 older people with moderate Alzheimer's disease (AD), and their family caregivers enrolled in the Up-Tech trial. We used bivariate analysis and multilevel regressions to investigate the association between independent variables-education, social class, and the availability of a care allowance-and three outcomes-employment of a MCW, hours of care provided by the primary family caregiver, and by the family network (primary and other family caregivers).The availability of a care allowance and the educational level were independently associated with employing MCWs. A significant interaction between education and care allowance was found, suggesting that more educated families are more likely to spend the care allowance to hire a MCW.Socioeconomic inequalities negatively influenced access both to private care and to care allowance, leading disadvantaged families to directly provide more assistance to AD patients. Care allowance entitlement needs to be reformed in Italy and in countries with similar long-term care and migration systems.
