The current pandemic SARS‐CoV‐2 has required an unusual allocation of resources that can negatively impact chronically ill patients and high‐complexity procedures. Across the European Reference Network on Pediatric Transplantation (ERN TransplantChild), we conducted a survey to investigate the impact of the COVID‐19 outbreak on pediatric transplant activity and healthcare practices in both solid organ transplantation (SOT) and hematopoietic stem cell transplantation (HSCT). The replies of 30 professionals from 18 centers in Europe were collected. Twelve of 18 centers (67%) showed a reduction in their usual transplant activity. Additionally, outpatient visits have been modified and restricted to selected ones, and the use of telemedicine tools has increased. Additionally, a total of 14 COVID‐19 pediatric transplanted patients were identified at the time of the survey, including eight transplant recipients and six candidates for transplantation. Only two moderate‐severe cases were reported, both in HSCT setting. These survey results demonstrate the limitations in healthcare resources for pediatric transplantation patients during early stages of this pandemic. Objetivo: evaluar la prueba de diagnóstico de Reacción en Cadena de la Polimerasa (PCR) cualitativo en tiempo real a partir del ADN proviral del VIH-1, en muestras de sangre seca en papel filtro. Métodos: realizamos un estudio observacional (octubre-diciembre 2016). Seleccionamos 56 muestras de sangre extraídos en tubos con EDTA (28 positivos a VIH y 28 negativos de niños expuestos a VIH (Gold Estándar: sistema COBAS® AmpliPrep/COBAS® TaqMan® HIV-1, v2.0 y PCR convencional HIV-1 ADN proviral). Las 56 muestras fueron impregnadas en papel filtro (DBS); añadiéndose 70 ul por cada circulo de la tarjeta 903 TM, se dejó secar (18-25°C) por tres horas y luego fueron almacenadas hasta su procesamiento por la prueba cualitativa de PCR-RT a partir del ADN proviral de VIH-1, extraídos del papel filtro con una solución fijadora de ADN. La sensibilidad, especificidad, límite de detección y precisión diagnóstica se calculó utilizando el programa Epidat v 3.1 y Excel. Resultados: la prueba cualitativa de PCR-RT a partir de ADN proviral de VIH-1 extraído de sangre seca en papel filtro mostró una sensibilidad y especificidad diagnóstica del 100% (IC 95%: 87-100) respectivamente. En cuanto a su límite de detección, esta prueba cualitativa también mostró buenos resultados al enfrentarlas 22 veces a cuatro muestras con diferentes valores de carga viral. Del mismo modo, se tuvo una alta precisión de la prueba al procesarse 11 repeticiones de estas cuatro muestras en dos días consecutivos. Conclusiones: los resultados nos muestran una buena eficiencia diagnóstica de la prueba molecular evaluada. En consecuencia, el uso de sangre seca en papel filtro de niños expuestos al VIH de lugares poco accesibles nos posibilitará realizar mejores intervenciones sanitarias dirigidas a eliminar la transmisión materno infantil del VIH.
