1. Reflections on Remaining Obstacles in a Primary-Care Oriented Pure PBL Curriculum after Twelve Years of Implementation

Abstract

A pioneer primary-care oriented pure PBL curriculum, based on constructivism and adult learning theories combined with Morin's complex thinking, was implemented in our medical school since 2002. Regardless of warnings opportunely made because the basic requirements for its successful implementation could not be fully fulfilled in practice, the experience was carried out and, while partially amended, still endures. This allows revealing several obstacles in many operational aspects, here recalled and/or characterized. Besides becoming helpful not only for counseling again to our own medical school about them but for warning and informing to those institutions with similar problems, the present analysis leads to a preventive final reflection: when designing and implementing a medical curriculum in general and particularly a pure PBL one, a thorough analysis of the contextual and operational factors, a flexible procedure and continuous objective evaluations for further adjustments become keystones for guaranteeing its fruitful implementation.

Abstract

Introducción: Considerando las dificultades halladas en el ámbito europeo y latinoamericano a este respecto, este trabajo intenta aportar ideas y una propuesta concreta a la problemática integración de las disciplinas básicas y clínicas en la carrera médica. Parte para ello de la polisémica definición de la integración conceptual y conforma un marco referencial esquemático centrado en la Medicina. Método: Se propone un camino posible con sus requisitos operativos básicos, se plantea un tema integrador con sus metas, variados objetivos y evaluación, y se rescata un tema integrador alternativo de la bibliografía especializada. Resultados: Se presentan resultados preliminares e intradisciplinares obtenidos en Histología y en Biología Celular y Molecular, respectivamente. Conclusiones: Una reflexión acerca del acto médico como proceso integrador y un interrogante sobre la integración de las perseguidas competencias médicas devienen conclusiones aproximativas en relación con un desafío cuya solución requiere todavía estudios y propuestas ulteriores., Introduction: Taking into account in this regard the noticeable difficulties perceived in Europe and Latin America, this paper intends to offer ideas and a definite proposal to the problematic integration of basic and clinic disciplines in medical career. Thus, it starts from the polysemic definition of conceptual integration and provides a schematic reference framework centered in Medicine. Method: A feasible way with its basic operative requisites is proposed. Likewise, an integrative theme with its goals, varied aims and evaluations is raised. Simultaneously, an alternative integrative theme is rescued from the specialized bibliography. Results: Preliminary and intradisciplinary results obtained in Histology and Molecular and Cellular Biology, respectively, are presented. Conclusions: A reflection about the medical practice as an integrative process and a question on the integration of the pursued medical competences become approximate conclusions related with a challenge whose solution still demands further studies and proposals.

5. La inoculación con Mycobacterium phlei inhibe la inflamación alérgica en un modelo de conejos sensibilizados con ovoalbúmina (OVA)

Abstract

The prevalence of atopic disease and other TH2 autoimmune disorders have increased in Western countries. One theory explains that fact by the hygiene hypothesis, which suggests that a decrease exposure to microbes in the environment leads to atopic disorders. The aim of the present study is to determine the effect of Mycobacterium phlei inoculation on allergic response from sensitized rabbits at early stages after birth in rectal mucosa. Thirty two rabbits were divided in 4 groups: G1: Normal control; G2: Ovalbumin (OVA) sensitized subcutaneously (SC); G3: OVA sensitized SC and OVA rectal challenged and G4: OVA sensitized SC and OVA rectal challenged previously inoculated with M. phlei at days 1; 30 and 60 after birth. The following monoclonal antibodies were used: anti-rabbit CD4 for CD4+ T cells identification; anti-rabbit CD8 and anti-rabbit CD25 (IL-2 receptor marker). Botin-streptavidin system was used for detection of marked cells and AEC as chromogen. Eosinophils: G1: 1,2 (0,6), G2: 3,1 (0,2), G3: 13,1 (0,8), G4: 1,33 (0,4); G4 vs G3 p<0,05. Mast cells: G1: 8,7 (1,3), G2: 17,6 (2,5), G3: 6,0 (1,8), G4: 7,2 (1,7); G4 vs G2 p<0,001. CD4: G1: 8,35 (0,7), G2: 13,4 (0,9), G3: 8,26 (0,6), G4 17,2 (1,29); G4 vs G1 and G3 p<0,05. CD8: G1: 6,28 (0,5), G2: 8,25 (0,8), G3: 10,77 (0,5), G4: 11,29 (1,1); CD25: G1: 13,2 (1,2), G2: 15,1 (1,9), G3: 20,0 (1,4), G4: 14,41 (1,9); G4 vs G3 p<0,05. Based on the evidence presented in this work, we conclude that early inoculation with M. phlei may inhibit by immune modulation allergic response in ovoalbumin sensitized and challenged rabbits., La prevalencia de enfermedades atópicas y otros desórdenes autoinmunes tipo Th2 se ha incrementado en los países occidentales. Una teoría lo explica mediante la “hipótesis higiénica”, la cual plantea que una disminución en la exposición a microbios del medio ambiente conduce a afecciones atópicas. El objetivo del estudio es determinar el efecto de la inoculación con Mycobacterium phlei sobre la respuesta alérgica en conejos sensibilizados en estadios tempranos post nacimiento en mucosa rectal. Treinta y dos conejos fueron divididos en 4 grupos: G1: Control normal; G2: Sensibilizado subcutáneo (SC) con ovoalbúmina (OVA); G3: Sensibilizado SC con OVA y desafiado por vía rectal con el mismo antígeno (OVA); G4: Sensibilizados SC con OVA y desafiados vía rectal con OVA previamente inoculados con M. phlei los días 1, 30 y 60 luego del nacimiento. Fueron utilizados los siguientes anticuerpos monoclonales: ratón anti-conejo CD4; ratón anti-conejo CD8 y ratón anti-conejo CD25 (receptor de Il-2). Las células marcadas se visualizaron mediante biotina-estreptavidina y AEC como cromógeno. Eosinófilos: G1: 1,2 (0,6), G2: 3,1 (0,2), G3: 13,1 (0,8), G4: 1,33 (0,4); G4 vs G3 p<0,05. Mastocitos: G1: 8,7 (1,3), G2: 17,6 (2,5), G3: 6,0 (1,8), G4: 7,2 (1,7); G4 vs G2 p<0,001. CD4: G1: 8,35 (0,7), G2: 13,4 (0,9), G3: 8,26 (0,6), G4 17,2 (1,29); G4 vs G1 and G3 p<0,05. CD8: G1: 6,28 (0,5), G2: 8,25 (0,8), G3: 10,77 (0,5), G4: 11,29 (1,1); CD25: G1: 13,2 (1,2), G2: 15,1 (1,9), G3: 20,0 (1,4), G4: 14,41 (1,9); G4 vs G3 p<0,05. En base a la evidencia presentada en este trabajo, concluimos que la inoculación temprana con M. phlei podría inhibir mediante modulación inmune la respuesta alérgica en conejos sensibilizados y desafiados con OVA.

Abstract

Background: Gut-associated lymphoid tissue (GALT) is mainly formed by the gut mucosa and associated lymphatic structures that under normal conditions induces hyporesponsiveness, a phenomenon termed oral tolerance. However, the potential brakeup of oral tolerance could otherwise lead to disorders such as food allergy. Objectives: The aim of the study is to characterise the histopathological and immunohistochemical modifications in intestinal gut mucosa in an animal model of food allergy. Methods: New Zealand rabbits were subcutaneously sensitized twice with ovalbumin (OVA), on day 30 after first sensitization, animals were oral challenged with the same antigen. Lymphatic cell population and accessory cells from gut mucosa were studied by conventional histology, histochemistry and immunohistochemistry. Results: An important increase in number of eosinophils were observed in sensitized and challenged group as well as CD25+ cells increase in sensitized animals without challenge. Conclusions: Data obtained demonstrated that subcutaneous sensitization and challenge with OVA induced generation of specific IgE antibodies and an anaphylactic inflammatory response. This pattern induced quantitative modifications in studied cells and structural changes in mucosa like oedema at intestinal villi in sensitized and challenged rabbits in this animal model of food allergy.
