Objectives: The Pasifika Prediabetes Youth Empowerment Programme (PPYEP) was a community‐based research project that aimed to investigate empowerment and co‐design modules to build the capacity of Pasifika youth to develop community interventions for preventing prediabetes. Methods: This paper reports findings from a formative evaluation process of the programme using thematic analysis. It emphasises the adoption, perceptions and application of empowerment and co‐design based on the youth and community providers’ experiences. Results: We found that the programme fostered a safe space, increased youth's knowledge about health and healthy lifestyles, developed their leadership and social change capacities, and provided a tool to develop and refine culturally centred prediabetes‐prevention programmes. These themes emerged non‐linearly and synergistically throughout the programme. Conclusions: Our research emphasises that empowerment and co‐design are complementary in building youth capacity in community‐based partnerships in health promotion. Implications for public health: Empowerment and co‐design are effective tools to develop and implement culturally tailored health promotion programmes for Pasifika peoples. Future research is needed to explore the programme within different Pasifika contexts, health issues and Indigenous groups.

Purpose To present a case study of the considerations of mandatory fortification with folic acid in Australia and New Zealand. Methods Review of published reports and consumer advocacy views. Results Australia and New Zealand jointly approved mandatory fortification of flour with folic acid to prevent neural tube defects in 2007. Fortification was fully implemented in Australia in 2009 and has resulted in reduction in NTD. At the last minute, industry lobbying led to the New Zealand government not proceeding with fortification. With continued consumer advocacy, mounting scientific evidence, and a change of government, approval was given in 2021 for mandatory fortification of flour with folic acid. Conclusion In large part as a response to consumer pressure, New Zealand has now joined with Australia (and around 70 other countries) in fortifying flour with folic acid for the prevention of NTD.

The New Zealand health system is data-rich, information-poor, and intelligence meagre. However, there is widespread confusion about the definitions of these terms, so they are often used synonymously. Like many jurisdictions, we continue to collect and collate vast quantities of data at an increasing rate. Many tools are available to “analyse” the data deluge with the false expectation that “intelligence” will be produced. Naively, such a data-driven, machine-analysed paradigm is often thought to produce the “evidence” for decision-making and policy development. Continuing such a blinded approach poses potential health risks to New Zealanders and remains a major impediment to improving our health statusCreating intelligence from information involves humans (perhaps in concert with AI) utilising their interpretative abilities, asking the “so what, “what does it mean” questions, and employing their communication skills to disseminate the answers. To move from information to intelligence requires agencies to employ, develop and maintain a sufficiently skilled workforce over a long period, rather than the quick and easy investment in more and faster machines and software.Only through a human-driven evaluation of intelligence-based decisions and policies will our knowledge about the environmental health system increase and ultimately yield better health outcomes.Environmental Health Intelligence NZ (EHINZ) provides intelligence as evidence for decision-making and policy development in environmental health. It is based on the interpretation, communication, and dissemination of information from the surveillance more than seventy environmental health indicators (EHIs) across twelve domains (e.g., air and water quality, climate change), exposure to hazardous substances, and social vulnerability indicators to environmental hazards (e.g., flooding, climate change, sea-level rise, wildfires, heat waves).The paper details our approach, with two case studies, in providing the NZ health system with “intelligence for environmental health decisions.”

Co-design is a relatively new method employed in public health-based interventions to identify problems, generate and implement solutions through harnessing knowledge and creativity of citizens and staff . Researchers use different co-design steps in the design and implementation of intervention programmes. The co-design approach has been successfully used in redesigning health care services to fit the needs of the consumers and has extended to develop health interventions for communities . In New Zealand, co-design methods have been used to develop health interventions among minority and indigenous groups. Previous research highlighted that co-design fits well when collaborating with these groups as it allows tool redevelopments and re-fining based on the socio-cultural needs of participants. This method captures and understands the needs of the Tongan community, as well as foster expression, reflection, and sharing to inform the development of the intervention. The generation of discussion in co-design aligns with the indigenous knowledge of systems, creation stories and oral stories which provide a culturally empowering way to generate discussion and insights from Tongan communities.

Melanoma is one of the most common cancers diagnosed in New Zealand, and New Zealand has one of the highest rates of melanoma incidence and mortality in the world. Monitoring by Environmental Health Intelligence NZ (EHINZ) has found a recent sharp decline in melanoma mortality rates in New Zealand. Since 2014 and 2015 (with 376 and 378 melanoma deaths, respectively), melanoma deaths have declined to 362 deaths in 2016, 308 deaths in 2017, and 296 deaths in 2018. We believe that two new PD-1 inhibitor drug treatments introduced in New Zealand in 2016-nivolumab (Opdivo, BMS) and pembrolizumab (Keytruda, MSD)-may have contributed to this decrease in melanoma mortality. Other factors are unlikely to have had such a major effect, with the drop unlikely due to random variation, and no major changes in melanoma registrations or melanoma thickness at diagnosis over the past decade. While our monitoring of the time trend is descriptive only, and cannot attribute causality, it does suggest a recent decrease in melanoma mortality rates at the population level. These national-level statistics reflect both what might be expected in the New Zealand situation with the introduction of PD-1 inhibitor treatments, based on clinical trials, and what oncologists are seeing at an individual level. Further studies could investigate this observational finding, to confirm whether PD-1 inhibitor drug treatments are having an impact on melanoma mortality and survival rates in New Zealand.

Background: Hypospadias is a common male birth defect that has shown widespread variation in reported prevalence estimates. Many countries have reported increasing trends over recent decades.Objective: To analyze the prevalence and trends of hypospadias for 27 international programs over a 31-yr period.Design, setting, and participants: The study population included live births, stillbirths, and elective terminations of pregnancy diagnosed with hypospadias during 1980-2010 from 27 surveillance programs around the world.Outcome measurements and statistical analysis: We used joinpoint regression to analyze changes over time in international total prevalence of hypospadias across programs, prevalence for each specific program, and prevalence across different degrees of severity of hypospadias.Results and limitations: The international total prevalence of hypospadias for all years was 20.9 (95% confidence interval: 19.2-22.6) per 10 000 births. The prevalence for each program ranged from 2.1 to 39.1 per 10 000 births. The international total prevalence increased 1.6 times during the study period, by 0.25 cases per 10 000 births per year (p Conclusions: Although there have been changes in clinical practice and registry ascertainment over time in some countries, the consistency in the observed increasing trends across many programs and by degrees of severity suggests that the total prevalence of hypospadias may be increasing in many countries. This observation is contrary to some previous reports that suggested that the total prevalence of hypospadias was no longer increasing in recent decades.Patient summary: We report on the prevalence and trends of hypospadias among 27 birth defect surveillance systems, which indicate that the prevalence of hypospadias continues to increase internationally. Social vulnerability indicators are a valuable tool for understanding which population groups are more vulnerable to experiencing negative impacts from disasters, and where these groups live, to inform disaster risk management activities. While many approaches have been used to measure social vulnerability to natural hazards, there is no single method or universally agreed approach. This paper proposes a novel approach to developing social vulnerability indicators, using the example of flooding in Aotearoa New Zealand. A conceptual framework was developed to guide selection of the social vulnerability indicators, based on previous frameworks (including the MOVE framework), consideration of climate change, and a holistic view of health and wellbeing. Using this framework, ten dimensions relating to social vulnerability were identified: exposure, children, older adults, health and disability status, money to cope with crises/losses, social connectedness, knowledge, skills and awareness of natural hazards, safe, secure and healthy housing, food and water to cope with shortage, and decision making and participation. For each dimension, key indicators were identified and implemented, mostly using national Census population data. After development, the indicators were assessed by end users using a case study of Porirua City, New Zealand, then implemented for the whole of New Zealand. These indicators will provide useful data about social vulnerability to floods in New Zealand, and these methods could potentially be adapted for other jurisdictions and other natural hazards, including those relating to climate change.

© The Author 2017. Background This article outlines the methods being used to evaluate a community-based public health intervention. This evaluation approach recognizes that not only is the intervention, Healthy Families NZ, complex, but the social systems within which it is being implemented are complex. Methods To address challenges related to complexity, we discuss three developing areas within evaluation theory and apply them to an evaluation case example. The example, Healthy Families NZ, aims to strengthen the prevention system in Aotearoa/New Zealand to prevent chronic disease in 10 different geographic areas. Central to the evaluation design is the comparative case method which recognizes that emergent outcomes are the result of 'configurations of causes'. 'Thick', mixed-data, case studies are developed, with each case considered a view of a complex system. Qualitative Comparative Analysis is the analytical approach used to systematically compare the cases over time. Conclusions This article describes an approach to evaluating a community-based public health intervention that considers the social systems in which the initiative is being implemented to be complex. The evaluation case example provides a unique opportunity to operationalize and test these methods, while extending their more frequent use within other fields to the field of public health.

Background Sexual function declines with age and erectile dysfunction (ED) is a common condition worldwide; however, prevalence rates vary markedly between populations and reliable data specific to New Zealand (NZ) are lacking. Aim To assess the prevalence of ED in NZ men using a population-based cross-sectional survey. Methods Postal questionnaires were sent, according to a modified Dillman method, to a randomly selected age-stratified population-based sample of 2,000 men 40 to 70 years old obtained from the electoral roll. Self-reported erectile function was assessed using the five-item International Index of Erectile Function (IIEF-5) and the single-question self-assessment tool. Outcomes The prevalence of ED is presented as crude, age-adjusted to the distribution of the NZ population, and standardized to the World Health Organization World Standard Population (WSP). Associations between sexual function and age were analyzed using χ2 test. Results The response rate was 30% (599) and 28% (562) were complete for analysis. The crude prevalence of ED was 42% (22% mild, 10% mild to moderate, 6% moderate, and 4% severe), the age-adjusted prevalence was 38%, and the WSP-adjusted prevalence was 37%. Among men reporting ED, 16% were medically diagnosed and 22% were treated. ED affected 24% of men in their 40s, 38% in their 50s, and 60% in their 60s (P < .001). Age was associated with a significant increase in diagnosed ED (P = .001), treated ED (P = .006), dissatisfaction with current sexual function (P < .001), associated anxiety or depression (P = .023), and a decrease in sexual activity (P < .001). Clinical Translation Approximately one in three NZ men 40 to 70 years old might have ED. Although comparable to overseas populations, this prevalence is high. Strengths and Limitations This study provides the most reliable, comprehensive, and current information on ED and its risk factors in NZ men. Strengths include the large sample, the use of random selection from a population-based sampling frame, established effective survey methods, and the validated IIEF-5. Limitations include the inability of cross-sectional data to determine causation, non-sampling errors associated with the population-based sampling frame, the low response rate, the inability to assess non-respondents, the possibility of men with ED who were sexually inactive not responding or not completing the IIEF-5, and the inherent inability to rule out recall bias. Conclusion ED is a marker of subclinical cardiovascular disease. The high prevalence and low levels of diagnosis and treatment indicate a lost opportunity for timely intervention to delay or prevent the progression toward clinical disease.

ObjectivesObesity among Pasifika people living in New Zealand is a serious health problem with prevalence rates more than twice those of the general population (67% vs 33%, respectively). Due to the high risk of developing obesity for this population, we investigated diet quality of Pacific youth and their parents and grandparents. Therefore, we examined the dietary diversity of 30 youth and their parents and grandparents (n=34) to identify whether there are generational differences in dietary patterns and investigate the relationship between acculturation and dietary patterns.MethodsThe study design of the overarching study was cross-sectional. Face-to-face interviews were conducted with Pasifika youth, parents and grandparents to investigate dietary diversity, that included both nutritious and discretionary food items and food groups over a 7 day period. Study setting was located in 2 large urban cities, New Zealand. Exploratory factor analyses were used to calculate food scores (means) from individual food items based on proportions consumed over the week, and weights were applied to calculate a standardised food score. The relationship between the level of acculturation and deprivation with dietary patterns was also assessed.ResultsThree distinctive dietary patterns across all participants were identified from our analyses. Healthy diet, processed diet and mixed diet. Mean food scores indicated statistically significant differences between the dietary patterns for older and younger generations. Older generations showed greater diversity in food items consumed, as well as eating primarily a ‘healthy diet’. The younger generation was more likely to consume a ‘processed diet’. There was significant association between acculturation and deprivation with the distinctive dietary patterns.ConclusionOur investigation highlighted generational differences in consuming a limited range of food items. Identified dietary components may, in part, be explained by specific acculturation modes (assimilation and marginalised) and high socioeconomic deprivation among this particular study population.

HBsAg seroclearance is the most desired endpoint in chronic hepatitis B (CHB) but occurs uncommonly. Recent studies have shown baseline HBsAg levels to be predictive of HBsAg loss up to 10 years. We report the 28-year rates of HBsAg loss and outcomes in the Kawerau study cohort from New Zealand, and assess the predictive value of baseline HBsAg levels to predict long-term HBsAg loss.The 1984 Kawerau community study identified 572 CHB patients, followed up for 28 years (41 % HBeAg-positive, median age 17 years, range 1-71 years). In 2012, surviving individuals attended a local clinic for an interview, blood tests and transient elastography.384/218 (74 %) surviving individuals attended the clinic in 2012. Spontaneous HBsAg loss occurred in 145 (33 %) after 12,702 person-years of follow-up (1.14 per 100 person-years). Liver stiffness measurements were significantly lower if HBsAg loss occurred 50 years (mean 6.1 kPa) versus 50 years (mean 11.6 kPa), p = 0.0002. No HCC occurred following HBsAg loss (median follow-up 72 months). Predictors of HBsAg loss were older age and lower baseline HBsAg level (HR for HBsAg loss at 28 years 2.7 (95 % CI 1.7-4.2), 6.7 (95 % CI 3.9-11.4) and 9.4 (95 % CI 5.2-16.9), respectively, for HBsAg 1000-9999, 100-999 and 100 IU/mL compared to HBsAg 10,000 IU/mL at baseline, (p 0.0001). Baseline HBsAg was a superior predictor of HBsAg loss compared to HBV DNA at all time-points: AUROC at 15 years: 0.87 (95 % CI 0.82-0.93) versus 0.73 (95 % CI 0.66-0.80) (p 0.0001) and AUROC at 28 years: 0.74 (95 % CI 0.69-0.79) versus 0.67 (95 % CI 0.62-0.72) (p = 0.0007). The optimal cut-off HBsAg level to predict HBsAg seroclearance at 28 years is HBsAg 10,000 IU/mL (sensitivity 72 %, specificity 64 %, NPV 88 %).Rates of HBsAg loss in our community cohort were high, and occurred earlier than previously reported. Earlier HBsAg loss was associated with less severe liver fibrosis. Baseline HBsAg level was a good predictor of long-term HBsAg loss up to 28 years and superior to HBV DNA.

Chronic hepatitis B infection is endemic in New Zealand and has high prevalence in New Zealand Māori. Previous longitudinal studies in populations with predominantly vertically acquired chronic hepatitis B have shown low spontaneous hepatitis B surface-antigen (HBsAg) seroclearance rates: 0.5-1.4% annually (mean age of clearance 48 years). We report the 28-year follow-up data on clinical and serological outcomes in indigenous New Zealand Māori with early horizontally acquired HBV.In 1984, community seroprevalence study identified 572 HBsAg-positive individuals, followed for 28 years. Liver-related mortality and hepatocellular carcinoma (HCC) incidence were compared between these 572 HBV carriers and 1140 HBsAg-negative matched case-controls. Surviving HBsAg-positive individuals have been followed up in 2012 with clinical assessment, blood tests and liver transient elastography. Rates of hepatitis B e-antigen (HBeAg) and HBsAg seroconversion were determined.After total 13 187.4 person-years follow-up, 15 HBsAg-positive patients have developed HCC compared with none of the HBsAg-negative controls (p0.001). 12 HBsAg-positive patients died from liver-related causes compared with none in the controls (p0.001). Spontaneous HBeAg-seroconversion occurred in 91% of HBeAg-positive patients. Spontaneous HBsAg loss occurred in 33% overall (annual clearance rate 1.34%), with higher rates at older ages (1.05% in patients20 years at entry vs 4.3% per annum40 years at entry, p0.0001). Median ages of HBeAg loss and HBsAg loss were 23 years (range 6-66 years) and 40 years (range 4-80 years), respectively.Horizontally transmitted HBV in Maori is similarly associated with increased risk of liver-related mortality and HCC compared with Chinese, although absolute incidence rates are lower. The rates of HBeAg and HBsAg loss are high, and occur at an earlier age than previously reported.

Purpose– The extent to which research into the design and development of environmental health indicators (EHIs) has translated into operational programmes is unclear. The purpose of this paper is to identify EHI initiatives worldwide, distil the EHIs and draw lessons from the experience.Design/methodology/approach– A systematic internet-based review was undertaken. Programmes were selected for inclusion if they: first, had the ability to monitor both the physical environment and associated health outcomes; and second, the parent agency had the ability to influence policies related to the environment and health.Findings– The small number of eligible programmes indicates EHI initiatives are not yet well established, especially in developing countries. The use of indicators was also limited by uncertainties in the exposure-response relationships that they implied, and the consequent inability to translate the indicators into a common measure of health impact. In addition, there is no information on the extent to which the indicators have been applied in decision making, nor on the policy implications of using indicators.Practical implications– More effort is needed to encourage the development and use of more balanced and informative sets of indicators, and to evaluate their use and outcomes in terms of health benefits.Originality/value– The time is right for a substantial review paper on EHIs as they are now being used by a number of organisations and to the knowledge this is the first review of operational EHI programmes worldwide.

A population-based cross-sectional study was conducted to follow-up an earlier study which estimated the prevalence of self-reported visual disability and its impact on the quality of life (QOL) of older persons living in a rural area of Northeast Thailand based on a secondary analysis of data collected for another purpose. Self-reported difficulty with seeing was used to determine prevalence of visual disability. The WHOQOL-BREF and WHOQOL-OLD were used to assess the impact of visual disability on QOL. Fifty nine percent reported having difficulty seeing to the extent that it interfered with their daily life. Those who reported having difficulty with seeing were found to be older, disproportionately female, have lower perception of health, and to score lower on QOL than those who did not. The findings of this study differed from the earlier study. It is asserted that the findings from this latter study are likely to be more representative of the situation due to the use of a more systematic and targeted sampling procedure.

To determine the incidence of orofacial cleft at birth in New Zealand over 10 years from January 2000.Comparison of data collected from cleft units and data held on the national minimum dataset.The overall incidence of OFC in New Zealand over a 10 year period was found to be 1.79 per 1,000 live births, higher than the norm for Western society. The major reason for this increased rate was an increased rate for the Māori 2.37 per 1,000 live births, specifically related to a Cleft Palate alone rate over twice that of the European (1.54 vs 0.73 per 1,000 live births). The rate for Pacific was half way between (1.04 per 1,000 live births). The rate of Cleft Lip alone was significantly lower in both Māori and Pacific populations. Different sex ratios were also seen in relation to Cleft Lip and Cleft Lip and Palate for Māori and Pacific compared to those normally reported.Māori have an increased incidence of Orofacial Cleft due to one of the highest rates of Cleft Palate alone in the world. Further aetiological studies involving genetic and environmental factors are required to elicit the reasons for this increased incidence.

In New Zealand, the burden of obesity is greatest among Pacific people, especially in children and adolescents. We investigated the factors of the obesogenic environment that were indigenous to Pasifika youths' social-cultural context, their food purchasing behaviours, and associated anthropometric measures.An exploratory study of 30 Pasifika youth aged 16-24 years in Wellington and Auckland, New Zealand.A large proportion of the participants were obese (mean body mass index: 31.0kg/m2; waistto-hip ratio: 0.84; waist-to-height ratio: 0.6), suggesting that the future health and wellbeing trajectory of the studied Pasifika youth is poor. Purchasing behaviours of food and snacks over a 7-day period provided meaningful insights that could be a useful future research tool to examine the role of their physical environment on food access and availability.From this exploratory study, we highlight the following: (i) the future health trajectory of Pasifika youth is poor. Developing the youths' healthy lifestyle knowledge may lend itself to developing culturally relevant intervention programmes; (ii) identifying the enablers and barriers within the Pasifika ontext of an obesogenic environment can provide very useful information; (iii) use of spatial analysis using purchased food receipts adds to the current knowledge base of obesity-related research, although this was an exploratory investigation. We need to address these highlights if we are to reverse the trend of obesity for this population.

The aim of the study was to use recent data to determine whether Community Water Fluoridation (CWF) remains a cost effective public health intervention in New Zealand, given a reduction in dental caries in all communities over time.Local authorities that fluoridated their water supplies were asked to complete a questionnaire regarding fixed and variable costs incurred from CWF. Cost savings were calculated using data from the 2009 New Zealand Oral Health Survey. The cost effectiveness of CWF in conjunction with treatment per dmft/DMFT averted was compared to an alternative of treatment alone. Calculations were made for communities with populations of less than 5,000, 5,000 to 10,000, 10,001 to 50,000 and greater than 50,000.CWF was cost effective in all communities at base case. CWF remained cost effective for communities over 5,000 under all scenarios when sensitivity analysis was conducted. For communities under 5,000 the there was a positive net cost for CWF under certain scenarios.In this study, CWF was a cost effective public health intervention in New Zealand. For smaller communities cost effectiveness would be more dependent upon the population risk profile of the community.

PurposeThis study seeks to measure the public knowledge, attitudes, and practices related to eye health and disease in New Zealand (NZ).Design/methodology/approachA 22‐item survey of 507 adults in NZ was conducted. The survey was developed using interviews and focus groups, as well as comparisons with other benchmark international studies.FindingsOverall awareness about the importance of eye health is high in NZ, although knowledge about associated diseases (e.g. macular degeneration) is low – this is particularly important, given the risk and cost of preventable vision impairment.Originality/valueThis consumer survey of eye health in NZ, which assessed awareness and access to eye care, provides a foundation for those involved in the provision of eye care health. It is also a first step to further exploring key issues and identifying longitudinal patterns for consumer, provider, and public health dialogue, as well as a baseline for public health campaigns.

Developing environmental health indicators is challenging and applying a conceptual framework and indicator selection criteria may not be sufficient to prioritise potential indicators to monitor. This study developed a new approach for prioritising potential environmental health indicators, using the example of the indoor environment for New Zealand. A three-stage process of scoping, selection, and design was implemented. A set of potential indicators (including 4 exposure indicators and 20 health indicators) were initially identified and evaluated against indicator selection criteria. The health indicators were then further prioritised according to their public health impact and assessed by the five following sub-criteria: number of people affected (based on environmental burden of disease statistics), severity of health impact, whether vulnerable populations were affected and/or large inequalities were apparent, whether the indicator related to multiple environmental exposures, and policy relevance. Eight core indicators were ultimately selected, as follows: living in crowded households, second-hand smoke exposure, maternal smoking at two weeks post-natal, asthma prevalence, asthma hospitalisations, lower respiratory tract infection hospitalisations, meningococcal disease notifications, and sudden unexpected death in infancy (SUDI). Additionally, indicators on living in damp and mouldy housing and children&rsquo, s injuries in the home, were identified as potential indicators, along with attributable burden indicators. Using public health impact criteria and an environmental burden of disease approach was valuable in prioritising and selecting the most important health impacts to monitor, using robust evidence and objective criteria.

Background There are ethnic disparities in cervical cancer survival in New Zealand. The objectives of this study were to assess the associations of screening history, ethnicity, socio-economic status (SES) and rural residence with stage at diagnosis in women diagnosed with cervical cancer in New Zealand during 1994-2005. Methods The 2323 cases were categorized as 'ever screened' if they had had at least one smear prior to 6 months before diagnosis, and as 'regular screening' if they had had no more than 36 months between any two smears in the period 6-114 months before diagnosis. Logistic regression was used to estimate the associations of screening history, ethnicity, SES and urban/rural residence with stage at diagnosis. Results The percentages 'ever screened' were 43.3% overall, 24.8% in Pacific, 30.5% in Asian, 40.6% in M (a) over bar ori and 46.1% in 'Other' women. The corresponding estimates for 'regular screening' were 14.0, 5.7, 7.8, 12.5 and 15.3%. Women with 'regular screening' had a lower risk of late stage diagnosis [odds ratio (OR) 0.16, 95% confidence interval (CI) 0.10-0.26], and the effect was greater for squamous cell carcinoma (OR 0.12, 95% CI 0.07-0.23) than for adenocarcinoma (OR 0.32, 95% CI 0.13-0.82). The increased risk of late-stage diagnosis (OR 2.72, 95% CI 1.99-3.72) in M (a) over bar ori (compared with 'Other') women decreased only slightly when adjusted for screening history (OR 2.45, 95% CI 1.77-3.39). Conclusions Over half of cases had not been 'ever screened'. Regular screening substantially lowered the risk of being diagnosed at a late stage. However, screening history does not appear to explain the ethnic differences in stage at diagnosis.

© 2015 Springer-Verlag Berlin Heidelberg Purpose: To quantify serum concentrations of 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) and dioxin-like compounds in former phenoxy herbicide production plant workers and firefighters, 20 years after 2,4,5-T production ceased. Methods: Of 1025 workers employed any time during 1969–1984, 430 were randomly selected and invited to take part in a morbidity survey and provide a blood sample; 244 (57 %) participated. Firefighters stationed in close proximity of the plant and/or engaged in call-outs to the plant between 1962 and 1987 also participated (39 of 70 invited). Reported here are the serum concentrations of TCDD and other chlorinated dibenzo-dioxins, dibenzofurans, and polychlorinated biphenyls (PCBs). Determinants of the serum concentrations were assessed using linear regression. Results: The 60 men who had worked in the phenoxy/TCP production area had a mean TCDD serum concentration of 19.1 pg/g lipid, three times the mean concentration of the 141 men and 43 women employed in other parts of the plant (6.3 and 6.0 pg/g respectively), and more than 10 times the mean for the firefighters (1.6 pg/g). Duration of employment in phenoxy herbicide synthesis, maintenance work, and work as a boilerman, chemist, and packer were associated with increased serum concentrations of TCDD and 1,2,3,4,7-pentachlorodibenzo-p-dioxin (PeCDD). Employment as a boilerman was also associated with elevated serum concentrations of PCBs. Conclusions: Occupations in the plant associated with phenoxy herbicide synthesis had elevated levels of TCDD and PeCDD. Most other people working within the plant, and the local firefighters, had serum concentrations of dioxin-like compounds comparable to those of the general population.

Background: Injuries resulting in admission to hospital provide an important basis for determining priorities, emerging issues, and trends in injury. There are, however, a number of important issues to be considered in estimating person based injury incidence using such data. Failure to consider these could result in significant overestimates of incidence and incorrect conclusions about trends. Aim: To demonstrate the degree to which estimates of the incidence of person based injury requiring hospital inpatient treatment vary depending on how one operationally defines an injury, and whether or not day patients, readmissions, and injury due to medical procedures are included. Method: The source of data for this study was New Zealand's National Minimum Dataset. The primary analyses were of a dataset of all 1989–98 discharges from public hospital who had an external cause of injury and poisoning code assigned to them. Results: The results show that estimates of the incidence of person based injury vary significantly depending on how one operationally defines an injury, and whether day patients, readmissions, and injury due to medical procedures are included. Moreover the effects vary significantly by pathology and over time. Conclusions: (1) Those using New Zealand hospital discharge data for determining the incidence of injury should: (a) select cases which meet the following criteria: principal diagnosis injury only cases, patients with day stay of one day or more, and first admissions only, (b) note in their reporting that the measure is an estimate and could be as high as a 3% overestimate. (2) Other countries with similar data should investigate the merit of adopting a similar approach. (3) That the International Collaborative Effort on Injury Statistics review all diagnoses within International Classification of Diseases 9th and 10th revisions with a view to reaching consensus on an operational definition of an injury.

The spectrum of the VATER association has been debated ever since its description more than two decades ago. To assess the spectrum of congenital anomalies associated with VATER while minimizing the distortions due to small samples and referral patterns typical of clinical series, we studied infants with VATER association reported to the combined registry of infants with multiple congenital anomalies from 17 birth defects registries worldwide that are part of the International Clearinghouse for Birth Defects Monitoring Systems (ICB-DMS). Among approximately 10 million infants born from 1983 through 1991, the ICB-DMS registered 2,295 infants with 3 or more of 25 unrelated major congenital anomalies of unknown cause. Of these infants, 286 had the VATER association, defined as at least three of the five VATER anomalies (vertebral defects, anal atresia, esophageal atresia, renal defects, and radial-ray limb deficiency), when we expected 219 (P

A study was conducted in a cohort of New Zealand births on the prevalence of anencephalus and spina bifida in relation to various chemical elements in the drinking water. Cases were ascertained from national infant mortality and late fetal death records, and public hospital records and consisted of still and live births (of 28 or more weeks gestation) during 1978-1982 with anencephalus or spina bifida diagnosed either at birth or during the first year of life. The 460 cases were matched with their official birth registration records and a sample of 5000 control births was randomly selected from all non-NTD still- and livebirths. Drinking water data were obtained from a national surveillance program. Iron was the only water element to be included in the stepwise multiple logistic regression model of anencephalus prevalence, and magnesium, sulphate, and nitrate nitrogen were retained in the spina bifida model. This indicates a statistically significant association between these elements and the respective outcome. The results for iron and nitrate nitrogen were contrary to findings from previous studies. Despite the methodological problems associated with such ecological studies, there are indications that the constituents of a water supply may have an impact on the risk of NTDs.

We have established and/or upgraded cancer registries in four Pacific countries, a region where few cancer registries exist. We report age-standardised cancer incidence in Tonga (2000-2005), Fiji (2002-2005), Cook Islands (2000-2005) and Niue (2000-2005), and in Pacific people in New Zealand (2000-2005).In each country we identified incident cancer cases by reviewing hospital discharge, death registration, cancer registration records, and pathology reports. The primary site and morphology data were coded using ICD-0, and age-standardised incidence rates were calculated.Age-standardised cancer incidence rates for Pacific people in New Zealand (315 per 100,000 person-years in females, 379 in males) were similar to those for New Zealand overall (322 in females, 404 in males); incidence rates were lower in the Pacific, with rates of 195 and 151 per 100,000 person-years for females and males respectively in Tonga, 231 and 126 in Fiji, 165 and 142 in the Cook Islands, and 228 and 131 in Niue. However, some specific cancers were elevated in the Pacific including cervical cancer (16 per 100,000 in Tonga, 51 in Fiji, 17 in Cook Islands, and 26 in Niue compared with 10 in Pacific people in New Zealand, and 8 in New Zealand overall), liver cancer (rates of 8, 5, 19, 0, 7, and 2 respectively) and uterine cancer (rates of 24, 18, 47, 19 and 12 respectively).Cancer incidence in the Pacific is lower than for Pacific people living in New Zealand. Environmental rather than genetic factors are most likely to explain these patterns, and cancer incidence in the Pacific is likely to increase to rates similar to those in New Zealand as the region becomes more 'westernised' The high rates of cervical cancer and liver cancer in the Pacific indicate an important role of

Objective: There are substantial ethnic inequalities in stage at diagnosis and cervical cancer survival in New Zealand. We assessed what proportions of these differences were due to screening history (for the analyses of late stage diagnosis), stage at diagnosis (for the analyses of survival), comorbid conditions (for the analyses of survival), and travel time to the nearest General Practitioner and cancer centre. Methods: The study involved 1594 cervical cancer cases registered during 1994–2005. We used G-computation to assess the validity of the estimates obtained by standard logistic regression methods. Results: Māori women had a higher risk of late stage diagnosis compared with ‘Other’ (mainly European) women (odds ratio (OR) = 2.71; 95% confidence interval 1.98, 3.72); this decreased only slightly (OR 2.39; 1.72, 3.30) after adjustment for screening history, and travel time to the nearest General Practitioner and cancer centre. In contrast, the (non-significantly) elevated risk in Pacific women (1.39; 0.76, 2.54) disappeared almost completely when adjusted for the same factors (1.06; 0.57, 1.96). The hazard ratio of mortality for cervical cancer for Māori women was 2.10 (1.61, 2.73) and decreased to 1.45 (1.10, 1.92) after adjustment for stage at diagnosis, comorbid conditions, and travel time to the nearest General Practitioner and cancer centre; the corresponding estimates for Pacific women were 1.96 (1.23, 3.13) and 1.55 (0.93, 2.57). The G-computation analyses gave similar findings. Conclusions: The excess relative risk of late stage diagnosis in Māori women remains largely unexplained, while more than half of the excess relative risk of mortality in Māori and Pacific women is explained by differences in stage at diagnosis and comorbid conditions.

To estimate occupational mortality rates in New Zealand males for the period 2001-2005.Occupation information noted in the free text of death records of males aged 15-64 years during 2001-2005 was classified to the New Zealand Standard Classification of Occupation 1999 and age and deprivation standardised mortality rates and ratios were calculated for the major ICD10-AM categories of disease and different occupational groups.A total of 12,713 male deaths were included in the study. There were marked differences in mortality between occupations. Plant and machine operators and assemblers and agriculture and fishery workers had the overall highest rates. The former had the highest rates for ischemic heart disease, other diseases of the circulatory system, diseases of the respiratory system, and endocrine, nutritional and metabolic diseases, while the latter had the highest rate for external causes.The last published study investigating occupational disease mortality in New Zealand is now over 20 years old and currently no regular monitoring of occupation-related mortality is occurring in New Zealand. This paper shows that there continues to be marked differences in mortality between occupations in New Zealand and that many of these differences persist following adjustment for socioeconomic deprivation.

The 2009 Cancer Care Survey aimed to gather information from patients about their experiences receiving outpatient cancer care.In mid-2009, Cancer Control New Zealand sent an NRC+Picker postal survey to a stratified sample of 3251 eligible adults, who had received outpatient cancer care between October 2008 and March 2009. Eight cancer treatment facilities across New Zealand provided patient lists from which potential respondents were selected.The final response rate to the survey was 68%. Most of the patients surveyed responded very positively to questions related to specialist care coordination (91% positive response; 95%CI: 90-93), the level of privacy (87% positive response; 95%CI: 85-89), and the dignity and respect provided by healthcare professionals (86% positive response; 95%CI: 85-88). However, patients tended to be much less positive about the level of information they received on the effects of cancer treatment on their day-to-day life (responses ranging between 30% and 40% positive) and the level of emotional support provided (36% positive response; 95%CI: 33-39). Responses from different cancer services tended to follow similar patterns, although for twelve questions there was at least a 20% difference in response between services.Overall, patients rated their outpatient cancer care experiences as positive, but important gaps exist in the provision of information, emotional support, and treating patients within the context of their living situation. Cancer patient experience surveys can achieve high response rates and generate useful information on patient perceptions of their care. This data can be used to inform quality improvement efforts at both national and cancer treatment service levels.

Despite the plethora of epidemiological research conducted on anencephalus and spina bifida, few of the studies have used multiple source case ascertainment and controlled for the effect of possible confounding factors. This paper reports the results from a study of the relationships between various risk factors and the prevalence of anencephalus and spina bifida in New Zealand during 1978-82, using case data obtained from multiple sources and a national cohort of births as the denominator. The rates of anencephalus and spina bifida in New Zealand were 0.78/1000 and 0.94/1000 total births, respectively. The rate of a neural tube defect (NTD) birth for Maori parents was less than for their non-Maori counterparts. Paternal ethnic origin and maternal ethnic origin made similar contributions to the model of anencephalus rates, but the results suggest that paternal ethnic origin is a less important risk factor in the prevalence of spina bifida. The rate of both NTD was high among female infants and low among births to women born in countries other than the British Isles and New Zealand. The rate of anencephalus showed a distinct north-south gradient, but there was no evidence of effects for maternal or paternal age, parity, urban-rural place of residence, nuptiality, social class or season of birth in the prevalence of either NTD in New Zealand.

Objective: To investigate whether the rates of all cancers and four cancers (soft tissue sarcoma, non-Hodgkin's lymphoma, Hodgkin's disease and chronic lymphocytic leukaemia) associated with dioxin exposure are higher in New Plymouth, the site of a former 2, 4, 5-T manufacturing plant, than for the rest of New Zealand. Methods: Analysis of 1970–2001 cancer data from the New Zealand Cancer Registry was undertaken for New Plymouth and the rest of New Zealand. Results: There is no evidence of an increased cancer risk apart from one period (1970-74), which falls partly outside the 1962–1987 manufacturing period if 10-year latency is assumed. For 1970-74, there was an elevated risk for all cancer incidence (SIR=111, 95% CI 104–119), and for two of the four specific cancers that are associated with dioxin exposure (non-Hodgkin's lymphoma SIR=175, 95% CI 121–246 and chronic lymphocytic leukaemia SIR=251, 95% CI 144–408). Conclusions and Implications: The results do not suggest an increased cancer risk among the New Plymouth population related to the period of 2, 4, 5-T manufacture, although the study's limitations mean the possibility of an undetectable small elevation in cancer risk cannot be excluded. Although TCDD exposure in the first few years of 2, 4, 5-T manufacture may have contributed to cancer incidence in 1970-74, unknown exposure(s) before the start of 2, 4, 5-T manufacture and chance are also possible explanations.

An integrated environmental health impact assessment of road transport in New Zealand was carried out, using a rapid assessment. The disease and injury burden was assessed from traffic-related accidents, air pollution, noise and physical (in)activity, and impacts attributed back to modal source. In total, road transport was found to be responsible for 650 deaths in 2012 (2.1% of annual mortality): 308 from traffic accidents, 283 as a result of air pollution, and 59 from noise. Together with morbidity, these represent a total burden of disease of 26,610 disability-adjusted life years (DALYs). An estimated 40 deaths and 1874 DALYs were avoided through active transport. Cars are responsible for about 52% of attributable deaths, but heavy goods vehicles (6% of vehicle kilometres travelled, vkt) accounted for 21% of deaths. Motorcycles (1 per cent of vkt) are implicated in nearly 8% of deaths. Overall, impacts of traffic-related air pollution and noise are low compared to other developed countries, but road accident rates are high. Results highlight the need for policies targeted at road accidents, and especially at heavy goods vehicles and motorcycles, along with more general action to reduce the reliance on private road transport. The study also provides a framework for national indicator development.

The International Clearing-house for Birth Defects Surveillance and Research, formerly known as International Clearinghouse of Birth Defects Monitoring Systems, consists of 40 registries worldwide that collaborate in monitoring 40 types of birth defects. Clearinghouse activities include the sharing and joint monitoring of birth defect data, epidemiologic and public health research, and capacity building, with the goal of reducing disease and promoting healthy birth outcomes through primary prevention. We discuss 3 of these activities: the collaborative assessment of the potential teratogenicity of first-trimester use of medications (the MADRE project), an example of the intersection of surveillance and research; the international databases of people with orofacial clefts, an example of the evolution from surveillance to outcome research; and the study of genetic polymorphisms, an example of collaboration in public health genetics.

To predict differences in prostate cancer rates between New Zealand's major ethnic groups using community-based levels of elevated serum prostate-specific antigen (PSA).This study was undertaken in the Wellington region of New Zealand. 1425 subjects with no clinical history of prostate cancer had serum PSA levels measured--728 New Zealand European, 353 Maori, and 344 Pacific Island men. Age-standardised elevated PSA prevalences were calculated by standardising for population proportions. Prostate cancer prevalence ratios were predicted using a previously published method.There was no significant difference between New Zealand's ethnic groups in the prevalence of elevated PSA (p0.05). The overall age-standardised elevated PSA prevalence (3.9%) was lower than for all other community-based studies that were compared. Predicted cancer prevalence ratios were 1.1 across all New Zealand ethnic comparisons.The prevalence of elevated PSA in New Zealand men is lower than found in other community-based studies, and not significantly different between the three New Zealand ethnic groups. However, levels of elevated PSA may be useful for predicting prostate cancer incidence rates in ethnic groups. Available incidence data show New Zealand European men to have a higher prostate cancer incidence rate than both Maori and Pacific Islands men; however, this study found that prostate cancer incidence ratios between these groups are more likely to be closer to 1. Findings may indicate cultural barriers in the health system for Maori and Pacific Islands men; highlighting the need for clinicians to further consider cultural appropriateness in practice, and to target prostate health promotion for these groups.

STUDY OBJECTIVE: To assess spatial clustering of childhood leukaemias and lymphomas in New Zealand, using a national dataset from a country with no nuclear installations. DESIGN: New Zealand Map Grid coordinates, derived from the birth addresses of cases and controls were used in clustering analyses that applied Cuzick and Edwards' method. SETTING: The whole of New Zealand. PARTICIPANTS: The cases were ascertained from the New Zealand Cancer Registry. They were diagnosed with leukaemia or lymphoma at ages 0-14 years during the period 1976 to 1987. For Hodgkin's disease, the age range was extended to include those aged from 0-24 years. The cancer registrations were linked with national birth records, to obtain the birth addresses of the cases. The controls were selected at random from birth records, with matching to cases (1:1) on age and sex. The analyses included 600 cases and 600 controls. MAIN RESULTS: There was no statistically significant spatial clustering for any tumour group overall, including acute lymphoblastic leukaemia, acute nonlymphoblastic leukaemia, other leukaemias, non- Hodgkin's lymphomas, Hodgkin's disease, and all these combined. Significant clustering was found in a sub-analysis for one of three age specific subgroups of acute lymphoblastic leukaemia (ages 10-14 years, p = 0.003). CONCLUSION: The subgroup finding may have been real or a chance association, as several comparisons were made. This study found little evidence for spatial clustering of leukaemias or lymphomas in a population with no nuclear installations.

Background Increasing incidence rates for adenocarcinomas of the oesophagus and gastric cardia have been reported from the United States, Denmark, United Kingdom, Switzerland, and Sweden. This paper reports on the incidence of adenocarcinomas of the oesophagus and gastric cardia in New Zealand in the Maori (Polynesian), and non-Maori (predominantly European) populations. Methods Incidence data from the National Cancer Registry for 1978 through 1992 were used to compute age-adjusted rates by sex, ethnic group, anatomic subsite, morphology, 14 area health districts, and for three periods: 1978-1982, 1983-1987 and 1988-1992. Statistical tests for significance of trends and differences in frequencies were employed. Results Incidence rates for adenocarcinoma of the oesophagus are increasing in non-Maori men and women, but at a lesser rate than that reported for the US. The rate of 2.3 per 100000 population (1988-1992) for non-Maori men is similar to the rate for US white men of 2.5 (1988-1990). Rates for adenocarcinoma of the gastric cardia in non-Maori men declined from 2.5 in 1983-1987 to 1.9 in 1988-1992, and were stable at 0.4 in non-Maori women. However, rates for cases with unspecified anatomic subsite fluctuated over the 15-year period and probably caused a deflation in rates in the most recent 5-year period. Conclusions The incidence patterns of adenocarcinomas of the oesophagus and gastric cardia in New Zealand should be monitored over the next decade for confirmation of the trends observed here. There is need to review the quality of the data in the New Zealand registry.

We used New Zealand data on occurrence of different types of brain cancer to investigate: (i) a possible secular increase which has been seen worldwide and has generated considerable debate; (ii) possibly higher rates among Maori; and (iii) possibly higher risks related to social class and occupation. Data from the NZ Cancer Registry on the 5,684 brain cancers diagnosed among NZ residents from 1948-88 were used to study the pattern of occurrence by gender, age, race, calendar year, social class, occupation, and histology. Age-standardized brain-cancer incidence rates per 100,000 more than doubled over the 41-year period (from 2.9 to 6.9 in males and from 2.1 to 5.1 in females). A strong trend of increasing incidence with increasing social class is seen in males (Ptrend = 0.01). Among Maori, the proportion of all brain cancer that is medulloblastoma is four times that among non-Maori, and the proportion of all brain cancers that lack histologic confirmation is about 40 percent higher. Elevated risks are seen among: dairy farmers (odds ratio [OR] = 3.4, 95 percent confidence interval [CI] = 1.9-6.0); sheep handlers (OR = 2.7, CI = 1.4-5.3); livestock workers (OR = 3.8, CI = 1.7-8.4); and farm managers (OR = 3.2, CI = 1.4-7.2); as well as among electrical engineers (OR = 8.2, CI = 20-34.7); electricians (OR = 4.6, CI = 1.7-12.2); and other electrical workers. Brain cancer rates in NZ have increased steadily since 1948, but this increase has leveled off in the most recent five-year period. Although brain cancer rates are likely to be underestimated among the Maori, an excess of medulloblastoma is evident in this group.

Social class differences in mortality from causes of death amenable to medical intervention were examined. All deaths in New Zealand males aged 15-64 years during the periods 1975-1977 and 1985-1987 were identified. Strong social class gradients in mortality from causes of death amenable to medical intervention were observed during both periods. Furthermore, social class inequalities were more pronounced for amenable causes of mortality than for non-amenable causes. However, a marked decline in the age-standardized mortality rate from amenable causes was observed, with the rate falling by 30% over the 10-year study period. This decline was twice as large as the drop in the non-amenable mortality rate. Despite the fall in the death rate from amenable causes, social class inequalities in mortality persisted among New Zealand men, with the lowest socioeconomic group experiencing a death rate from amenable causes of mortality that was 3.5 times higher than men in the highest socioeconomic group.

The geographical distribution of child injury morbidity in Auckland between 1982 and 1987 was examined. Analysis of total injury, pedestrian injury and vehicle occupant injury, with the census area unit as the basic spatial entity revealed distinct variations in child injury morbidity by census area unit. Morbidity rates were above average in parts of the central urban area and South Auckland and below average on the North Shore. Total injury morbidity and pedestrian injury morbidity rates were strongly correlated with census area unit unemployment rates, which were used as a measure of socio-economic deprivation. Geographical areas with high rates of child injury morbidity, to which injury prevention resources can be directed, were identified. In particular, the results suggest that injury prevention programmes should be targeted at socio-economically disadvantaged communities.

Despite extensive research, little progress has been made in elucidating the etiologies of anencephalus and spina bifida. International and national distributions of disease occurrence have often been used as a basis for generating etiological hypotheses (e.g., potato blight, tea consumption, and zinc deficiency hypotheses). However, few of the epidemiological studies of neural tube defects (NTDs) have been conducted with scientific rigor in design, and most are of dubious validity, often with low precision in the estimates. This paper shows that the accepted geographic patterns of NTDs may be attributable to variations in the validity of studies used to describe these patterns. The nonuniformity in the duration and diligence of case ascertainment, the lack of a standardized nomenclature and classification, and the definition of the denominator remain principal problems in evaluating the epidemiology of NTDs. For example, the distinction between incidence and prevalence is not always made, and there is no consistency in the placement of the gestational boundary between late fetal deaths and spontaneous abortions. Findings are compared from studies conducted at different times, without due regard to the effect of secular trends, and using studies that have varying levels of case ascertainment. In etiological research, it is important to perform studies that are accurate and precise, but the literature used to define the spatial distribution of NTDs has often been accepted without due regard to the effect of these factors.
