The patient was known to have HIV infection and did not comply with therapy. Analytically he presented: Haemoglobin 92 g/L; white blood cells 2.3×109 /L; platelets count 19×109 /L; alanine aminotransferase 28u/L; aspartate aminotransferase 102 u/L; bilirubin 1.67 mg/L; lactate dehydrogenase 236u/L; ferritin 1481ng/mL; C-reactive protein 693.3 nmol/L and procalcitonin 5.67ng/mL.

Infection start primarily by inhalation of environmental basidiospores or poorly encapsulated yeast cells (with less than 5 μm), that can disseminate, after a latent period within lung lymph nodes. There are two principal species of Cryptococcus spp related with this disease: Cryptococcus neoformans and Cryptococcus gattii. Cryptococcus albidus and Crytpococcus laurentii are rarely associated with cryptococcosis in humans.
