1. The Effect of an Experiential Learning Program on Middle School Students' Motivation toward Mathematics and Science

Abstract

A mixed methods design was used to evaluate the effects of four experiential learning programs on the interest and motivation of middle school students toward mathematics and science. The Expectancy-Value model provided a theoretical framework for the exploration of 336 middle school student participants. Initially, participants were generally positive and had relatively high mathematics and science motivation (Eccles et al., 1983). Overall interest in mathematics increased after completing the program, but a decrease in the importance of mathematics on students' sense of self and some gender differences were detected, with males showing more gains than females. While few significant differences were found on the pre-post student self-ratings, other evidence suggests the program brought about meaningful change. Several potential reasons for the lack of detectable changes on self-reported student motivation measure are discussed. This article concludes with a discussion about the implications for the evaluation of science enrichment programs. (Contains 5 tables.)

2. The Effect of an Outdoor Nature Investigation Program on Young Children's Ability To Transfer Knowledge.

Abstract

This study examined the effect of using an outdoor nature investigation program on the ability of third-grade students to transfer knowledge. Specifically, the study determined whether the program enhanced students' ability to transfer declarative (facts and concepts), procedural (process skills), and schematic (experience) knowledge to a set of transfer situations. Forty-five third-grade students from an urban elementary school were divided into 2 groups, both of which were taught science by the same teacher in consecutive 7-week units. The main theme of each unit was habitats. The treatment program, used only with the second student group, involved students becoming scientists by reading about a particular topic, posing questions, collecting data in their schoolyard, and analyzing the data by creating graphs and charts. Students were interviewed after watching video vignettes that presented situations requiring generalization from what they had learned during the unit. Results indicate that both groups effectively transferred knowledge to the more similar scenario, but only the group that received the treatment was able to transfer knowledge to the less similar scenario. In addition, the treatment group transferred declarative knowledge better than students in the control group on the more similar problem but not on the less similar problem. The treatment group appeared to transfer procedural knowledge better in both scenarios. Transfer of schematic knowledge appeared to favor the treatment group, but schematic knowledge was low in both groups. Contains 13 references. (LP)

Abstract

Drusen are hallmarks of early and intermediate age-related macular degeneration (AMD) but their quantification remains a challenge. We compared automated drusen volume measurements between different OCT devices. We included 380 eyes from 200 individuals with bilateral intermediate (iAMD, n = 126), early (eAMD, n = 25) or no AMD (n = 49) from the MACUSTAR study. We assessed OCT scans from Cirrus (200 × 200 macular cube, 6 × 6 mm; Zeiss Meditec, CA) and Spectralis (20° × 20°, 25 B-scans; 30° × 25°, 241 B-scans; Heidelberg Engineering, Germany) devices. Sensitivity and specificity for drusen detection and differences between modalities were assessed with intra-class correlation coefficients (ICCs) and mean difference in a 5 mm diameter fovea-centered circle. Specificity was > 90% in the three modalities. In eAMD, we observed highest sensitivity in the denser Spectralis scan (68.1). The two different Spectralis modalities showed a significantly higher agreement in quantifying drusen volume in iAMD (ICC 0.993 [0.991–0.994]) than the dense Spectralis with Cirrus scan (ICC 0.807 [0.757–0.847]). Formulae for drusen volume conversion in iAMD between the two devices are provided. Automated drusen volume measures are not interchangeable between devices and softwares and need to be interpreted with the used imaging devices and software in mind. Accounting for systematic difference between methods increases comparability and conversion formulae are provided. Less dense scans did not affect drusen volume measurements in iAMD but decreased sensitivity for medium drusen in eAMD.Trial registration: ClinicalTrials.gov NCT03349801. Abstract

This study aimed to determine the retest variability of quantitative fundus autofluorescence (QAF) in patients with and without age-related macular degeneration (AMD) and evaluate the predictive value of patient reliability indices on retest reliability. A total of 132 eyes from 68 patients were examined, including healthy individuals and those with various stages of AMD. Duplicate QAF imaging was conducted at baseline and 2 weeks later across six study sites. Intraclass correlation (ICC) analysis was used to evaluate the consistency of imaging, and mean opinion scores (MOS) of image quality were generated by two researchers. The contribution of MOS and other factors to retest variation was assessed using mixed-effect linear models. Additionally, a Random Forest Regressor was trained to evaluate the extent to which manual image grading of image quality could be replaced by automated assessment (inferred MOS). The results showed that ICC values were high for all QAF images, with slightly lower values in AMD-affected eyes. The average inter-day ICC was found to be 0.77 for QAF segments within the QAF8 ring and 0.74 for peripheral segments. Image quality was predicted with a mean absolute error of 0.27 on a 5-point scale, and of all evaluated reliability indices, MOS/inferred MOS proved most important. The findings suggest that QAF allows for reliable testing of autofluorescence levels at the posterior pole in patients with AMD in a multicenter, multioperator setting. Patient reliability indices could serve as eligibility criteria for clinical trials, helping identify patients with adequate retest reliability. [ABSTRACT FROM AUTHOR]

5. Universities, Schools, and Communities: A New Generation of Professional Development School Roles, Structures, and Governance

Abstract

What differentiates the professional development school (PDS) from other schools are the people and the roles they have created to enact the functions or goals unique to the PDS model: preparation of teacher candidates; enhanced professional learning for educators; improved student achievement through the simultaneous renewal of university and K-12 curriculum; and inquiry designed to continuously improve teaching and learning. As schools and universities have established PDS models around the country, the roles, structures, and governance established by each model vary greatly, some enhancing, others limiting the partnership's ability to truly fulfill the functions and ultimate promise of PDSs when they were first conceptualized nearly three decades ago. In this chapter, the authors briefly examine the literature related to roles, structures, and governance and the typical players in PDSs, while making a case for the rediscovery of the community as having a critical role in PDS work. They then turn their attention to the discussion of examining the structure of the PDS as a social learning space. And finally, they look at how this new structure has implications for governance and roles within PDS partnerships. (Contains 1 note.)

Abstract

A Professional Development School (PDS) offers unique university-school relationships that can change the culture of learning and add value to students and the community. Initially created in the 1980s, the PDS movement is growing across the country and is now a respected teacher education model. In this book, Carole G. Basile has collected stories written by people connected to the University of Colorado Denver, which has one of the longest-standing PDS partnerships (established in 1993). The site professors, site coordinators, teachers, and others describe in engaging detail the work they do and its impact. By providing a framework situated in the notion of intellectual capital, PDS faculty from the university and K-12 share how the PDS model adds value to schools and students. 7. Teachers as Advisors: Fostering Active Citizens in Schools

Abstract

When considering democratic education, educators talk about teaching the "whole child," creating caring classrooms, building communities of learning where every student has access to knowledge, and teaching students how to be active citizens (Goodlad and Keating 1994; Noddings 2002; Apple 1993). These are not practices that are done "to" students but "with" students in ways that promote understanding different points of view and problem solving, resulting in action that is equitable and forward looking. With the pressures to close the achievement gap and improve school performance, how do teachers find time to embed these practices in the daily activities of the school community? More importantly, how can they afford not to include students' voices in constructive ways? This article describes how this is accomplished at the Jefferson County Open School, a public alternative school near Denver, Colorado, where teachers work to reach and build relationships with students to establish resiliency, connectedness, and democratic practices.

Abstract

Quantification of the relative ellipsoid zone reflectivity (rEZR) might be a structural surrogate parameter for an early disease progression in the context of age-related macular degeneration (AMD). Within the European multicenter, cross-sectional MACUSTAR study, we have devised an automatic approach to determine the mean rEZR [arbitrary units, AU] at two independent visits in SD-OCT volume scans in study participants. Linear mixed-effects models were applied to analyze the association of AMD stage and AMD associated high-risk features including presence of pigmentary abnormalities, reticular pseudodrusen (RPD), volume of the retinal-pigment-epithelial–drusenoid-complex (RPEDC) with the rEZR. Intra-class correlation coefficients (ICC) were determined for rEZR reliability analysis. Within the overall study cohort (301 participants), we could observe decreased rEZR values (coefficient estimate ± standard error) of − 8.05 ± 2.44 AU (p = 0.0011) in the intermediate and of − 22.35 ± 3.28 AU (p < 0.0001) in the late AMD group. RPD presence was significantly associated with the rEZR in iAMD eyes (− 6.49 ± 3.14 AU; p = 0.0403), while there was a good ICC of 0.846 (95% confidence interval: 0.809; 0.876) in the overall study cohort. This study showed an association of rEZR with increasing disease severity and the presence of iAMD high-risk features. Further studies are necessary to evaluate the rEZR’s value as a novel biomarker for AMD and disease progression.

14. Nature at Your Doorstep: Real World Investigation for Primary Students.

Abstract

"Nature at Your Doorstep" is an outgrowth of a successful environmental education program offered as a school field trip at a Nature Discovery Center. It was developed and used with thousands of students throughout Bellaire, Texas and the surrounding Houston metroplex. The purpose of this program is to kindle students' curiosity and wonder about the interdependent world of nature of which they are a part, and to foster their responsibility toward the environment. Each unit is designed to guide students through discovery of basic biological concepts using the scientific method as a logical method of inquiry. Students explore questions, formulate hypotheses based on current knowledge, collect data relevant to the questions, and, by creating various forms of representations, analyze their data to formulate conclusions and compare those conclusions with their hypotheses. The 10 units are as follows: (1) Senses; (2) Trees; (3) Birds; (4) Insects and Neighbors; (5) Ants; (6) Biodiversity; (7) Habitats; (8) Communities; (9) Food Webs; and (10) Endangered Species. Each unit provides focus and background material, activities to introduce the topic, suggestions for teaching each data sheet, and extensions for further study. Student data sheet masters are included. All units are integrated with opportunities for learning experiences in science, mathematics, and language arts. (PVD)

This open‐label study investigated the impact of hepatic impairment on ivosidenib pharmacokinetics (ClinicalTrials.gov: NCT03282513). Otherwise healthy participants with mild (n = 9) or moderate (n = 8) hepatic impairment (Child‐Pugh score) and matched participants with normal hepatic function (n = 16) received 1 oral dose of 500‐mg ivosidenib. Mild hepatic impairment had a negligible effect on total ivosidenib plasma exposure, with geometric mean ratios (90% confidence interval [CI]) of 0.933 (0.715‐1.22) for maximum concentration (Cmax) and 0.847 (0.624‐1.15) for area under the plasma concentration–time curve (AUC) in participants with mild hepatic impairment versus matched controls. Moderate hepatic impairment reduced total ivosidenib exposure by 28% to 44%, with geometric mean ratios (90%CI) of 0.565 (0.419‐0.763) for Cmaxand 0.716 (0.479‐1.07) for AUC, although the 90%CI for AUC included 1.00. The ivosidenib unbound fraction was concentration dependent and higher in participants with mild/moderate hepatic impairment compared with matched controls. There was no apparent trend to increasing unbound Cmaxwith increased hepatic impairment severity. A single 500‐mg ivosidenib dose was well tolerated, with no serious or severe adverse events and no adverse events leading to discontinuation. We conclude that mild/moderate hepatic impairment did not lead to clinically relevant changes in ivosidenib exposure following a single 500‐mg dose.

Abstract

Tumor necrosis factor-α (TNF-α) is a pro-inflammatory mediator of the immune response to allogenic and infectious stimuli. Non-immunosuppressed individuals possessing a NcoI restriction enzyme site in the TNF gene locus produce less TNF-α in vitro and in vivo compared with individuals lacking this restriction site. We performed polymerase chain reaction amplification and restriction enzyme fragment length analysis of the TNF locus from 86 liver transplant recipients to determine if presence of the NcoI site is associated with the frequency of rejection or infection, time to rejection or infection, and patient and graft survival. We controlled for recipient primary diagnosis, age, sex, United Network for Organ Sharing status, year of transplant, type of immunosuppression, use of anti-lymphocyte agents, and graft ischemia time. Fifty-six recipients possessed the NcoI+/low TNF-α genotype and 30 were NcoI-/high TNF-α genotype. In the first year after transplant, there were no significant differences in the frequency, or time to first rejections or the overall number of rejection episodes between the two genotypes. NcoI+/low TNF-α genotype recipients had significantly more infections (1.52 vs. 0.87, P=0.014). In a linear regression, multivariate model controlling for all marginally significant variables, the NcoI+/low TNF-α genotype was still associated with significantly more infections (P=0.0031). Patient and graft survival were equal for the two groups. One implication of this study, in individuals genetically predetermined to be low TNF-α producers, is that additional inhibition of TNF-α production by routine immunosuppression may be excessive, rendering these individuals less able to respond to infectious stimuli. These patients may benefit from lower doses or with-drawal of corticosteroids.

Abstract

Infection with a sexually transmitted disease (STD) increases the risk for human immunodeficiency virus (HIV) infection. Polymorphonuclear leukocytes (PMNs) are recruited into the genital tract by STD pathogens, such as Chlamydia trachomatis. Semen of HIV-infected men contains HIV associated with mononuclear cells. This study investigated the interaction among PMNs from HIV-uninfected persons, C. trachomatis, and HIV-infected cells and examined the mechanisms for enhanced HIV replication. We demonstrated that PMNs from HIV-seronegative donors induced HIV replication in mononuclear cells from 17 HIV-infected patients in medium without exogenous IL-2. HIV in the cell-free supernatants from cocultures of PMNs and patients' peripheral blood mononuclear cells (PBMCs) was replication competent, as indicated by their capacity to propagate HIV in a second round of culture using PBMCs from HIV-seronegative individuals and by the fact that proviral DNA was found in these cells. PMNs from HIV-seronegative donors increased HIV replication over 100-fold in chronically HIV-infected cell lines of the monocytic, T, and B cell lineages. Moreover, PMNs increased U1 cells' production of p24 antigen by as much as ninefold when compared with U1 cells cocultured with PBMCs. The addition of C. trachomatis to PMN and U1 coculture increased HIV replication by an additional ninefold at 24 h, whereas C. trachomatis alone had no effect on p24 antigen production by U1 cells. Thus, C. trachomatis serves not only to recruit PMNs, but also to interact with PMNs to increase HIV replication. HIV replication is triggered by contact of HIV-infected cells with PMNs, by the generation of reactive oxygen intermediates (ROIs), and by soluble factors such as TNF-alpha and IL-6. This is based on the findings that production of p24 antigen, IL-6, and TNF-alpha induced by PMNs is abrogated by disrupting or partitioning PMNs from HIV-infected cells; is inhibited by superoxide dismutase and catalase, enzymes that destroy ROIs; is enhanced by differentiated HL60 cells capable of producing ROIs; and is induced by PMNs tested negative for CMV. Furthermore, the production of ROIs is independent of HIV infection of mononuclear cells, since PMNs cocultured with HIV-uninfected parental monocytic and T cell lines generated ROIs. Therefore, the increased risk for acquiring HIV infection associated with chlamydia cervicitis may be related to the local recruitment of PMNs by C. trachomatis and the induction of infectious virus from mononuclear cells present in semen. These observations provide a rationale for strategies to reduce HIV transmission by control of STD.

Abstract

We present the classical and quantum versions of collision induced absorption spectra for three exactly solvable 1-D models, and obtain the asymptotic behaviour at high frequency. It is found that the quantum spectra fall off more slowly than the classical, and for realistic systems, they both fall off more slowly than exponential. An improved version of the Landau-Lifshitz WKB method is suggested as a numerically viable method for calculating the high frequency wing in quantum spectra and is tested on the exactly solvable models. The error is found to be negligible in two models and tolerable in the third model, even for the least classical system. Our discussion of an extension of the method using exact wavefunctions increases its scope beyond the collision induced absorption problem.

Abstract

The aim of this study was to evaluate the utility of the brachial plexus block using an axillary route employing the technique in which the needle is inserted into the sheath at an angle parallel to the neuro-vascular bundle with a sole modification: using a G22 spinal needle and without evoking paresthesia. The results obtained show that this method ensures an improved and more widespread analgesia. The flexibility and small size of the G22 spinal needle allow traumas to the axillary guaina and brachial plexus to be reduced to a minimum. In addition its length enables the anesthetic solution to be diffused around the first rib, including the axillary and musculo-cutaneous nerves, thus ensuring e total sensory and motor block of the upper limb.

We have reported the results of 700 general anesthesia induced and maintained by Propofol in women who have undergone IVG operation. The anesthesia-method used has guaranteed an adequate level of anesthesia with the characteristics of a quick induction, non significant variations of the cardiovascular parameters, a quick recovery of consciousness, low incidence of side effects, the abolition of the azote-peroxide and the halogen-anesthetics, a quick discharge of the patients.

Abstract

A case of severe anaphylactic reaction provoked by suxamethonium during the induction of general anaesthesia is reported. Diagnosis of reaction and the trigger effect of suxamethonium was identified by intradermal testing. Diagnosis of anaphylactic reaction was confirmed on serial blood estimations of IgE and C3-C4 of complement fractions.
